FA5205-17-Q-MG01 Nurse Utilization Manager Amendment 0003
Amendment 0003 – Changes: Period of Performance, Evaluation Criteria, PWS, Points of Contact, and Quote Due Date.
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice.
This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is Combo FA5205-17-Q-MG01 Nurse Utilization Manager; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-91, DPN 20161021, and AFAC 2016-0603. This acquisition will be full and open competition.

*Period of performance dates are subject to change after selection of awardee. At no time will the period of performance be greater than a combined 2 years 6 months.
Award shall be made on an all or none basis. The Government reserves the right to award the contract without discussions.

This Request for Quote will close out at 1500 on 23 Mar 2017 (JST).
All firms must be registered in the System for Award Management (SAM) database at https://www.sam.gov/portal/public/SAM/ to be considered for award. All proposals must be directly submitted by the firm registered in SAM that is intended to be the successful awardee.
Proposal submitted by another firm on behalf of a SAM registered company with the intention of being award "care of" will not be accepted.
EVALUATION CRITERIA: Evaluation of quotations will be completed in accordance with FAR 13.106-2. The Government will award the contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation and represents the best value to the Government. Best value is expected to result from selection of the technically acceptable proposal with the lowest evaluated price also known as Lowest Price Technically Acceptable
(LPTA).
Evaluation Factors: (1) Technical and (2) Price.
FACTOR I – TECHNICAL
Table A-1. Technical Acceptable/Unacceptable Ratings Adjectival Rating Description Acceptable Proposal meets the minimum requirements of the solicitation.
Unacceptable Proposal does not meet the minimum requirements of the solicitation.
FACTOR II – PRICE QUOTE
An offeror’s proposed prices will be verified on the Solicitation CLIN Pricing Schedule by multiplying the quantities identified for each item number in the price schedule by the proposed unit price to confirm the extended amount for each price schedule item. The extended price for each line item will then be added together to arrive at a total evaluated price. As this requirement is a “commercial” service that will result in a “firm-fixed-priced” contract, a comparison of offeror’s proposed, competitive prices will satisfy the requirement for price analysis
Options: The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
