STATEMENT OF WORK

REPAIR BASE PAVEMENTS

TYNDALL AFB FLORIDA

Base Project Number: XLWU 18-1010 15 June 2018

1.0 INTRODUCTION

This project intent is to repair select Base pavements within the Tyndall Air Force Base (AFB) properties. The Contractor shall perform the services herein and as described in this Statement of Work (SOW).

2.0 SCOPE OF WORK

The Contractor shall be responsible for all professional services, permits, equipment, labor, tools, materials, and ancillary items necessary to complete the tasks defined by the SOW. The performed tasks shall be based on the project description, other data furnished in this SOW, and information covered during a pre-construction/criteria review meeting to be held prior to Contractor mobilization.

2.1 Scope & Applicable Documents: This SOW sets forth the requirements for a Sustainment, Restoration & Modernization (SR&M) Acquisition contract for a site specific construction effort for repair of select pavements located on Tyndall AFB, Florida. The Contractor shall adhere to the following guidance during construction phase(s) of this project: Unified Facilities Criteria (UFC) 3-250-01, UFC 3-250-03, UFC 3-600- 01, United States Army Corps of Engineers, Uniform Building Code (UBC), those sections of the Southern Standard Building Codes not incorporated in the UBC/International Building Code (IBC) for our respective area, Air Force Standards, Department of Defense (DOD) Directives, Florida State Statutes, Florida Department of Transportation (FDOT), the Tyndall AFB Facility Excellence Plan (Base Regulations), 325th Fighter Wing Bird Aircraft Strike Hazard (BASH) Plan 910, National Electric Code (NEC), Occupational Safety and Health Administration (OSHA) Requirements, Department of Environmental Protection (DEP) Regulations, Environmental Protection Agency (EPA) Regulations, and any related Americans with Disabilities Act (ADA) Compliance Requirements. If there is a conflict between these codes, regulations and specifications the most stringent requirements shall apply at all times. Documents listed by and/or provided by the Government are for reference only and do not relieve the Contractor from adherence with applicable codes not specifically addressed.

2.2 Description of Work: The project intent is to rehabilitate various road and parking lot pavements located on Tyndall AFB. Repair will primarily involve mill and overlay procedures, new permanent pavement markings and select tasks requiring complete reconstruction. The contractor shall perform corrective measures to address encountered isolated areas of instability, potholes or root damage which is estimated to be up to 2% of the combined pavement area. Any of the aforementioned conditions encountered shall be quantified and communicated to the COR upon discovery. Milling and asphalt depths stated in the task descriptions are average values. Underlying base materials/subgrade disturbed during construction activities shall be reworked to achieve stable bearing surface for the pavement section. Task areas containing parking blocks shall reuse the existing parking blocks. The Government has assumed that up to 30% of the parking blocks will require replacement with new to account for existing damage or damage incurred during removal. Pavement markings shall match existing unless specifically noted otherwise. The Contractor to notify government if existing markings conflict with MUTCD or FDOT standards. The Contractor shall implement means and methods that will limit disturbance to the natural vegetation, dune systems, water bodies and base activities to the greatest extent practicable throughout the project performance. FDOT Management of Traffic (MOT) guidelines shall be followed. Unless noted otherwise, work is anticipated to be performed during normal working hours. The government provided documents and existing field conditions shall be reviewed by the contractor to identify any potential discrepancies and develop necessary Requests For Information (RFI) prior to construction mobilization.

2.2.1 TASK 1 (Building 950 Truck Delivery): Repair the Building 950 truck delivery pavement as shown in the Criteria Drawings attached to the SOW. The facility is currently operational with delivery traffic typical to retail business. Work shall be coordinated with the facility management to limit disruption to retail delivery activities. After hours or weekend work shall be anticipated for this task. The approximate repair area is estimated to be 2,675 square yards. Anticipated repair efforts shall include the following;

A. Mill 1.5 inches from the existing asphalt pavement;

B. Prepare the milled pavement surface for overlay;

C. Place 1.5 inches of SP12.5 asphalt pavement in accordance with industry standards;

D. Paint pavement markings to match existing for the new pavement and concrete pavements at the dock;

2.2.2 TASK 2 (Building 950 Side Parking): Repair the Building 950 side parking lot pavement as shown in the Criteria Drawings attached to the SOW. The facility is currently operational with employee and customer traffic typical to retail business. The contractor may close the task area and perform the work during normal business hours. The approximate repair area is estimated to be 1,805 square yards. Anticipated repair efforts shall include the following;

A. Mill 1 inch from the existing asphalt parking lot pavement;

B. Prepare the milled pavement surface for overlay;

C. Place 1 inch of SP9.5 asphalt pavement in accordance with industry standards;

D. Paint pavement markings to match existing;

2.2.3 TASK 3 (Mall Lane): Repair the Mall Lane pavement as shown in the Criteria Drawings attached to the SOW. The road experiences light to moderate traffic. Vehicle access to adjacent facilities shall be available during performance of the work. The approximate repair area is estimated to be 2,115 square yards. Anticipated repair efforts shall include the following;

