This is a combined synopsis/solicitation for commercial items prepared in accordance with the format inSubpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.

The Request for Quotation (RFQ) number FA481423TF001 shall be used to reference any written quote provided under this

RFQ.

This competition will be 100% Small Business set-aside.

The RFQ document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2015 – 4571 Revision No. 19 Effective 09 July 2022.

The North American Industry Classification System (NAICS) code for this project is 611710 with a size standard of $16.5M.

The purpose of this combined synopsis and solicitation is for the services of a Catholic Religious Education Coordinator (see attached Statement of Work). The bid schedule and contract CLIN schedule are below.

Quote Preparation and Submission The following will be submitted for a quote:

1. RFQ Cover Letter -

RFQ Cover Letter shall include an offer acceptance period good through 01 April 2023. Quotes offering less than this acceptance period will be deemed unacceptable. The RFQ Cover Letter shall include the company name, address, contract number (if applicable), cage code, UEI number, small business status/ categories, a specific primary and alternate point of contact (POC) to include phone numbers and email addresses and who can contractually obligate the company, a statement that the company understands the requirements specified and will meet the performance standards and requirements therein. The RFQ Cover Letter must also include all pricing information filled out on the CLIN schedule.

2. Sample Resume/CV

Sample Resume/CV must demonstrate the Offeror’s ability to understand the level of education/experience/certification as required to complete the services identified in the SOW. Sample Resume/CV submission is limited to 10 pages, Times New Roman 12 point font.

FAR Provision *52.212-2, Evaluation -- Commercial Items (Oct 14), applies to this acquisition and the following evaluation is applicable:

(i) technical capability of the services offered to meet the Government requirement in accordance with the

Statement of Work;

(ii) price;

Technical and price, when combined, are equally important.

(b) Options. The Government will evaluate quotes for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that a quote is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

(c) A written notice of award or acceptance of a quote, mailed or otherwise furnished to the successful vendor within the time for acceptance specified in the quote, shall result in a binding contract without further action by either party. Before the quote’s specified expiration time, the Government may accept a quote (or part of a quote), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

FAR Provision *52.212-3, Offeror Representations and Certifications -- Commercial Items (Jan 2021), with its Alternate I (Oct 14), applies to this acquisition. All vendors must be registered in System for Award Management at https://www.sam.gov/portal/public/SAM/ at the time of Quote submittal.

FAR clause at 52.212-4, Contract Terms and Conditions -- Commercial Items (Oct 2018), applies to this acquisition with the following Addendum: Paragraph (c) of this clause is tailored as follows: Changes in the terms and conditions of this contract may be made only by written agreement of the parties with the exception of administrative changes, such as changes in the paying office, appropriations data, etc., which may be changed unilaterally by the Government.

A vendor must be determined to be responsible by the Contracting Officer in order to be eligible for award.Responsibility is described in Federal Acquisition Regulation (FAR) Subpart 9.1, “Responsible Prospective Contractors.” Part of the determination addresses financial capability. Please provide the names, addresses and points of contact for all financial institution organizations utilized to determine if contractor has adequate financial resources to perform the contract or the ability to obtain them to include written authority to contact the provided financial institution.

The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

(i) price;

(ii) technical capability of the quote to meet the Government requirement.

BASIS FOR AWARD:

First, the government will rank all responsive offers by price. Then the Government will evaluate lowest priced Offeror for technical acceptability. The Government shall evaluate the offeror on an acceptable/unacceptable basis IAW Table A-1 (as provided below). Should the lowest priced Offeror be rated “Technically Unacceptable”, evaluation of the next lowest priced Offeror will continue, until technical acceptability can be determined for two quoted offers, at that point evaluations will cease without consideration of any other offers.

In order to be deemed technically acceptable, the following evaluation criteria must be met:

a. Offeror must submit a RFQ Cover Letter and Sample Resume/CV IAW the Quote Preparation and Submission section of this solicitation.

b. Sample Resume/CV must demonstrate the Offeror’s ability to understand the level of education/experience/ certification as required to complete the services identified in the SOW.

