PERFORMANCE WORK STATEMENT
for Cable Services at
Mike O’Callaghan Federal Hospital (MOFH) 99 Medical Group (MDG)
Nellis Air Force Base (AFB) Las Vegas, Nevada 89191
23 August 2017
1.0 Description of Service/General Information
The Contractor shall construct, install, operate and maintain a television system for 99 MDG, Mike O’Callaghan Federal Hospital, Nellis AFB, Nevada in accordance with the terms, conditions, and standards set forth in this performance work statement (PWS). The Contractor shall be capable of providing service to buildings 1300 (main hospital), 340 (Satellite Pharmacy, Mental Health and Flight Medicine/Public Health) and 60 (Bio-Environmental). All materials required for installation, operation, and maintenance of the television system to the building must be furnished by the Contractor at its own expense.
1.1 Installation Services
The Contractor shall provide all personnel, equipment, tools, materials, supervision, and other items and services necessary to provide television services to Buildings 1300, 340, 60, 1032 and 1033. The Contractor shall furnish at its own expense new equipment and materials needed to provide television service to required areas. This includes but is not limited to equipment that is needed to provide access to multiple drops within the same building. The Contractor shall provide the capability to change the channel independently at each television. The number of television drops requiring service is 100. It is possible that the government may require up to 7 additional drops each year. These items may be purchased via Government Purchase Card or through bilateral modification to the contract. Any additional monthly cable service resulting from the installation of additional drops will be incorporated into the contract via bilateral modification at the prices established by the contractor for that given year.
1.2 Maintenance Service
The Contractor shall maintain, measure, inspect, and test the cable system in accordance with industry standards and as required by Federal Communications Commission (FCC) regulations.
If trouble is determined to be caused by the Contractor’s equipment, the Contractor shall notify the Government immediately. The Contractor shall inform the Government of the nature of the outage and the estimated time of repair. If trouble is determined to be caused by factors not attributed to the Contractor’s equipment or service, the Contractor shall notify the Government immediately. If the Contracting Officer gives authorization to proceed, the Contractor shall perform the repair or replacement of equipment within 24 hours from the authorization. The CO is the only individual allowed to direct the Contractor to proceed with the repair. If repairs/replacement of equipment are completed without CO approval the contractor may bear all fiscal risk.
1.2.1 Signal Leakage/Interference
The Contractor shall recognize signal leakage or other interference, including emission, radiation, or induction, from the television system that affects the Government's air navigational, communications, or surveillance facilities, or other electronic device or equipment located on the installation which may disrupt or endanger military activities and may pose a threat to the safety of both the public and military personnel. The Contractor shall take all necessary actions, including testing, maintenance, and coordination with Government personnel to prevent any such leakage or interference. Upon receiving notice of such leakage or interference from whatever source, including the Contractor's own testing and monitoring activities, the Contractor shall immediately take corrective action (with-in 8 hours) to eliminate the leakage or interference.
1.3 Channel Capacity
The system shall provide one tier of service:
The MDG Facilities - Bldgs 1300 (main hospital), 340 (Satellite Pharmacy, Mental Health and Flight Medicine/Public Health) and 60 (Bio-Environmental) will have only one tier of service.
The estimated number of television drops requiring tier one services is 100.
1.3.1 Service Tier Description
Service will have a minimum of thirty (30) channels. The service will include but not limited to one (1) 24 hour news channels, one (1) 24 hour weather channel, one (1) sports channel, one
(1) childrens programming channel, and five (5) local channels. Providing more than 30 channels is acceptable but not required.
1.4 Channel Guide
A laminated channel guide shall be provided by the Contractor to all users, and be updated whenever programming change at no additional cost to the government. A ‘guide’ channel that viewers can watch is acceptable and meets this criteria, but will not count as a channel to meet the requirements of the cable packages.
1.4 Service Drop Equipment
The Contractor shall provide all materials and equipment necessary to connect the Government owned hardware to the service network. Satellite dishes, receivers, converter boxes, cable boxes, and modems are all example of service drop equipment. Ownership of these items will remain with the Contractor. Any upgrades to the service drop equipment shall be provided and exchanged out within 90 days of release to the public. Rental/lease/usage fees will not be a separate cost and must be included with the monthly service CLIN. If the cost of the additional drops are less than the micropurchase limit, then the customer will purchase this item via their Government Purchase Card on an as need basis.
1.4.1 Remote Controls
The Contractor shall provide universal remote controls for all drops at installation. The Contractor shall provide assistance in troubleshooting remote control operations as necessary to facility personnel. Ownership of the remote controls will remain with the Government at termination of this contract, unless the Government relinquishes that ownership to the Contractor. It is possible that the government may require up to 10 additional remotes each year. These items may be purchased via Government Purchase Card or through bilateral modification to the contract. Any additional monthly cable service resulting from the installation of additional drops will be incorporated into the contract via bilateral modification.
1.7 Addition and Termination of Service
The Government may add or terminate television services in any outlet or drop. When adding a drop or outlet, the Contractor shall provide and install any required materials and/or equipment to provide this service. There shall be no “term” or minimum length of service required for services at any location, and there shall be no termination fee for terminating service. The Contractor shall add television service or remove television service from any drop within 5 days notice from the Contracting Officer Representative (COR) or Contracting Officer (CO).
