I DESCRIPTION OF SERVICES

1.1 SCOPE OF WORK. The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items or services necessary to perform the operation and maintenance of the base’s 0.85 Million Gallon per Day (MGD) Sequencing Batch Reactor (SBR) wastewater treatment facility as defined in this PWS.

1.1.1 Transition Plan. Each offeror, with the exception of the incumbent, shall submit a detailed plan describing methods they intend to employ to smoothly transition from the incumbent contractor to themselves as the potential O&M contractor. The plan shall be submitted in conjunction with the quote. The plan shall detail; names of employees, positions, duties, certificates held, and perspective employment dates, methods and timelines for the transfer of contractor owned property, if any is to be transferred. If no equipment is to be transferred then provide a list of equipment owned or to be purchased, anticipated date of arrival to the site and its intended purpose.

1.2 OPERATIONS

1.2.1 General. The contractor shall operate the Mountain Home AFB Class III wastewater treatment plant (WWTP), and reuse water systems, IAW industry standards, federal, state and local regulations. The contractor shall adhere to requirements of the National Pollution Discharge Elimination System (NPDES) permit, State of Idaho Wastewater Reuse permit and all additions and or amendments to all permits and rules throughout the life of the contract. The contractor shall adhere to all permit associated plans pertaining to wastewater collections and treatment.

1.2.1.1 Operation of Wastewater Reuse Systems. The contractor shall be responsible for the operation of the wastewater reuse systems (within plant compound and the Golf Course reuse system) IAW permits and manuals to include the following: filters, strainers, meters, pumps, controls, samplers, monitoring systems, disinfection systems, chemical addition systems, chemicals and back flow prevention devices. Specifically, the contractor shall conduct routine maintenance of the golf course reuse system pump and motor, the flow meter/totalizer, minor control panel maintenance such as indicator bulb replacement, and control corrosion on panels, pumps and piping located at the WWTP storage lagoon. The chlorination system located at the WWTP storage lagoon used to chlorinate the effluent pumped to the golf course shall be maintained by the contractor. The chlorination system includes the hypochlorite storage tank and all of the chlorination system components (fittings) downstream from the tank to and including the injection point. The government shall maintain the pipe line from its connection to the effluent pump to the golf course pump station.

1.2.1.2 Operation and Maintenance of Cloth Media Filter System. The contractor shall be responsible for operation and maintenance of the cloth media filter and all associated pumps, piping, valves, tanks, gauges, and meters etc. provided to ensure compliance with effluent wastewater reuse permit requirements issued by Idaho Department of Environmental Quality (IDEQ) in the current and future permits. Operation shall include daily startup, monitoring and shutdown as required or, 24 hour. Operation and monitoring as required to meet the installations water reuse goals, current and future. Operation and maintenance shall be IAW manufacturer’s manuals, instructions, best industry practices. Maintenance shall be scheduled IAW applicable paragraphs in section 1.2 of this PWS.

1.2.2 Sampling and Analysis. The contractor shall be responsible for proper sampling protocol, analysis and laboratory quality assurance in relation to the National Pollution Discharge Elimination System (NPDES) permit, and the State of Idaho Wastewater Reuse Permit. If the contractor uses an outside laboratory to meet this requirement, the lab must be certified in the state in which the lab is located. Soil sampling required in the Wastewater Reuse Permit shall be the responsibility of the government.

1.2.3 Sludge Processing. The contractor shall process, haul and dispose of all screenings, grit waste and, sludge/bio solids IAW the permits and the (WWTP) Operations Plan, Appendix G Bio solids Management Plan. The Simco Rd. landfill operated by Idaho Waste Systems in Elmore County is the nearest permitted landfill.

1.2.4 Records. A log shall be maintained indicating daily operating activities. All operations and laboratory records shall be maintained for five (5) years and originals of each left on site upon contract completion.

