PART 1. DESCRIPTION OF SERVICES

1.1. SCOPE OF WORK: The Contractor shall furnish all personnel, equipment, tools, materials, supervision and other items and services necessary to perform laundry cleaning services on government furnished property located at Nellis AFB Linen Exchange, Visiting Quarters and Child Development Center Centers I II & III (CDC I II& III) Nellis AFB, Nevada. All work shall be accomplished in accordance with the recommendations set forth by the International Fabricare Institute (http://www.ifi.org), Institute of Industrial Launders, and standards established by the Occupational Safety and Health Administration (OSHA Standard 1910.264) for the commercial healthcare laundry industry.

1.2. BASIC SERVICES: The Contractor shall accomplish laundry cleaning services to meet the requirements of the Performance Work Statement (PWS) and the Service Delivery Summary (SS).

1.3. QUALITY STANDARDS: The contractor shall perform laundry service in accordance with manufacturer’s recommendations/instructions, and commercial standards and practices. The contractor shall ensure items are clean, free of soil and stains, and odors. Items damaged by the contractor shall have tears and ripped seams and holes repaired to match existing thread and color, at no additional cost to the government. Repairs are to be made only to tears and ripped seams that are 1 inch or less. Repairs are required on holes that are 1/2 inch or less, in diameter.

1.4. CONTRACT MANAGER: The service provider shall provide a contract manager at the cleaning location who shall be responsible for the performance of the work. The name of the primary, as well as an alternate contract manager, who shall act for the service provider when the manager is absent, shall be designated in writing to the contracting officer. The contract manager or alternate shall be available during normal duty hours within 3 hours to meet on the installation with government personnel designated by the contracting officer to discuss problem areas or emergency requirements. After normal duty hours the manager or designated alternate shall be available by phone within 1 hour for emergency actions which cannot wait until the next normal duty day. The contract manager and alternate(s) must be able to read, write, speak, and understand English.

1.5. PICK UP AND DELIVERY: Contractor shall pick-up and deliver to Building 466 - 4075 Duffer Drive, Building 536 - 4425 N. Washington Blvd, Building 556 - 5941 Fitzgerald Blvd, Building 2945 - 5641 Stafford Drive, Building 4842 - 28 Kinley Drive, Child Development Center I (CDCI ) - Building 2966 - 5085 John Chapman Parkway, and Child Development Center III (CDCIII) - Building 600 - 4207 Duffer Drive, Nellis AFB, Nevada, 89191.

All pick-up of lodging linen for cleaning shall be between the hours of 2:00 PM – 4:00 PM. Delivery of clean lodging linens shall be accomplished between the hours of 9:00 AM and 4:00 PM. The contractor shall insure the correct mix of items and quantities are returned to the location where they were picked up, i.e. items picked up at building 466 will be delivered to 466, items picked up at building 536 will be delivered to 536, items picked up at building 556 will be delivered to 556, items picked up at building 2945 will be delivered to 2945 and items picked up at building 4842 will be delivered to 4842. The mix of linens picked up must be the same as the mix of linens delivered. It is essential that the mix of clean linen returned be comparable to the soiled lined picked up to ensure continued and uninterrupted hotel operations to meet guest requirements and Air Force Lodging Standards.

All pick up and deliveries shall be accomplished between the hours of 8:00 AM and 3:00 PM. The contractor shall ensure the correct items and quantities are returned to the location they were picked up from. When a holiday occurs on a scheduled pick up or delivery day, the scheduled pickup and delivery shall be performed on the first workday following the holiday (Mon through Sat.).

1.6. HOLIDAY OBSERVATION: CDCI and CDCIII will observe holidays falling on a Saturday on the preceding Friday and holidays falling on a Sunday the following Monday. Lodging will not observe holidays due to the nature of the business and standard business practices.

1.7. FEDERAL HOLIDAYS: The following federal holidays are observed by this base and apply to CDCI and CDCIII:

The CDC’s may also close on authorized and approved Nellis AFB Family Days. The CDC’s will notify the contractor of any closures due to family days at least three days in advance of the closure.

1.8. SUPPLY OF LAUNDRY TICKETS AND ITEMIZED RECEIPT FORMS: Contractor shall provide all necessary laundry tickets and itemized receipt forms (one for every pick up day of the month, sample attached) to Linen Exchange, Visiting Quarters and CDC I & III. Adequate stock of laundry tickets and itemized receipt forms shall be provided to Linen Exchange, Visiting Quarters and CDC I & II prior to contract start date and throughout the entire contract period.