1.7.1 Equipment/Supplies/Manpower for Installations and Removals The Contractor will provide all equipment, material, tools, labor, and supplies for all official drop installations. All materials and equipment will remain the property of the contractor with the exception of wires, cable, plating, etc. At the completion of this contract, the Contractor shall remove all Contractor owned equipment and materials within seventy-five (75) days of written notification from the CO. If approved by the CO, the Contractor may elect to abandon the equipment and materials and allow the Government to take possession.
1.8 Customer Service
The Contractor shall provide quality customer support services to Nellis AFB personnel. The service includes proactive maintenance, repair management, timely response, identification of facility needs, property management and fiscal administration.
1.9 Normal Hours of Operation
The Contractor shall perform the services required under this contract 24 hours per day, 7 days a week, 365 days a year. The transition period may require the Contractor to perform services on evenings and/or weekends. The Contractor shall respond to official service outages when reported within 24 hours, regardless of whether on a weekend or holiday, at no additional cost to the government.
1.10 Service and Repair Log
The Contractor shall maintain for the duration of the contract a log showing the date and time of all customer service calls. The log shall indicate: The nature of complaint with time and date, repair action with time and date. The Contractor shall provide a copy of the log to the COR upon request. Contractor shall also maintain and provide a list of all cable drops per building and listing room numbers upon request.
1.11 Interruption of Service
The Contractor shall at all times maintain the system in good condition and repair to provide uninterrupted service in conformance with all standards and requirements. The COR and CO shall be notified by e-mail and telephone within ten working days in advance of interruption of service. Service shall not be interrupted/down for more than eight hours per month.
1.12 Contractor Support
The Contractor shall provide a point of contact (POC) and an alternate POC that shall be responsible for ensuring that the contract services are provided satisfactorily. The POC and alternate POC must be able to read, write, speak, and understand English and have the authority to act as a binding agent in regards to this contract. The name of this person, and their alternate or alternates, shall be designated in writing to the CO and the COR at the start of the contract and for the duration of the contract.
1.13 Equipment Installation
Work Orders will be issued by the COR for all new installations of official TV cable drops.
Contractor shall coordinate with the 99 MDG during all equipment installations.
1.14 Interior Installation
Cable/Wires shall be concealed behind ceiling and walls and not be run along interior walls or baseboards, unless approved by the COR. Television set boxes shall be installed in accordance with commercial practices.
1.15 Exterior Installation
Unless otherwise directed or permitted in writing by the Contracting Officer, the Contractor shall use existing Government underground facilities for carrying cable. Mounting shall follow building lines, where possible (i.e., brick, mortar, siding), in straight horizontal and vertical lines, with bends tied to building corners. Aerial service drops are prohibited in new installations. The diameter of any holes passing through walls shall not exceed the diameter of the cable or conduit by more than one-eighth inch. All exterior holes shall be sealed to prevent entry of moisture or insects into the facility. The contractor shall coordinate with the 99th Civil Engineering Squadron (CES) to ensure that all required permits are obtained (dig permits, dust permits, asbestos permits, etc.). The Contractor shall coordinate with 99 CES to ensure proper installation of all system equipment and materials. The contractor shall coordinate with 99 Communications Squadron (CS) for frequency waivers in the event a satellite/antenna is installed on a building.
1.16 Additional Capabilities
The Contractor and the Government recognize that, over the term of the PWS, television technology, the regulation of television, and the programming services available for distribution will undergo continual change; the technology, equipment, and services required by this PWS may become obsolete, uneconomical, or otherwise disadvantageous. The Contractor and the Government, therefore, agree to consider in good faith proposals submitted by either party for modification of the PWS to permit the introduction of new equipment and technology or to otherwise improve television service to the bases.

4.0 General Information
4.1 Entry/Escort Service.
The Government point of contact will arrange for entry and escort service if needed for Nellis AFB when required. In order to acquire access to the installation, the REAL ID Act does apply.

4.2 Points of Contact
Telephone numbers for the Nellis AFB points of contact shall be provided to the Contractor no more than 10 days after award of contract.
4.3 Refuse Collection
Waste generated by the Contractor in performance of the services specified with this PWS shall be deposited in Government dumpsters, excluding hazardous materials in which proper disposal instructions shall be complied with. The Contractor shall comply with all base recycling regulations.
4.4 Contractor Employees
The Contractor shall employ competent and qualified personnel to accomplish the work performed under this contract. The Contractor shall not hire off-duty Air Force personnel whose employment would result in a conflict of interest or would otherwise violate the Joint Ethics Regulation, DOD Directive 5500.7-R.
4.5 Dress and Appearance
Contractor personnel shall present a clean, neat and professional appearance and be easily recognized as Contractor personnel. This may be accomplished by wearing distinctive clothing showing the name of the company or by wearing appropriate badges that contain the company name and employee name. No item of military clothing shall be worn by a Contractor as an outer garment.
4.6 Safety Requirements
Contractor shall take such additional immediate precautions as may reasonably require for safety and mishap prevention purposes. Provide protection to Government property to prevent damage during the period of time the property is under the control or in possession of the Contractor.
4.7 Definitions
“Contracting officer” means a person with the authority to enter into, administer, and/or terminate contracts and make related determinations and findings. This is the only individual authorized to make any changes or modify the contract. “Contracting Officer’s Representative” means an individual who is designated and authorized in writing by the contracting officer to perform specific technical or administrative functions on contracts or orders.
