PERFORMANCE WORK STATEMENT

FOR

THE SOUTH GATE ARCH SHELTER

AT

EIELSON AFB, ALASKA

(REVISED)

PEFORMANCE WORK STATEMENT

1. INTRODUCTION:

1.1. Eielson AFB (EAFB) is creating a temporary construction gate at the southern entrance of the base in support of the F-35 beddown. Temporary facilities are required for this area.

1.2. LOCATION. EAFB is located approximately 23 miles southeast of Fairbanks, Alaska.

2. PROJECT OBJECTIVES:

2.1. The objective of this project is for the Contractor to provide, deliver and install an Arch

Shelter for Eielson AFB, AK.

2.2. Description of Work.

CLIN Item Description

- Gable Truss Shelter

(OR EQUAL); 1 EACH

- Shelter must be at minimum 40ft X 80ft long.

- Fabric Cover (Tan*)

- 5-ton ECU ports; 6 EACH ECU ports no longer required

- Electrical ports; 6 EACH

- Two End Panels, each including the following:

1. 20ft wide X 16ft high metal roll-up door(OR EQUAL); 1 EACH

2. 30in X 80in hard door entry (OR

EQUAL); 1 EACH

3. 42 inch exhaust fan; 1 EACH

4. Each end panel will include an additional one electrical port.

- Anchorage Package (Spike) (OR

EQUAL)

1. The structure shall be anchored in place using spikes that can be driven through asphalt and anchored into the base course material of the asphalt pad.

2. The structure shall be anchored in such a fashion that it can withstand sustained wind speeds of 70 MPH with gusts up to 90

MPH.

- The structure must have stamped drawings by a licensed engineer showing that it is capable of withstanding 50 PSF snow loads and sustained 70 MPH winds with gusts up to 90 MPH.

* Alternate colors to be approved through Q&A solicitation process.

2.3. Delivery. Delivery and installation shall commence within 60 calendar days from contract award be accomplished by 03 April 2017.

3. INSTRUCTIONS TO THE CONTRACTOR:

3.1. The Contractor shall install all necessary equipment to complete the requirement.

Government Representatives from both 354th Secuirty Forces Squadron and 354th Civil Engineer

Squadron will be supplied upon contract award. The installer shall validate the system performance is in full working condition upon completion of the contract.

3.2. Quality Assurance. The Government shall periodically inspect the Contractor during installation. Any discrepancies discovered will be reported to the Contractor for remedial action.

The Government Representative will have the final decision on the acceptance of work carried out and ensure that the final installation of the arch shelter is fully operational. The arch shelter must be 100% compatible and in working order with the Eielson AFB provided utility hookups.

GOVERNMENT SPECIAL INSTRUCTIONS, TERMS AND CONDITIONS

4. GOVERNMENT FURNISHED EQUIPMENT/PROPERTY:

4.1. The Government shall provide the following:

i. The South Gate asphalt pad for staging purposes.

4.2. The Contractor shall provide the following:

i. Tools, labor, and equipment needed for installation.

4.3. Toilet Facilities. Use of existing toilet facilities in the immediate work area will be permitted.

4.4. Emergency Service. The government will provide emergency service, emergency medical treatment, and emergency patient transportation service for Contractor personnel who are injured or become critically ill while at Eielson AFB. Treated personnel will be responsible for reimbursing the government for the cost of medical treatment and patient transportation. For all emergencies using an on-base phone, please dial 911. If using a cell phone, please dial (907)

377-5130 (Security Forces Law Enforcement Desk/BDOC). Fire emergencies/routine calls can also be reported at the before mentioned phone numbers.

4.5. Telephone. The government shall not provide telephone services. If available, the

Contractor may use government phones in a case of emergencies during field and site work. No long distance telephone calls will be authorized.

4.6. Damage to Property or Equipment. The Contractor, at his own expense, shall repair any damage to Government property or equipment that can be attributed to the Contractor. Said repairs shall be made by a skilled workman, and to the satisfaction or the Contracting Officer or a designated representative.

5. GENERAL INFORMATION:

5.1. Hours of Operation. Working hours for the contractor shall normally be between the hours of 0730-1630 excluding Saturdays, Sundays, and Federal holidays. If the Contractor desires to work during periods other than above, a government representative may need to be present. The

Contractor shall make his/her request to the Contracting Officer five (5) calendar days in advance of his/her intention to work during other periods to allow assignment of a Government

Representative. If such force is reasonably available from assigned contract administrators or quality assurance personnel, the Contracting Officer may authorize the contractor to perform work during periods other than normal duty hours/days.

5.2. Disposition of Waste, Excess, Scrap, and Salvage Materials. The title to all waste, excess, scrap, and salvage materials generated as a direct result of this contract is vested in the

Contractor - unless specifically excepted below. Further, these materials shall be disposed of off the base by the Contractor in accordance with all applicable Federal, State, and Local

Regulations.

5.3. Clean Up. The Contractor shall, keep the work site and storage area(s) free from accumulation of waste, rubbish, or construction debris. All loose or lightweight materials shall be secured to prevent blowing or scattering. The burning of trash or construction debris is strictly prohibited on Eielson AFB. Prior to final inspection, the Contractor shall remove all construction debris, tools, equipment, and materials not the property of the Government. Upon completion of the work, the Contractor shall leave the work site area(s) in a clean, neat and workmanlike condition satisfactory to the Contracting Officer.

6. SERCURITY REQUIREMENTS:

6.1. Levels of Security. Eielson AFB security may be viewed as being one of three broad levels, the level in effect being dependent on the location or AREA of Eielson AFB in question.

6.1.1. The lowest level of security exists in “GENERAL ACCESS” areas. These areas are all of Eielson AFB outside of the "USAF Controlled" and "USAF Restricted" areas.

6.1.2. “USAF Controlled” access areas are the mid-level security areas. Presence within the CONTROLLED ACCESS AREA is by authorization; however, the movement of authorized personnel in and out of these AREAS is generally not impeded.

6.1.3. The highest level of security on Eielson AFB is that within the “USAF

RESTRICTED AREAS”. Work schedule requests or changes to existing schedules for work to be performed in a “USAF RESTRICTED AREA” shall be submitted at least 7 calendar days in advance for approval and coordination of Eielson AFB Security Forces.

6.2. Base Access. The Contractor shall be responsible for and shall pay for obtaining all necessary clearances, cards, passes, decals, or other items required for access of Contractor personnel and vehicles to the base or other areas where work will be performed. The Contractor shall be required to apply for vehicle and personnel identification media at the Base Pass and

Registration Section (Visitor Control Center). Upon Completion or termination of the contract, SOUTH GATE ARCH SHELTER

PWS – South Gate Arch Shelter (REVISED) 16 September 2016 Page 4 of 4 the Contractor shall ensure all vehicle and personnel identification media are surrendered to the

Base Pass and Registration Section (Visitor Control Center). The Contractor shall obtain the following pass and identification items: Eielson AFB Vehicle Pass. For all further inquiries, please contact the Visitor Control Center at (907) 377-3807.

6.2.1. The Contractor shall maintain a current listing of employees. The list will include employee names, last four digits of social security numbers, and driver license numbers.

This list shall be provided to the Contracting Officer (CO), and servicing Security Forces organization. An updated listing shall be provided upon change of employees. The

Contractor must provide all names, last four digits of social security numbers and duty hours of all personnel who will be on site at the start or anytime during this project for identification purposed in case of emergencies to base security.

7. MISCELLANEOUS:

7.1. Use of cell phones while operating any motor vehicle on base is strictly prohibited. Driving privileges will be suspended for 30 days for the first violation.

7.2. The on-base speed limit while passing military troops performing physical training is 10

MPH and is strictly enforced.
