Performance Work Statement for

Medical Delivery Courier Services

1.0. DESCRIPTION OF SERVICES/GENERAL INFORMATION

1.1. Description of Services. The contractor shall provide all transportation, personnel, labor, supervision, and management to perform Medical Delivery Courier services for the 355th

Medical Group (355 MDG), Davis-Monthan AFB (DMAFB), AZ.

1.1.1. Scope. The contractor shall provide specimen transport of frozen, refrigerated, or ambient samples and supplies from the 355 MDG to Government-designated local hospitals, referral laboratories in a timely manner. Government personnel will provide the contractor with final destination(s) when the specimens and supplies are picked up by the contractor. The contractor will also provide pickup of laboratory supplies and radiology items from various Tucson area locations and deliver them to the 355 MDG. Government personnel will provide the contractor with pickup locations via phone call before 10:00 a.m.

1.1.1.1. Basic Services. The contractor will pick up items from designated Tucson locations and deliver items once a day to the Main Laboratory Shipping Department in building 400 on

DMAFB at 2:00 p.m., Monday through Friday. The contractor shall pick up specimens once a day at 2:00 p.m., Monday through Friday, from the Main Laboratory Shipping Department in building 400 on DMAFB, and make deliveries directly to the location(s) designated by

Government personnel. Government personnel will notify contractor personnel at least one day in advance, not later than 1:00 pm, if services are not required on any specific day, and when the

Main Laboratory is scheduled for closure.

1.1.1.2. Additional Services. There will be occasions when the Government requires same day services, in addition to the basic once a day pick up at 2:00 p.m., between the hours of 7:30 a.m.

- 4:30 p.m. Government personnel will place calls to the contractor when additional services are required.

1.1.1.3. Delivery Requirements. The contractor shall perform deliveries in accordance with the

Health Insurance Portability and Accountability Act (HIPAA), Occupational Safety and Health

Act (OSHA), and Air Force Occupational Safety, and Health (AFOSH) standards.

1.1.1.4. Working Hours. The working hours for the 355 MDG are 7:30 a.m. and 4:30 p.m.

Monday through Friday, excluding Saturdays, Sundays, and Federal Holidays.

1.1.1.5. After-Hours Requests. The contractor shall provide after-hours services (On-Demand

Deliveries) as requested by designated Government personnel, outside of the 355 MDG working hours of 7:30 a.m. - 4:30 p.m. Upon contract award, the contractor shall provide the

Contracting Officer with a telephone number(s) that Government personnel will call to request after-hours requests.

1.1.1.6. Contractor Employee Qualifications. Contractor employees performing delivery services are required to have training in accordance with HIPAA regulations and blood-borne pathogens. The contractor shall provide proof of training upon Government request.

1.2. General Information

1.2.1. Federal Holidays. The following Federal Holidays are observed at Davis-Monthan AFB, AZ. The contractor is not required to perform services on Federal Holidays, or the day the holiday is observed.

1.2.2. Safety. The contractor shall comply with all applicable laws and safety regulations.

Compliance with OSHA and other applicable laws and regulations for the protection of employees is exclusively the obligation of the contractor, and the government will assume no liability or responsibility for the contractor’s compliance or non-compliance with such responsibilities. The contractor shall provide all personal protective clothing and equipment for contractor employees.

1.2.2.1. Fire Prevention and Safety Standards. Contractor employees shall comply with the

Occupational Safety and Health Act (OSHA), Davis-Monthan AFB Fire Protection Program, Air

Force Occupational Safety, and the Fire Prevention and Health Program.

1.2.3. Smoking In AF Facilities. Contractors are advised that the AF has placed restrictions on the smoking of tobacco products in AF facilities. AFI 40-102, Tobacco Use in the Air Force, outlines the procedures used by the commander to control smoking in our facilities. Contractor employees and visitors are subject to the same restrictions as Government personnel. Smoking is permitted only in designated smoking areas.

