Statement of Work (SOW)

CONSTRUCT PACKAGE TREATMENT PLANT, MAFR

Project Number: PXLY14-0471 Cannon Air Force Base, New Mexico August 11, 2016

1. BACKGROUND: The Melrose Air Force Range is located approximately 30 miles west of Cannon AFB and southwest of the village of Melrose, NM. The location of the future development within the NWDA lacks a sustainable water source to provide potable and non- potable water. The intent of this project is to install one (1) new well. The existing wells do not comply with the New Mexico Environment Department (NMED) Construction Programs Bureau’s “Recommended Standards for Water Facilities”.

2. OBJECTIVES: The objectives for this project include:

a. Drilling and installation of a potable water supply well meeting all NMED-DWB regulations for a Potable Water Supply System within the northwest area of Melrose Air Force Range as specified in the New Mexico Administrative Code (NMAC) 20.7.10, as well as NMAC 19.27.4.29 and NMAC 19.27.30, “Well Drilling”. The well will replace the existing agricultural well designated as G-1. As a point of clarification, the new well will produce water for a non-potable supply and not be used for human consumption; however, the contractor should construct the well as if it were to be potable.

b. Conduct alignment and plumbness testing of the well after installation of the well screen and riser per American Water Works Association (AWWA) standard A100-06.

c. After completion of the well and well development, conduct a step-drawdown test analyses to investigate the performance and efficiency of the well. Provide detailed report of analyses and findings.

d. Submit a well installation plan describing the well drilling methodology, well construction materials/product data to be employed such as well screen/riser, centralizers, pump/motor, and all other components of the well installation. Plan will include management and disposition of soil cuttings and development fluids.

e. Disinfect the well per AWWA standard C654-13.

f. Install a totalizing flow meter, water sampling spigot/port, as well as a diversion pipe to effectively flush the well of sediments as necessary, ensure a backflow preventer device is installed on the diversion pipe ; reference the New Mexico Environment Department Construction Programs Bureau “Recommended Standards for Water Facilities” guidance document.

g. Install a port for the installation of a depth measurement probe.

h. Provide a health and safety plan/activity hazard analyses describing safety precautions and engineering controls to be implemented during the drilling and installation of the well. Conduct and document daily “tailgate” safety meetings describing work to be performed and means of mitigating risks to personnel.

i. Provide a detailed soil boring lithology log utilizing the Unified Soil Classification System for the characterization of sub-surface geology.

j. Provide a well construction diagram /as built detailing all well construction components such as well casing, well screen, filter pack material, pump and motor, drop pipe, etc. Provide redlined drawings and specifications indicating any departures from the original government provided design contract documents.

k. Provide all necessary information to complete the “Totalizing Meter Report” form and the “Proof of Completion of Well” form for submission to the New Mexico Office of the State Engineer (NMOSE). The contractor shall submit to the government the “Totalizing Meter Report” within 10 calendar days of meter installation. Section 1., Permittee, information shall be left blank and the government representative will sign under “Submitted by.” A copy of the “Totalizing Meter Report” is included as Attachment 2.

l. Plug and abandon existing agricultural well designated G-1 meeting all NMED requirements as specified in NMAC 19.27.4.30 as well as in accordance with the NMOSE. Will include the preparation and submission of a “Well Plugging Plan of Operations” to the NMOSE prior to the plugging and abandoning of the existing well as well as a “Plugging Record” within twenty (20) days of completion of plugging activities. A copy of the “Plugging Record” form is included as Attachment 3.

m. The new well shall be drilled by a water well driller who is licensed in the State of New Mexico in accordance with Section 72-12-12 New Mexico Statues Annotated.

3. CONSTRUCTION REGULATIONS: The contractor shall operate in accordance with the 27th SOAOS Standard Operating Procedure 13-212, MELROSE AFR ADMINISTRATIVE MISSION AND CONTRACTOR ACCESS.

a. MEETINGS: The contractor shall attend one project kickoff meeting within one week of Notice to Proceed and one post-survey briefing following the site visit. The contractor shall prepare and distribute meeting minutes from each meeting.

b. GOVERNMENT FURNISHED PROPERTY: Upon the Contractor’s request, the Government will provide electronic and/or hard copies of available documents and correspondences pertinent to the Work. In addition, the Government will provide assistance with base passes, identification badges, and registrations.

c. DAILLY WORK SCHEDULE: Workdays shall be from 0730 to 1700, (0800 to 1700 for any “hot work” activity including welding, brazing, and cutting due to the availability of fire protection personnel), Monday through Friday. No weekend work allowed unless approved by the Contracting Officer or the Contracting Officer’s Representative. The Contractor will provide written notification to the Contracting Officer 14 calendar days lead time for proposed work schedule changes.

d. Concrete Trucks: Cleaning out of concrete trucks on MAFR is prohibited. Concrete truck chutes, only, may be rinsed at the construction site. Wastewater and concrete from this risnse shall be collected in a high-density polyethylene (HDPE) plastic-lined box or pit provided by the Contractor at the site. At the end of pouring operations, the Contractor shall excavate all the waste and liner and properly dispose of same. The Contractor shall dispose of all concrete debris to an authorized off base site and shall remove any and all concrete debris and residue at the end of the project at no additional cost to the Government. The pit shall be completely backfilled and the site restored to original conditions.

