1.0 Description of Service. The contractor shall furnish all personnel, vehicles, tools, supplies and equipment required to operate and maintain the Wastewater Treatment Plant (WWTP) at Shaw Air Force Base (SAFB) located in Sumter County SC and the Wateree Recreation Facility located in Kershaw County SC, and to pump and haul wastewater for disposal from Wateree Recreation Facility (WRF) to SAFB. Operations and maintenance shall be in accordance with South Carolina Department of Health and Environmental Control (DHEC), National Pollutant Discharge Elimination System (NPDES) Permit Numbers SC0024970 and SC0044440, the Clean Water Act, the Pollution Control Act, the plant’s operation and maintenance (O & M) manuals and all applicable federal, state and local laws, regulations, standards, and commercial practices as described in the performance work statement (PWS) with the focus on safety and customer service. The contractor is encouraged and expected to use innovative approaches to efficiently and effectively accomplish contract requirements in a timely manner at reduced costs, and in a way, which fosters pride and ownership in the work, performed. Historical chemical usage is listed in Appendix A, Historical Data. All chemicals must be coordinated through Hazmart (building 231) for entry into Enterprise Environmental, Safety, and Occupational Health Management Information System (EESOH-MIS).

The Contactor shall have all permits and licenses required by the South Carolina Department of Health and Environmental Control (DHEC) to pump, haul and dispose of wastewater, septage, and wastewater sludge prior to contract award. All required permits and licenses shall be valid and current. The contractor shall travel to the Wateree Recreational Facility daily, 365 days per year to determine if waste must be pumped and hauled. Waste shall be pumped and hauled any time the level is above 50% inside the aeration chamber; the level shall not exceed 50% after pumping. Clarifier shall remain empty and shall be pumped out any time waste is present. In the event that a regulatory agency assesses a monetary fine against the Government for violations caused by the contractor, the contractor shall reimburse the Government for the amount of that fine.

1.1

SPECIFIC FUNCTIONS

1.1.1 OPERATE WASTEWATER TREATMENT PLANTS. The operation of the wastewater plants, associated equipment, and specific systems shall be conducted in a manner that is safe, effective and efficient; providing continuous, cost effective treatment of all wastewater delivered to the facility. Wastewater treatment plant and wastewater pumping system operations shall be maintained to provide continuous, cost effective, and efficient conveyance of all generated waste. Continuous flow shall be maintained to prevent the cause of nuisance odors and to prevent interruptions of service. All systems shall be maintained to minimize unacceptable service conditions including flooding conditions and pump station failures. The SAFB WWTP shall be manned 24 hours per day, 7 days per week, 365 days per year and will be in compliance with all federal, state and local laws and regulations and all relevant permits. The contractor shall comply with NPDES Permit No SC0024970, effective 1 June 2003, for Shaw Air Force Base and NPDES Permit No SC004440, effective 1 Nov 2005, for Wateree WWTP. Contractor shall comply with applicable operation and maintenance manuals and the Wastewater Best Management Practices Plan (BMP) for Shaw AFB. Shaw WWTP operation and maintenance manuals are located at each plant; the contractor is responsible for ensuring permit posting at each location.

1.1.2 OPERATE WASTEWATER TREATMENT LABORATORY. The laboratory will be maintained to meet South Carolina DHEC Certification. The contractor is responsible for the operation and maintenance of the wastewater treatment plant laboratory including all safety items, chemical storage and disposal, mixing of reagents, indicators, buffers, and other instruments required for analysis. Calibration of laboratory equipment is the responsibility of the contractor and shall be accomplished in accordance with the NPDES permit. The contractor shall maintain a log of the inspections, items inspected, deficiencies found and corrective actions taken.

1.1.3 SAMPLING AND LABORATORY ANALYSIS. The contractor shall provide sampling and laboratory analytical services for process control and to support regulatory operating requirements. Such sampling and testing procedures shall be accomplished in accordance with applicable operating permit requirements and conditions as directed by the contracting officer. The contractor shall keep a complete set of laboratory records for all laboratory tests to include; date and time of sampling, type of sample, name of sample, location of sample, test performed, test results, date and time of calibration, name of operator, and chain-of- custody documentation. Results will be recorded as indicated on the NPDES permit and submitted to 20 CES/CEIEC not later than the 10th of each month. Analytical result sheets will be submitted with monthly Discharge Monitoring Reports (DMR) to 20 CES/CEIEC. Copies of all testing records and associated correspondence shall be maintained on file and available for review by the Contracting Officer Representative (COR) upon request.