1.2.5 Operations Reports. The contractor shall provide the government with the following reports in a contractor format acceptable to the COR:

Electronic Discharge Monitoring Report (eDMR) - No later than the fifth working day of each month the contractor shall electronically input permit required data to the Environmental Protection Agency’s eDMR, once complete notify the installation’s Water Quality Manager that the submission is complete for the preceding month.

Monthly Operations Report - No later than the second Wednesday of each month the contractor shall submit to the Contracting Officers Representative (COR) and CO a report explaining operations for the preceding month specifically including flow data, chemical usage, bio solids activities and a detailed discussion of operational problems and corrective actions.

Annual Report - a report summarizing operations for the period as described in the Wastewater Reuse Permit to include a summarization of operational problems and solutions for the period shall be submitted to the CO and COR by the second Wednesday in December of each year.

Laboratory Summary Report - a report summarizing laboratory results/data for the prior month shall be submitted to the CO and COR by the second Wednesday of each month.

1.2.6 Notice of Non-Compliance. The contractor shall notify the CO, COR, and 366 FW/A7I/A7IE (Environmental section) as soon as possible but not longer than 12 hours after an event having the potential to invoke a Notice Of Non-Compliance or Notice of Violation (NOV) from the state or federal authorities. The contractor shall also notify CO, COR, and 366FW/A7IE in writing as soon as possible, but not greater than two (2) calendar days after the date the contractor became aware of any non-compliance. The government may hold the contractor liable for all fines incurred as a result of non-compliance or notices of violation due to negligence on the part of the contractor.

1.3 MAINTENANCE/REPAIR

1.3.1 Preventive Maintenance. Preventive maintenance shall mean labor and materials predicted and scheduled for equipment or systems to prevent equipment failure, to extend equipment or parts life to the equipment’s or parts manufacturer’s standard life expectancy or beyond. A preventative maintenance and inspection (PMI) program shall be established for all wastewater related equipment and systems to prevent and correct deficiencies with process equipment and systems.

1.3.1.1 Corrective Maintenance/Repair. Corrective maintenance shall mean labor and parts or installation of new equipment necessary to repair or restore equipment or systems to normal operating functionality. The contractor shall be responsible for the repair or repair by replacement of wastewater related equipment. The government shall be responsible for the retrieval and install of pumps within tanks or vaults for which no retrieval systems exist.

Wastewater equipment or systems requiring the use of specialized equipment or engineering and construction services to restore the system or equipment to normal functionality shall be reviewed on a case by case basis by the COR and CO. See paragraph 1.3.2

1.3.1.2 Building Maintenance. The government shall be responsible for preventive and corrective maintenance on equipment not directly related to wastewater treatment i.e. Heating Ventilation and Air Conditioning (HVAC), potable water systems, backflow prevention devices, electrical systems, wiring, Motor Control Centers (MCC), variable drives, Supervisory Control and Data Acquisition (SCADA) systems, and controls except field sensors, and probes.

Structural components of buildings i.e. doors, windows, locks, roofing systems. Pavements i.e.

roads, sidewalks, drainage. Pest management i.e. weed control in turf areas, rock areas, roads sidewalks and rodent control in and around government facilities.

1.3.1.3 Sewer Truck Wash Pad Maintenance. The contractor shall monitor and maintain the wash pad and connected sump used by the government plumbing shop for day-to-day dumping of the government owned sewer truck. Maintenance shall include the interior of the pad area up to where the concrete meets the surrounding soil. Maintenance shall mean spreading wet material if necessary, piling dried material and hauling material following passage of a paint filter test to a certified landfill, periodic pumping of the sump pit and maintaining the catch trough free of solids. The government user shall be responsible for reporting, cleanup and disposal of any spillage caused by the government anywhere outside the concrete pad area.

