Statement of Work (SOW) –Amended: 30 Nov 2020

1.0 SCOPE

1.1 To provide annual preventive maintenance for one (1) EATON 9355-20-30, VRLA Sealed Uninterruptable Power Supply (UPS) Serial No. BL141KXX06, and its related components inside the MacDill Command Post (building 54) located on MacDill Air Force, 7709 Hangar Loop Rd Tampa, FL 33621.

2.0 PREVENTIVE MAINTENANCE (PM)

2.1 On-Site maintenance by EATON Authorized Service provider will:

• Check inlet and electronic air filters for debris.

• Check electrical connections.

• Verify power draw from connected devices.

• Check status of batteries.

• Conduct system test to verify functionality for a minimum of 10 minutes.

3.0 TRAINING (CLIN 0002)

3.1 Provide on-site training, on the proper operation, of the EATON 9355-20-30, VRLA Sealed Uninterruptable Power Supply for up to four (4) government personnel.

Provide written Operating Procedures (1 copies). Demonstrate system test of the UPS from the within the Command Post utilizing imbedded user interface. Training will be conducted one time per (annual/option) contract period and during the first Maintenance on-site visit.

4.0 REPAIR SERVICES

4.1 The contractor will respond by phone to repair calls as soon as possible or within 2 hours of initial notification. Repairs include system failure, battery modules issues or anything that prevents the UPS from operating properly. Repairs will be charged separate from the Preventive Maintenance. Response time to on-site repairs will be within 8 hours unless otherwise directed by the POC.

5.0 SCHEDULE

5.1 Annual Maintenance will be performed once per annual contract period.

Maintenance, Repair and Training will be performed during standard working hours, between 0730-1630 hours, Monday-Friday, excluding holidays and down days.

6.0 DELIVERABLES

6.1 Provide documentation on the maintenance performed and any observations made during the maintenance. Provide the documentation in memo form (E-mail is acceptable) within seven (7) days after site visit to:

7.0 DISPOSAL OF MATERIAL

7.1 The contractor will be responsible for disposing of material in compliance with all applicable environmental regulations.

8.0 GOVERNMENT/CONTRACTOR FURNISHED PROPERTY AND SERVICE:

8.1 Contractor Furnished Parts and Materials: The contractor will provide a proposal to the site POC and the Command Post Leadership for the required parts and materials, identified as needing replacement or repair per annual maintenance visit.

8.2 Government Property Protection: The contractor will provide protection to government property to prevent damage during the period of time the property is under the control or possession of the contractor.

8.3 Government Property Damage: The contractor will record and report promptly to the Program Manager and designated Government Site POC, all available facts relating to each instance of accidental damage to government property or injury to either contractor or government personnel. The contractor will not perform any work on the damaged equipment/property until released by an authorized government representative.

If the government elects to conduct an investigation of the accident, the contractor will cooperate fully and assist the government personnel until the investigation is completed.

9.0 GENERAL INFORMATION:

9.1 Contractor Employee: The contractor will not employ persons for work on this contract if such employee is identified to the contractor by the Government Site POC as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population. Contractor personnel will meet site access requirements. The contractor personnel may be required to undergo a background investigation to perform their duties at restricted sites.

10.0 SECURITY REQUIREMENTS:

10.1 The Contractor: will follow all security procedures established by MacDill AFB Security Forces and MacDill AFB Command Post at all times.

10.2 Employee List: The contractor will, following award of the project, provide a list of each employee who will require access to the base, including the employees of all subcontractors. The list must include employee name (first, middle, last), DOB, driver’s license/identification card number or other government issued photo identification card, social security number issued in the same name as the photo identification card.

10.3 Vehicles: All contractors' vehicles must have current registration and proof of insurance; the vehicle operator must have a valid driver's license. Vehicles may not remain parked at or in the vicinity of the Visitor's Reception Facility, or at any MacDill Air Force Base gate for longer than 30 minutes. Vehicles left unattended for more than 30 minutes are subject to being towed at the owner's expense. ALL VEHICLES

ENTERING OR ON MACDILL AFB ARE SUBJECT TO INSPECTION BY

SECURITY FORCES PERSONNEL.

10.4 AF Form 75, Visitor/Vehicle Pass: The contractor will be responsible for the return of all AF Form 75’s issued to any employee working on the project to the 6 Contracting Squadron/LGCB after completion of the contract.

10.5 Recording Media: The Contractor will not allow cameras or recording devices to be on the site, nor photographs or recordings to be taken without the written permission from MacDill AFB Security Forces and/or the MacDill AFB Command Post. The contractor may request photographic support from the MacDill Command Post.

10.6 Cell Phones: The use of cell phones is prohibited within MacDill AFB Command Post facilities. Cell phone slots are provided on a first come first serve basis for anyone entering the facility; however, it is recommended to leave cell phones in the contractor’s vehicles.

10.7 Laptop computers, Notebook and handheld computers are not allowed in MacDill AFB Command Post without prior written/verbal authorization. Contractor must request authorization for such, 2 business days prior to entry to the MacDill AFB Command Post. To facilitate the process, the contractor should disable wireless and Bluetooth functions.

10.8 Violations: Violations of these security requirements will revoke any or all passes.

The Government reserves the right to deny access to any person, with no consequence to the contract.