1.1.4 OPERATE AND MAINTAIN LIFT STATIONS. The contractor shall operate and maintain 21 lift stations including performing operational checks of each station on pumps and motors, wet wells, control panel, bubbler systems, and other related equipment. The contractor shall perform preventative and corrective maintenance on all lift stations. (See Appendix B, Lift Stations, for locations of lift stations.)

1.1.5 HOUSEKEEPING AND GROUNDS MAINTENANCE. The contractor is responsible for housekeeping and cleaning of the interior of the assigned facilities, equipment, and other components on a regular and recurring basis to prevent corrosion, health, safety, environmental, and other hazardous conditions. The facilities are to have a clean and neat appearance on the inside and outside and be free from health and safety hazards. The contractor shall perform grounds maintenance service on and around the immediate vicinity of the exterior of the facilities to ensure the grounds remain free of litter and trash. Cleaning and maintenance supplies and equipment are the responsibility of the contractor.

1.1.6 ENVIRONMENTAL COMPLIANCE. Safely handle, remove, transport, and dispose of hazardous/nonhazardous materials in accordance with federal, state and local regulations. Contractor must comply with all safety requirements and regulations when removing hazardous materials. The contractor is responsible for any permit or fees associated with the use of off-base disposal locations.

1.1.6.1 Waste oil. Waste oils shall be put into Central Accumulation Point (CAP) Contractor provided drums and will be picked up by the CAP contractor. The contractor is responsible for notifying the CAP contractor when drums are ready for pickup.

1.1.6.2 Sludge. The contractor is responsible for drawing digested sludge from holding tanks into a tank truck and transporting to the Pocotaligo Wastewater Treatment Facility. The contractor is responsible for coordinating disposal times. Drivers are responsible for properly completing a “Load Verification Form” for each load and entering the correct number of gallons of sludge to be dumped. The operator shall randomly verify the volume and /or sample the load content. Contractor must maintain and make available to the Government, a copy of the load verification form. Sludge generated at the Wateree Recreational Wastewater Treatment Plant shall be transported back to Shaw Air Force Base for complete digestion prior to transporting to the Pocotaligo Wastewater Treatment Facility for disposal.

1.1.6.3 Grit and Screenings. After drying, grit and screenings shall be put into contractor provided containers and disposed of at an appropriate landfill site as specified in the NPDES permit.

1.1.7 TEMPORARY AND BYPASS PROCEDURES. Temporary and by-pass services may be necessary to accomplish certain repairs and maintenance. Such temporary and by-pass services shall be coordinated with the contracting officer and COR. Services shall be accomplished using methods to avoid service interruptions, where possible, or to minimize system downtime where such interruptions are unavoidable. Notification of scheduled anticipated temporary by-pass shall be at least 15 days before the date of the by-pass.

1.1.8 EMERGENCY SERVICES. Upon Government notification, the contractor has 60 minutes to respond to emergency situations. Emergency services are defined as an unforeseen occurrence, a sudden and urgent occasion(s) for action. These situations require quick action and sound judgment. For purposes of this PWS, an emergency situation is defined as any condition that requires immediate action to eliminate life threatening hazards or other hazards representing risks of serious injury to personnel; or damage to Government property; to restore essential services; prevent violation of discharge permit or a violation of the permit.

1.1.9

PREVENTATIVE MAINTENANCE

1.1.9.1 Preventative Maintenance Inspection (PMI). The contractor shall maintain the Shaw AFB and Wateree Wastewater Treatment Plants by providing preventative maintenance on all system equipment as required by Unified Facilities Criteria (UFC) 3-240-03N, US Department of Defense Augmentation Handbook for Operation and Maintenance of Wastewater Treatment Systems. The contractor shall implement a preventive maintenance inspection program to help prevent and correct deficiencies with equipment items, minimizing breakdowns and service interruptions, extending component service life, and maximizing operating efficiency. This program should incorporate methods to monitor trends for normal, abnormal or unusual conditions that would indicate problems that could arise. The contractor’s PMI plan shall include inspections and services of lubrication, minor adjustment, and minor repair of components. A completed Preventive Maintenance Inspection (PMI) plan shall be submitted to the contracting officer (CO) for acceptance within thirty (30) calendar days after contract award. This plan shall include a 52 consecutive week schedule indicating specific maintenance/inspection items to be accomplished. At the end of each year, each equipment item within every system included in this contract will have been addressed. The PMI program is to be established based upon manufacturers' recommendations, handbooks, operating/service manuals, contractor expertise, and general engineering judgments. The submitted plan shall include equipment components to be inspected and maintained, inspection and maintenance techniques, inspection and maintenance frequencies, and reporting methodology. The contractor shall update schedules as necessary to reflect any changes in equipment inventory.