e. Contractor Storage: 27th SOCES shall designate Contractor storage and parking area. All project storage areas shall be kept free of debris, leaks, stains, or splashes and kept in a neat, clean and safe condition. Any contamination of the storage area by a hazardous substance shall be immediately remediated by the Contractor, in accordance with specifications and environmental regulations stated at pre-construction briefing, at no additional expense to the Government. All hazardous materials shall be secured when not in use.

f. Utilties: Water shall be made available to the Contractor by the Government from a well specified by 27th SOCES. The Contractor shall provide a water truck(s) to haul water from well to site. The Contractor shall make the well available to the fire department when required. The water provided is not fit for human consumption. Bottled water must be brought by the Contractor from consumption. Electrical power is not available on site. The Contractor shall provide own rest facilities for works.

g. Range Precautions: All construction personnel shall be required to view range safety briefing prior to beginning work. Laser protective eye wear must be available for each worker. Site supervisor must be in contact with Range Control Tower, and each vehicle operating under supervisor’s supervision. Range Operations shall coordinate for one radio at each work site.

1. Protective Laser Eyewear: The Contractor is required to use/wear protective laser eyewear when instructed to do so (when laser operations are active) by the Range Control Officer. Laser eye protection shall be in accordance with ANSI Z136.1 and CE En207 and shall protect from the following laser types: Argon, NDGA/YAG. Utmost care should be taken to maintain the integrity of these glasses as minor scratches can render them ineffective for use in protecting eyes from laser damage. The Contractor shall propose to purchase one set of eyewear for each prime and subcontractor employee who will be working on this project. The Government will only reimburse the Contractor for the first set of eyewear to be issued to each employee. Any damaged eyewear shall be replaced immediately and the Contracting Officer (CO) shall be notified of such damage and replacement. The Contractor shall be responsible for the costs of replacing the damaged eyewear and will not be reimbursed by the Government. Upon completion of the project, the Contractor shall turn in all undamaged sets of eyewear to the Contracting Officer (CO) such that the Government receives one set of usable laser protective goggles/glasses for every set purchased by the Government through the contract resulting from this solicitation.

h. Welding, Cutting, and Brazing: Fire Protection shall complete inspection of all welding, cutting, and brazing operations prior to any operation. The Contractor shall provide the appropriate operable fire extinguisher. Contractor shall comply with OSHA Std29 CFR 1910.252 Welding, Cutting and Brazing (General Requirements) and AFI 91-203 (June 2012) Air Force Consolidated Occupational Safety Instruction. Air Force Form 592 USAF Welding, Cutting and Brazing permits will be issued prior to any operation and shall be kept on site till completion of operation or permit expires. Contact the Construction Manager or Fire Protection at (575) 784-2578 for issuance of permit.

i. Soil: Soil conditions vary across the MAFR. Contractor shall assess existing soil. When existing soil does not meet soil specifications or compaction requirements as stated in the SOW, Drawings, and/or Specifications; the Contractor is responsible for importing soils which meet all specification and requirements. Origins of material and soil tests are required, and both must be approved by Government. Contactor shall have an approved plan and location for excavated and/or excess soil. Soil shall not negatively impact the existing project construction, accessibility of facility upon completion, or the environment.

1. Borrow Material: Borrow material shall be selected to meet the requirements and conditions of the particular fill or embankment for which it is to be used. Borrow material shall be obtained from approved sources, either private or within the limits of the project site, selected by the Contractor. Unless otherwise provided in the contract, the Contractor shall obtain from the owners the right to procure material, pay all royalties and other charges involved, and bear all expense of developing the sources, including rights-of-way for hauling. Borrow material from approved sources on Government-controlled land may be obtaining without payment of royalties.

2. Overburden Material: Overburden and other spoil material shall be transported off Base. Borrow pits shall be neatly trimmed and graded to drain after the excavation is completed. The Contractor shall ensure that excavation of any area, operation of borrow pits, or dumping of spoil material results in minimum detrimental effects on natural environmental conditions.

3. Backfill: Backfill shall be placed and compacted to at least 90 percent maximum density for cohesive materials or 95 percent maximum density of cohesion less materials, in such a manner as to prevent wedging action or eccentric loading upon or against any structure.