A. Mill 1.5 inches from the existing asphalt pavement;

B. Repair pavement, base material, subgrade and remove root at tree root damage – est. 15 sy;

C. Prepare the milled pavement surface for overlay;

D. Place 1.5 inch of SP12.5 asphalt pavement in accordance with industry standards;

E. Paint reflective pavement markings to match existing;

2.2.4 TASK 4 (Oklahoma Avenue & Building 912): Repair the Oklahoma Avenue and Building 912 parking lot pavements as shown in the Criteria Drawings attached to the SOW. The road experiences light to moderate traffic. Vehicle access to adjacent facilities shall be available during performance of the work. The contractor may close the Building 912 parking areas during construction. Pedestrian access to the facility shall be maintained at all times. Anticipated repair efforts shall include the following;

Oklahoma Avenue (est. 2,130 square yards) A. Mill 1.5 inches from the existing asphalt pavement;

B. Prepare the milled pavement surface for overlay;

C. Place 1.5 inches of SP12.5 asphalt pavement in accordance with industry standards;

D. Paint reflective pavement markings to match existing;

Building 912 Parking Lots (est. 870 square yards) E. Mill 1 inch from the existing asphalt parking lot pavements;

F. Prepare the milled pavement surface for overlay;

G. Place 1 inch of SP9.5 asphalt pavement in accordance with industry standards;

H. Paint pavement markings to match existing;

2.2.5 TASK 5 (Virginia Lane & Wisconsin Street): Repair Virginia Lane and Wisconsin Street as shown in the Criteria Drawings attached to the SOW. The roads experience moderate traffic activity. Vehicle access to adjacent facilities shall be available during performance of the work. The approximate repair area is estimated to be 1,565 square yards for Virginia Lane and 355 square yards for Wisconsin Street. Anticipated repair efforts shall include the following;

A. Mill 1.5 inches from the existing asphalt pavement;

B. Prepare the milled pavement surface for overlay;

C. Place 1.5 inches of SP12.5 asphalt pavement in accordance with industry standards;

D. Paint reflective pavement markings to match existing;

2.2.6 TASK 6 (Building 914 Parking Lot): Repair Building 914 parking lot as shown in the Criteria Drawings attached to the SOW. The facility is currently operational with employee and customer traffic. The Contractor shall phase the work or perform work outside of the facilities normal business hours to limit disruption to regular operation. The approximate repair area is estimated to be 4,550 square yards. Anticipated repair efforts shall include the following;

A. Mill 1 inch from the existing asphalt parking lot pavement;

B. Prepare the milled pavement surface for overlay;

D. Paint pavement markings similar to existing;

2.2.7 TASK 7 (Building 909 Parking Lot): Repair Building 909 parking lot as shown in the Criteria traffic. The Contractor may close the parking lot to vehicle traffic during work performance provided that the Building 914 parking lot is accessible for employee use. Pedestrian access to the facility shall be maintained at all times. The approximate repair area is estimated to be 1,330 square yards.

Anticipated repair efforts shall include the following;

A. Mill 1 inch from the existing asphalt parking lot pavement;

B. Prepare the milled pavement surface for overlay;

2.2.8 TASK 8 (Louisiana Avenue): Repair the Louisiana Avenue pavement as shown in the Criteria Drawings attached to the SOW. The road experiences moderate traffic activity. Vehicle access to adjacent facilities shall be available during performance of the work. The work shall be coordinated with Base Security Forces to occur during a period when the Louisiana Gate is not open for use. The approximate repair area is estimated to be 10,720 square yards. Anticipated repair efforts shall include the following;

A. Mill 1.5 inches from the existing asphalt pavement;

B. Prepare the milled pavement surface for overlay;

C. Place 1.5 inch of SP12.5 asphalt pavement in accordance with industry standards;

E. Repaint stop bars & double center lines on Minnesota Avenue and Mississippi Avenue;

2.2.9 TASK 9 (Building 1542 Parking Lot): Repair Building 1542 parking lot as shown in the Criteria Drawings attached to the SOW. The facility is currently operational with employee, customer traffic and motorcycle safety track activities. The Contractor shall coordinate the work with the Motorcycle Track Users to occur in between scheduled training activities. The approximate repair area is estimated to be 8,030 square yards. Anticipated repair efforts shall include the following;

A. Mill 1 inch from the existing asphalt parking lot pavement;

B. Prepare the milled pavement surface for overlay;

C. Place 1 inch of SP9.5 asphalt pavement in accordance with industry standards;

D. Paint parking stall markings to match existing;

E. Coordinate with Cycle Track Users to mark up to 1,000 square feet of markings (ex: 3,000 lf of 4”line);

2.2.10 TASK 10 (Mississippi Road #1): Repair the Mississippi Road pavement as shown in the Criteria Drawings attached to the SOW. The road experiences moderate traffic activity. Vehicle access to adjacent facilities shall be available during performance of the work. The approximate repair area is estimated to be 9,105 square yards. Anticipated repair efforts shall include the following;