1.3.1.4 Grounds Maintenance. Grounds maintenance shall be conducted by the government’s grounds contractor, for improved and semi-improved mowing, trimming and edging including the lagoon and Rapid Infiltration Basin (RIB) roadways. Lagoon piping and pump station area to be maintained by the WWTP contractor. Non-potable irrigation to be operated and maintained by the licensed land application wastewater operator. Contractor shall be responsible for tilling RIB including all basins no less than annually or more frequently as necessary to ensure proper infiltration rates.

1.3.2 Over and Above (O&A) Work Procedures. O&A work is routine or emergency maintenance or repair with a projected cost of materials and labor greater than $2,500 and or, requiring equipment or services estimated to be outside the normal scope of routine repairs or preventive maintenance as determined by the CO. The contractors project manager (PM) shall notify the COR 14 calendar days in advance of routine work meeting the O&A definition. The following procedures shall apply for all O&A work requests pertaining to this contract:

(1) The contractors PPM shall submit an O&A work request form (see appendix G) for work at any dollar amount per occurrence. The Contractor shall submit the form, with all applicable fields completed, to the COR and the PM within five business days for non-emergencies and within the requested timeframe for emergencies.

(2) All work requests submitted with a total value of less than $2,500 shall be reviewed and approved by the COR. All O&A work requests of $2,500 or more will be reviewed by the COR, and approved by the CO in order to make the determination of a fair and reasonable price. Work shall not commence on any O&A work until the Contractor receives written permission from the government.

(3) The COR for this contract shall –

(a) Promptly review the O&A work request for technical acceptability and the reasonableness of the price within five (5) working days.

(b) Verify that the proposed work is required, appropriate, and is not covered under the basic contract line items.

(c) Verify that funding is available for the O&A work request.

(4) The COR shall inform and coordinate with other appropriate government agencies on base as necessary in a timely and professional manner.

(5) Approval will be done in writing, unless under emergency conditions where oral approval may be used. All oral approvals will be followed up in writing. The approval will establish a firm-fixed price for the work request. Failure of the Contractor to provide the information required on the O&A work request, within the specified timeframe, may be grounds for the government to openly compete the O&A work. Additionally, if the government and the Contractor cannot agree upon a firm-fixed price for the O&A work request, the government has the right to openly compete the O&A work.

1.3.3 Maintenance Reports. The contractor shall submit to the CO and COR, a report on the second Wednesday of each month detailing the preventive maintenance activities for the prior month, repair activities and associated costs. The contractor shall submit to the CO and COR, an itemized list of all reimbursable labor, parts, materials and supplies.

1.3.4 Removed Parts and Materials. The contractor shall notify the COR when any item with a replacement cost over $250 is removed from equipment or facilities.

SERVICE DELIVERY SUMMARY

SECTION III

GOVERNMENT FURNISHED PROPERTY (GFP) AND SERVICES

3.0 REAL PROPERTY

SBR’s -34900 Utility Dump Pad -73491 Equipment and parts storage areas are those available within the buildings located at the WWTP compound. No other buildings are provided for contractor use.

3.1 Incidental Government Furnished Services

3.1.1. Telephones. Government telephone facilities shall be made available for local calls to the contractor for official business use only.

3.1.2. Security Forces and Fire Protection: The government will provide general on-base security and fire protection services. 366th Security Forces telephone is 8-2256 (on-base) and 208-828-2256 (from all other phones) for both emergency and routine calls. Fire protection emergency phone is 8-0911(from on-base phone) and 208- 828-0911(from all other phones).

3.1.3. Utilities: All reasonable quantities of electricity will be made available to the Contractor without charge.

3.1.4. Toilet facilities: Use of existing toilet facilities in the immediate work area will be permitted. The Contractor will ensure facility cleanliness is maintained at all times to the extent of cleaning up after Contractor personnel only.

