Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or sub factors. Failure to meet a requirement will result in an offer being ineligible for award. The offeror shall prepare submissions as set forth in the organizational table below. The titles and contents of the volumes are defined in the below table, all of which shall be within the required page limits and with the number of copies shown below.

b. The proposal shall be clear, concise, and shall include sufficient detail for effective evaluation and for substantiating the validity of stated claims. The proposal should not simply rephrase or restate the Government’s requirements but rather shall provide convincing rationale to address how the offeror intends to meet these requirements. Offerors shall assume that the Government has no prior knowledge of their capabilities and experience and will base their evaluation on the information provided in the offeror's proposal.

c. Page Limitations – Page limitations shall be treated as maximums. If exceeded, the excess pages will not be read nor considered in the evaluation of the proposal and (for paper copies) will be returned to the offeror as soon as practicable. Each page shall be counted except the following: cover page, table of contents, tabs, and glossaries.

d. Glossary of Abbreviations and Acronyms – Each volume shall contain a glossary of all abbreviations and acronyms used, with an explanation for each. Glossaries do not count against the page limitations for their respective volumes.

e. Page Size and Format – A page is defined as each face of a sheet of paper containing information. When both sides of a sheet display printed material, it shall be counted as two pages. Page size shall be 8.5 X 11 inches, not including foldouts. Pages shall be single spaced and text size shall be no less than 12 point. Use at least 1-inch margins on top and bottom and ¾ inch side margins. Pages shall be numbered sequentially by volume.

Foldout pages shall not be used for text. These limitations shall apply to both electronic and hard copy submissions.

f. Labeling – Submit each volume as separate files to permit rapid location of all portions, including sub factors, exhibits, annexes, and attachments if any. Electronic copies shall be submitted in a format readable by Microsoft Office.

g. Electronic Offers – The content and page size of electronic copies must be identical to the hard copies. For electronic copies, indicate on each the volume number and title. Use separate files to permit rapid location of all portions, including sub factors, exhibits, annexes, and attachments if any. Electronic copies shall be submitted in a format readable by Microsoft Office. As part of the provided electronic, offerors must provide the completed Excel document (not a scanned or copied) for evaluation.

4. General Format and Content

The offeror shall submit the following volumes of material. A complete proposal consists of the following volumes:

a. Volume I – SF 1449, Representations and Certifications, and any parts of the request for proposal with fill-in blanks and solicitation attachments as applicable, i.e. excel spreadsheets of Attachment 2, Bid Schedule.

Complete the Representations and Certifications IAW 52.212-3 and/or SAM website (https://www.sam.gov/portal/SAM/). Fill in blanks, when appropriate in any sections of the request for proposal. All pricing information shall be included in this volume.

b. Volume II – The technical proposal shall be prepared in accordance with these instructions and shall be evaluated in accordance with the evaluation criteria and evaluation standards in Addendum to 52.212-2.

Technical proposals shall be used by the evaluators to determine offeror's ability to perform the required services in the PWS. As part of an offerors Technical Approach, each Subfactor must be individually responded to. Responses must demonstrate a clear understanding of the Performance Work Statement (PWS) and provide specific evidence of compliance. Failure to address all Sub factors will result in that offeror being determined non-responsive and remove them from consideration.

c. Volume III – Past Performance.

3.1.3 The past performance evaluation results are an assessment of the offeror’s probability of meeting the solicitation requirements. The past performance evaluation considers each offeror’s demonstrated recent and relevant record of performance in supplying products and services that meet the contract’s requirements. In accordance with FAR 15.305(a)(2), the currency and relevance of the information, source of the information, context of the data, and general trends in contractor’s performance shall be considered. These are combined to establish one performance confidence assessment rating for each offeror.

3.1.3.1 There are three aspects to the past performance evaluation: recency, relevancy (including context of data), and quality (including general trends in contractor performance and source of information)

Table 1. Past Performance Rating Method

Adjectival Rating Description Acceptable Based on the offeror’s performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror’s performance record is unknown.

Unacceptable Based on the offeror’s performance record, the Government does not have a reasonable expectation that the offeror will be able to successfully perform the required effort.

http://www.sam.gov/portal/SAM/)

3.1.3.1.1 Recency. The first is to evaluate the recency of the offeror’s past performance. Recency is generally expressed as a time period during which past performance references are considered relevant and is critical to establishing the relevancy of past performance information. With appropriate parameters to assess past performance recency, descriptions and ratings of “acceptable” or “unacceptable” provided in Table 1, may be used if past performance is not a discriminating factor in the source selection. Each offeror shall submit past performance information for up to five (5) recent considered most relevant in demonstrating an ability to perform the proposed effort.

3.1.3.1.2 Relevance. The second is to determine how relevant a recent effort accomplished by the offeror is to the effort to be acquired through the source selection. The criteria to establish what prior performance is recent and relevant shall be unique to each source selection and shall be stated in the solicitation. In establishing what is relevant for the acquisition, consideration should be given to those aspects of an offeror’s history of contract (or subcontract) performance that would provide the most context and give the greatest ability to measure whether the offeror will successfully satisfy the current requirement. Common aspects of relevancy include, but are not limited to, the following: similarity of product/service/support, complexity, dollar value, contract type, use of key personnel (for services), and extent of subcontracting/teaming.

Table 2. Past Performance Relevancy Rating

Adjectival Rating Description Relevant Present/past performance effort involved essentially the same scope and magnitude of effort and complexities this solicitation requires. (i.e., Reference describes experience in fulfilling same market segment and describes in detail how closely the provided services performed under identified contracts relate to the Task Order Performance Work Statement).

Not Relevant Present/past performance effort involved little or none of the scope and magnitude of effort and complexities this solicitation requires. (i.e., Reference does not describe experience in fulfilling same market segment and/or provides no detail resembling how closely the provided services performed under those contracts relate to the Task Order Performance Work Statement).

3.1.3.1.3 Quality of Products or Services. The third aspect of the past performance evaluation is to establish the overall quality of the offeror’s past performance. The Government will consider the performance quality of recent, relevant efforts. For each recent, relevant past performance reference reviewed, a performance quality rating (PQR) will be assigned by the performance raters in accordance with the Performance Quality Ratings.

There are two levels of relevancy as shown in Table 2. When source selections require a greater level of discrimination within the past performance evaluation, the Source Selection Team (SST) shall use the relevancy ratings identified in Table 2. However, for those source selections requiring less discrimination in the past performance evaluation and with appropriate parameters to assess past performance relevancy, descriptions, and ratings of “acceptable” or “unacceptable” provided in Table 1, may be used. The Source Selection Plan (SSP) shall clearly identify the treatment of relevancy within the past performance evaluation. With respect to relevancy, past performance of greater relevancy will typically be a stronger predictor of future success and have more influence on the past performance confidence assessment than past performance of lesser relevancy.

Furnish the following information for each reference provided:

- Cage Code/UEI

- Product/Service

- Contracting Agency/Customer

- Contract Number

- Contract Dollar Value
