PERFORMANCE WORK STATEMENT

Repair 2006 International R-11 Reg Number 06L0078 Date: 27 August 2024 I. Summary Repair 2006 International R-11 Reg Number 06L0078 in accordance with manufacturer’s recommendations and specifications to return this vehicle to original operational condition. Work must include: Prep unit for repairs, Remove damage from left side, straighten bent baffles, straighten tank panels, sand and prep tank, repaint tank the manufacturers original color.

1. Procurement of all parts and accessories required to repair this 2006 International R-11.

2. Provide all labor required to inspect, diagnose and repair this vehicle. See sample damage photos below.

3. Perform operational checks to ensure proper function and reliability as per manufacturer’s design.

II. Submittals: N/A

III. Products:

All materials and products provided by the contractor in the performance of this contract shall be in accordance with Manufacturers Specifications.

IV. Quality Control:

The Contractor shall be responsible for all quality control and all inspection and test that are required unless otherwise specified, arrange for factory tests and inspections when they are required.

1. The contractor shall perform all work according to accepted Industry Standards, Safety Procedures and Manufacturers Specifications.

2. The contractor shall schedule work to cause the least amount of interference with station operations.

3. The contractor is responsible for any damage to the work area or adjacent areas due to negligence and will immediately repair at the contractors own expense.

4. The Contractor shall perform field tests and provide labor, equipment, and incidentals required for testing. Also, the Contractor shall be able to produce evidence, when required, that each item of work has been constructed properly in accordance with the specifications, and product manufacturers’ instructions.

5. Work shall be performed by licensed personnel. Note: Required certification/license to be made available when requested. At the conclusion of the project, required certification shall be performed by Licensed Personnel to test and verify for proper operation.

V. Execution:

Scope of Work: Repair 2006 International R-11 Reg Number 06L0078 in accordance with manufacturer’s recommendations and specifications to return this vehicle to original operational condition. Work must include:

1. Period of Performance: Return 2004 International R-11 Reg Number 06L0078 within 60 days after receipt of the vehicle.

VI. Environmental & Safety:

1. The contractor shall perform all work according to accepted Industry Standards, Safety Procedures and Manufacturers Specifications.

2. Ensure all workers are wearing the proper PPE for the assigned task.

3. The contractor will dispose of all hazardous waste per local, state, and federal regulations.
