SECTION L - INSTRUCTIONS

PROPOSAL PREPARATION INSTRUCTIONS

A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The response shall consist of four (4) separate parts; Part I - Summary and Miscellaneous Data, Part II - Price Proposal, Part III - Technical Proposal, and Part IV - Past Performance Proposal.

B. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer’s opinion, adequate price competition exists no additional cost information will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists offerors may be required to submit information to the extent necessary for the contracting officer to determine the reasonableness and completeness of the price.

C. Specific Instructions:

1. PART I – SUMMARY AND MISCELLANEOUS DATA. Submit original plus 4 copies.

(a) The offeror shall provide a concise summary and master table of contents of the entire proposal.

(b) Complete blocks 13, 15, 16, 17 and 18 of the RFP Section A, SF33. In doing so, the offeror accedes to the contract terms and conditions as written in the RFP Sections A through K. These sections constitute the model contract.

(c) Complete block 14 of the RFP Section A, SF 33, if any solicitation amendment(s) to the original solicitation documents was/were issued. Amendments will not be counted against the page limits.

(d) Complete the necessary fill-ins and certifications in Sections I through K. Section K shall be returned in its entirety. For Sections C through I, the offeror shall submit only those pages that require a fill-in.

(e) If applicable, provide a copy of the letter from the Small Business Administration (SBA) showing proof of Section 8(a) status.

(f) If none proposed, submit an Affirmation of no Exceptions, Deviations, or Waivers.

(g) Representations, certifications, and other statements of offerors or respondents shall be returned in its entirety. For the remainder of the solicitation, the offeror shall submit only those pages that require a fill-in.

2. PART II – PRICE PROPOSAL - Submit original plus 4 copies.

(a) This part shall contain a completed Section B (Supplies or Services and Prices/Costs).

(b) Insert proposed unit prices and extended amounts in Section B for each Contract Line Item Number (CLIN), including all option periods. Please note that the Unit Prices in the following CLINs will be provided by the Government, and will be used for evaluation purposes only: 0002, 0003, 0005, 0006, 1002, 1003, 1005, 1006, 2002, 2003, 2005, 2006, 3002, 3003, 3005, 3006, 4002, 4003, 4005, and 4006. The extended amount must equal the unit price multiplied by the number of units (do not back into the Unit Price by dividing an Extended Amount by the Units). It is imperative that mathematical calculations are correct; particular care should be taken to ensure the amounts match when using the EXCEL program and the rounding of figures, limited to two decimal places.

(c) Extreme care should be exercised to ensure that no price data of any kind are included in any other Part of the proposal.

(d) Complete all necessary fill-ins.

3. PART III – TECHNICAL PROPOSAL - Limited to no more than 25 pages. Submit original plus 4 copies.

Sub-Factor 1 - Recruitment/Retention: This sub-factor shows the offeror’s proposed methodology to meet the unique staffing and manning requirements of this contract, including attracting and retaining qualified personnel for hard-to-fill positions.

Sub-Factor 2 – Training: This sub-factor shows the offeror’s ability to meet the training demands of the contract, including without limitation training foreign and USAF personnel from basic familiarization to advanced weapons-based and/or maintenance instruction.

Sub-Factor 3 - Quality Management: This sub-factor demonstrates the offeror’s plan to ensure quality services are offered under this contract over the stated period of performance, in accordance with all applicable laws and regulations.

Sub-Factor 4 – Experience: This sub-factor demonstrates the offeror’s familiarity with 4th-generation fighter aircraft, including maintenance and/or operations support and training experience.

4. PART IV – PAST PERFORMANCE INFORMATION - Limited to no more than 5 pages per contract reference (not to exceed five contract references) containing information described in paragraphs (a) and (b) below. Only references for relevant past efforts/contracts (similar in scope and magnitude of effort and complexities this solicitation requires). Submit one paper copy, one CD-ROM, and one electronic copy via AMRDEC.

(a) Quality and Satisfaction Rating of Contracts Completed in the Past Five Years from the Date of This Solicitation’s Issuance: Provide any information currently available (letters, metrics, customer surveys, independent surveys, etc.) that demonstrates customer satisfaction with overall job performance and/or quality of completed and/or ongoing contracts. In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems, such as cost overruns, extended performance periods, numerous warranty calls, etc.

(b) Performance Surveys/Questionnaires: Past Performance Questionnaires (PPQs) shall be used by the offeror in obtaining and providing past performance information. The government requires the offeror send out a PPQ to each of the Points-of-Contact (POCs) identified in the Past Performance Proposal (paragraph (c) below).