Discussions: The government intends to award a purchase order without discussions with respective vendors/offerors. The government however, reserves the right to conduct discussions if deemed in its best interest.

Note: The vendor acknowledges that should the quote terms and conditions and/or agreement conflict with mandatory provisions of the Federal Acquisition Regulation (FAR) and other Federal law applicable to commercial acquisitions, to the extent of such conflict the FAR and Federal law govern andconflicting vendor terms and conditions and/or agreement are unenforceable and are not considered incorporated into any resultant contract.

AFFARS CLAUSES

5352.201-9101Ombudsman - OMBUDSMAN (OCT 2019)

(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes.The ombudsman may refer the interested party to another official who can resolve the concern.

(b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsmandoes not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).

(c) If resolution cannot be made by the contracting officer, the interested party may contact the ombudsman, Mrs. Susan Madison, AFICC OL AMC, 510 POW/MIA, Scott AFB, IL 62225-5022, 618-229-0267, fax 618- 256-5724, email:

Concerns, issues, disagreements, and recommendations that cannotbe resolved at the Center/MAJCOM/DRU/HQ AFICC/AFISRA/SMC ombudsman levels, may be brought by the interested party for further consideration to the Air Force ombudsman, Associate Deputy Assistant Secretary(ADAS) (Contracting), SAF/AQC, 1060 Air Force Pentagon, Washington DC 20330-1060, phone number (571) 256-2395, facsimile number (571) 256-2431.

(d) The ombudsman has no authority to render a decision that binds the agency.

(e) Do not contact the ombudsman to request copies of the solicitation, verify offer due date, or clarify technical requirements. Such inquiries shall be directed to the contracting officer.

Air Force 5352.223-9001 Health and Safety on Government Installations Basic

As prescribed in 5323.9001, the contracting officer must insert AFFARS clause 5352.223-9001 in solicitationsand contracts, other than for construction, which require performance on a Government installation.

HEALTH AND SAFETY ON GOVERNMENT INSTALLATIONS (OCT 2019)

(a) In performing work under this contract on a Government installation, the contractor shall:

(1) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of thiscontract; and

(2) Take such additional immediate precautions as the contracting officer may reasonably require for health andsafety purposes

(b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health (AFOSH) Standards and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract.

(c) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract.

5352.242-9000 Contractor Access to Air Force Installations

As prescribed in AFFARS 5342.490-1, insert a clause substantially the same as the following clause in solicitations and contracts:

CONTRACTOR ACCESS TO AIR FORCE INSTALLATIONS (OCT 2019)

(a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished,contractor identification badges while visiting or performing work on the installation.

(b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following:

contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized tosign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver s license, current vehicle registration, valid vehicle insurance certificate, and [insert any additional requirementsto comply with local security procedures] to obtain a vehicle pass.

(c) During performance of the contract, the contractor shall be responsible for obtaining required identificationfor newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who nolonger requires access to the work site. If resolution cannot be made by the contracting officer, the interested party may contact the ombudsmen, Mrs. Susan Madison, AFICC OL AMC, 510 POW/MIA, Scott AFB, IL 62225-5022, 618-229-0267, fax 618-256-5724, email: susan.madison@us.af.mil. Concerns, issues, disagreements, and recommendations that cannot be resolved at the Center/MAJCOM/DRU/HQ AFICC/AFISRA/SMC ombudsman levels, may be brought by the interested party for further consideration to the Air Force ombudsman, Associate Deputy Assistant Secretary (ADAS) (Contracting), SAF/AQC, 1060 Air Force Pentagon, Washington DC 20330-1060, phone number (571) 256-2395, facsimile number (571) 256- 2431.

(d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractorshall comply with [insert any additional requirements to comply with AFI 31-101, Integrated Defense, and DODMAN5200.02_AFMAN 16-1405, Air Force Personnel Security Program] citing the appropriate paragraphs as applicable.

(e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees arereturned to the issuing office.

(f) Failure to comply with these requirements may result in withholding of final payment.
