52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

The Government will accept the lowest price that meets the requirements of the PWS. All offerors shall submit two Master and one Senior certificates showing acknowledgement of personnel required to fill the positions.

(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. 52.216-18 ORDERING. (OCT 1995)

(a) Any supplies and services to be furnished under this contract shall be ordered by issuance of delivery orders or task orders by the individuals or activities designated in the Schedule. Such orders may be issued from 30 Sep 2018 through 29 September 2020.

(b) All delivery orders or task orders are subject to the terms and conditions of this contract. In the event of conflict between a delivery order or task order and this contract, the contract shall control.

(c) If mailed, a delivery order or task order is considered "issued" when the Government deposits the order in the mail. Orders may be issued orally, by facsimile, or by electronic commerce methods only if authorized in the Schedule.

52.216-19 ORDER LIMITATIONS (OCT 1995)

(a) Minimum order. When the Government requires supplies or services covered by this contract in an amount of less than $10,000 the Government is not obligated to purchase, nor is the Contractor obligated to furnish, those supplies or services under the contract.

(b) Maximum order. The Contractor is not obligated to honor -

(1) Any order for a single item in excess of $150,000.00 ;

(2) Any order for a combination of items in excess of $2,000,000; or

(3) A series of orders from the same ordering office within 30 days that together call for quantities exceeding the limitation in paragraph (b) (1) or (2) of this section.

(c) If this is a requirements contract (i.e., includes the Requirements clause at subsection 52.216-21 of the Federal Acquisition Regulation (FAR)), the Government is not required to order a part of any one requirement from the Contractor if that requirement exceeds the maximum-order limitations in paragraph (b) of this section.

(d) Notwithstanding paragraphs (b) and (c) of this section, the Contractor shall honor any order exceeding the maximum order limitations in paragraph (b), unless that order (or orders) is returned to the ordering office within 30 days after issuance, with written notice stating the Contractor's intent not to ship the item (or items) called for and the reasons. Upon receiving this notice, the Government may acquire the supplies or services from another source.

52.216-22 INDEFINITE QUANTITY. (OCT 1995)

(a) This is an indefinite-quantity contract for the supplies or services specified, and effective for the period stated, in the Schedule. The quantities of supplies and services specified in the Schedule are estimates only and are not purchased by this contract.

(b) Delivery or performance shall be made only as authorized by orders issued in accordance with the Ordering clause. The Contractor shall furnish to the Government, when and if ordered, the supplies or services specified in the Schedule up to and including the quantity designated in the Schedule as the "maximum". The Government shall order at least the quantity of supplies or services designated in the Schedule as the "minimum".

(c) Except for any limitations on quantities in the Order Limitations clause or in the Schedule, there is no limit on the number of orders that may be issued. The Government may issue orders requiring delivery to multiple destinations or performance at multiple locations.

(d) Any order issued during the effective period of this contract and not completed within that period shall be completed by the Contractor within the time specified in the order. The contract shall govern the Contractor's and Government's rights and obligations with respect to that order to the same extent as if the order were completed during the contract's effective period; provided, that the Contractor shall not be required to make any deliveries under this contract after 29 September 2020.

(a) Definitions. As used in this clause--

Department of Defense Activity Address Code (DoDAAC) is a six position code that uniquely identifies a unit, activity, or organization.

Document type means the type of payment request or receiving report available for creation in Wide Area WorkFlow (WAWF).

Local processing office (LPO) is the office responsible for payment certification when payment certification is done external to the entitlement system.

(b) Electronic invoicing. The WAWF system is the method to electronically process vendor payment requests and receiving reports, as authorized by DFARS 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports.

5352.201-9101 OMBUDSMAN (JUN 2016)

(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern.

5352.223-9001 HEALTH AND SAFETY ON GOVERNMENT INSTALLATIONS (JUN 1997)

(a) In performing work under this contract on a Government installation, the contractor shall:

(1) Comply with the specific health and safety requirements established by this contract;

(2) Comply with the health and safety rules of the Government installation that concern related activities not directly addressed in this contract;

(3) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and

(4) Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes.

(b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health Standards (AFOSH) and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract.

(c) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract.

PERFORMANCE WORK STATEMENT

Statement of Work for

Parachute Rigger Support Services

1.0. DESCRIPTION OF SERVICES/GENERAL INFORMATION

1.1. Description of Services. The contractor shall provide all equipment, tools, parts, transportation, supervision, and management to provide parachute rigger support services, in the form of two (2) Federal Aviation Administration (FAA) Master riggers and one (1) FAA Senior rigger to perform parachute rigging services and instruction in support of the 68th

Rescue Squadron (68 RQS) at Davis-Monthan AFB (DMAFB), AZ.

1.1.1. Parachute Riggers. Contractor personnel shall maintain unit level advanced parachute and oxygen training equipment, support logistical requirements, and provide courses of instruction to assist in maintaining fully mission capable Aircrew Flight Equipment technicians.

a. Maintain premeditated and cargo parachute systems.

b. Maintain high altitude oxygen equipment and accessories.

c. Provide courses of instruction to maintain training requirements for all Aircrew Flight Equipment technicians.