The offeror shall send out the PPQ (Attachment 4), accompanied by the Past Performance Evaluation Cover Letter, to each of its references.

THE RESPONSIBILITY TO SEND OUT THE PAST PERFORMANCE QUESTIONNAIRE RESTS SOLELY WITH THE OFFEROR.

Once the questionnaires are completed by the POCs, the information contained therein will be considered source selection sensitive and will not be released outside the Government. The POCs shall forward their completed questionnaires directly to the government – NOT BACK TO THE OFFEROR.

(c) Past Performance Information: Provide a list of no more than five (5) of the most relevant contracts performed for any customer within the last five (5) years from the issuance date of the solicitation.

The evaluation of past performance information will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the instant acquisition. Furnish the following information for each contract referenced in Section 1 of Attachment 4, Past Performance Questionnaire:

(i) Company/Division name

(ii) Product/Service

(iii) Contracting Agency/Customer

(iv) Contract Number

(v) Contract Dollar Value

(vi) Period of Performance

(vii) Verified, up-to-date name, address, email address, FAX & telephone number of the contracting officer

(viii) Comments regarding compliance with contract terms and conditions

(ix) Comments regarding any known performance deemed unacceptable to the customer, or not in accordance with the contract terms and conditions.

Include rationale supporting your assertion of relevance and identify aspects of the contracts deemed relevant and how they relate to the proposed effort. If an offeror has no past performance history of relevance, the offeror must affirmatively state it possesses no past performance history of relevance.

(d) The evaluation of past performance information (i) will take into account past performance regarding predecessor companies, (ii) will take into account past performance of key personnel who have relevant experience, and (iii) will take into account past performance regarding subcontractors that will perform major or critical aspects of the requirement when such information is relevant to this acquisition. Major or critical aspects are identified in PWS paragraphs 2.1.1, 2.1.3, 2.2.1.3, 2.2.12, 3.2.

(e) If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past performance information on previous teaming arrangements with same partner. If this is a first time joint effort, each party to the arrangement must provide a list of past contracts of relevance.

(f) Subcontractor Consent: Past performance information pertaining to a subcontractor cannot be disclosed to the prime offeror without the subcontractor’s consent. Provide with the proposal a letter from each subcontractor that will perform major or critical aspects of the requirement, as defined above, consenting to the release of its past performance information to the prime contractor.

C. Documents submitted in response to this RFP must be consistent with the following:

1. Requirements of the solicitation: FA4897-17-R-0006 (CLINs and PWS) and government standards and regulations pertaining to the PWS.

2. Evaluation Factors for Award in Section M of this solicitation.

3. If a Part in a proposal exceeds the specified page limit cited in the Part Content, the excessive pages will not be evaluated.

4. Format for proposal Parts I, III, and IV shall be as follows:

(a) Each written Part of the proposal shall be separately bound and labeled in standard three ring loose-leaf binders and clearly identified on the front and side. Each Part shall contain a Table of Contents, Cross References, and the List of Tables and Drawings (if applicable). Cover pages, Table of Contents, and Tabs will not be counted against the page limits. However, pages depicting tables, charts, graphs and figures will count toward the page limit. The number of copies of each part is specified in paragraph B, Specific Instructions, above.

(b) The proposals will be on 8 1/2” x 11” paper, except for fold-outs used for charts, tables, or diagrams, which may not exceed 11” x 17”.

(c) A page is defined as one face of a sheet of paper containing information. Two-sided pages are desired.

(d) All printing shall be single-spaced. Spacing characteristics are applicable to charts and graphs.

(e) Font shall not be less than 10 pitch. Size of print is applicable to charts and graphs.

(f) Page margins shall be a minimum of 1 inch top, bottom, and each side.

(g) In addition to the paper copies specified for each Part, electronic copies of each Part of the entire offer must be submitted. The electronic version of the proposal shall be submitted in a format readable by Microsoft Office 2007, 2010 and 2013 which includes software programs such as WORD, EXCEL, or PowerPoint. For those pages of the proposal that cannot be submitted using Microsoft Office, such as the Standard Form 33, offerors shall include those pages electronically using Adobe Acrobat portable document files (.pdf) format. The Price Part shall be submitted in Microsoft Excel format, readable by Microsoft Excel 2007, 2010 and 2013. The paper copies and electronic versions shall be identical in all respects. In the event there are any discrepancies between the paper and electronic copies, the paper copies take precedence and will be used for evaluation. The electronic version shall be submitted both (1) on a CD-ROM and (2) via the AMRDEC electronic file submission service at https://safe.amrdec.army.mil/safe/.