3.2 Joint Inventory The contractor and COR shall conduct a joint inventory of all GFP where GFP is being provided. The initial inventory shall be conducted and forwarded to the CO NLT 10 calendar days following contract performance start date and at the beginning of each period of performance. Upon expiration of the contract, a joint inventory shall be accomplished with the successor contractor in the event the incumbent contractor is not awarded a new contract. In this case, the incumbent contractor will be provided disposition instructions for the GFP. A second joint inventory will not be required if the incumbent contractor is awarded the new contract. All joint inventories shall be conducted and documented by the contractor, signed by the contractor and the COR, and forwarded to the CO. Additions and/or deletions from the joint inventories will not be made without the consent of the CO and will be documented in the form of a modification and will be provided to the contractor.

SECTION IV

GENERAL INFORMATION

4.0 PROGRAM MANAGEMENT

4.1. Contract Manager. The Contractor shall appoint in writing to the CO within 10 business days following contract award a contract manager and alternate(s). Any changes to this letter of appointment shall be provided to the CO within three (3) business days. These individual(s) will be responsible for contract performance IAW the PWS. The CM or alternate shall function as the responsible charge operator and shall and be available to government personnel within 30 minutes of notification during normal duty hours and one hour after normal duty hours to discuss wastewater treatment plant and contract issues, have full authority to act for the contractor on all contract matters. The name of this person, and an alternate or alternates, will act for the contractor when the primary is absent. The CM and alternate shall have three years’ experience in activated sludge treatment, and must possess minimum certification as an Idaho Class III Wastewater Treatment Plant Operator and Land Application Operator on the contract start date, as well as, the entire contract period. Copies of Idaho certificates shall be provided with the quote package.

4.1.1 Contract Employees. The contractor shall maintain a sufficient number of qualified personnel on site to operate and maintain the WWTP as specified in the PWS. All treatment plant operators shall be certified in accordance with State of Idaho wastewater certification rules throughout the life of the contract. Personnel performing laboratory analysis for the purposes of regulatory compliance shall be trained and certified to a level of class II. Certification records shall be maintained on site, up to date and valid at all times. All initial and follow-on contractor personnel shall be fully qualified, trained, and knowledgeable on the operation and maintenance of waste water treatment plant and reuse water systems. All contractor personnel must present a neat appearance and be easily recognized as contractor employees. This may be accomplished by wearing contractor-furnished clothing bearing the company name and/or logo. Dress of office workers will be in good taste and conducive to working with the general public. The contractor will identify himself as a contractor in meetings, telephone conversations, and e-mails and in any correspondence with the. All employees shall fluently read, write, speak, and understand the English language

4.1.2 Contractor Licensure and Permits. The contractor shall, without any additional expense to the government, be responsible for obtaining any necessary licenses and permits. Additionally, the contractor is responsible for complying with any applicable Federal, State, and municipal laws, codes, and regulations in connection with the performance of work, as specified in the PWS, which are not provided, in advance, by the government. The contractor shall be liable for all personnel injuries and equipment damage that result from their negligence or failure to follow safety/health guidelines. The contractor shall take proper safety and health precautions to protect work, the workers, the public, and the property of others, to include appropriate insurance.

4.1.3 Problem Identification. The contractor’s CM shall implement procedures to encourage early problem identification and expeditious COR notification IAW the contractor’s quality assurance plan.

4.2 Government Program Management

4.2.1 Government Program Manager. The government’s FC and COR responsibility is assigned to 366th Financial Acquisition Squadron, 366 Gunfighter Ave., Suite 2102, Mountain Home Air Force Base, Idaho 83648-5258

4.3 OPERATIONAL HOURS AND EMERGENCY SERVICES

4.3.1 Normal Hours of Operation. The contractor shall operate and maintain the WWTP, reuse water systems (see paragraph 1.2.1.1), and pump stations 24 hours per day, 365 days per year.

At a minimum, the contractor shall physically man these facilities Monday - Friday, 0700 - 1600 and two (2) hours per day on weekends and holidays. When the facilities are not physically manned, continuous remote sensing shall be provided by the government. Facilities not configured with remote sensing capability or during system downtime regardless of cause, will require the contractor to perform periodic checks, to be described in the PMI plan.

4.3.2 Operational Emergencies. After normal hours of operation the contractor shall respond with all necessary equipment, i.e. emergency backup pumps, hoses, etc. (within 60 minutes of acknowledgement) to the site of an emergency involving a contracted facility or system.

4.3.3 Recognized Federal Holidays: A minimum of ten paid holidays per year.

A contractor may substitute for any of the named holidays another day off with pay in accordance with a plan communicated to the employees involved.)

4.4 QUALITY MANAGEMENT

4.4.1 Contractor Quality Control. The contractor shall develop, document, implement, maintain, and continuously improve a comprehensive Quality Control Plan that ensures WWTP operations and maintenance provide continuous, cost effective, and efficient treatment of wastewater, as well as compliance with Federal, State, local regulations, and all requirements of this PWS. The plan shall be submitted to the CO not later than the pre-performance conference.

The contractor shall develop and implement procedures to identify and prevent recurrence of deficiencies. As a minimum, the contractor shall develop quality control procedures addressing the areas identified in the Service Delivery Summary. The contractor shall make appropriate modifications and obtain acceptance of the plan by the contracting officer before the contract start date.

4.4.2 Government Quality Assurance. The government shall evaluate the contractor’s performance under this contract. Evaluations will be provided to the contractor, in writing, on a monthly basis. The COR shall inspect by validating actual work performance, physically checking an attribute of a completed task, checking a management information report, conferring with facility managers, or otherwise inspecting the task or its results to determine whether or not performance meets the standards contained in this PWS. When possible the contractor shall re-perform tasks that have been determined by the COR to not meet the standards outlined in this PWS. When a performance threshold has not been met or contractor performance and or re-performance has not been accomplished, the COR will initiate and provide the CO a Contract Discrepancy Report (CDR) DD Form 2772 for issuance to the contractor. Contractor performance will also be recorded annually in the contractor Performance Assessment Reporting System (CPARS) database. These evaluations may be used by other government agencies as a record of contractor performance history for the purpose of evaluation in other competitive or noncompetitive acquisitions.

4.4.3 Post Award Multi-Functional Team (MFT) Meetings. The CO, COR, other government personnel, as appropriate, and the contractor will meet quarterly or more often if necessary, to identify opportunities to improve performance. The MFT will discuss contract performance, identify initiatives, assess risk associated with the initiatives, and monitor the success of implementation. The government will also apprise the contractor of how the government views the contractor’s performance overall. Failing to meet the performance threshold as outlined in the SS of these performance objectives in any combination for any two, or more consecutive or non-consecutive months during a contract period shall constitute an immediate MFT.

4.5 Security Requirements. The contractor shall be responsible for safeguarding all government property provided for contractor use. When unoccupied, all government facilities, equipment, and materials shall be secured. The contractor shall establish and implement methods to ensure all keys issued to the contractor by the government are not lost or misplaced and are not used by unauthorized persons. The contractor’s PM shall be responsible for complying with the on-site security requirements as established by the 366th Fighter Wing Commander. The contractor shall support annual government security inspections and unscheduled spot inspections. A spot inspection is defined as any unscheduled security inspection.

4.5.1 Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their contractor-owned or privately-owned vehicle while on Mountain Home AFB.

4.5.1.1 Contractor/Employee Base Pass and Identification: The contractor shall provide a completed 366th SFS Form 30 for each employee requiring installation access to the CS within five (5) business days following contract award. After approval, the CS will notify the contractor that the employees may get their installation access ID at the Visitor’s Center.

The contractor shall retrieve all identification media, including vehicle passes from employees who depart for any reason before the contract expires i.e. terminated for cause, retirement, etc.

4.5.2 Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware which may pose a threat to the security of DoD or contractor personnel, resources, and classified or unclassified defense information.

