Course History:
• Two Rivers o Climbing/Rappelling Tower – Built by XXXXXXXXXXX in 2016.
o Dynamic Ropes Course – Catwalk, Vine Traverse, Giant Swing, Criss Cross, and Postman’s
Walk were built by XXXXXXXXXXX in spring 2016.
o Dynamo Tower – Built by XXXXXXXXXXX in 2010.
o Indoor Climbing Wall – Built by XXXXXXXXXXX in 2004.
o Low Ropes Course – Built by XXXXXXXXXXX in 2016.

Design & Engineering History:
• Two Rivers – The Indoor Climbing Wall (built 2004), Dynamo Tower (built 2010), Dynamic
Ropes Course (built 2016), and Climbing/Rappelling Tower (built 2016) were all designed by
XXXXXXXXXXX.

General Reminder:
• According to OSHA 3845-08 2016: Employers are responsible for providing free protective equipment such as:
o Harnesses or lanyards attached to an anchor or line. Seat harnesses may only be used in conjunction with safety nets or with lines that prevent workers from moving beyond the edge.
o Guardrails o Helmets o Safety nets o Gloves o When potential free fall is greater than 2 feet:
§ Full body harness attached at center back with a shock absorbing lanyard When potential free fall is less than 2 feet:
§ Full body harness attached at center back or front.
This release is aimed directly at Zip Line workers at Parks, Summer Camps, Amusement Parks, and Resorts, but the OSHA regulations that they are based on apply to all employees who work at height. Employers are also responsible for providing training on how to properly use and inspect the protective equipment. XXXXXXXXXXX strongly recommends that all facilities who employ people that work at height ensure that they are meeting OSHA regulations.
Annual/Quarterly Inspection Reminders:
• This inspection is an assessment of the physical condition of the course on the day of the inspection only. The physical condition of the course cannot be guaranteed after the day of inspection; weather, unauthorized use, vandalism, or changes in tree health or conditions can cause problems to occur. XXXXXXXXXXX does not take responsibility for changes in elements or for repairs to elements performed by the client or anyone other than XXXXXXXXXXX unless said changes or repairs are inspected by XXXXXXXXXXX prior to use.
• Facility staff and/or facilitators should be performing regular internal inspections of the course. These inspections should include thorough quarterly inspections of all components, monthly hands on inspections, and daily pre-use inspections. All inspections should be documented and be able to be presented to the inspector upon request. According to the current ANSI/ACCT Standards, “The organization shall implement an ongoing system of monitoring of all components at an interval specified by the designer, manufacturer or other qualified person.” (ANSI/ACCT 03-2019 DPI Standard B.1.3.)
• If your course utilizes trees, annual inspection of those trees by a Certified Arborist is highly recommended. Annual inspections by a Certified Arborist not only identify dead or dying trees, but can also identify potentially harmful conditions that can be remediated or treated to prevent the death of trees that support Challenge Course Elements.
Equipment Documentation:
• It is strongly recommended that the use of all life support systems be accurately documented in an equipment log. According to the current ANSI/ACCT Standards, “Equipment shall be inspected at intervals specified by the manufacturer or qualified person for correct operation. Supporting information may include date of purchase, use log, or other records”. (ANSI/ACCT 03-2019 DPI Standard I.3.2.1) If the age and or amount of use of these life-support systems cannot be accurately determined, they should be replaced.
• Each piece of program equipment that is used on your course (harnesses, belay devices, carabiners, auto belays, zip line braking systems, etc.) comes with a user’s manual. It is critically important that these manuals be read and understood thoroughly as well as kept for future reference and periodically ensure that you have the most recent version of these manuals.
Staff Training:
• One of the most critical parts of a safe challenge course is a properly trained staff. It is highly recommended that all staff that will help operate the course throughout the year receive a formal challenge course training. According to the current ANSI/ACCT Standards, “Training shall occur annually, or more frequently, contingent upon staff turnover, seasonal fluctuations, operational changes or as a response to incident/accident analysis.” (ANSI/ACCT 03-2019 Training Standard B.1.2.) & “Training shall be delivered by a qualified person.” (ANSI/ACCT 03-2019 Training Standard B.1.3.)
Operational Audit:
• External review of all aspects of a challenge course operation is an important tool to ensure that your organization is meeting or exceeding all applicable standards. According to the current ANSI/ACCT Standards, “The organization shall engage in a review of its practices by an external qualified person(s), at least once every five (5) years. (ANSI/ACCT 03-2019 Operation Standard B.2.7.)