CD-ROM: Indicate on each CD the Part number(s) and title(s) of all files included on the CD. Ensure the electronic documents stored on the CD can be opened. Use separate files and folders to permit rapid location of all portions, including exhibits and attachments, if any. Replacement CDs will be required to update the final proposal resulting from any discussions and updates, if applicable.

AMRDEC: Indicate in the title of each uploaded file the Part number and document title. The paper copies and uploaded electronic versions shall be identical in all respects. Prior to uploading, ensure the electronic documents uploaded to AMRDEC can be opened. Use separate files to permit rapid location of all portions, including exhibits and attachments, if any. Replacement files uploaded to AMRDEC will be required to update the final proposal resulting from any discussions and updates, if applicable. The AMRDEC system may be accessed at the following: https://safe.amrdec.army.mil/safe/

(h) Each Part shall be complete in itself and shall not reference information contained in other Parts. Page 1 of a Part is defined as the first page after the Table of Contents. All pages shall be numbered.

(i) Price data of any kind shall be presented only in the Price Proposal Part.

(j) Elaborate formats, bindings, and color presentations are not desired or required.

D. General Information:

2. The sealed envelope or package used to submit your proposal must show the time and date specified for receipt, the solicitation number, and the name and address of the offeror.

3. Offerors are cautioned that Mountain Home AFB has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass prior to entrance. SOME DELAY SHOULD BE EXPECTED WHEN HAND-CARRYING PROPOSALS. Offerors should allow sufficient time to obtain a visitor pass and arrive at 366 Gunfighter Ave, Suite 498, Mountain Home AFB 83648, prior to the time specified for receipt. Late proposals will be processed in accordance with FAR 52.215-1(c)(3), “Submission, modification, revision, and withdrawal of proposals.”

SECTION M - EVALUATION FACTORS FOR AWARD

The following have been modified:

SECTION M

Section M - Evaluation Factors for Award

BASIS FOR CONTRACT AWARD: This is a competitive Lowest Price Technically Acceptable (LPTA) source selection conducted in accordance with Federal Acquisition Regulation (FAR) Part 15, Department of Defense SOURCE SELECTION PROCEDURES (1 Apr 16), Defense FAR Supplement Subpart 215.3, and Air Force FAR Supplement (AFFARS) Mandatory Procedure (MP) 5315.3. By submission of its offer in accordance with the instructions provided in clause FAR 52.215-1, Instructions To Offerors – Competitive Acquisition, the offeror accedes to the terms of this model contract. Failure to meet a requirement may result in an offer being determined unacceptable. Award will be made to the lowest Total Evaluated Price (TEP) offer that meets the acceptability standards for non-price factors.

A. Evaluation Methodology:

1. Initial Proposals. The Government will evaluate a minimum number of proposals for efficiency and to maintain adequate competition, as specified in this section. Upon receipt of the Technical, Past Performance and Price proposals, the Government will:

1. Step 1: Order by Initially Proposed Total Price. The Government intends to initially order all proposals by the initially proposed total price, from lowest to highest. The initially proposed total price includes the sum of all performance years (the total price listed in schedule B). Evaluation and determination of the proposed Total Overall Evaluated Price as fair, reasonable, and balanced will occur as specified in Step 2 below, paragraph A(1)(b)(iii), and paragraph E. The Government will select the three (3) lowest priced proposals for further evaluation. Those outside the initial evaluation pool will not be evaluated unless necessary to obtain adequate competition. If the Government receives fewer than three (3) proposals, the Government will evaluate all proposals.

b. Step 2: Factor Evaluations. Upon establishment of the evaluation pool described above, the Government will evaluate each factor simultaneously, as follows:

0. Technical Evaluation. The Government will evaluate the technical proposals of Offerors in the evaluation pool as specified in Technical Factors, paragraph C, below. If any of the proposals receives any “Unacceptable” ratings in the subfactors, the Government reserves the right to evaluate the next lowest priced proposal(s) to ensure adequate competition, or until all proposals are evaluated.

0. Past Performance Evaluation. The Government will evaluate the past performance proposals of Offerors in the evaluation pool as specified in paragraph D, below. If any of the proposals receives an “Unacceptable” rating in the subfactors, as described in paragraph D(3), the Government reserves the right to evaluate the next lowest priced proposal(s) to ensure adequate competition, or until all proposals are evaluated.

0. Price Evaluation. The Government will evaluate the price proposals of Offerors in the evaluation pool as specified in paragraph E, below.

If additional proposals are pulled in on a one-for-one basis under any of the factors, the Government will evaluate those proposals in all three factors: Technical, Past Performance and Price.

