COMBINED SYNOPSIS/SOLICITATION
FA4877-21-Q-0011
Roman Catholic Priest Davis-Monthan AFB, AZ.
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation;
quotes are being requested and a written solicitation will not be issued.
The Request for Quotation (RFQ) number FA4877-21-Q-0011, Roman Catholic Priest shall be used to reference any written quote provided under this RFQ.
The RFQ document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021- 05; Effective 3/10/21.
This solicitation is being solicited full and open. NAICS Code 813110, with a small business size standard of $7.5M. All prospective offerors must be registered in the System for Award Management (SAM) at www.sam.gov. Lack of SAM registration will make an offeror ineligible for award.
Acquire Auxiliary Catholic Priest to provide religious and spiritual leadership to meet the needs of Catholic military personnel, their family members, and other authorized personnel assigned to Davis-Monthan Air Force Base (DMAFB).
The contractor shall, under the direction of the Wing Chaplain or designated representative, oversee all Catholic Community exercise of religion as defined by the Archdiocese for Military Services. The government request quotes for following line items:

Provisions 52.212-1, Instructions to Offerors – Commercial and 52.212-2, Evaluation – Commercial Items, apply to this acquisition along with the listed addenda’s to the provisions.
Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items. For offerors with a current registration in the System for Award Management (SAM) complete paragraph (b) of the provision and submit with the response to the RFQ.
FAR clause at 52.212-4, Contract Terms and Conditions -- Commercial Items (Jan 17), applies to this acquisition with the following Addendum: Paragraph (c) of this clause is tailored as follows: Changes in the terms and conditions of this contract may be made only by written agreement of the parties with the exception of administrative changes, such as changes in the paying office, appropriations data, etc., which may be changed unilaterally by the Government.
Note: The vendor acknowledges that should the quote terms and conditions and/or agreement conflict with mandatory provisions of the Federal Acquisition Regulation (FAR) and other Federal law applicable to commercial acquisitions, to the extent of such conflict the FAR and Federal law govern and conflicting vendor terms and conditions and/or agreement are unenforceable and are not considered incorporated into any resultant contract.
Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders – Commercial Items and selected clauses within the clause apply to this acquisition.
52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS (OCT 2018) –ADDENDUM
To ensure timely and equitable evaluation of the quotation, the offeror must follow the instructions contained herein. The quote must be complete, self-sufficient, and respond directly to the requirements of this solicitation.
Offerors must be currently registered in the System for Award Management (https://sam.gov) and agree to remain registered while performing services in the support of Davis-Monthan AFB, AZ. The offerors response shall consist of a:
1. Pricing: Offerors shall provide pricing for all Line Items. The extended amount for each Line Item must equal the unit price multiplied by the number of units. It is imperative the mathematical calculations are correct. Numeric rounding shall be limited to two decimal places. Offerors shall clearly state if any discount terms are offered.
2. Fill-In Clauses/Provisions: Offerors are required to complete all clauses/provisions that require Fill-ins and submit them with their quote. Note: For 52.212-3, If the Offeror has completed the annual, representations and certification electronically in the System for Award Management (SAM) accessed through https://www.sam.gov. Only paragraph (b) of the provision is required.
3. Technical Documents: Offerors must provide written documentation DD Form 2088, Statement of Ecclesiastical Endorsement from the Archdiocese for the Military Services (AMS), USA (http://www.milarch.org)
Quotes Submissions: Quotes are due no later than, 2 July 2021, at 2:00 PM (PST). Offerors are responsible for verifying receipt of their quotes to this office before offer due date and time.

The Government will not provide contract financing for this acquisition. Upon award and satisfactory performance, payment will be through Wide Area Workflow.

52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014) – ADDENDUM (Using 13.106 Procedures)
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
(i) technical capability of the item offered to meet the Government requirement;
(ii) price;
BASIS FOR CONTRACT AWARD: Two factors will be used to evaluate offerors’ quotes: Technical Acceptability and Price. Contract award will be made to the offeror who submits the lowest total evaluated price quote achieving a rating of “Acceptable” for the Technical Acceptability factor. Past performance will be evaluated to assist in determining contractor responsibility in accordance with FAR 9.104-1 but will not be included as a subfactor or rated. By submission of its offer, the offeror accedes to all solicitation requirements including: terms and conditions, representations and certifications, technical requirements, and the evaluation factors and subfactors contained herein. Failure to meet a requirement may result in an offer being determined unacceptable for award. The offeror must clearly identify any exception to the solicitation terms and conditions with accompanying rationale. The evaluation shall proceed as follows:

1. Technical Acceptability – The Government technical evaluation team shall evaluate the technical quotes at the factor level, assigning an “Acceptable” or “Unacceptable” rating as defined below.

2. Price Evaluation – The Government will evaluate the offeror’s price quote, to include all options, for price completeness and reasonableness as described below.
(a) Completeness – The offeror’s price quote will be evaluated for completeness by confirming the offeror has provided unit and total price for all CLINs.
(b) Reasonableness - The offeror’s price will be evaluated for reasonableness through one or more of the methods specified in FAR 13.106-2 & 3.
(c) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
(d) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
