PERFORMANCE WORK STATEMENTSTATEMENT OF WORK
REMOVE AND INSTALL FUME HOOD
MOUNTAIN HOME AFB (MHAFB), IDAHO
1.0. OBJECTIVE: Remove and replace the fume hood in the 366 LRS fuels laboratory. The fuels laboratory is located in building 1319. The contractor must complete the removal and installation in a 48 hour period that is scheduled with the flight commander or superintendent.
2.0. GENERAL SCOPE: Remove old fume hood and replace with a new fume hood already possessed by MHAFB in accordance with (IAW) the attached Laboratory Fume Hoods Manual and appendix and verified to fulfill the bases’ needs.
2.1. Disconnect the old fume hood from building power.
2.2. Disconnect the fume hood venting from the roof ductwork.
2.3. Remove the existing vacuum source tubing without damage.
2.4. Remove the old vent hood from the building. The existing cabinet pedestal shall remain in place for the new hood to rest upon. The lab has an outside door.
2.5. Verify that there are no blockages in the ductwork up and outside the building prior to installing the new hood.
2.6. Test that any fume hood motorized venting, located on the roof, is working properly, if present. Repairs, if needed, will be completed by base civil engineering.
2.7. Move the new hood inside the lab and prepare for installation per the manufacturer’s instructions.
2.8. Mechanically install the new vent hood on top of the existing cabinet pedestal. The building ductwork is roughly square, approximately 12 inches on a side. The fume hood exhaust vent is circular, approximately 13 inches in diameter.
2.9. Reroute the vacuum source tubing into the new hood.
2.10. Electrically connect the new hood to the existing circuit. Verify that all electrical switches function properly.
2.11. Ensure proper grounding throughout the entire installation. Electrical grounds must meet relevant electrical codes. The ductwork shall be grounded if required by manufacture’s installation guidelines or applicable codes.
2.12. Verify that there are no vapor leaks in the ductwork.
2.13. Working with base bioenvironmental, confirm that the air flow up and out of the building meets or exceeds bioenvironmental guidelines when the work is complete.
3.0. ENVIRONMENTAL CONTROLS
3.1. Hazardous Material Inventory Material Safety Data Sheets, and Safety Data Sheets: Approval is required before bringing any hazardous materials (HAZMAT) onto the installation. Consult with the Hazardous Materials Program Manager (366 FW/A7IE) at 208-828-1684/6351 for a determination of whether or not a product is a HAZMAT and what additional requirements may be applicable/necessary.
3.2. Material Storage and Use. The contractor shall follow manufacturer's guidelines and professional recommendations for control of humidity, temperature, cleanliness, and materials handling. This includes hazardous materials.
3.3. Notification of Environmental Spills. If the contractor spills or releases any substance into the environment regardless of quantity, the contractor or its agent shall immediately report the incident to MHAFB Fire Department at 208-828-6292 and all emergencies dial 911. The liability for the spill or release of such substances rests solely with the contractor and its agent.
3.4. Hazardous Waste. Waste. Identify, label, handle, store, and dispose of hazardous waste or debris in accordance with federal, state, and local regulations. Ensure all waste has undergone the appropriate hazardous waste determination and documentation is retained for these determinations. Do not dispose of hazardous waste on Government property. Do not discharge any waste to a sanitary sewer, storm drain, or to surface waters or conduct waste treatment or disposal on Government property without written approval of the Contracting Officer and Environmental. Consult with the Hazardous Waste Program Manager (366 FW/A7IE) at 208-828-1684/6351 prior to disposal of any waste.
4.0. GENERAL INFORMATION
4.1. Period of Performance: The contractor shall have two (2) days to complete removal and installation of the new unit. All work must be completed prior to 1600 HRS on 30 August 2019.
4.2. Base Hours of Operation: The contractor is responsible for performing the responsibilities of the SOW during business days. The contractor shall coordinate with MHAFB for scheduling. Duty Hours. Base hours of operation are 7:30 – 4:30 Monday through Friday. The contractor may find it necessary to deviate from the normal base hours of operation, to ensure timely completion of work under this SOW at no additional cost to the Government. Work scheduled but not accomplished because of base closure due to weather, exercise, or actual alert, will be accomplished as soon as possible after reopening the base.
Recognized Federal Holidays. A minimum of ten paid holidays per year:

4.3. Contractor Licensure and Permits. The contractor shall, without any additional expense to the government, be responsible for obtaining any necessary licenses and permits. Additionally, the contractor is responsible for complying with any applicable Federal, State, and municipal laws, codes, and regulations in connection with the performance of work, as specified in the SOW, which are not provided, in advance, by the government. The contractor shall be liable for all personnel injuries and equipment damage that result from their negligence or failure to follow safety/health guidelines. The contractor shall take proper safety and health precautions to protect work, the workers, the public, and the property of others, to include appropriate insurance.