1.9. SEPARATING, WEIGHING, AND COUNTING OF ARTICLES, AND ITEMIZED RECEIPT FORM:

Weighing/counting of items to be laundered shall be accomplished by the government representative at each location prior to pick-up. The contractor shall verify the amounts, sizes, and weights at time of pick-up and sign the Count Sheet (Appendix B, C & D.) The signature constitutes agreement with the government weights/counts. The contractor shall receive the bottom copy of the weight/count sheet and the government will retain the original along with all additional copies of the form. Upon return, the government representative shall record the quantity of items laundered and returned on the original Count Sheet (Appendix B, C, & D) to verify the weights/counts and mix sizes at the time of delivery and sign on the sheet. The government representative at each location will retain the original copy of the Weight/Count Sheet and provide a copy to the contractor. The original itemized receipt form will be retained by the Government for computing variance in count when clean/dry items are delivered. Contractor personnel shall be responsible for physically unloading clean linen. Contractor personnel shall be responsible for loading soiled linen.

1.10. MAINTAINING SEPARATION OF GOVERNMENT PROPERTY:All lodging property, CDC property, other government accounts property, commercial accounts property, or private accounts property shall be handled, laundered and maintained separately. Separation between properties belonging to different or other customers shall be maintained at all times.

1.11. INCORRECT COUNTS: The contractor shall make no adjustments to the counts or weights. The contractor shall not launder and invoice items not originally counted/weighed on the tickets. If additional items are found, they shall be returned to the authorized user without being laundered. If items are laundered and not originally weighed/counted, the contractor shall not be reimbursed for those laundered items. If incoming items returned to the government are not government property or less than those sent to the contractor, and these lesser quantities are greater than the variance allowed in the service summary or are not the government property the government reserves the right to recoup the value of these lost linens from the contractor (REF: DFARS 252.237-7014).

1.12. LINEN EXCHANGE BEDLINEN, BATHLINEN, AND RAGS: Bed linen, bath linen and rags specified within this PWS shall be laundered as bulk laundry items. They shall be weighed and or counted (please reference Appendix B & D to determine whether item will be measured by weight or individual count). by Government personnel prior to pick up. Items shall be processed by the contractor by codes identified in Appendix A. Contractor shall verify weight/count at time of pick up/delivery, annotate any discrepancies and sign the itemized receipt form. The contractor shall be given a copy of the itemized receipt form at time of pick up/delivery. The original itemized receipt forms shall be retained by the

Government for computing variance in weight when clean/dry rags are delivered. Laundered rags shall be returned sorted by color. Contractor personnel shall be responsible for physically unloading clean linen. Contractor personnel shall be responsible for loading soiled linen.

1.13. SPECIFIC REQUIREMENTS FOR PACKAGING CLEAN LINEN AND LAUNDERING SOILED LINEN: All items shall be separated by size, color, content and clearly marked with the quantity of each item in the bundle. Linen Exchange has four different sizes of sheets Twin, Full, XL Full, and Queen Size. Bundles shall be of a weight between 5 and 10 lbs per bundle and be covered in a protective covering.

1.14. WATER REPELLENT ITEMS: Items that are water repellent treated will be identified at the time of pick up. Such items shall be treated in accordance with acceptable industry procedures for water repellent treatment stated in paragraph 1.1.

1.15. DAMAGED, SALVAGED AND LOST LAUNDRY: Damaged (ripped/torn) or excessively stained linen discovered by the contractor at the laundry facility, shall be cleaned, identified, and packaged separately to ensure accountability before returning to the Government. For the purposes of this contract, excessively stained shall be defined as items marked with boot polish, excessive amounts of makeup, or any other substance that cannot be removed by multiple cleanings. Salvage, or damaged laundry, shall be cleaned, conspicuously marked by dying a different color, and packaged separately when requested by the government. Dye colors to be used are red, dark green and dark blue. Upon completion salvaged items will be returned to Linen Exchange and CDC I II and III for proper disposition. The contractor shall repair all rips, tears, open seams, holes and loss of items caused by contractor processing as Non-Reimbursable Repairs. This is accomplished by using thread and materials of the same quality, color, colorfastness, and texture as that used in manufacture of the articles. Such repairs and replacements are performed at no additional cost to the government. Lost items and items damaged that exceed the parameters identified in section 1.3 shall be replaced by the contractor. The government will provide specific information on the item and possible source for items to be replaced.

1.16. STAIN TREATMENT: Stained linen discovered by the Government will be sent out to the contractor and shall be cleaned with a special chemical treatment. Linen that can’t be cleaned by Stain Treatment shall be packed separately and marked “STAIN” before returning it to the Government.

1.17. REJECTED LAUNDRY: Linen is rejected when it fails a quality assurance inspection at the time of delivery or when unpacked. Rejected laundry shall be collected by Linen Exchange and CDC I II and III as discovered and returned to the contractor who shall re-perform and deliver re-cleaned laundry and/or dry cleaning by the next scheduled delivery day. The Government will reject by individual piece, package, or bin(s). The Contractor shall re-perform at no cost to the Government.

