GENERAL REQUIREMENTS

SCOPE OF WORK:

Materials to be abated may include all types and configurations of asbestos in various concentrations and quantities and any additional items identified during the mandatory site visit. Contractor shall be responsible for obtaining any documents not attached as part of the RFP but referenced as criteria for the project. The projects shall include, but is not limited to the following major items of work:

a. Abatement material sampling and testing was conducted by DGM Environmental and a copy of the survey results is provided (as Attachment 1) Perform abatement using proper asbestos removal and disposal procedures.

b. After abatement and proper disposal of hazardous materials, Contractor is required to demolish the structures completely..

1. RKMF 05-0025ask projnum "What is Project Number?", Demo Base Cold Storage, BLDG 841; see attached specification and drawing for this project.

2. RKMF 95-0064ask projnum "What is Project Number?", Demo Steam Plant , BLDG 10207; the work includes but is not limited to the following:

1. Demolish all the concrete walls, block walls, roof decking, and standing seam metal decking.

2. Protect gas lines, sewer lines and other utilities in the area during demolish and cap as required. Disconnect and cap all utilities appropriately.

3. Demolish power pole and transformer. Keep and protect the pads mounted transformers and keep them enegized.

4. Block and reroute the storm drain as required for a proper drainage.

5. Pad mounted A/C unit to be removed and disposed.

6. Level the area with appropriate fill material and landscape the site to match the adjacent area.

7. Ensure continues utility connection to other facilities in the area.

HAZARDOUS MATERIAL ABATEMENT SEGMENT

1. Contractor is responsible for field verifying all information given.

a. Contractor’s Proposal & Site Visit: A site visit will be scheduled after Contractor’s receipt of this RFP. Contractor shall provide a cost proposal only within 30 days after the site visit.

b. Notice to Proceed (NTP) with abatement: After award and receipt of bonding, a NTP shall be provided to the Contractor to proceed with the Abatement Segment of the Contract. The contractor shall complete the abatement to 100% as specified below. The abatement segment shall be 45 calendar days.

c. Clearance sampling and documentation shall be provided by an independent third party Nevada approved consultant, and use of Government approved (National Voluntary Laboratory Accreditation Program (NVLAP) laboratory prior to removal of the containment/enclosure barriers.

NAC 444 Disposal of Asbestos

Containment of the Work Area: Workers shall be required to wear persona protective equipment capable of providing necessary levels of protection from existing asbestos at all times when working in areas where suspect asbestos materials may have previously been disturbed and during any work taking place within the containment area.

Protection of Abatement Workers: All contractor personnel scheduled to work within the containment area shall provide current copies of the following listed documents to the Contracting Officer prior to the start of work.

1. Current certificate verifying EPA/OSHA asbestos supervisor or worker training

2. Current certificate of medical qualification for accomplishing the required work and wearing the appropriate protective equipment.

3. Current Nevada Department of Industrial Relations permit to do asbestos abatement work.

DEMOLITION SEGMENT

1. NTP for Demo and Removal: Upon completion of the hazardous material abatement, the Contracting Officer will issue a NTP for the Demolition & Removal Segment of the contract. Period of performance to demolish and properly dispose the abated structures is 112 calendar days after the receipt of NTP

2. Contract Closeout Requirements: The contract will not be considered final or closed out until all requirements have been completed. This includes but is not limited to the correction of the inspection punch list items to the satisfaction of the Project Engineer / Architect and the Contracting Officer.