1.1.9.1.1 Reimbursable Costs Government reimbursable costs shall apply to repair or replacement to return a system or components to proper operating condition. The Government shall provide an annual allowance for reimbursable costs for the basic year and each additional option year. Government reimbursable costs shall apply to repair/replacement of materials, including labor.

1.1.9.1.2 Reimbursable Procurement and Billing. The contractor shall submit an estimate and RS Means cost to the COR for approval. The COR will validate cost by comparing the contractor’s estimate, the Government estimate and “RS Means” book value. The contractor shall invoice for reimbursable equipment/materials each month for each completed job order. The contractor shall maintain original delivery tickets, sales slips and other documents identifying items purchased under the cost reimbursement portion of the contract. All purchase documents will be made available for review by the COR.

1.1.9.2 Preventative Maintenance Inspection Records. The contractor will be responsible for maintaining preventative maintenance records for each item of equipment included within this contract. These records will include detailed descriptions of periodic maintenance performed, the schedule and completion dates and the person who performed the work. The contractor shall ensure the records are accurate and current. These records shall be made available to the COR for review during normal duty hours.

1.1.10 NOTICES OF VIOLATIONS (NOV). The contractor shall notify the COR and 20 CES/CEIEC (895-9978/983-9194) within 1 hour of non-compliance discharge. The contractor shall notify the Government (20 CES/CEIEC) within 1 business day of any NOV received from the state or other regulatory agency. The contractor shall prepare and submit to the Government within 2 business days, a written description of the non-compliance discharge. This information shall include a description of the discharge, the cause of noncompliance and the period of noncompliance including exact dates and times (or if not corrected, the anticipated time the noncompliance is expected to continue) and steps taken to reduce, eliminate and prevent the recurrence of the non-complying discharge. In the event that a regulatory agency assesses a monetary fine against the Government for violations caused by the contractor, the contractor shall reimburse the Government for the amount of that fine.

NOTE: Cross Connections to systems used for irrigation, cooling, or other purpose shall not be permitted. All repairs, maintenance, alterations, and extensions to water treatment facilities shall be designed to avoid cross connections. Constant care must be exercised to prevent contamination from infiltration, flooding, and back siphoning.

1.2

PLANS AND REPORTS.

1.2.1 Contractor is required to submit the following plans and/or reports prior to the contract start date.

1.2.4 Operation Reports. The contractor shall maintain a copy of all operating reports and make them available for review by the COR. A logbook containing daily operations information shall be maintained at the Shaw AFB facility and the Wateree Recreation Area. The logbook must be signed daily by a certified operator of the required grade as required by the permit.

2.0 SERVICE SUMMARY. The Service Summary (SS) captures the service requirements that can be measured and are considered to be high risk and critical operational requirements. The SS includes Performance Objectives that describe the desired end result or outcome the contractor shall achieve. The Performance Thresholds states the minimum acceptable level of the performance in terms of quality, timeliness and quantity in realistic and achievable standards. These thresholds are critical to mission success. The PWS paragraphs reference the details of the Performance Objective in the PWS. Each requirement includes a Government-determined performance objective. (Note that the Performance Objective is merely a reference point for Government personnel and does not relieve the contractor of any other contractual requirements.)

2.1.

QUALITY CONTROL. The contractor shall employ commercial quality control program/procedures to identify, prevent and ensure non-recurrence of defective services. Through implementation of the contractor’s quality control program/procedures, the Government (Shaw AFB) will receive quality services meeting the contract requirements. As a minimum, the contractor shall develop quality control procedures that address the areas identified in paragraph 2.0, Services Summary. The government Contracting Officer’s Representative (COR) must have a specific quality control manager to notify in case of deficient service or customer complaints. The contractor’s quality control plan shall be provided to the contracting officer for acceptance not later than the post award conference.

2.2.

QUALITY ASSURANCE. The Government will inspect and evaluate the Contractor’s performance to ensure services are received IAW requirements set forth in this PWS. The COR will periodically inspect by watching actual task performance, physically checking an attribute of the completed task, checking a management information report, investigating customer complaints, or otherwise inspecting the task or its result to determine whether or not the performance meets the standards contained in this PWS. The COR will document the results of the surveillance and this documentation will become the official Air Force (AF) record of the Contractor’s performance. The COR shall conduct, document, track, and analyze Contractor performance via monthly scheduled surveillances. Surveillance results will be used to develop metrics to track contractor’s overall compliance with contract terms and conditions. The surveillance results shall be maintained as past performance information and utilized to submit information into the Contractor Performance Assessment Report System (CPARS).