A. Mill 1.5 inches from the existing asphalt pavement;

B. Prepare the milled pavement surface for overlay;

C. Place 1.5 inch of SP12.5 asphalt pavement in accordance with industry standards;

E. Paint new stop bar and repaint double center lines at the Suwannee Road intersection.

2.2.11 TASK 11 (Mississippi Road #2): Reconstruct Mississippi Road as shown in the Criteria Drawings attached to the SOW. The road experiences low to moderate traffic activity consisting primarily of small truck and truck traffic. Pedestrian and limited vehicle access to adjacent facilities shall be available during performance of the work. Vehicle access shall be maintained with limited duration blockage. The approximate repair area is estimated to be 1,337 square yards. Anticipated repair efforts shall include the following;

A. Remove existing pavement, base materials and excess soils;

B. Compact exposed subgrade to 95% of the laboratory Modified proctor value;

C. Install 8” limerock base material compacted to 98% of the laboratory Modified Proctor value;

D. Place 2.5 inches of SP12.5 asphalt pavement in accordance with industry standards;

E. Paint new reflective pavement markings to match existing;

2.2.12 TASK 12 (Building 1149 Parking Lots): Repair Building 1149 parking lots as shown in the Criteria Drawings attached to the SOW. The facility is currently operational with employee and resident traffic.

The Contractor may coordinate short duration closure of the parking lots to vehicle traffic with the approval of facility management during work performance or phase the work to permit partial vehicle access. Pedestrian access to the facility shall be maintained at all times. The approximate repair area is estimated to be 4,260 square yards. Anticipated repair efforts shall include the following;

Rear Parking Lot – South East (est. 1,816 square yards) A. Prepare existing pavement surface for overlay;

B. Place 1 inch of SP9.5 asphalt pavement in accordance with industry standards;

C. Taper on to adjoining pavements for smooth transition;

D. Paint pavement markings to match existing;

Side Parking Lots - North East & South West (est. 2,444 square yards) E. Mill 1 inch from the existing asphalt parking lot pavement;

F. Prepare the milled pavement surface for overlay;

G. Place 1 inch of SP9.5 asphalt pavement in accordance with industry standards;

H. Paint parking stall markings similar existing;

2.2.13 TASK 13 (Building 662 Parking Lot): Repair Building 662 parking lot as shown in the Criteria traffic. The Contractor shall phase the work to maintain vehicle and pedestrian access to the facility at all times. The approximate repair area is estimated to be 20,045 square yards. Anticipated repair efforts shall include the following;

A. Mill 1 inch from the existing asphalt parking lot pavement;

B. Prepare the milled pavement surface for overlay;

2.2.14 TASK 14 (Building 650 Parking Lot): Repair Building 650 parking lots as shown in the Criteria traffic. The Contractor shall phase the work to maintain vehicle and pedestrian access to the facility at all times. The approximate repair area is estimated to be 2,650 square yards. Anticipated repair efforts shall include the following;

A. Remove parking blocks for reuse;

B. Mill 1 inch from the existing asphalt parking lot pavement;

C. Prepare the milled pavement surface for overlay;

D. Place 1 inch of SP9.5 asphalt pavement in accordance with industry standards;

E. Install parking blocks;

F. Paint pavement markings to match existing;

2.2.15 TASK 15 (California Road): Repair the California Road pavement as shown in the Criteria Drawings attached to the SOW. The road experiences low to moderate traffic activity. Vehicle access to adjacent facilities shall be available during performance of the work. The approximate repair area is estimated to be 2,350 square yards. Anticipated repair efforts shall include the following;

A. Mill 1.5 inches from the existing asphalt pavement;

B. Prepare the milled pavement surface for overlay;

C. Place 1.5 inch of SP12.5 asphalt pavement in accordance with industry standards;

2.2.16 TASK 16 (Building 462 Parking Lots): Repair Building 462 parking lots as shown in the Criteria traffic. The Contractor shall phase the work to maintain vehicle and pedestrian access to the facility at all times. The approximate repair area is estimated to be 1,780 square yards. Anticipated repair efforts shall include the following;

A. Mill 1 inch from the existing asphalt parking lot pavement;

B. Prepare the milled pavement surface for overlay;

2.2.17 TASK 17 (Building 449 Parking Lots): Repair Building 449 parking lots as shown in the Criteria traffic. The Contractor shall phase the work to maintain vehicle and pedestrian access to the facility at all times. Anticipated repair efforts shall include the following;

Asphalt Parking Lots (est. 3,070 square yards) A. Remove parking blocks for reuse;

B. Mill 1 inch from the existing asphalt parking lot pavement;

C. Prepare milled pavement surface for overlay;

D. Place 1 inch of SP9.5 asphalt pavement in accordance with industry standards;

E. Install parking blocks;

F. Paint pavement markings to match existing;

Gravel Parking Lot (est. 180 square yards) G. Grade and compact existing gravel parking area H. Place 7 cubic yards of gravel;

2.2.18 TASK 18 (Building 427 Parking Lot): Repair Building 427 parking lot as shown in the Criteria traffic. The Contractor may close the work area to non-critical vehicle traffic during performance.

Pedestrian access to the facility shall be maintained at all times. The approximate repair area is estimated to be 3,070 square yards. Anticipated repair efforts shall include the following;

A. Mill 1 inch from the existing asphalt parking lot pavement;

B. Prepare the milled pavement surface for overlay;

D. Paint pavement markings similar to existing to include adjacent concrete pavement areas;

2.2.19 TASK 19 (Building 227 Parking Lot & Arizona Avenue): Repair Building 227 parking lot and Arizona Avenue as shown in the Criteria Drawings attached to the SOW. The facility is currently operational with employee and customer traffic. The Contractor shall phase the work to maintain vehicle and pedestrian access to the facility at all times. Anticipated repair efforts shall include the following;

Building 227 Parking Lot (est. 7,810 square yards) A. Remove parking blocks for reuse;

B. Mill 1 inch from the existing asphalt parking lot pavement;

C. Prepare milled pavement surface for overlay;

D. Place 1 inch of SP9.5 asphalt pavement in accordance with industry standards;

E. Install parking blocks;

F. Paint pavement markings to match existing;

Arizona Avenue (est. 815 square yards) G. Mill 1.5 inches from the existing asphalt pavement;

H. Prepare milled pavement surface for overlay;

I. Place 1.5 inches of SP12.5 asphalt pavement in accordance with industry standards;

J. Paint pavement markings to match existing;

2.2.20 TASK 20 (EOD Range): Repair the EOD Range gravel roads as shown in the Criteria Drawings attached to the SOW. The gravel road experiences low traffic activity. Vehicle access to range shall be available during performance of the work. The approximate repair area is estimated to be 6,896 square yards. Anticipated repair efforts shall include the following;

A. Grade existing gravel road to level ruts;

B. Compact exposed surface materials;

C. Place, grade and compact 800 cubic yards of gravel on prepared surface to restore 10 foot width minimum;

2.2.21 TASK 21 (Boyscout Road): Repair the Boyscout Road and Scout Center pavements as shown in the Criteria Drawings attached to the SOW. The road experiences low to moderate traffic activity.

Vehicle access to adjacent facilities shall be available during performance of the work. Anticipated repair efforts shall include the following;

Boyscout Road (est. 18,336 square yards) A. Mill 1.5 inches from the existing asphalt pavement;

B. Prepare milled pavement surface for overlay;

C. Place 1.5 inches of SP12.5 asphalt pavement in accordance with industry standards;

D. Paint reflective pavement markings to match existing;

Scout Center Parking Lot (est. 345 square yards) E. Remove parking blocks for reuse;

F. Mill 1 inch from the existing asphalt parking lot pavement;

G. Prepare milled pavement surface for overlay;

H. Place 1 inch of SP9.5 asphalt pavement in accordance with industry standards;

I. Install parking blocks;

J. Paint pavement markings to match existing;

Areas disturbed by construction activity shall be restored and/or stabilized to equal or better than previously existing conditions. All work shall be in compliance with all Public Laws (P.L.), Executive Orders (E.O.), Military Standards (MIL-STD), Air Force Instruction (AFI), Code of Federal Regulations (CFR), Department of Defense Instructions (DODI) and Department of Defense Directives (DODD) or other higher authority documents as applicable, as listed in MIL-STD-3007F.

2.3 Security Requirements:

MIL-HDBK-1013: DOD Military Handbook Design Guidelines for Physical Security of Facilities.

AFI 31-101 Air Force Instruction 31-101: The Air Force Installation Security Program.

DOD 5200.1.R, Department of Defense 5200.1.R: Information Security Program Construction Standards UFC 4-010-01 Unified Facilities Criteria Antiterrorism Standards for New and Existing Facilities

2.4 Permits: (Where Applicable) The Contractor shall develop, coordinate, and be responsible for labor and other costs associated with acquiring any and all required state, local, and other applicable permits; base access (including off-base easements and leases), agreements, licenses, and certificates required to perform and complete the project. Before the start of work, the Contractor shall process an Air Force Form 103, Work Clearance Request, through the local Base Civil Engineer (BCE) office. The Contractor shall maintain a library of these documents on site as well as at the corporate facility handling each project. The Contractor shall comply with applicable permit conditions, environmental protection policies and appended guidelines.

2.5 Construction: Upon approval by the CO and receipt of all required permits (if applicable) and phasing plans, the Contractor may commence on-site construction activities. The Contractor shall conform to all specifically referenced and other applicable requirements during performance of the work as described in the SOW:

1. Mobilization: Mobilize equipment, labor, tools, materials, and ancillary items necessary to complete the work defined in the SOW.

2. Demolition / Construction: Perform awarded tasks as defined in section 2.2 Description of Work.

a. Protect work area with barricades, warning tape, signs and/or other appropriate methods for the work being completed to ensure the safety of pedestrians, contractor personnel, vehicles and property;

b. Remove and dispose of all demolition and general construction debris from the site (off base);

c. Perform ancillary efforts required to complete the work necessary to provide a clean, complete and functional facility;

d. Limit disturbance to the natural vegetation, persons and property to the greatest extent practicable.

2.6 Government Furnished Equipment: None

3.0 GENERAL REQUIREMENTS

Applicable provisions of the Contract Clauses in the contract will govern work under each section of the specifications.

3.1 Pricing Schedule: The Contractor shall complete a Pricing Schedule and include the completed schedule with the submitted bid documents.

3.2 Kick-Off Meeting: A kick-off meeting shall be accomplished with the Contractor, Contracting Officer, Civil Engineering Squadron (CES), and user in accordance with the Table of Deliverables. The meeting objective is to review project details, determine construction start date, security procedures for site, dumpster locations, and any applicable requirements during construction. The Contractor shall visit the job site to verify location, conditions, and dimensions prior to starting work.

3.3 Facility Occupancy: The road and parking facilities covered by this contract are currently open to public and experiences low to moderate pedestrian and vehicle traffic. During performance of this contract, non-work areas will remain functional to normal activities unless noted otherwise. Refer to the Task Descriptions for task specific occupancy conditions. Base personnel will relocate military vehicles/equipment from the work areas. The Contractor is responsible for all management of traffic (MOT) in the work areas for the duration of the performance period.

3.4 Identification of Employees: The Contractor shall be responsible for obtaining security clearances and badges for each employee, and for requiring each employee engaged on the work to display identification as approved and directed by the CO and Security Forces. The contractor shall submit personnel background check applications for badging within fourteen (14) days after award. Badge processing typically requires fourteen (14) days to complete and badges are issued to the applicant in-person at the Tyndall AFB Visitor’s Center. During performance of the contract, prescribed identification shall immediately be delivered to the CO for cancellation upon release of any employee. When required, the Contractor shall obtain and provide fingerprints of persons employed on the project. All Contractor and sub-contractor personnel shall wear identifying markings clearly defining the company for whom the employee works.

3.5 Employee Parking: Contractor employees shall park privately owned vehicles in an area designated by CES and approved by the CO.

3.6 Job Site Tools and Equipment: All hand tools and equipment shall be maintained in good working conditions and properly stored when not in use. Construction attire shall be in accordance with applicable OSHA regulations.

3.7 Notification Requirements: The Contractor shall notify the Government a minimum of ten (10) days in advance of mobilization. The Contractor is required to notify the (CO) and Government Project Manager of critical issues that may affect the contract performance and/or human health and the environment. For critical issues pertaining to human health and the environment, the Contractor shall immediately notify the CO and stop work. Work shall only resume at the direction of the CO following resolution of the issue.

3.8 Environmental Protection: Activities shall be planned and implemented in a manner that protects existing site utilities, structures, surface features, service operations, critical wildlife, and the general site environment. This includes the protection of trees, shrubs and other vegetation not in the affected zone from dust damage, soil compaction, and physical contact with machines and equipment. If appropriate, the Contractor shall conserve uncontaminated topsoil by removal, storage, or redistribution. Soil redistribution practices are anticipated for this project. All reasonable measures shall be taken to minimize and suppress fugitive emissions of dust, vapors, and other site materials during site work. All fill materials shall be non-contaminated. The Contractor shall conduct operations and activities with the intent of reducing the amount of pollution generated. Specific areas to be focused on are generation of solid waste, use of hazardous materials, use of ozone depleting chemicals, generation of hazardous waste, and use of energy and water.

The Contractor shall plan, construct, operate, maintain, optimize, and commission systems necessary to control storm water run-on and runoff. At a minimum, the Contractor shall employ best management practices consistent with the Florida Development Manual, Chapter 6, Storm-water and Erosion and Sediment Control Best Management Practices for Developing Areas.

3.9 Hazardous Waste Management: CO will notify 325 CES/CEA if any hazardous waste is encountered or generated by the Contractor. The Contractor shall stop all work in the event 325 CES/CEA identifies noncompliance with federal and state regulations and shall correct any discrepancies immediately within 2 hours of notification by 325 CES/CEA. All hazardous waste shall be labeled and an inventory management system will be initiated to insure timely removal and proper disposal. No on-base disposal will be allowed.

All drums will be labeled with a hazardous waste label. The label shall include the proper Department of Transportation (DOT) shipping name, United Nations (UN) or North American (NA), EPA waste number, generator information, and accumulation start date. The label shall be placed on the side of the drum. All drums used to store hazardous waste shall be non-leaking and safe to handle. Contractor shall be responsible for over packing drums that are rusted, dented, or leaking. Drums and/or over packs shall be provided by the Contractor. All drums shall be "new" DOT approved containers. The storage location for the hazardous waste drums shall be approved by the 325 CES/CEA prior to the generation of hazardous waste. The Contractor shall document inspection of drums for leaks on a daily basis or if not working in the area daily, then a weekly inspection will suffice. A copy of the inspection checklist shall be forwarded to 325 CES/CEA every Friday. Hazardous waste transportation and disposal shall be coordinated through 325 CES/CEA. The Contractor shall be responsible for transportation and disposal of all hazardous waste at an EPA approved treatment, storage, disposal facility (TSDF). The transportation and disposal facilities shall be approved by 325 CES/CEA prior to their use. Manifests shall be signed only by 325 CES/CEA. Drums shall be disposed of within 90 days of placing the first drop in the container.

3.10 On-Site Field Oversight: The Contractor shall provide the manpower, equipment, material, services, and transportation necessary to ensure oversight services throughout accomplishment of the construction period for the IFB. It is the responsibility of the Contractor to establish and maintain open communications between salient authorities associated with accomplishment of this contract.