1.2.4. Illegal Substances. Illegal substance possession and/or use will not be tolerated on any military installation, including DMAFB. The contractor shall ensure that contractor employees who have base access do not possess and/or use illegal substances. All contractors and subcontractors are required to adhere to all local, state and federal laws concerning possession and/or use of illegal substances. Contractors found to be in possession of illegal substances will be barred from the installation, and subject to arrest and prosecution by local authorities.

1.2.5. Illegal Aliens. IAW Air Combat Command (ACC) supplement to AFI 31-101, The Air

Force Installation Security Program (FOUO), illegal aliens are unauthorized on DMAFB. The contractor shall not hire illegal aliens to perform contract services on DMAFB, nor attempt to bring illegal aliens on the installation. Corrective/punitive action will be taken against the contractor and the illegal alien if the contractor hires illegal aliens to perform contract services on DMAFB.

1.2.6. Contractor Travel on DMAFB. Contractor employees shall not loiter in any working area before or after performing services. All contractor employees shall limit their travel on

DMAFB only to the specific areas required for performance of services or in travel directly to and from these locations. Upon completion of contract services, contractor employees shall promptly depart DMAFB.

1.2.7. Contractor Employee Conduct. The Government reserves the right to require removal from the installation any contractor employee who endangers persons or property, whose actions are inconsistent with professional conduct, whose continued employment is inconsistent with the interest of military security or who is found to be incapacitated or under the influence of alcohol, drugs, or other substances. Removal of employees for any reason does not relieve the contractor of the requirement to perform services specified herein.

1.2.8. Security Requirements

1.2.8.1. List Of Employees. Within 10 days of contract award, the contractor shall provide the

Contracting Officer a list of contract personnel that require access to DMAFB. The list shall include the employee’s name and social security number. The contractor shall provide the

Contracting Officer the list of employees every six months and when an employee’s status or information changes.

1.2.8.2. Pass And Identification Items. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees and non-Government owned vehicles.

1.2.8.3. Retrieving Identification Media. The contractor shall retrieve all identification media, including vehicle passes from employees who depart for any reason before the contract expires;

e.g. terminated for cause, retirement, etc. The contractor shall notify the Contracting Officer in writing when an employee departs, and return the employee’s identification to the office that issued it.

1.2.8.4. Traffic Laws. Contractor employees shall comply with all DMAFB traffic regulations and state traffic laws. Contractor employees are subject to random vehicle speed control checks.

Failure to adhere to base traffic regulations may result in the loss of base driving privileges, debarment from the base, or other administrative action. The use of cell phones is strictly prohibited while driving on Davis-Monthan AFB, unless the phone is hands free. Seat belt use is mandatory for all drivers and passengers of vehicles.

1.2.8.5. Random Personnel and Vehicle Searches. Contractor employees are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base, and may result in loss of base driving privileges, debarment from the base, or other administrative action.

1.2.8.6. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor owned vehicle or privately owned vehicle while on Davis-Monthan AFB.

1.2.8.7. For Official Use Only (FOUO). The contractor shall comply with DoDD 5400.7, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material. FOUO or FOIA material discovered by the contractor or contractor employees shall not be retained by the contractor and surrendered immediately to an appropriate Government authority.

1.2.8.8. Reporting Requirements. Contractor employees shall immediately report to an appropriate government authority any information or circumstances of which they are aware that may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information.

1.2.8.9. Physical Security. Contractor employees are responsible for safeguarding all

Government property and controlled forms provided for the contractor’s use. At the end of each work period, the contractor shall secure all DMAFB facilities, equipment, and materials.

2.0. GOVERNMENT FURNISHED PROPERTY AND SERVICES

2.1. Government Furnished Emergency Services. The Government will provide Security

Forces Squadron services (call 228-4444, for emergencies only), and fire protection services (call

228-3333), while the contractor is physically on DMAFB. There are no emergency medical services available on DMAFB, therefore the contractor must dial 911 for emergency medical services.