2.3.

GOVERNMENT REMEDIES. The Contracting Officer (CO) shall follow the requirements of FAR 52.212-4, Contract Terms and Conditions for Commercial Items, for contractor’s failure to correct nonconforming services.

2.4.

PERIODIC PROGRESS MEETINGS. The Contracting Officer, Contracting Officer’s Representative, and other Government personnel, as appropriate, will meet periodically with the contractor to review the contractor's performance. At these meetings the contracting officer will apprise the contractor of how to the government views the contractor's performance and the contractor will apprise the Government of problems, if any being, experienced. The contractor will also notify the contracting officer in writing, of any work being performed, if any, that the contractor considers over and above the requirements of the contract. Appropriate action shall be taken to resolve outstanding issues.

These meetings shall be held as needed, but not less than QUARTERLY.

The minutes of these meetings will be reduced to writing, signed by the Contracting Officer and any other signatures as deemed appropriate, and distributed to the functional area and the contractor. Should the contractor not concur with the minutes, the contractor will set out in writing to the contracting officer any areas of non-concurrence.

2.5. CONTRACTOR PERFORMANCE ASSESSMENT REPORTING SYSTEM (CPARS). The contractor shall designate representatives to whom the annual evaluations will be sent automatically and electronically. The name, title, e-mail address and phone number of the designated contractor representative shall be provided to the Contracting Officer who will, in turn, provide that information to the Activity CPARS Focal Point for authorization access. Any changes in designated contractor personnel shall be the sole responsibility of the contractor to inform the CO and the Activity CPARS Focal Point. The designated contractor representative has the authority to receive the Government evaluation, review, comment and, return evaluation to Assessing Official/CO within 30 calendar days.

30. GOVERNMENT-FURNISHED FACILITIES/SERVICES:

3.1 Government Furnished Services. The Government will furnish utilities to include, gas, electric, water, sewer, refuse collection and local telephone service.

3.2 Government Furnished Facilities. The Government will provide the following facilities. The contractor is advised that Government furnished facilities shall be used only in the performance of this contract.

Shaw AFB – Bldg 304, office, lab and rest room (1271 sq ft)

Bldg 305, storage building (215 sq ft)

Bldg 306, break room (614 sq ft)

Bldg 307, chlorination/dechlorination building Bldg 311, pump room (370 sq ft)

Bldg 312, lime silo (179 sq ft)

Reserved parking space for contractor vehicles is on the west side of bldg 304

Wateree Recreation Area – Bldg 200 WWTP and a covered shed used for protection from the elements (18 sq ft), located at the plant.

NOTE: (See Appendix D Aerial View of WWTP)

3.3 Government Furnished Equipment/Supplies. The COR will initially furnish all Plant Operation and Maintenance Manuals. The contractor must keep the manuals up to date. See Appendix C for Government furnished equipment/supply list. The contractor is advised that Government furnished equipment/supplies shall be used only in the performance of this contract.

3.4 Training. The Government will provide the following training to contractor personnel: The COR will coordinate dates and times of all training with the contractor.

4.0 GENERAL INFORMATION

4.1 Normal Hours of Operation. Normal hours of operation for Shaw AFB are Monday through Friday, 7:00 am through 5:00 pm EST. The Shaw AFB WWTP will be manned 24 hours per day, 7 days per week including holidays.

4.2.

SECURITY REQUIREMENTS FOR UNCLASSIFIED CONTRACTS

4.2.1. Listing Of Employees. The contractor shall maintain a current listing of employees. The list shall include the employee's name, social security number and type of investigation if contract work involves unescorted entry to Air Force restricted areas or other sensitive areas designated by the installation commander. The list shall be provided to the Program Manager and sponsoring agency’s Security Manager. An updated listing shall be provided when an employee's status or information changes.

4.2.2. Security Training. The contractor ensures all contractor employees receive initial and recurring security education training from the sponsoring agency’s security manager. Training must be conducted IAW DOD 5200.1-M, Information Security Program, Volumes 3, and AFI 31-401, "Information Security Program Management". Contractor personnel who work in Air Force controlled/restricted areas must be trained IAW AFI 31- 101, Integrated Defense.

4.2.3. Pass And Identification Items. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees.

4.2.4. Background Checks. All contractor employees shall submit to a criminal background check accomplished by Shaw Air Force Base Security Forces prior to the employee being issued base access credentials. Failure to submit requested Contractor Listing of Employees as indicated can result in denial or delay of issuing base access credentials. Processing criminal background checks takes, at a minimum, 7-10 days from time of submission.

