Block 4: Report shall be provided using MS Office suite and posted on contractor’s website. Contractor shall notify the Government via Email once report is posted.
Block 5: Plans shall include, but not limited to: Drinking Water and Waste Water System Management, Hazardous Material Handling and Storage (HazMat Program), Hazardous Waste Program, Hazardous Waste Collection and Disposal, Pollution Prevention Program, POL, Air Quality Ozone Depleting Chemicals (ODCs), Pesticides and Herbicides Management, Special Health Considerations, Spill Prevention and Response, Training, Environmental Protection Committee and Site Specific Environmental Closure Plans. The closure plans shall include, but not limited to: Phased approach to site closure based on relevance of ongoing activities, Listing of all sites/areas that have a potential for environmental contamination, Listing of all known spills over 25 gallons of POL or over the RQ of other hazardous materials, Listing of previous Environmental Baseline Studies and reports, Procedures for the removal of all HazMat and HazWaste, Procedures for operating the WRM site in an environmentally sound manner until final closure, and Procedures for assisting USAFCENT with drafting the Environmental Site Closure Survey (ESCS) as directed by CENTCOM Reg 200-2.
Block 8: The document will be accepted or rejected by the Government within 30 business days after receipt. Contractor shall revise and resubmit the document within 10 business days after receipt unless otherwise directed by the CO.
Block 9: Distribution authorized to U.S. Government agencies only (administrative or operational data, 22 May 07). Other requests for this document shall be referred to HQ ACC AMIC/PKB.
Block 10/11/13: The contractor shall review and update their plans IAW with the approved QMS. When a plan is revised it shall be submitted to the CO for approval NLT 10 business days after revision.