4.5.3 For Official Use Only (FOUO). The contractor shall comply with DoD 5400-7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

4.5.4 IAW Department of Defense Instruction (DoDI) 3020.37. Continuation of Essential contractor Services During Crisis, and the Air Force implementation thereof, unless otherwise directed by an authorized government representative, it is determined WWTP services under this PWS are essential and must be performed during a crisis. The contractor shall continue providing service to the requesting organization 24 hours a day until the crisis is over. The contractor shall submit a contingency plan for operating under said conditions. This plan is due to the CO and COR within 30 days after contract award and shall include how the contractor will ensure continuation of services, contain the employee’s name, address, home phone number, cell phone number, social security number, security clearance, and duty title. Contractor employees, with approved background checks will be allowed unescorted access to the installation for performance of work in a FPCONs except “Delta”. The CO, PM, or COR shall notify the contractor of a “Delta” condition as soon as possible after security conditions have been implemented.

4.6 SPECIAL QUALIFICATIONS. Contractor employees must be in possession at all times of a valid state required driver licenses and appurtenant endorsements for all equipment used during the performance of this contract. The contractor and its employees shall comply with base traffic regulations.

4.7 HAZARDOUS MATERIAL IDENTIFICATION. Safety Data Sheets (SDS) are required as specified in the latest version of Federal Standard No. 313 (including revisions adopted during the term of the contract) for any material designated by a government technical representative as potentially hazardous and requiring safety controls. SDSs must be submitted by the contractor at the pre-performance conference. Failure to provide SDSs or certificates when requested could result in unsatisfactory performance ratings and possible termination of the contract.

4.7.1 Hazardous Material Handling. The contractor shall have approval from the base HAZMART section prior to purchasing, handling, using, and storing any chemicals, solvents, lubricants, and other products that require SDSs. The contractor shall identify these materials and products on Air Force Form 3952 Chemical/Hazardous Material Authorization Request, Process Identification Form, and Shop Disposal Form. The contractor shall provide one copy of the SDS for each item to the HAZMART section for review prior to any chemicals being brought onto Mountain Home Air Force Base. The contractor shall maintain one copy of the SDS for each hazardous material line item used within the work center. The contractor must provide one copy of the SDS for each item to the Environmental Flight prior to the use of any materials on base. The contractor shall comply with the requirements of the MHAFB HazMat Monitor’s Handbook. The contractor shall not use products that are or contain Toxic 17 chemicals, Extremely Hazardous Substances (EHS), Ozone Depleting Substances (ODS), and/or Persistent Bio-accumulative and Toxic (PBT) chemicals. Any hazardous materials containing one of these banned substances will not be allowed on base.

4.7.2. Spill Response. The contractor shall be briefed on Mountain Home AFB spill response procedures at the pre-performance conference. The contractor shall report and promptly clean up all spills in a manner consistent with current environmental regulations. In the event that it is necessary to utilize government material, equipment, or personnel to clean up a contractor caused spill, the contractor shall reimburse the government for all associated costs. All trucks shall be in good working condition to ensure leaks do not cause spill response actions.

Secondary containment for parked vehicles may be required.

4.8 SAFETY. The contractor shall provide a copy of their establish safety program under this contract to the CO and COR NLT the pre-performance conference. This plan shall identify preventative measures and delineates safety goals and objectives. The contractor shall support all government safety inspections, scheduled or unscheduled.

4.9 CONTRACTOR SUPPORT. In addition to support furnished by the contractor elsewhere in this PWS, the contractor shall:

4.9.1 Notification. Document incidents (suspected illegal discharges or overloads to the plant), accidents, threats, events, and disasters. Such notification shall be submitted to the CO and COR as soon as possible, but not later than 24 hours of the occurrence.