1.18. CARTS: Contractor shall provide an adequate number of plastic laundry carts to Linen Exchange and CDC I & II to transport daily linen. Contractor shall also provide sufficient covered metal laundry carts for dirty rags. Historically Linen Exchange has used up to 100 carts. However, contractor shall provide an adequate number sufficient to perform operations. Additional carts are required on heavy checkout for soiled linen collections. Linen Exchange will contact the contractor in advance to identify heavy check out days. Historically CDC I & II utilizes three carts each Monday through Friday. Carts shall be in good operating conditions at all times to include proper operating condition of wheels. There are also ten carts that are tied up on a rotational basis. Each cart shall have an individual identification letter and the tare weight clearly stenciled on two opposing sides.

1.20. CONTINGENCY PLAN: Contractor shall have a contingency plan, in the event of equipment failure, to ensure no disruption of services. The contingency plan shall be submitted at the pre-performance conference for acceptance by the Government.

2.1. Contractor Manpower Reporting Application, CMRA.

2.1.1. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil.

2.1.2. Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October* of each calendar year. Contractors may direct questions to the CMRA help desk.”

2.1.3. Reporting Period: Contractors are required to input data by 31 October of each year. However, due to the delayed implementation, input of FY12 data is required by 31 Mar 13.

2.1.4. Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.

2.1.5. User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link. However, user manuals for government personnel and contractors are available at the

Army CMRA link at http://www.ecmra.mil.

PART 3. GOVERNMENT FURNISHED PROPERTY

3.0 GOVERNMENT FURNISHED PROPERTY AND SERVICES

3.1. Government-Furnished Property and Services. The Government will provide the items listed below.

3.1.1. Telephone. Contractor personnel shall be allowed the use of a Government telephone for local calls upon request.

3.1.2. Commercial Grade Scales. Scales will be of a type capable of weights up to 2,000 pounds. Scale is located Building 556, Nellis AFB, Nevada. A scale may be located in Bldg 466 in the future. The government will calibrate the scales for accuracy at least annually.

3.1.3 Linens. In accordance with DFARS 252.237-7018, all linens listed in Appendix B are government-owned property.

PART 4. GENERAL INFORMATION

4.1. QUALITY CONTROL. The Contractor shall develop and maintain a quality program to ensure laundry services are performed in accordance with commonly accepted commercial practices. Contractor shall immediately notify the government when they receive garbage in the laundry picked up. Notification will be made via telephone followed up with a written report by email or fax. The Contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of non-conformance to PWS requirements. At a minimum the Contractor shall develop quality control procedures addressing the areas identified in paragraph 2, Service Summary (SS). The Contractor shall submit their quality control plan at the post-award conference for acceptance by the Government.

4.2. QUALITY ASSURANCE. The Contracting Officer will periodically evaluate the Contractor’s performance by appointing a representative(s) to monitor performance to ensure services are received. The Contracting Officer will evaluate the Contractor’s performance through intermittent on-site inspections of the Contractor’s quality control program and receipt of customer feedback from Lodging and CDC I II and III personnel. The Contracting Officer may inspect each task as completed or increase the number of quality assurance inspections if deemed appropriate because of repeated failures discovered during quality assurance inspections or because of repeated customer complaints. Likewise, the Contracting Officer may decrease the number of quality assurance inspections if performance dictates. The Contracting Officer will also receive and investigate complaints from various customers located at Lodging and CDC I II and III. The Contracting Officer will be responsible for initially validating customer complaints.

4.3. GOVERNMENT REMEDIES. The Contracting Officer shall follow FAR 52.212.4, Contract Terms and Conditions- Commercial Items (May 1997), for Contractor’s failure to perform satisfactory services or failure to correct non-conforming services.

4.4. HOURS OF OPERATION. Normal work hours for lodging are 7:30 AM to 4:30 PM, Monday through Saturday and 8:30 AM to 4:30 PM on Sunday. All pick up of lodging linen for cleaning shall be between the hours of 2:00 PM – 4:00 PM. Clean linens may be delivered between the hours of 9:00 AM and 4:00 PM. Pick up and deliveries are accepted between 7:00 A.M. - 5:00 P.M. for CDCI and CDCIII, Monday through Friday, excluding federal holidays or other approved closures. If the contractor desires to pick up soiled laundry or deliver clean laundry during hours other than scheduled times in the PWS, the request shall be submitted in writing to the Contracting Officer. The Contractor shall list the desired dates and identify the reason(s) for the change of pick-up and delivery times. The Government shall be notified at least three days in advance. Contractor shall maintain close coordination with the Government. If such notification is not approved in advance, the Contractor bears the risk of possible non-payment of work and/or payment for damages incurred.

