Questions/notes from Melrose Air Force Range Site Visit:

1. If the contractor has to get the exploratory dig permit from the Office of State Engineer, then they will have to have power of attorney of MAFR.

a. Contractor to fill out forms and submit to CE?

b. The exploratory well form that needs to be submitted to the Office of the State Engineer is WR-07, "Application for Permit to Drill A Well with No Consumptive Use of Water."

There's a $5 filing fee for this form; the contractor is responsible for that. The contractor must complete the form; we will have 27SOCES/CC sign page 3. After

27SOCES/CC signs it, we'll give back to the contractor for him to submit (along with the

$5.00 filing fee) to OSE. I've attached a partially completed form. The WR-07 form can be turned around within a day or two provided the form is e-mailed to him and payment of the $5.00 filing fee is made via credit or debit card. If the form is mailed to him, allow

10 days to 2 weeks processing.

2. Well not to be built for potable water, but to be built to be able to meet potable water standards?

a. Yes. Future expansion of the system requires the Well to meet potable standards.

3. Processes of paperwork could potentially take much longer than the PoP provided. 45 days could potentially not be long enough.

a. Extending to 100 days to account for paperwork issues.

4. Not approved potable water source.

a. As answered in question 2. Initial usage will be for providing fire suppression water.

Future expansion requires the Well to meet potable standards.

5. Contractor to drill exploratory hole – plug hole – and drill in same or different location?

a. The same hole for the exploratory well can be used for the new well (obviously a wider bit will be used for the new well).

6. Can the contractor use water from one well to drill will?

a. Everybody receives water from the same location. – Well 14 – best pressure.
