PERFORMANCE WORK STATEMENT
FOR
HEMATOLOGY ANALYZER
1. SCOPE: The government estimates the performance of 15 cost-per-tests per day, 7 days a week, Monday - Friday, on a 24-hour basis (or when dictated by mission requirements). The contractor shall provide a hematology analyzer that has a self-contained waste management, does not require water or plumbing. The analyzer will report, at a minimum the following parameters: WBC, RBC, HGB, HCT, MCV, MCH, MCHC, PLT, neutrophils (# and %), lymphocytes (# and %), monocytes (# and %), eosinophils (# and %), basophils (# and %), RDW-SD, RDW-CV, and MPV. This analyzer will be capable of processing both adult and pediatric samples (microsample capability) and require no more than 20 uL for minimum specimen volume (open or closed). The contractor shall maintain, replace, repair, provide supplies, warranty and replacement as required, in order to ensure the equipment is fully operational and within manufacturer specifications.

2. REQUIRED SERVICES:
a. The contractor shall lease to the government a primary hematology analyzer over a period of 1 base year + 4 option years, with a fixed annual cost price compatible with the government’s existing stock of supplies, reagents and materials. The lease must include: 1) Equipment Rental, 2) 24/7 Technical/Specialist Support, 3) Equipment Maintenance and Repair. All items and services shall be provided in accordance with all federal, state, local laws, and regulations, manufacturer’s recommended standards, commercial requirements, and this Statement of Work
(SOW).
b. Preventative Maintenance Inspections (PMIs). The contractor shall provide scheduled preventative maintenance inspections (PMIs), calibrations, and on-site emergency services for the equipment listed below. Service shall include travel, labor, parts, materials, updates, technical data, and required tools and test equipment. Services shall be performed during normal duty hours of 0730 to 1630, Monday thru Friday.
Response time shall not exceed 24 continuous clock hours. These inspections will include quality assurance checks for proper operation, detection and correction of incipient failures, actions pertaining to periodic technical inspections, lubrications, adjustments, calibrations, controls, cleaning, and replacement of worn parts.
(1) Two (2) PMIs shall be performed biannually (6-months) per contract year with the optimal schedule as follows:
(a) During the month of February
(b) During the month of August
(2) PMIs should include:
(a) Consulting with equipment operators regarding equipment performance
(b) Performing operational checks on all systems and correcting noted deficiencies
(c) Replacing worn or defective parts (those parts that may fail before the next PMI)
(d) Cleaning and lubricating IAW manufacturer’s service manual.
(e) 6-month calibration of analytes IAW College of American
Pathologists (CAP) standards using manufacturer Calibrators
(f) All services include associated travel expenses, associated labor costs, needed cleaning, control, and calibration material.
(3) Scheduled PMIs shall be coordinated with the Mountain Home Laboratory at least one week in advance.
a. Emergency Service Calls. On site emergency service is the repair necessary to return the system to its intended functional state of operation. The contractor will provide on-site repair to correct all malfunctions that occur. The service technician will continue uninterrupted service until the system is operational according to manufacturer’s specifications. Travel and overtime will be included at no additional cost. All tools, test equipment, parts, and supplies will be the responsibility of the contractor.
(1) Response time to emergency service requests shall not exceed 24 continuous clock hours, excluding weekends and holidays.
(2) The equipment shall be repaired within 24 continuous clock hours after arrival to the equipment site, excluding federal holidays. The need for unavailable parts will suspend the time limit until parts are obtained. These parts shall be obtained in the swiftest manner at no additional cost to the government.
(3) Use of a service hotline. The service hotline should be available for questions/concerns and troubleshooting advice 24 hours per day.
c. Equipment Updates. The contractor shall furnish and install all software updates and equipment modifications provided by the manufacturer. These updates are provided at no additional cost and installed no later than the next PMI or emergency visit.
3. ADMINISTRATION:
a. The contractor will receive PMI and on-site emergency service requests from Medical Equipment Repair (MERC). Upon arrival, the contractor will contact MERC before performing any work specified in this agreement.
b. The service technician will prepare a service report for each visit and obtain the signature of a supervisor within the laboratory or by a COR certifying acceptance of services performed. The report shall describe the service performed in sufficient detail and include the following:
(1) System name, model, serial number, location, and site number if applicable
(2) Purpose of the visit or description of the malfunction
(3) Start/Stop times and dates
(4) Break down of travel and labor hours
(5) Description of all services rendered – including calibration validation & linearity verification data for College of America Pathologists (CAP) evidence of compliance.
(6) Complete list of replacement parts used
(7) Attached copy of test records or checklist used to prove completion of PMI
(8) Signatures of the person performing the service and of the person accepting the service.
c. The contractor will deliver one copy of the service report to the MERC office located in the 366 Medical Group.
d. Replacement Parts. The contractor shall use only new original or factory refurbished replacement parts. The service personnel should carry or have the manufacturer’s “recommended stock” of parts for the system readily available.
e. Training/Experience. Service technicians should have received factory training on this or like-systems. Individuals should be actively engaged in repairing like-systems for 12 of the last 18 months.
f. Test Equipment. The contractor shall ensure that all test equipment used has been calibrated according to manufacturer’s specification against a traceable standard.
g. Technical Data. The contractor’s service personnel will maintain a complete file of technical documentation for the system serviced to include schematics, service manuals, update memos, and service history records.
4. INSTALLATION: Placement and installation of a clinical hematology analyzer and all equipment components inclusive, but not limited to, the entire laboratory associated mechanical, electromechanical parts and supplies. Installation shall conform to industry Standards and shall meet or exceed with all applicable codes to include local/state regulations, NFPA, building codes, communications codes, and other codes for each utility identified. The contractor shall provide the following services for staff which shall include the following tasks:
a. The contractor shall provide an Uninterrupted Power Supply (UPS) for each analyzer and must be installed in the same room, to be positioned on the analyzer’s side. This UPS is a self-operated unit that provides enough power, during a commercial power outage to the analyzer, to ensure that the hematology analyzer system remains operational thus prevents mechanical noise of the system, therefore, producing accurate patient results. The bidding vendor/contractor shall provide the specific type of UPS required for their emergency use at the 366 MDG.
b. Complete daily clean-up and a final (upon installation completion) clean-up of the work area. To include any need for cosmetic repairs on walls, floorings, ceiling, etc.
c. The contractor shall provide a project manager to perform/oversee system installation, correlation, validation. The contractor shall provide all validation reagent/materials needed to perform these tasks.
d. The contractor is responsible for the removal of the clinical hematology analyzers at the end of the contract period.
5. TRAINING: The Contractor shall provide training for at least two government users at contract start, and one additional user with each contract option period.
6. CONTRACTING OFFICERS REPRESENTATIVE: The designated representatives authorized to place calls for this agreement will be the Laboratory, MERC or Contract Services personnel. CORs may be contacted at:
