52.212-1 ADDENDUM

Instructions to Quoters

1. SUMMARY: This is a request for quotation (RFQ). Responses to this solicitation are considered quotes and not offers. In those instances where the words offer, offeror, and proposal are used, the reader should understand that the use of such words does not change the nature of this RFQ. The resulting award will be for a firm fixed-priced, multiple year purchase order contract for Laboratory Testing Services for the 452d Aerospace Medicine Squadron (AMDS) at March Air Reserve Base (MARB), California.

2. SUBMISSION OF QUOTES: Quotes must be submitted electronically to the email listed Faxed quotes will below no later than 15 August 2023 / 9:00 AM Pacific Daylight Time (PDT).

3. CONTRACT AUTHORITY: The Government will solicit and award this contract using Simplified Acquisition Procedures, in accordance with FAR parts 12 and 13. As a result of this authority, certain requirements for soliciting, awarding, and notifying Submitters are streamlined. In accordance with FAR 12.102(c), when a policy in another part is inconsistent with FAR Part 12, Part 12 shall take precedence.

4. SYSTEM FOR AWARD MANAGEMENT (SAM): Submitters' registration must be active. For information and registration go to www.sam.gov.

5. PERIOD OF ACCEPTANCE OF SUBMITTERS: The pricing terms and conditions of the quotes should be valid for 120 days.

6. COMPLETE QUOTE: The Government will award a firm fixed-priced, multiple year purchase order contract resulting from this solicitation to the responsible Submitter whose quote conforming to the solicitation represents the Lowest Price Technically Acceptable. Please read the PWS carefully! This RFQ is for Laboratory Testing Services for the 452d Aerospace Medicine Squadron (AMDS). A complete quote must be received for consideration. A complete quotation consists of

(a) - (e) as follows:

(a) Certify Submission: Signed Solicitation.

(b) Price: Completion of all CLIN(s) Pricing Sheet on this SF 1449 Solicitation and pricing estimate worksheet.

(c) Solicitation Provisions - Completion of all solicitation provisions, as required. If information is "None" or "Not Applicable" then so state.

(d) Acknowledgment of all amendment(s). (Fill out Block 8 and fill out and sign Blocks 15A, 15B and 15C.)

(e) Technical Acceptability-- See 52.212-2 Addendum (Evaluation Factors)

Incomplete quotes may not be considered. The Submitter agrees to hold the quote firm for 120 calendar days from the date specified for receipt of quotes.

7. QUESTIONS: All questions shall be submitted by email no later than 8 August 2023 at 9:00 AM Pacific Daylight Time (PDT) to: Amy.Crain.1@us.af.mil No phone questions will be .

accepted. The deadline is necessary to ensure timely award and the Government may, in its sole discretion, choose not to respond to questions received after the deadline. All questions will be reviewed and responded to by an amendment to the solicitation. It is recommended that Submitters often check www.sam.gov for updates.

Evaluation Factors for Award

Additional Quote Evaluation Information -- Addenda to 52.212-2 Evaluation - Commercial Products and Commercial Services:

A. This requirement is currently being procured as an unrestricted competition.

B. The Government intends to make an award without further discussions. Discussions, if necessary, will be conducted in accordance with FAR 52.212-1(g). Award will be made to the Submitter when the quotation conforms to all required terms and conditions, submits all required representations and certifications, meets all requirements set forth in the RFQ, and provides a fair and reasonably quoted Total Evaluated Price (TEP) to the Government based on the results of the evaluation as described in the paragraphs below.

Evaluation Process:

All quotes will be placed in price order from lowest to highest based on the Total Evaluated Price (TEP). The lowest ranked TEP Submitter's technical quote will then be evaluated and receive a rating of Acceptable or Unacceptable. To be rated as acceptable, the technical quote must meet the Government's minimum requirements. Quotations deemed Unacceptable are those that fail to meet the Government's minimum requirements.

If the lowest priced Submitter's technical quote is rated acceptable, the evaluation process will stop at this point as that quote represents the best value to the Government. Award shall be made to that Submitter without further consideration of any other quotes. If the lowest ranked TEP Submitter's technical quote is assigned an unacceptable rating or if the price is determined not fair and reasonable and poses an unacceptable risk to the Government, or if the quote is found to be non-responsible, the next lowest placed TEP quote will be evaluated. This process will continue (in order of ascending TEP) until a Submitter is evaluated to have an acceptable technical rating, fair and reasonable price, and the Submitter is deemed responsible or until all quotes have been evaluated.

Evaluation Factors: The following factors shall be used to evaluate quotes:

Factor 1: Technical Capability. Each Submitter's technical quote shall be evaluated as Acceptable or Unacceptable as defined below:

Technical Acceptable/Unacceptable Ratings

Rating Description

Acceptable Quote clearly meets the minimum requirements of the solicitation.

Unacceptable Quote does not clearly meet the minimum requirements of the solicitation.

To be rated Technically Acceptable, the Submitter must provide a technically acceptable quote which clearly meets the requirements of the Performance Work Statement (PWS) objectives.

When evaluating the Submitter's technical quote, the Government will determine if the Submitter has met the following minimum requirements:

See Section 3.5 Quality Control for a Quality Control Plan (QCP) in the PWS.

Factor 2: Price: The Submitter's price quote will be evaluated to determine price completeness and reasonableness.

(a) Award will be made to the Submitter whose quote represents the best value to the Government, based on a Lowest Price Technically Acceptable (LPTA) basis between price and non-price factors - Technical Capability.

(b) Apparent mathematical errors. Overriding formulas within the Government provided pricing estimate worksheet may result in calculating errors and cause the price quotation to be inaccurate. It is highly encouraged that Submitters ensure that all pricing is entered correctly prior to submission to the Contracting Office. Quotes cannot be altered after submission.