Comments According to a statement released by the ACCT on December 8, 2009, “MSA issued a stop use notice on October 14, 2009 and an update on November 3, 2009. MSA has now stated that they will discontinue the Redpoint Descender.” The statement goes on to list part numbers and this auto belay does match one of the part numbers listed. Required Action: Stop using the Redpoint Descender IMMEDIATELY as it has FAILED the inspection.

Two Rivers Adjustments & Comments Structural Components Guy Systems:
All of the ground guy and internal guy systems are too loose. According to the current standards, “Guy systems shall be designed by a qualified person. The designer shall consider the elative support provided by structure, guys, and the interaction between them.” (ANSI-ACCT 03-2019 Standard D.4.2.)
Required Adjustments: Tighten the guy systems within 12 months.
There are 4 bolts that support the guy systems at the Vine Travers and Pirates Crossing Pole that are not completely secured in the pole. According to the current standards, “Inspection shall include an assessment for integrity based on the appropriateness of the termination (application), proper installation and fastener torque, negative effects from deflection, distortion, wear in the clamp or bolt or its connecting components, rust, corrosion, pitting that may affect the ability of the clamp or bolt to support the expected load, quality of welds, and misalignment with the expected load.” (ANSI-ACCT 03-2019 Standard E.4.2.2).
Required Adjustment: Tighten bolts completely to seat the head against the washers next to the pole within 12 months.
Utility Poles & JRDs:
It appears that an animal has dug around the utility poles that support the Climbing/Rappelling Tower. According to the current standards, “On wood poles, the inspector shall visually inspect for vertical checks and through-splits, horizontal cracks, decay pockets, shakes, shell rot, and other defects that may affect pole strengths and integrity. Additional consideration shall be made for potential ground line decay, pole top degradation or shrinkage that may result in loosening of hardware. Sub-grade inspection shall be done on older poles or poles in high stress environments using techniques and at intervals determined by a qualified person.” (ANSI-ACCT 03-2019 Standard D.3.3.2).
Required Adjustments: Fill in the holes at the base of the Climbing/Rappelling Tower within 60 days.
Climbing/Rappelling Tower General Comments:
The Climbing/Rappelling Tower would benefit greatly from treatment with a weather sealant/wood preservative. Treating the wood with a product such as T.W.P. (Total Wood Preservative) on a regular basis will help prolong the usable life of these components.
Recommended Adjustments: Treat wood surfaces with wood preservative.
There are several decking boards on the top deck of the Climbing/Rappelling Tower that are not fully secured to the structure. According to the current standards, “The inspector shall visually assess the supports, frame, joists, decking, and fasteners used in platform construction and assess platform integrity based on the negative effects from rot and wood degradation, component defects and deformities, and overgrowth. The inspector shall additionally assess platform size and strength, construction materials and preservatives, and any other potential hazard or environmental impact.”
(ANSI-ACCT 03-2019 Standard G.1.3).
Required Adjustments: Secure the decking boards within 12 months.
North Face:
The rapid links supporting the SRD’s are not fixed creating a possible wear point on the belay cable.
It is the recommendation of the inspector that the rapid links be fixed to the belay cable.
Recommended Adjustment: Fix the rapid links to the belay cable within 12 months.
South Face:
The rapid links supporting the SRD’s are not fixed creating a possible wear point on the belay cable.
It is the recommendation of the inspector that the rapid links be fixed to the belay cable.

West Face:
The rapid links supporting the SRD’s are not fixed creating a possible wear point on the belay cable.