3.11 Federal Holidays: Tyndall AFB observes the following Federal Legal Holidays;

3.12 Working Hours: The normal duty hours are from 7:00 AM to 5:00 PM, Monday thru Friday excluding Federal Holidays. The Contractor may submit a request to the CO for approval to perform work outside of normal duty hours, on weekends, and on holidays. This request shall be submitted no less than three (3) workdays prior to the time requested.

3.13 Availability and Use of Utility Services: The Government will make all reasonably required utilities available to the Contractor from existing outlets and supplies, as specified in the contract.

3.14 Sanitation: The Contractor shall provide and maintain within the construction area minimum field-type sanitary facilities approved by the CO. Government toilet facilities will not be available to Contractor's personnel.

3.15 Clean Up: At the end of each working day the Contractor shall clean up the work site by the removal of all construction debris, waste materials, packaging material and the like. If the before mentioned items could become airborne they shall be placed in refuse containers immediately. Clean up of the work site shall include the construction area(s), construction office area(s), material storage area (s), parking and eating area(s), and any other area(s) affected by the construction process. Any dirt or mud which is tracked on to any Base street, parking area, sidewalk, patio, or driveway shall be cleaned away daily. Materials resulting from demolition activities which are salvageable shall be stored within the work area or at a supplemental storage area. Stored material not in trailers, whether new or salvaged, shall be neatly stacked when stored in an approved location.

3.16 Contractor’s Management and Use of Electromagnetic Emission Devices on Base: (Electromagnetic emission devices include: Radio/Radar Transmitters; Navigational Aids/Instrumentations; Signaling, Intrusion Detecting, and Identification Devices; Mobile and Fixed Business Radio Communications Equipment; and MARS, CBs, and Amateur Radio Stations.) The Contractor shall comply with the Air Force Military Training Center (AFMTC) Regulation 700-14, Jul 89.

3.17 Preparation of Progress Schedules and Reports: The reports contemplated by Clause 52.236-15, Schedules for Construction Contracts, shall be accomplished on and in accordance with instructions pertaining to AF Form 3064, Contract Progress Schedule, and AF Form 3065, Contract Progress Report.

Contract Schedules and Reports shall run from Monday through Sunday and be submitted to the CO per Table of Deliverables.

4.0 GENERAL SAFETY REQUIREMENTS

4.1 Workplace: Provide a safe and healthful workplace for employees and the general public while performing the contract work.

4.2 Personal Protective Equipment: Supply each employee with personal protective equipment. This equipment includes but is not limited to protection for the eyes, face, head, ears and extremities. Use the proper protective equipment for the work performed.

4.3 Roads: Obey the posted base speed limits. Wear Seat belts on base in accordance with AFI 91-207.

4.4 Accidents: Report Contractor accidents involving injury to Air Force personnel or damage to Government property to the Base Law Enforcement Desk and the CO.

4.5 Safety Regulations: Familiarize yourself and comply with the general safety requirements in accordance with DOL regulations, OSHA 29 CFR Part 1926, as well as applicable Air Force Office of Safety and Health (AFOSH) standards and Air Force Regulations (AFR). These standards and AFRs are available in the Wing Ground Safety Office, Building 662.

4.6 Required:

4.7 OSHA Inspections: The Contractor shall be subject to no-notice inspections under OSHA program by inspectors of the Department of Labor.

4.8 Protection and Maintenance of Traffic: During construction the Contractor shall provide and maintain MOT equipment and personnel in accordance with FDOT standards to ensure the safety of pedestrians and vehicles. The Contractor shall maintain and protect traffic on all affected roads/pedestrian paths during the construction period except as otherwise specifically directed by the CO. Measures for the protection and diversion of traffic, including the provision of watchmen and flagmen, erection of barricades, placing of lights around and in front of equipment and the work, and the erection and maintenance of adequate warning, danger, and direction signs, shall be as required by the State and local authorities having jurisdiction. The traveling public shall be protected from damage to person and property. The Contractor's traffic on roads selected for hauling material to and from the site shall interfere as little as possible with public traffic. The Contractor shall investigate the adequacy of existing roads and the allowable load limit on these roads. The Contractor shall be responsible for the repair of any damage caused by construction operations.

4.9 Barricades: The Contractor shall erect and maintain temporary barricades to limit public access to hazardous areas as required by OSHA, Florida Department of Transportation, and/or like Agencies. Such barricades shall be required whenever safe public access to paved areas such as roads, parking areas, sidewalks or other work areas are prevented by construction activities or as otherwise necessary to ensure the safety of both pedestrian and vehicular traffic. The minimum required number of barricades shall be securely placed in a neat and orderly fashion, clearly visible with adequate illumination to provide sufficient visual warning of the hazard during both day and night. Barricades shall be maintained in an orderly fashion during the duration of their placement.

