MEMORANDUM FOR RECORD

SUBJECT: Site Visit (SV) Meeting Minutes of 49 Comptroller Squadron (CPTS) Carpet Cleaning (Solicitation FA480123Q0409)
1. The site visit began at 0915 on 3 February 2023 at Bldg 29, Holloman AFB Visitor Center.
The listing of attendees is attached.
2. Personnel went to the following locations at the times listed in the table below:

3. The site visit ended at 10:15 AM at Holloman AFB Visitor Center, Bldg 29.
4. During the site visit the following conditions were discovered and additional information uncovered. This information was verified with the technical expert and is now provided here in an official format. Any information discussed at or during the site visit and no stated below is not a requirement or standard the government will require. Additionally, any information provided below which aligned with information discussed at the site visit was not official nor a requirement until the posting of this document.
• All chemicals used must not be banned nor used in an illegal way according to the laws of New Mexico or the federal government.
• There are four utility sinks with a standard faucet that are available as a water source. Additionally on the north-west side of the building there is a spicket with a sillcock control mechanism which may provide water. These four utility sinks can be used for draining liquid waste only, with no undissolved solids. All solids must be disposed of by the contractor off base.
• Vehicles may unload on the north-west side of the building, but vehicles may not park there. All vehicles must park on the south-east side of the building. Off loading may be accomplished against the sidewalk on the south-east side of the building, providing a ramp like surface for wheeled equipment. No loading dock is available on site and the contractor must have the capability to unload and load equipment on their own.
• All work must be completed by 30 March 2023 and must only be done on Saturdays and Sundays. Offerors must include a tentative schedule for suggested days and work hours subject to government technical acceptability.
• Carpet condition must be returned to state that allows foot traffic during work week without creating slip or fall hazard nor affect the final cleaned condition.
• Contractors must clean carpeted areas in hallways up to the closed doors, however closed and locked doors are inaccessible and cleaning process must not create waste on other side of closed doors.
• Some photos of the carpets are included below. Except for conference room with blue carpet, all other carpet not photographed is of same or similar condition and style. Any rugs that are not permanently affixed to the floor are not required to be cleaned and may be moved by either the government or the contractor.
• The government will move all major furniture to allow the contractor access to the flooring. Except for minor items such as wheeled chairs, small trashcans or similar, any furniture left in place is not the responsibility of the contractor to move nor clean, however the contractor will clean all carpeting exposed around the furniture.
• Outlets will be available for fans to expedite drying of carpet if desired by the contractor.
• A passenger elevator is available for transporting equipment to the second floor however, it has a rated weight limit of two thousand pounds. Contractor use will not exceed the rated weight.
• Additional photos are attached. Floor plan with detailed locations to be cleaned will be posted COB 8 February 2023 as an amendment.