Dynamic Course Catwalk:
The Catwalk is currently using 2 Auto Deadends to suspend the Catwalk. In April 2021, Hubbell along with their subsidiaries Fargo, Chance, and Anderson released a statement stating that their products are not designed for use in the Challenge Course Industry. These products include automatic deadends, suspension clamps, and pole bands. While all of their products have been used successfully for many years, they can no longer be used in this manner.
Required Adjustments: Replace 2 Auto Deadends with components that meet the current standards within 9 months.
Giant Swing:
The Giant Swing is currently using 2 Suspension Clamps to suspend the element. In April 2021, Hubbell along with their subsidiaries Fargo, Chance, and Anderson released a statement stating that their products are not designed for use in the Challenge Course Industry. These products include automatic deadends, suspension clamps, and pole bands. While all of their products have been used successfully for many years, they can no longer be used in this manner.
Required Adjustments: Replace 2 Suspension Clamps with components that meet the current standards within 9 months.
Dynamo Tower General Comments:
The Dynamo Tower would benefit greatly from treatment with a weather sealant/wood preservative.

Giant’s Ladder:
One of the vertical cables that support the Giant’s Ladders is twisted around the 5th rung. This can cause damage to the wood and the cable.
Required Adjustments: Un-twist the cable from the element before use.

Horizontal Access Belay Cable, North:
The belay cable is currently using 2 Auto Deadends to secure the belay cable to the structure. In April 2021, Hubbell along with their subsidiaries Fargo, Chance, and Anderson released a statement stating that their products are not designed for use in the Challenge Course Industry. These products include automatic deadends, suspension clamps, and pole bands. While all of their products have been used successfully for many years, they can no longer be used in this manner.
Required Adjustments: Replace 2 Auto Deadends with components that meet the current standards within 9 months.
There is 1 rapid link that is used for the backup on the belay cable’s east side termination that is showing signs of corrosion. According to the current standards, “The inspector shall assess the severity if any degradation on the integrity of the connector. The inspector shall assess connector integrity with consideration given to scoring, cracking, corrosion, area reduction, material incompatibility, defect in gate operation, hinge, locking mechanism, and deformation.” (ANSI-ACCT 03-2019 Standard I.3.5.4).
Required Adjustments: Replace the rapid link within 12 months.
Horizontal Access Belay Cable, South:
The belay cable is currently using 2 Auto Deadends to secure the belay cable to the structure. In April 2021, Hubbell along with their subsidiaries Fargo, Chance, and Anderson released a statement stating that their products are not designed for use in the Challenge Course Industry. These products include automatic deadends, suspension clamps, and pole bands. While all of their products have been used successfully for many years, they can no longer be used in this manner.

Leap of Faith, North:
There is 1 wire rope clip on the belay cable that is corroded. According to the current standards, “Wire ropes clip terminations shall be inspected for appropriateness of the termination (application) and negative effects from broken wires at the turn of the eye (if no thimble is present), corrosion, deformities, poor thread condition, nicking damage, etc.” (ANSI-ACCT 03-2019 Standard E.2.6.1.1.2).
Required Adjustment: Replace the 1 corroded wire rope clip within 12 months.
Leap of Faith, South:
There is 1 wire rope clips on the belay cable that is corroded. According to the current standards, “Wire ropes clip terminations shall be inspected for appropriateness of the termination (application) and negative effects from broken wires at the turn of the eye (if no thimble is present), corrosion, deformities, poor thread condition, nicking damage, etc.” (ANSI-ACCT 03-2019 Standard E.2.6.1.1.2).

All of the wood used in the construction of the Low Challenge Course Elements would benefit greatly from treatment with a weather sealant/wood preservative. Treating the wood with a product such as T.W.P. (Total Wood Preservative) on a regular basis will help prolong the usable life of these components.
Recommended Adjustments: Treat wood surfaces with wood preservative.
Islands:
The dimensional lumber used for the bridge on Islands is showing significant signs of wear.
According to the current standards, “The inspector shall visually assess the supports, frame, joists, decking, and fasteners used in platform construction and assess platform integrity based on the negative effects from rot and wood degradation, component defects and deformities, and overgrowth.
The inspector shall additionally assess platform size and strength, construction materials and preservatives, and any other potential hazard or environmental impact.” (ANSI-ACCT 03-2019 Standard G.1.3).
Required Adjustments: Replace the dimensional lumber within 9 months.
Mohawk Walk / Triangle Traverse / Wild Woosey:
There are several bolts that support the foot cables that are not completely secured in the poles.
According to the current standards, “Inspection shall include an assessment for integrity based on the appropriateness of the termination (application), proper installation and fastener torque, negative effects from deflection, distortion, wear in the clamp or bolt or its connecting components, rust, corrosion, pitting that may affect the ability of the clamp or bolt to support the expected load, quality of welds, and misalignment with the expected load.” (ANSI-ACCT 03-2019 Standard E.4.2.2).