4.10 Hurricane Season: Normally from 1 June through 30 November is the premier season for hurricanes, Tropical Storms and Tropical Depressions for Tyndall AFB, FL and surrounding areas. In the event of a hurricane and at the direction of the Contracting Officer, the Contractor shall be responsible for:

4.10.1 Securing all equipment, materials and cleaning up the work area;

4.10.2 Continuing normal operations until directed otherwise by the Contracting Officer;

4.10.3 Evacuating the base as directed by the Contracting Officer;

4.10.4 Provide a POC to the Contracting Officer to recall the contractor after the danger is over;

4.10.5 The work site should be free of objects capable of becoming airborne and causing a hazard. For construction contracts, per FAR clauses 52.236-9 and 52.236-12, you are required to maintain your work and in a safe and secure condition at the completion of each work day with particular care during the hurricane season.

4.11 Severe Weather: Upon receipt by the CO of a severe weather warning with anticipated winds of 35 knots or higher, the following sequence of actions shall be carried out:

4.11.1 The CO or the Contracting Officer's Representative (COR) will notify the Contractor of the severe weather warning.

4.11.2 The CO or the COR will instruct the Contractor to secure all materials and equipment.

4.11.3 The Contractor shall take immediate action to tie down, remove, protect, and/or secure construction materials, equipment and refuse to the satisfaction of the Air Force Inspector in order to reasonably assure that Government property will not be damaged. If the Contractor fails or refuses to secure materials and equipment to the satisfaction of the Air Force Inspector, the work will be accomplished by Air Force personnel and the cost thereof charged to the Contractor.

4.12 Fire Regulations and Preventative Practices: The Contractor shall comply with fire prevention practices as set forth by the National Fire Protection Association (NFPA), other recognized fire prevention agencies, and base regulations.

4.13 Explosive Operated Hand Tools: The Contractor shall comply with OSHA Standard 1910.215(c) when utilizing explosive operated hand tools. Storage of explosive cartridges on the installation shall be in metal containers and limited to one days' supply. The Contractor shall provide adequate controls to prevent loss/theft of cartridges used and stored on the installation.

4.14 Welding: Welding shall not be performed without first obtaining a welding permit issued by the Base Fire Department.

5.0 GENERATED REFUSE

5.1 Refuse: The Contractor shall take positive action to prevent work-generated refuse from entering the sewer system and water bodies. All Contractor generated refuse and waste shall be hauled from the construction site daily to a disposal area located outside the physical boundaries of the installation. This refuse and waste material shall be hauled in with tight-fitting beds and covers to prevent spillage on roadways and waterways. The construction site shall be kept neat, orderly, and safe for workmen at all times.

5.1.1 All waste materials other than uncontaminated recyclable metals generated by any work under this contract shall become the property of the Contractor and shall be handled, transported, stored and disposed of off-base by the Contractor at the end of each day's work. Such material shall be hauled OFF BASE to a Contractor selected disposal area and disposed of in accordance with applicable Florida Administrative Code 62-701, federal, state and local laws, ordinances, regulations, court orders or other type of rules or rulings having the effect of law. The Contractor shall provide the CO with the waste manifest tickets for all disposal runs associated with the project.

5.1.2 Salvageable, goods shall be turned in to the appropriate base agency. The Contractor shall separate and turn in recyclable materials to the 325th Services Squadron Recycling Facility, building 6029, in accordance with base policies. Precious scrap and waste metals (e.g. steel, iron, etc.) shall be recycled and/or disposed in accordance with base policies.

6.0 UTILITY OUTAGES

6.1 Definition: Utility outages shall be defined as a stoppage in electric, gas, potable water, sewer, chilled or hot water, steam, telephone, Energy Monitoring and Control System (EMCS), or any other similar utility whether serving all or a part of a single facility or of several facilities.

6.2 Outage Coordination: All utility outages shall be coordinated with the Using Agency through the CO or the COR. Notice of the necessity for an outage shall be provided by the Contractor to the CO, in writing, not less than seven (7) working days prior to the time of the required outage and written approval for the outage will be issued by the Contracting Officer. All work shall be coordinated and arranged to ensure that the outage will be of minimum duration.

6.3 Utility Restoration: Once a utility outage is arranged and work begun, work shall continue until all utilities are restored to the affected facility or facilities.

6.4 Cancellation: In the event a scheduled utility outage is cancelled by the Government, notification will be given to the Contractor at least 24 hours in advance of the time for the outage to start. Rescheduling of a cancelled outage must be coordinated with written notification as described above.

7.0 EXISTING UTILITIES

7.1 Reference Drawings: Record drawings showing existing underground utilities are located in Building 421, Tyndall AFB. The Contractor shall avail himself of the drawings. Any utility line shown on the record drawings (or made known to the Contractor) and damaged during construction work shall be repaired immediately by the Contractor at no cost to the Government.

8.0 DELIVERABLES / SUBMITTALS

8.1 Construction Phasing Plan: The Contractor shall submit a Construction Phasing Plan a minimum of ten

(10) days in advance of mobilization. The plan should define in detail work phasing intent of the Contractor with dates and duration for each phase of work. Note that the government anticipates that the work will be primarily continuous after mobilization and that portions of the work may be in progress simultaneously. As an example, milling operations may be underway at one location while paving operations are occurring at a previously milled location. This plan will be used as the basis for base populous notification and planning.