5.0. SECURITY REQUIREMENTS
5.1. Identification and Credentials. All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the government installation, shall abide by all security regulations of the installation.
5.1.1. Identification. The contractor shall provide a completed 366th SFS Form 30 for each employee requiring installation access to the CS within five (5) business days following contract award. After approval, the CS will notify the contractor that the employees may get their installation access ID at the Visitor’s Center. Employees must demonstrate vehicle registration, proof of insurance, and a valid driver's license in order for vehicles to be registered at Mountain Home AFB, Idaho. All employees shall fluently read, write, speak, and understand the English language.
5.1.2. Safeguarding: The contractor shall be responsible for safeguarding all Government property and controlled forms provided for contractor use. At the end of each work period, all Government facilities, equipment, and materials shall be secured.
5.2. Escorting: Contractor personnel without appropriate security clearance shall be escorted by authorized personnel within controlled areas.
5.3. Traffic Laws: The contractor shall comply with all MHAFB traffic regulations while on MHAFB.
5.4. Weapons, Firearms, and Ammunition: Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor owned vehicle or privately owned vehicle while on MHAFB.
5.5. For Official Use Only (FOUO): The contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Action (FOIA) Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting and safeguarding FOUO material.
5.6. Reporting Requirements: Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware that may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.
5.7. PHYSICAL SECURITY
5.7.1. The contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.
5.7.2. The CO may direct the contractor to remove, and the contractor shall remove, an employee from any assignment under this contract for reasons of security or misconduct. All contractor personnel are subject to security forces jurisdiction and the authority of the Installation Commander while on MHAFB.
6.0. ADMINISTRATIVE MATTERS
6.1. Records Management: All records, files, and documents, regardless of media (e.g., paper, electronic, etc.), as described in this SOW, which are the responsibility of the Contractor are the property of the Government and shall remain so upon termination or completion of the contract.
6.1.1. The contractor shall keep these item(s) current and maintain and dispose of them in accordance with the requirements established in AFI 33-322, Records Management Program, paragraph 10, AFMAN 33-363, paragraph 6.4, AFMAN 33-364, paragraph 2.10 and the Air Force Records Disposition Schedule which may be accessed from the following web address: https://www.my/af.mil/afrims/afrims/afrims/rims.cfm.4
6.1.2. Records shall be turned over to the government upon completion of the contract unless otherwise stated. All records are subject to the Freedom of Information and Privacy Act.
6.2. Safety Notification and Reporting: Notify the CO and any onsite 366 LRS point of contact of any mishap involving contractor personnel, AF assets, or AF operations, which result in injury, illness, or damage. Establish internal mishap notification procedures and brief all contractor personnel. Document the mishap notification briefing as part of initial safety orientation. Document and report any mishap using local base procedures. Provide a copy of all mishap reports to the CO within 7 calendar days following mishap. Injuries to contractor personnel shall be reported IAW OSHA requirements. Maintain copies of all mishap reports for one year.
6.3. Performance Evaluation: Contractor performance outputs are subject to surveillance by government personnel.
6.4. Licenses and Permits: The contractor shall, without any additional expense to the Government, be responsible for obtaining any necessary licenses and permits and for complying with any applicable Federal, State, and municipal laws, codes, and regulations in connection with the performance of work as outlined in this SOW, that are not provided, in advance, by the Government.
6.5. Injuries and Damages: The contractor shall be liable for all personnel injuries and equipment damage that result from their negligence or failure to follow safety/health guidelines.
The contractor shall take proper safety and health precautions to protect work, the workers, the public, and the property of others, to include appropriate insurance.
6.6. Telephones: Government telephone facilities shall be made available for local calls to the contractor for official business use only.
6.7. Security Police and Fire Protection: The Government will provide general on-base security and fire protection services. Security Police telephone extension is 8-2256 (on-base) and 208-828-2256 (from all other phones) for both emergency and routine calls. Fire protection emergency phone is 8-0911(from on-base phone) and 208- 828-0911(from all other phones).
6.8. Utilities: All reasonable quantities of electricity will be made available to the Contractor without charge.
6.9. Toilet facilities: Use of existing toilet facilities in the immediate work area will be permitted. The Contractor will ensure facility cleanliness is maintained at all times to the extent of cleaning up after Contractor personnel only.
6.10. Contractor Manpower Reporting: The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract via a secure data collection site. The contractor is required to completely fill in all required data fields using the following internet address: http://www.ecmra.mil/. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2016.
