Project WZVV 02-8009 DEMO CIVILIAN CAMP TTR BLDG 723 Project WZVV 02-8010 DEMO CIVILIAN CAMP TTR BLDG 738 (01 April 2009)

GENERAL REQUIREMENTS

SCOPE OF WORK:

The Contractor shall furnish all labor, supervision, materials, tools, equipment, permits, legal notifications, third party clearances, incidental works, as necessary to remove and properly dispose of asbestos at the following two (2) existing structures Bldg 723 and Bldg 738 located at Tonopah Test Range, Nevada. Materials to be abated may include all types and configurations of asbestos in various concentrations and quantities and any additional items identified during the mandatory site visit. Contractor shall be responsible for obtaining any documents not attached as part of the RFP but referenced as criteria for the project. The project shall include, but is not limited to the following major items of work:

a. Abatement material sampling and testing was conducted by Macrotec Consulting, LLC and a copy of the survey results is provided (Atch 1). Perform abatement using proper asbestos removal and disposal procedures.

b. After abatement and proper disposal of hazardous materials, Contractor is required to demolish the structures completely.

HAZARDOUS MATERIAL ABATEMENT SEGMENT

1. Contractor is responsible for field verifying all information given.

a. Contractor’s Proposal & Site Visit: A site visit will be scheduled after Contractor’s receipt of this RFP. Contractor shall provide a cost proposal within 30 days after the site visit.

b. Notice to Proceed (NTP) with abatement: After award and receipt of bonding, a NTP shall be provided to the Contractor to proceed with the Abatement Segment of the Contract. The contractor shall complete the abatement to 100% as specified below. The abatement segment shall be 45 calendar days.

c. Clearance sampling and documentation shall be provided by an independent third party Nevada approved consultant, and use of Government approved (National Voluntary Laboratory Accreditation Program (NVLAP) laboratory prior to removal of the containment/enclosure barriers.

NAC 444 Disposal of Asbestos

Containment of the Work Area: Workers shall be required to wear personal protective equipment capable of providing necessary levels of protection from existing asbestos at all times when working in areas where suspect asbestos materials may have previously been disturbed and during any work taking place within the containment area.

Protection of Abatement Workers: All contractor personnel scheduled to work within the containment area shall provide current copies of the following listed documents to the Contracting Officer prior to the start of work.

1. Current certificate verifying EPA/OSHA asbestos supervisor or worker training

2. Current certificate of medical qualification for accomplishing the required work and wearing the appropriate protective equipment.

3. Current Nevada Department of Industrial Relations permits to do asbestos abatement work.

DEMOLITION SEGMENT

1. NTP for Demo and Removal: Upon completion of the hazardous material abatement, the Contracting Officer will issue a NTP for the Demolition & Removal Segment of the contract. Period of performance to demolish and properly dispose the abated structures is 90 calendar days after the receipt of NTP

2. Contract Closeout Requirements: The contract will not be considered final or closed out until all requirements have been completed. This includes but is not limited to the correction of the inspection punch list items to the satisfaction of the Project Engineer / Architect and the Contracting Officer.
