(5) Location in the general geographical area of the project and knowledge of the locality of the project; provided, that application of this criterion leaves an appropriate number of qualified firms, given the nature and size of the project.

(6) Acceptability under other appropriate evaluation criteria, as indicated below.

Mountain Home Air Force Base will provide one architect-engineer evaluation board composed of its members who, collectively, have experience in architecture, engineering, construction, and Government and related acquisition matters. One Government member of each board shall be designated as the chairperson.

Under the general direction of the 366th Contracting Squadron Chief of the Contracting Office (COCO) the evaluation board shall:

(1) Review the current data files on eligible firms and responses to the public notice.

(2) Evaluate the firms in accordance with the criteria listed.

(3) Hold discussions with at least three of the most highly qualified firms regarding concepts and the relative utility of alternative methods of furnishing the required services.

(4) And prepare a selection report for the 366th Civil Engineering Squadron Base Civil Engineer recommending, in order of preference, at least three firms that are considered to be the most highly qualified to perform the required services. The report shall include a description of the discussions and evaluation conducted by the board to allow the selection authority to review the considerations upon which the recommendations are based.

The final selection decision shall be made by the Base Civil Engineer.

The selection authority shall review the recommendations of the evaluation board and shall, with the advice of appropriate technical and staff representatives, make the final selection. This final selection shall be a listing, in order of preference, of the firms considered most highly qualified to perform the work. If the firm listed as the most preferred is not the firm recommended as the most highly qualified by the evaluation board, the selection authority shall provide for the contract file a written explanation of the reason for the preference. All firms on the final selection list are considered "selected firms" with which the contracting officer may negotiate in accordance with FAR 36.606.

The selection authority shall not add firms to the selection report. If the firms recommended in the report are not deemed to be qualified or the report is considered inadequate for any reason, the selection authority shall record the reasons and return the report through channels to the evaluation board for appropriate revision.

The board shall be promptly informed of the final selection.

The final selection authorizes the Contracting Officer to begin negotiations, beginning with the most preferred firm in the final selection.

The Contracting Officer will request priced proposals for fully burdened contract rates that will be used to price orders made under each contract. Refer to the table of categories for which rates will be requested.

If a mutually satisfactory contract cannot be negotiated, the contracting officer shall obtain a written final proposal revision from the firm and notify the firm that negotiations have been terminated. The contracting officer shall then initiate negotiations with the next firm on the final selection list. This procedure shall be continued until a mutually satisfactory contract has been negotiated. If negotiations fail with all selected firms, the contracting officer shall refer the matter to the selection authority who, after consulting with the contracting officer as to why a contract cannot be negotiated, may direct the evaluation board to recommend additional firms in accordance with FAR 36.602.

Instructions.

To be considered for contract award, firms must file a Standard Form 330. The most current version is available for download at https://www.gsa.gov/reference/forms/architectengineer-qualifications

PROFESSIONAL QUALIFICATIONS POINTS (100 MAX): ________________________________

(2) Past performance on contracts with government agencies and private industry in terms of cost control, quality of work, compliance with performance schedules.

The Government will evaluate the past performance of the firm on its three most recent DoD or other federal contracts using Contractor Performance Assessment Rating System (CPARS) categories: Quality, Schedule, Cost Control, Management, and Regulatory (see provided table below for definitions). Past performance questionnaires may be used if CPARS assessments do not exist provided they utilize the categories and ratings below.

Each category will be rated with one of the following: Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory (see provided table below for definitions and scores). Based on the ratings that have been assigned to each category, scores will be averaged, providing an average score between 0 and 4. Points will then be assigned based on this average score.

Category: Definition:

Quality Quality reflects the Contractor's management of the quality control program, as well as the quality of the work itself. Questions which should be addressed are as follows: Has a quality product been provided? For example:

- Ability to maintain quality control

- Ability to address and review comments

- Independent Technical Review

- Whether plans are coordinated with specifications

- Coordination between disciplines

- Compliance with design criteria

Schedule The timeliness of the Contractor against the completion of the contract, task orders, milestones, delivery schedules, and administrative requirements (e.g., efforts that contribute to or affect the schedule variance).

Cost Control The Contractor's effectiveness in forecasting, managing, and controlling contract or order cost. For contracts or orders where task or contract sizing is based upon contractor-provided person hour estimates, the relationship of these estimates to ultimate task cost should be assessed. In addition, the extent to which the Contractor demonstrates a sense of cost responsibility, through the efficient use of resources, in each work effort should be assessed.

Management The integration and coordination of all activity needed to execute the contract or order, specifically the timeliness, completeness and quality of problem identification, corrective action plans, proposal submittals, the Contractor's history of reasonable and cooperative behavior (to include timely identification of issues in controversy), customer satisfaction, timely award and management of subcontracts.

Regulatory Compliance with all terms and conditions in the contract/order relating to applicable regulations and codes. Considering aspects of performance such as compliance with financial, environmental (example: Clean Air Act, Clean Water Act), safety, and labor regulations as well as any other reporting requirements in the contract terms and conditions.

Rating: Definition: Point Value:

Exceptional Performance meets contractual requirements and exceeds many to the Government's benefit. The contractual performance of the element or sub-element being evaluated was accomplished with few minor problems for which corrective actions taken by the Contractor were highly effective.

Very Good Performance meets contractual requirements and exceeds some to the Government's benefit. The contractual performance of the element or sub-element being evaluated was accomplished with some minor problems for which corrective actions taken by the Contractor were effective.

Satisfactory Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the Contractor appear or were satisfactory.

Marginal Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being evaluated reflects a serious problem for which the Contractor has not yet identified corrective actions. The Contractor's proposed actions appear only marginally effective or were not fully implemented.

Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the Contractor's corrective actions appear or were ineffective.
