MEMORANDUM FOR RECORD

SUBJECT: Contract FA4819-18-Q-7004, Repair Lift Station, Site Visit Report/Questions and Answers

1. The purpose of this memorandum is to document the site visit held 16 May, 2018 along with the questions and answers that resulted from it.

2. ON 16 May, a site visit was held to explore the site conditions for the Repair Lift Station requirement for Tyndall AFB. While the site visit was conducted, multiple questions were presented to the Government representatives and more were submitted later on. They are listed below.

a. Is there any lead paint located on the existing metal work at the site?

a. A survey was conducted and no lead paint was found.

b. The lift station currently in place drawing has 208V 3ph power, however someone on site mentioned 230V1ph. Please confirm which will be available.

a. The power at the site is 208 volts @ 3-phase. This power is available to be used following the installation of new pumps

c. Do the DEP panel features attached provide all options needed in the control panel?

a. In review of the DEP list, it is desired to have a quick disconnect in the valve box for an emergency pumping condition.

3. Along with these questions, the Governement technical representative also noted the following.

a. The generator receptacle should match the cofiguration of that avaliable in the Utilities Department. (The contractor who receives the award is expected to coordinate with J&J Utilities)

b. The use of mercury type floats in the wetwell is preferred.

c. As mentioned on site, the wetwell shall be concrete.

d. FRP manholes will not be accepted.