4.5. CODES. Contractor shall comply with industry standards and all work shall be accomplished in accordance with the International Fabricare Institute, Institute of Industrial Launders, and all guidelines established by the Occupational Safety and Health Administration (OSHA).

4.6. PERFORMANCE OF SERVICES DURING CRISIS. Nellis Air Force Base requires continued performance of services during any crisis declared by the National Command Authority or 99th Air base Wing Commander, or HQ Air Combat Command. Laundry services are deemed as “vital to mission continuance” and essential for continued quality service during any crisis. Schedule and hours of operation for contract laundry services will be maintained as normal. The Quality Assurance personnel will notify the laundry services contractor as soon as possible after implementation of heightened security posture.

4.7. COMPLIANCE WITH INSTALLATION SECURITY. All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the government installation, shall abide by all security regulations of the installation.

4.8. SAFEGUARD GOVERNMENT PROPERTY. The contractor shall be responsible for safeguarding all government property provided for contractor laundering/cleaning.

4.9. PERSONNEL. The contractor shall not employ any person who is an employee of the U.S. government if employing that person would create a conflict of interest. The contractor shall ensure all employees are qualified to perform the requirements of this statement of work. Contractor employees shall present a neat appearance and be easily recognized as contractor employees. Employees must have in their possession and present upon request, personnel and vehicle passes issued by the Government. Contractor personnel shall not wear military type uniforms such as battle dress uniforms (BDUs).

4.10. VEHICLE. All laundry items delivered under this contract shall be transported in clean and enclosed vehicles, provided by the contractor. The vehicle shall be maintained in a sanitary condition to prevent cross contamination.

4.11. EQUIPMENT. Laundry Carts/Bags. Contractor shall provide laundry carts and laundry bags in sufficient quantities at each location.

4.12. MANDATORY PUBLICATIONS. The contractor shall comply with the following:

4.12.1. AFI 23-111, Management of Government Property in Possession of the Air Force.

4.12.2. AFI 34-246, Chapter 6. Laundry and Dry Cleaning and Linen Exchange.

4.12.3. DOD 4145-19, Storage and warehousing of facilities and services

4.13. Safety

4.13.1. In performing work under this contract on a Government installation, the contractor shall:

4.13.1.1. Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and

4.13.1.2. Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes.

4.13.1.3. The contracting officer may, by written order, direct Air Force Occupational Safety and Health

(AFOSH) Standards and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract.

4.13.1.4. Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract.

(2) Examples. (i) The taxpayer has received a statutory notice of deficiency, under I.R.C. Sec. 6212, which entitles the taxpayer to seek Tax Court review of a proposed tax deficiency. This is not a delinquent tax because it is not a final tax liability. Should the taxpayer seek Tax Court review, this will not be a final tax liability until the taxpayer has exercised all judicial appeal rights.

(ii) The IRS has filed a notice of Federal tax lien with respect to an assessed tax liability, and the taxpayer has been issued a notice under I.R.C. Sec. 6320 entitling the taxpayer to request a hearing with the IRS Office of Appeals contesting the lien filing, and to further appeal to the Tax Court if the IRS determines to sustain the lien filing. In the course of the hearing, the taxpayer is entitled to contest the underlying tax liability because the taxpayer has had no prior opportunity to contest the liability. This is not a delinquent tax because it is not a final tax liability. Should the taxpayer seek tax court review, this will not be a final tax liability until the taxpayer has exercised all judicial appeal rights.

(iii) The taxpayer has entered into an installment agreement pursuant to I.R.C. Sec. 6159. The taxpayer is making timely payments and is in full compliance with the agreement terms. The taxpayer is not delinquent because the taxpayer is not currently required to make full payment.

(iv) The taxpayer has filed for bankruptcy protection. The taxpayer is not delinquent because enforced collection action is stayed under 11 U.S.C. 362 (the Bankruptcy Code).

(ii) The Offeror has ( ) has not ( ), within a three-year period preceding this offer, had one or more contracts terminated for default by any Federal agency.

(2) Principal, for the purposes of this certification, means an officer, director, owner, partner, or a person having primary management or supervisory responsibilities within a business entity (e.g., general manager; plant manager; head of a division or business segment; and similar positions).

(b) The Offeror shall provide immediate written notice to the Contracting Officer if, at any time prior to contract award, the Offeror learns that its certification was erroneous when submitted or has become erroneous by reason of changed circumstances.

(c) A certification that any of the items in paragraph (a) of this provision exists will not necessarily result in withholding of an award under this solicitation. However, the certification will be considered in connection with a determination of the Offeror's responsibility. Failure of the Offeror to furnish a certification or provide such additional information as requested by the Contracting Officer may render the Offeror nonresponsible.

(d) Nothing contained in the foregoing shall be construed to require establishment of a system of records in order to render, in good faith, the certification required by paragraph (a) of this provision.