4. Ground Surface: Ground surface on which fill is to be placed shall be stripped of live, dead, or decayed vegetation, rubbish, debris, and other unsatisfactory material; plowed, disked, or otherwise broken up; pulverized; moistened or aerated as necessary; thoroughly mixed; and compacted. The prepared ground surface shall be scarified and moistened or aerated as required just prior to placement of embankment materials to assure adequate bond between embankment material and the prepared ground surface.

j. PROTECTION OF HISTORICAL AND ARCHEOLOGICAL RESOURCES: All items having any apparent historical or archeological interest outside of designated areas that are discovered in the course of any construction activities shall be carefully preserved. The contractor shall protect the “find-in-place” by leaving the archeological find undisturbed and by using flags to mark a fifty foot radius around the find. The find shall be immediately reported to the Contracting Officer and 27 SOCES/CEIEN so that proper authorities may be notified. All work shall be stopped in the immediate area of the discovery until directed by the Contracting Officer in coordination with 27 SOCES/CEIEN that work can be resumed.

k. ENVIRONMENTAL IMPACT: All waste materials generated by any work under the contract performed on a government installation shall be handled, transported, stored, and disposed of by the contractor at any time in accordance with all applicable Federal, state, and local regulations including, but not limited to, Executive Order 12088, 13 October 1978; the Federal Pollution Control Act, as amended (33 USC Sec 1251 et seq); the Clean Air Act, as amended (42 USC Sec 1857 et seq); the Endangered Species Act, as amended (16 USC Sec 1531 et seq); the Toxic Substance Control Act, as amended (15 USC Sec 2601 et seq); and the Solid Waste Disposal Act, as amended (42 USC 6901 et seq). Any hazardous materials brought on site in order to perform work require coordination through 27 SOCES/CEIE hazardous materials program manager.

l. CLEANUP: The contractor shall collect any and all trash, debris, refuse, garbage, etc., that is generated and place it in appropriate containers with lids or approved covers on a periodic basis or as directed by the Contracting Officer’s Representative. The aforementioned materials shall be hauled from the site by appropriate means on a daily basis, unless otherwise approved by the Contracting Officer’s Representative. Disposal shall be outside the limits of Government property. Disposal shall be by sanitary landfill or other approved methods and shall conform to all state, local, and federal guidelines and regulations. Upon completion of the work, the contractor shall restore the worksite to its original and natural condition permitting the growth of vegetation. Land disturbance shall be minimized to the greatest extent possible in the performance of the work.

4. PERIOD OF PERFORMANCE: The period of performance is expected to be 45 calendar days or less after Notice to Proceed (NTP) Weekend and federal holiday work is not permitted unless otherwise approved by the Contracting Officer (CO) fourteen (14) calendar days lead time for proposed work schedule changes. The Prime Contractor superintendent shall remain onsite at all times during construction when workers are present.

a. Open work times on the Melrose AFR will be from 0730-0200. This is due to availability of medical personnel on the range.

5. GOVERNMENT FURNISHED INFORMATION: The Government will provide electronic and/or hard copies of available documents and correspondences pertinent to the project as needed.

6. DELIVERABLES: The following must be provided to the designated Contracting Officer (CO) and Contracting Officer’ Representative (COR) or Program Manager (PM):

a. Well Electrical Specification and Design: The contractor shall not provide any design, this invitation for Bid is all inclusive. All that is needed of the contractor is the price. The government has provided all documents necessary to the project.

b. Project Schedule: Within 10 days of notice to proceed, provide a legible project schedule in printed and PDF format for construction to include dates of anticipated site visits, team meetings, and other milestone dates.

c. As-Built Construction Drawings and Specifications: Upon completion of construction, provide as-built drawings and specifications incorporating any departures from the original contract documents.

1. Electronic As-built Drawings and Specifications: Provide electronic drawing files in a format compatible with AutoCAD 2010 including all external references required to view and print the drawing files properly. Include all necessary *.ctb, *.pc3 or similar files needed to properly set line weights for printing. Drawing files should be organized in general accordance with the Department of Defense A/E/C CAD Standard (ERDC/ITL TR-12-X) available in PDF format on many federal websites. In addition, provide drawings in PDF format for on-screen viewing and reproduction. Provide final project specifications in PDF and Microsoft WORD *.doc or *.rtf format.

2. The file naming convention and file structure shown in the As-built Submittal Guidelines shall be used in lieu of what is found in the Department of Defense A/E/C CAD Standard.

3. Hardcopy As-built Drawings and Specifications: Provide two (2) full size and two (2) half size printed copies of the as-built drawings and two (2) copies of the final specifications.

4. As-built Drawings and Specifications shall be stamped on each page with ‘As-Built’ marking prior to submission to government.

d. Operation and Maintenance Manuals: Provide O&M Manuals including all warranties, vendor contacts, relevant product data including proper maintenance and operation, and pertinent shop drawings describing all installed materials, systems and equipment in the project. Manuals must be organized by CSI specification sections.

e. Geographic Information Systems (GIS): All new facilities and utilities shall be located by the Contractor in an electronic format that is compatible with the current base GIS software and includes descriptive information required by the Air Force. When the Contractor is unable to provide GIS information as described, the Contractor shall notify the PM so that other arrangements can be made. In such cases, the Contractor is required to assist Air Force agents or personnel in the generation of GIS information if requested by the PM. Subject to change, the following are the basic requirements for GIS information to be delivered to the Air Force on CD:

· DATUM – WGS84

· Coordinate System – UTM

· Units – Meters

· Data in attribute Table will be SDSFIE version 2.6 Compliant

· Shapefile Geodatabase format