1.1.2. Standards/Training

a. Two FAA Master Rigger certifications with back and chest ratings.

b. One FAA Senior Rigger certification with back and chest ratings.

c. Maintain all Federal Aviation Administration (FAA) requirements, ratings, privileges, and performance standards in accordance with 14 CFR, Aeronautics and Space, Part 65, Certification: Airmen Other Than Flight Crewmembers, Subpart F, Section 65.111, 65.113, 65.115, 65.119, 65.125, 65.129, 65.131, and 65.133 for each respective certification and rating.

d. Maintain currencies on all 68 RQS premeditated and cargo parachute systems, to include RA-1 Military Free Fall (MFF), RA-1 Double Bag Static Line (DBSL), RA- 1 Bottom Of Container (BOC), T-11R, MC-6, Military Tandem Vector System (MTVS), Military Tandem Tethered Bundle (MTTB), T-10 cargo, G-12 cargo, 15ft extraction, J- 1 spotter, Instructor Raider, and Vector 3.

e. Maintain currencies on all 68 RQS high altitude supplemental oxygen systems, to include 122 ci bottle, PHANTOM mask, high capacity oxygen console, portable oxygen refill station, and PHANTOM test stand.

f. Maintain and repair ancillary equipment, to include night vision devices, altimeters, emergency breathing systems, Self-contained Hybrid Integrated Enhanced Life- Support Device rescue breathing systems, and floatation devices. Also rigging of aerial delivery loads after certification by the 68th RQS, to exclude signature authority to Joint Acceptance Inspection (JAI) forms or serve as the primary rigger. Currently, only active duty personnel can sign off on this equipment.

g. Prepare, maintain, and build aerial delivery equipment, to include, but not be limited to Rigging Alternate Method Boat (RAMB), Military Razor (MRZR), Advanced Rescue Craft (ARC), Quad, and door bundles.

h. (Master Rigger) Perform In-Process Inspections (IPI) or Rigger checks after meeting requirements in accordance with Air Force Instruction (AFI) 11-301 v1 ACC sup para.

i. (Master Rigger) Perform Task Evaluator/Certifier duties in accordance with AFI 11- 301 v1 para. 4.2.3 thru 4.2.3.4.

j. (Master Rigger) Perform Trainer duties in accordance with AFI 11-301 v1 para. 4.2.4 thru 4.2.4.3.

1.1.3. Knowledge, Skills and Abilities Qualifications

a. Knowledge, skill, and abilities in advance military parachute systems. Previous experience with United States Air Force (USAF) Battle Field Airman Air Force specialty code (Pararescue, Combat Control, Survival Evasion Resistance Escape, and Tactical Air Control Party) is highly desired.

b. Knowledge of Military Free Fall (MFF) and static line jump operations to assist with Military drop zone control officer (DZCO) duties.

c. Knowledge/application of Federal Aviation Administration (FAA) Senior Rigger experience, competencies and skill requirements in accordance with 14 CFR Part 65, Subpart F, Section 65.115, 65.125, 65.129, 65.131, and 65.133.

d. Certification as FAA Senior Rigger is required with chest and back ratings.

e. Certification as FAA Master Rigger is required with chest and back ratings.

f. Knowledge/application of Federal Aviation Administration (FAA) Master Rigger experience, competencies and skill requirements in accordance with 14 CFR Part 65, Subpart F, Section 65.119, 65.125, 65.129, 65.131, and 65.133.

g. Knowledge/application of data collection techniques, practices and statistical analysis principles for trend analysis.

h. Knowledge/application of USAF aviation flight safety program, safety mishap investigation/reporting procedures and associated systems.

i. Contractor personnel must be physically fit to include all fundamentals of fitness (strength, endurance, cardio, flexibility and body composition) and maintain fitness throughout the life of the contract. Contractor personnel shall perform recovery, washing and drying parachutes. This is a rigorous physical act that will require personnel to be fit enough to conduct these duties. Must be able to pack a minimum of four parachute systems a day, carry at least 50 pounds for 50 yards, and be able to pull at least 100 pounds on a pulley system.

j. Knowledge on preparation, maintenance, and building of aerial delivery equipment, to include, but not be limited to RAMB, MRZR, ARC, Quad, and door bundles.

k. Maintain assigned Aircrew Flight Equipment that will be inspected by an active duty 7-level Aircrew Flight Equipment 1P0X1 or equivalent. Contractor personnel will receive 100%, quality control on all parachute inspections performed to ensure the adherence of safety and military regulations.

1.2. General Information

1.2.1. Contract Manager. The contractor shall designate a contract manager who shall be responsible for contract performance and can act for the contractor on all matters relating to daily operation of the contract. The contract manager may be a contractor employee performing services. The contractor shall submit the name and phone number of the contract manager in writing to the Contracting Officer. The contract manager must read, write, speak, and understand English fluently.

1.2.1.1. Availability of Contract Manager. The contract manager shall be available within 60 minutes during normal duty hours (performance work statement (PWS) paragraph 1.2.3.) to meet on the installation with Government personnel (designated by the Contracting Officer) to discuss problem areas. After normal duty hours the contract manager shall be available within 3 hours. The contract manager shall be available for contact by telephone 24 hours a day, seven days per week, including holidays.

1.2.1.2. Contractor Employee Contact Information. The contractor shall provide the Government Contracting Officer Representative (COR) the telephone numbers for each contractor employee performing services, in the event a contractor employee must be contacted during non-duty hours. The contractor shall provide an updated list when changes in information or personnel occur.

1.2.2. Contractor Quality Control Plan. The contractor shall submit a Quality Control Plan to the Contracting Officer within 30 days after contract award. The Contracting Officer will notify the contractor of acceptance or required modifications to the plan. The contractor shall make appropriate modifications and obtain acceptance of the plan by the Contracting Officer. The contractor's quality control plan shall contain, as a minimum, the following items:

a. A description of the inspection system to cover all services. Description shall include specifics as to the areas to be inspected on a scheduled and unscheduled basis, frequency of inspections, and the title and organizational placement of the inspector(s).

b. A description of the methods to be used for identifying and preventing defects in the quality of service performed.

c. A description of how the records will be kept.