4.9.2 Corrective Action. The contractor shall take immediate action to correct noted deficiencies and reply promptly to evaluation, inspection, and survey reports. Corrective Actions shall be document on a Corrective Action Report Form available at https://cs2.eis.af.mil/sites/10059/afcc/knowledge_center/templates/corrective_action_report.pdf

4.9.3 Continuity. Continuous, uninterrupted contract service is vital to the government’s overall effort. Therefore, if the incumbent contractor is not awarded the new contract, the incumbent contractor shall, during the last 90 days of the contract, or any extension thereto, provide support to the government and the successor contractor to insure an orderly transition which will minimize any adverse impact on government operations. With regards to the successor’s access to incumbent employees, a recruitment notice may be placed in the facility.

4.10 REQUIRED REPORTS/DOCUMENTATION. The contractor shall maintain copies of the reports/documentation required under the terms of this PWS. The government maintains full rights to all such reports/documentation, which will be turned over to the government upon completion of the contract. The contractor shall submit the following reports as specified below:

4.11 RECORDS MANAGEMENT:

Records Management: All records, files, and documents, regardless of media (e.g., paper, electronic, etc.), as described in this PWS, which are the responsibility of the contractor are the property of the government and shall remain so upon termination or completion of the contract.

The contractor shall keep these item(s) current and maintain and dispose of them in accordance with the requirements established in AFI 33-322, Records Management Program, paragraph 10, AFMAN 33-363, paragraph 6.4, AFMAN 33-364, paragraph 2.10 and the Air Force Records Disposition Schedule which may be accessed from the following web address:

Records shall be turned over to the government upon completion of the contract unless otherwise stated. All records are subject to the Freedom of Information and Privacy Act.

Contractor Manpower Reporting. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract via a secure data collection site. The contractor is required to completely fill in all required data fields using the following internet address: http://www.ecmra.mil/. Reporting inputs will be for the labor executed during the period of performance during each government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2016. APPENDIX A

DEFINITION OF TERMS

WWTP: The base facility designated and designed to treat a combination of domestic wastewater, industrial wastewater, and storm water. Responsibility begins at the metering structure on the east side of perimeter road opposite the head works building and includes all appurtenances to the point of discharge and includes all discharge options listed in the NPDES permit. Also included is the storage lagoon, its piping and pump station. The Rapid Infiltration Basins and its piping are considered part of the treatment plant for the purposes of this statement of work. Piping between the plant compound and storage lagoon, and between the storage lagoon and RIB shall be the responsibility of the contractor. Effluent reuse system includes the storage lagoon wet well pump and motor used to pump treated effluent to the golf course. The contractor shall maintain the reuse system pump and motor. The flow meter/totalizer, including electrical components (controls) located at the WWTP storage lagoon wet well shall be the responsibility. The chlorination system located at the WWTP storage lagoon used to chlorinate the effluent pumped to the golf course shall be maintained by the contractor. The chlorination system includes, the hypochlorite storage tank and all of the chlorination system components (fittings) downstream from the tank to and including the injection point. The government shall maintain the pipe line from its connection to the effluent pump to the golf course pump station.

Operator Licensed personnel who operate and or maintain systems or components associated with the sewage collection system and or Wastewater Treatment Plant.

Wastewater Liquid discharged from domestic and industrial sources that has become contaminated through use and wasted to the plumbing and collection system.

Contracting Officer (CO): The CO is the only individual with the authority to enter into, administer, modify, and/or terminate contracts for the government. The CO is responsible for ensuring the performance and compliance with the terms of the contract and safeguarding the interests of the government in its contractual relationships.

Contract Specialist:

Contracting Officers Representative (COR) Designated government representative who shall perform surveillance of the contractor’s performance and is responsible for collecting all standardized evaluation and data collection forms for the purpose of a monthly evaluation of contractor’s performance.

Program Manager (PM) The contractor’s individual responsible for the overall performance of the contract appointed, in writing, by an officer of the company to the contracting officer.

Functional Commander (FC) The FC is the person responsible for a functional area.

Government Furnished Property (GFP) Property in the possession of or acquired directly by the government and subsequently delivered or made available to the contractor.