4.2.5.

Federal Investigations Requirements. (If Applicable) Contractor employees requiring initial issuance of a Common Access Card (CAC), Access to Restricted Areas, Use of Government Automated Information Systems/Networks, Child Care positions, or designated Positions of Trust shall complete submission of a National Agency Check with Inquiries (NACI) to the Office of Personnel Management (OPM), or a DoD-determined equivalent investigation. When required, the government (20 FW Information Protection Office) will submit NACI investigations to OPM at no cost to the contractor.

4.2.6.

Retrieving Identification Media. The contractor shall retrieve all identification media from employees who depart for any reason before the contract expires; e.g. terminated for cause, retirement, etc.

4.2.7.

Traffic Laws. The contractor and its employees shall comply with base traffic regulations.

4.2.8.

Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately-owned vehicle while on Shaw AFB SC.

4.2.9.

For Official Use Only (FOUO). The contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

4.2.10.

Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor employees shall be briefed by their immediate supervisor upon initial on-base assignment.

4.2.11.

Physical Security. The contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.

4.2.12.

Controlled/Restricted Areas. (If Applicable) The contractor shall implement local base procedures for entry to Air Force controlled/restricted areas where contractor personnel will work. An AF Form 2586, Unescorted Entry Authorization Certificate, must be completed and signed by the sponsoring agencies Security Manager before a Restricted Area Badge will be issued. Contractor employees must have a completed National Agency Check with Written Inquires (NACI) investigation before receiving a Restricted Area Badge. Interim access can be granted IAW AFI 31-501, Personnel Security Program Management.

4.2.13.

Key Control. The contractor shall establish and implement methods of making sure all keys issued to the contractor by the Government are not lost or misplaced and are not used by unauthorized persons. The contractor shall not duplicate any keys issued by the government.

4.2.13.1. The contractor shall immediately report to the Contract Officer’s Representative or Program Manager any occurrences of lost or duplicated keys.

4.2.13.2. In the event keys, other than master keys, are lost or duplicated, the contractor may be required, upon written direction of the contracting officer, to re-key or replace the affected lock or locks without cost to the government. The government may, however, at its option, replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the government and the total cost deducted from the monthly payment due the contractor.

4.2. 13.3. The contractor shall prohibit the use of keys, issued by the government, by any persons other than the contractor’s employees and the opening of locked areas by contractor employees to permit entrance of persons other than contractor employees engaged in performance of contract work requirements in those areas.

4.2.14 Photography Restrictions. No one is authorized to take any pictures or video recordings of Air Force operational aircraft or restricted areas without the approval of 20 FW/CVN regardless of location while on Shaw AFB. This includes, but is not limited to, personal electronic devices such as blackberries, iPADs, iPODs, cell phones, or any other devices with digital recording capabilities.

4.2.14.1. Any contractor requiring pictures or videotape of Shaw’s airfield to support their contract must request and possess written authorization prior entering the airfield area.

4.2.14.2. Written authorization letters to photograph or video of the flightline must be signed by the Maintenance Squadron Commander; a Fighter Squadron Top 3, Director of Operations, or Commander; Logistics Group Commander or Deputy; the Operations Group Commander or Deputy; 20 Operations Support Squadron Commander or Director of Operations or 20 FW/CVN.

4.3 CONTRACTOR PERSONNEL.

4.3.1 Employees. The contractor shall not employ persons for work on this contract if such employees are identified to the contractor, by the contracting officer, as a potential threat to health, safety, security, general well being, or operational mission of the installation and its population.

4.3.2 Appearance. Contractor personnel shall present a neat appearance and be easily recognized. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges that contain the company and employee name.

4.3.3 Military Personnel. The contractor is cautioned that off duty active military personnel hired under this contract may be subject to permanent change of station (PCS), change in duty hours or deployment. Military Reservist and National Guard members may be subject to recall to active duty. The abrupt absence of these personnel could adversely affect the contractor’s ability to perform. Their absence at any time shall not constitute an excuse for nonperformance under this contract.

1. Lime is received in bulk tankers and loaded into the silo; we used 36 tons of bulk lime in 2012.

Lime is also received in bags (500 lbs/month).

2. Chlorine bleach (12.5% solution). Note: the chlorine demand increases each summer during hot weather. Chlorine usage increases to during the months of June, July & August. In 2012 we used 12000 gallons of 12.5% liquid chlorine bleach.

3. Sodium Bisulfite is received in 35-gallon drums. During 2012 we use fifty-eight drums which is 2030 gallons.

4. In 2012 we used 5000 Lbs of Calcium Hypochlorite (HTH).