Spider’s Web:
The 4x4’s that are being used for the frame of the Spider’s Web is not appropriate for this application.
According to the current standards, “The inspector shall visually assess the supports, frame, joists, decking, and fasteners used in platform construction and assess platform integrity based on the negative effects from rot and wood degradation, component defects and deformities, and overgrowth.
The inspector shall additionally assess platform size and strength, construction materials and preservatives, and any other potential hazard or environmental impact.” (ANSI-ACCT 03-2019 Standard G.1.3). Over time we have come to realize that 4x4’s simply do not withstand the test if time for this application. They root out quickly below grade and they do not have the structural integrity to hold a person if someone were to fall onto the web.
Required Adjustments: Replace/rebuild the structure within 12 months.
Whale’s Watch:
The Whale’s Watch is being hindered bushes growing up through the element. As the bushes grow, they can damage the element.
Required Adjustments: Remove the bushes before use.
There are several decking boards on the Whale’s Watch that are not fully secured to the structure.
According to the current standards, “The inspector shall visually assess the supports, frame, joists, decking, and fasteners used in platform construction and assess platform integrity based on the negative effects from rot and wood degradation, component defects and deformities, and overgrowth.

The Wall:
There is a board on the right side of the face that is not fully secured to the structure. According to the current standards, “The inspector shall visually assess the supports, frame, joists, decking, and fasteners used in platform construction and assess platform integrity based on the negative effects from rot and wood degradation, component defects and deformities, and overgrowth. The inspector shall additionally assess platform size and strength, construction materials and preservatives, and any other potential hazard or environmental impact.” (ANSI-ACCT 03-2019 Standard G.1.3).
Required Adjustments: Secure the board within 12 months.

Black Diamond equipment was designed and intended to be used in the recreational climbing market.
The manufacturer does not wish to be part of the Challenge Course and/or Zip Line industry. It is recommended that Black Diamond products not be used in a Challenge Course or Zip Line application.
Required Action: Follow the manufacturer’s recommendations for the use of this equipment.
Black Diamond D None Locking:

The Black Diamond D None Locking Carabiners are not appropriate for use in a personal safety system because they gates do not have any kind of locking mechanism. According to the current standards, “Life safety systems shall be engineered systems or be designed by a qualified person and be manufactured and/or installed by a competent person.” (ANSI-ACCT 03-2019 Standard E.1.1.)
Required Adjustment If these carabiners are being used as part of a personal safety system, then they should be replaced with an auto locking carabiner, twist locking carabiner, or a screwgate locking carabiner.

It is the inspector’s responsibility to gather information regarding equipment purchase and/or implementation dates.” (ANSI-ACCT 03-2019 Standard I.3.2.2.2.). To comply with manufacturer’s recommendations, these harnesses need to be retired next year.
Required Adjustment: Retire these harnesses in 2022.
There are 2 Robertson Cypress Seat harnesses that have a manufacture date of 2010. According to the manufacturer, they have a maximum 10-year life. According to the current ANSI-ACCT Standards, “Manufacturer’s instructions and equipment implementation shall be referenced in determining synthetic material retirement. It is the inspector’s responsibility to gather information regarding equipment purchase and/or implementation dates.” (ANSI-ACCT 03-2019 Standard I.3.2.2.2.). To comply with manufacturer’s recommendations, these harnesses need to be retired.

Rescue Rope:
Leaving knots tied in rope will contribute to a ropes wear. The continued use of the same knot puts unnecessary stress on that point of the rope and can lead to an unexpected failure. According to the current standards, “Inspection shall include assessment of ropes cordage integrity with consideration given to wear, cuts, discoloration, or glazing; stiffness, softness, or inconsistency; change in diameter or bend radius; unknown or suspect history; age; use.” (ANSI-ACCT 03-2019 Standard I.3.11.2).
Another added benefit of tying and untying a knot every use it that it allows a full inspection of the rope before and after every use.
Required Adjustments: Remove the knots after every use.