1. Award Decision. One award will be made to the offeror with the lowest priced proposal and ratings of “Acceptable” as to the Technical and Past Performance factors. The Government reserves the right to award with or without discussions to an offeror with an “Acceptable” rating for each technical subfactor, an “Acceptable” Past Performance rating, and fair and reasonable pricing, based upon SSA determination that it is in the Government's best interest.

C. Technical Acceptability Factor. The government will evaluate technical proposals on a pass/fail basis and assign an Acceptable or Unacceptable rating as described in Table 1 below. An Unacceptable rating in any subfactor will results in an overall Unacceptable rating for the overall Technical Acceptability Factor.

The proposals will be evaluated against the following subfactor(s):

Sub-Factor 1 - Recruitment/Retention: This sub-factor is met when the offeror provides a comprehensive written plan to recruit and retain qualified personnel for all positions described in the PWS, giving due attention to the relative scarcity of qualified F-15 pilot-instructors and WSO instructors, as well as to the position qualifications for other instructor personnel described in the PWS. Acceptable submissions will identify the following number(s) of qualified contacts (made within sixty days prior to proposal submission) for the following positions, each of which is identified by AFSC in the PWS, Table 7-2, Appendix 1:

For the purpose of this sub-factor, a “qualified contact” is an individual (1) who is at the time of proposal submission fully qualified to perform the duties set forth for each position as described in the PWS, and (2) who has been contacted by the offeror within the past sixty (60) days. Identification of each qualified contact shall be made as follows:

Instructor-Pilot:Provision of the individual’s most recent AF
Form 8, or equivalent document.
Instructor-WSO:Provision of the individual’s most recent AF
Form 8, or equivalent document.
Simulator/Platform Instructor:Provision of the individual’s most recent AF
Form 8, or equivalent document.
Training Instructor/Manager:Provision of the individual’s DD214 (or
equivalent) along with training records and
EPR/OPR. Alternatively, qualifications may be evidenced by official training records, teaching certificates or formal training certifications sufficient to prove the individual has the requisite skills and experience to fill the position.

Sub-Factor 2 – Training: This sub-factor is met when the offeror provides a training plan to meet all required training needs as described in the PWS, paragraphs 2.1.3.1, 2.1.3.1.1, 2.1.3.1.2, and 2.1.3.2, including due consideration for the number of hours spent by qualified personnel providing classroom instruction as well as curriculum development. Additionally, each offeror will need to describe its plan for integrating its instructor personnel with the proximate USAF AMXS instructor cadre in the Maintenance Training Section (MTS) to provide instruction to USAF, RSAF and other personnel as required in paragraph 2.1.3 of the PWS.

Sub-Factor 3 - Quality Management: This sub-factor is met when the offeror provides a Quality Control Plan (QCP) which acknowledges ability to meet (1) all applicable AFIs in the PWS, (2) the SDS as listed in the PWS, and (3) all applicable OSHA requirements. Each offeror also must provide draft plans described in DCMA Instruction 8210.1C, Change 1 and DFARS 252.228-7001.

Sub-Factor 4 – Experience: This sub-factor is met when the offeror demonstrates its familiarity with 4th-generation fighter aircraft, including maintenance and/or operations support and training experience. That familiarity may be demonstrated by (1) an offeror as prime contractor, (2) an offeror’s subcontractor subject to a signed agreement for the purpose of fulfilling this requirement, (3) a teaming arrangement participant subject to a signed agreement for the purpose of fulfilling this requirement, and/or (4) a joint venture participant subject to a signed agreement for the purpose of fulfilling this requirement. To demonstrate that familiarity, the prime contractor, subcontractor, teaming arrangement participate, and/or joint venture participant must provide documentation of the following:

(a) involvement as a party to at least one contract with the United States Government, or any agency or Department thereof, at any time in the last five (5) years in which maintenance support and operations support for 4th-generation fighter aircraft were critical objectives; and

(b) employment for at least two (2) years out of the last five (5) years of each of the following:

1. At least one (1) qualified pilot-instructor who possessed all proper qualifications, certifications and currencies to instruct student pilots on 4th-generation fighter aircraft; and

1. At least one (1) qualified maintenance training instructor with an AFSC of 2A6X1, engine run experience on 4th-generation fighter aircraft, and flight-line experience.

D. Past Performance Factor. The government will evaluate recent and relevant performance information on all offerors based on (i) the references provided by the offeror, (ii) any past performance information obtained from survey/questionnaires (Attachment 6), and (iii) any data independently obtained by the government. If a reference identified by the offeror in accordance with Sec L, paragraph C.4(c), does not submit a survey/questionnaire, the government will follow up with the reference POC, but the government is not responsible for the failure of a reference POC to provide a survey/questionnaire. See Table 4, Note, below regarding lack of submission in any subfactor of Past Performance.