8.2 Contract Progress Schedule (AF 3064): The Contractor shall submit a signed AF 3064 (or approved equivalent), Contract Progress Schedule, depicting an overall contract progress schedule for the main elements of work for the period of performance as described in the Table of Deliverables. The Contractor shall also provide a line graph depicting actual construction progress (solid line) versus scheduled construction progress (dotted line) throughout the period of performance. The progress schedule shall be signed by the either the COR or the government contract inspector recommending approval and approved by the CO. If the Contractor requires reimbursement of a high dollar value item, the item shall be identified on the AF 3065, an appropriate percentage applied, and the item must be received and stored on site.

8.3 Contract Progress Report (AF 3065): The Contractor shall submit AF 3065 as described in the Table of Deliverables. This report shall be signed by the Contractor’s on-site representative in the Remarks section of the AF 3065. The work elements and percentages of the total job identified on the AF 3065 shall be identical to the work elements and percentages on the AF 3064. The Contractor shall obtain either the COR or the Government contract inspector’s acceptance by signature of the actual reported progress prior to submission to the CO. Additionally, the Contractor shall submit the updated AF 3064 as an attachment to the AF 3065.

8.4 Work Clearance Request / Digging Permit: Obtain a blank AF Form 103, BASE CIVIL ENGINEERING WORK CLEARANCE REEQUEST (often called a digging permit) from 325 CES Customer Service, (850) 283-4949. Contractors shall request a digging permit at least ten (10) working days in advance.

8.5 Hazardous Material and Hazardous Waste: The Contractor will submit TAFB Form 81 (Contractor Questionnaire) and TAFB Form 82 (Chemical Inventory) in ten (10) days upon issuance of the notice to proceed and prior commencement of work on site. The Contractor should note that Tyndall AFB is required to report chemicals such as (but not limited to) compressed gases, adhesives, aerosol cans, sealants, paints, lubricants, solvents, oils, cleaners, degreasers and pesticides. Copies of manufacturer-specific Material Safety Data Sheets must be attached to TAFB Form 82. After submission, CEA will notify the Project Manager and/or CONS of the reportable chemicals and of any special instructions. The Contractor is required to submit TAFB Form 83 (Reporting Entry Form) at the end of the project or as indicated by CEA. CEA must be notified if anything changes from the original submittal (i.e. new chemical is added, size of container or unit of issue changes or if the manufacturer changes).

8.6 Transfer And Acceptance of Military Real Property (If Applicable): The Contractor shall prepare and submit a DD Form 1354, Transfer and Acceptance of Military Real Property document. The draft and final copies shall be submitted in accordance with the Table of Deliverables.

8.7 Pre-Final Inspection: The Contractor shall conduct a pre-final walk through inspection with Base personnel and publish the pre-final inspection findings in a pre-final inspection (punch list) report. The Contractor shall include a draft DD Form 1354, Transfer and Acceptance of Real Property to the COR for review.

8.8 Final Inspection: The Contractor shall conduct a final inspection with base personnel and publish the findings in a final inspection report. The inspection shall concentrate on the items identified at the pre-final inspection and recorded in the pre-final inspection (punch list) report. A final inspection shall not be performed until the pre-final inspection (punchlist) report has been resolved. At the final inspection, the Contractor shall present a completed DD Form 1354, Transfer and Acceptance of Real Property to the BCE or other appropriate organization for signature and acceptance, if required.

8.9 Delivery/Warranty: The Contractor shall complete all inspection and commissioning requirements prior to final inspection. Following final inspection, the Contractor shall deliver to the Government the warrant documentation. The warranty shall be issued in accordance with FAR 52.246-21.

8.10 Table of Deliverables: Project deliverables shall be as shown in the following table.

9.0 CONTRACTOR’S QUALITY CONTROL

9.1 Quality Control: The Contractor is responsible for quality control and shall establish and maintain an effective quality control system in compliance with FAR Clause 52.246-12, Inspection of Construction. The quality control system shall consist of plans, procedures, and organization necessary to manage all delivery orders to produce end products which comply with the contract requirements. The system shall cover all construction operations, both on site and off site, and shall be keyed to the proposed construction sequence. The Government will hold the project manager responsible for the quality of work on the job and is subject to removal by the (CO) for non-compliance with quality requirements specified in the contract.

9.2 Field Measurements: The Contractor shall be required to make his own field investigations to verify dimensions and other information shown on Government furnished reference drawings and final contract documents.

10.0 INSPECTION AND ACCEPTANCE

10.1 Provisions: Government inspections and final acceptance shall be scheduled by the Contractor as the work is completed with the following additional provisions:

a. The Contractor shall give the Government a minimum of three working days notice for all requested inspections.

b. The Contractor shall make sure that the work is ready for all inspections. The presence of an unreasonable number of deficiencies, as determined by the COR, may cause the inspection to be rescheduled.

c. The Contractor will be given a certificate of acceptance by the COR for each successfully completed step of the work. The certificate will be signed by the COR and shall be initialed and dated by the Contractor’s approved representative. One copy of the certificate will be given to the Contractor.

d. Contractor shall complete a CES approved Contractor Daily Report form and submit to COR…