Past performance regarding predecessor companies of the offeror and/or subcontractors, key personnel, and subcontractors that will perform major aspects, as identified in PWS, Paragraph 1.1 of the requirement, if applicable (see Sec L, paragraph C.4(d)), will be rated as highly as past performance information for the principal offeror.

(1) Recent past performance information includes contracts performed and/or being performed for any customer within the last five (5) years from the issuance date of the solicitation.

(2) Relevant past performance is that which involved similar scope, magnitude of effort, and complexity that this solicitation requires. The government will assess relevancy for each contract and assign a rating as described in Table 2 below:

(3) Performance Confidence Assessment: For each of the recent and relevant past performance information citations reviewed, the performance quality of work will be assessed based on the (i) Quality Assessment Criteria evaluated under paragraph 4 of Present/Past Performance Survey/Questionnaire (Solicitation Attachment 3) and (ii) the evaluation ratings in the Federal Past Performance Information Retrieval System (PPIRS), if available. Each recent and relevant past performance information citation reviewed will be assigned a rating as described in Table 3 below:

During the contract period, contractor performance is not meeting (or did not meet) contract requirements. For any problems encountered, corrective action appeared only marginally effective, not effective, or not fully implemented. Customer involvement was required.

The past performance evaluation is an assessment of the offeror’s probability of meeting the solicitation requirements. Offerors must receive a past performance rating of “Acceptable” to be eligible for award. For offerors in a teaming arrangement where two or more companies form a partnership or joint venture (JV), the following will apply: (i) for a JV at least one partner must be rated “Acceptable;” and (ii) for prime-subcontractor(s) partnerships, the prime must be rated “Acceptable” in order for the partnership to be rated “Acceptable” as a whole.

As a result of the recency, relevancy, and performance quality assessments, offerors will receive an integrated past performance rating as described in Table 4 below:

Note: In the case of an offeror without a record of recent/relevant past performance or for whom past performance is not available, or so sparse that a meaningful past performance rating cannot be reasonably assigned, the offeror cannot be evaluated favorably or unfavorably on past performance (see FAR 15.305(a)(2)(iv)). Therefore, the offeror will be determined to have an unknown past performance. In the context of acceptability/unacceptability, “unknown” will be considered “Acceptable.”

An offeror with more than two (2) “Unsatisfactory” Quality Assessment Ratings based on its past performance information citations will receive an “Unacceptable” Past Performance Evaluation Rating.

E. Price Evaluation Factor. The government will rank all offers by TEP. The price evaluation will document the fairness and reasonableness of the TEP. Separately priced line items or subline items shall be analyzed to determine if the prices are unbalanced.

Total Evaluated Price: A TEP will be computed for each offer in order to meet the requirements of FAR Parts 6 and 17. The price of a six-month extension authorized by 52.217-8, Option to Extend Services, will be added to the proposed price for each potential contract performance period, based on the prices proposed for each period of performance.

The TEP is a two-part computation. First, the offeror’s total proposed price will be determined by multiplying the quantities identified in Section B by the unit price, limited to two decimal places, for each Contract Line Item Number (CLIN) to confirm the extended total amount for each CLIN; the sum of all extended amounts will be the offeror’s total proposed price. Second, the total proposed price (total amount for CLINs 0001 – 4006) plus the sum of Option to Extend Services clause 52.217-8 formula (shown below) will constitute the TEP.

Please note that CLINs 0002, 0003, 0005, 0006, 1002, 1003, 1005, 1006, 2002, 2003, 2005, 2006, 3002, 3003, 3005, 3006, 4002, 4003, 4005, and 4006 are being evaluated under the following formula for government purposes only, as each will contain a Unit Amount provided by the Government.

For TEP evaluation purposes only, the formula for the Option to Extend Services amount is as follows:

Evaluating the Option CLINs does not obligate the government to exercise the Options.

When applicable, the price evaluation adjustment for HUBZone small business concerns will be applied in accordance with FAR 52.219-4, Notice of Price Evaluation Preference for HUBZone Small Business Concerns, to arrive at an evaluated price.

F. Offerors are cautioned to submit sufficient information and in the format specified in Section L of this solicitation. Offerors may be asked to clarify certain aspects of their proposals. Exchanges conducted to resolve minor or clerical errors will not constitute discussions and the contracting officer reserves the right to award a contract without the opportunity for proposal revision. The government intends to award a contract without discussions with respective offerors. The government, however, reserves the right to conduct discussions if deemed in its best interest.

G. Award will be made to the lowest priced offer, based on TEP, that meets the acceptability standards for non-price factors.
