INSTRUCTIONS TO OFFERORS

L-1 MAGNITUDE OF CONSTRUCTION PROJECT:

Maximum: The total aggregate maximum amount of all contracts awarded under this Multiple Award Construct

Contract, with all options exercised, shall not exceed $50 Million.

The Magnitude of Seed Projects are as follows:

L-2 DISPOSITION OF UNSUCCESSFUL PROPOSALS:

In compliance with FAR Subpart 4.8 the Government will retain one copy of all unsuccessful proposals. Extra copies of unsuccessful will be destroyed by the Government. No destruction certificate will be provided.

L-3 PREPARATION OF PROPOSALS:

General Instructions: This section of the Request for Proposal (RFP) provides general guidance for preparing proposals as well as specific instructions on the format of the proposal. The Offeror's proposal must include all data and information requested in the RFP.

ONLY THE REQUIRED MINIMUM AMOUNT OF INFORMATION IS REQUESTED TO PROVIDE FOR PROPER

EVALUATION. EFFORTS SHOULD BE MADE TO KEEP OFFERS AS BRIEF AS POSSIBLE, CONCENTRATING ON

SUBSTANTIVE INFORMATION ESSENTIAL FOR PROPER EVALUATION. The proposal shall be clear, concise, and shall include sufficient detail for effective evaluation and for substantiating its validity. Elaborate format and color representations are not desirable.

Restrictions: Incomplete proposals: Failure to submit all the data indicated and required in this section may be cause for determining a proposal incomplete and, therefore, not considered for technical evaluation, or award.

Who May Submit: (1) Proposals may be submitted by: Service-Connected Disabled Veteran-Owned Small

Businesses, HUBZONE firms or 8(a) firms as certified by the SBA. A single design firm or construction contractor may offer more than one proposal by entering into more than one such association. For the purpose of this solicitation, no distinction is made between formally organized design/build entities and project-specific design/build associations. Both are referred to as the design/build offeror, (or simply "offeror"), or the design/build contractor, (or simply "Contractor"), after award of a contract. ONLY LEGALLY BINDING JOINT

VENTURES, AS APPROVED BY THE SMALL BUSINESS ADMINISTRATION WILL BE

CONSIDERED.

(2) Any legally organized offeror may submit a proposal, provided that the offeror, or offeror's subcontractor, has on its permanent staff professional architects and engineers registered in the appropriate technical disciplines and provided that the requirements specified in the solicitation are met. All designs must be accomplished under the direct supervision of appropriately licensed professionals.

Preparation Instructions: Proposals shall be prepared in seven volumes; Specific guidance regarding content of these volumes is discussed below.

(1) Construction of Volumes: Each volume shall not require extensive searching throughout the document(s) to locate and evaluate any items or area. Sub-categories of information in each of the volumes should be highlighted for ease of evaluating the information contained therein. All proposals shall contain the evaluation requirements stated herein. All written information and data shall be in an 8 ½” x 11” format and shall be provided in a standard

3–ring binder (maximum 3-inches). Every binder shall contain: Table of Contents, List of Tables or Figures (if required), and List of Appendices. Font size shall be not less than 12 point. Each page shall be identified with the appropriate page number centered at the bottom of the page. Sheet size of the proposal contents shall be 8 ½ by 11 inches. 11 by 17 inch sheets will be allowed for charts and tables. Legibility, clarity, coherence, and the contents are important. Contents shall follow the order of the evaluation criteria and pages shall be numbered. The offeror shall not submit verbatim sections or attachments of this solicitation as part of their proposal. Only written proposals will be accepted. Oral, electronic, and facsimile proposals will not be accepted. Offers that do not meet these requirements may be subject to rejection.

a. A cover sheet identifying the offeror and the project shall be provided and must include the following:

(1) Solicitation number.

(2) Name, address, e-mail, and telephone and facsimile numbers of the Offeror.

(3) Names, titles, e-mail, and telephone and facsimile numbers of persons authorized to negotiate on the

Offeror‟s behalf with the Government in connection with this solicitation.

(4) Name, title, and signature of the person authorized to sign the proposal.

(5) Acknowledgement of all amendments to the solicitation (if applicable).

NOTE: THIS IS THE ONLY PLACE THAT SHALL HAVE ANY IDENTIFYING FACTOR OF THE

OFFERORS COMPANY/ OR LOGO. NO OTHER PAGE SHALL CONTAIN THIS INFORMATION.

The terms identifying factor/ or logo, mean a company logo or company name imposed into the background of the pages of the proposal. This is only authorized on the Cover Page of Volume 1. The offeror may use their company name as text on all pages of all “Original Volumes” of the proposal. However, the

Government ask that all four copies of the offerors proposal shall omit or blackout any reference to the offeror’s company name for evaluation purposes.

b. Table of Contents. The proposal shall contain a detailed Table of Contents. The complete Table of Contents shall be included in each binder used.

c. Materials submitted but not required by this solicitation (such as company brochures) shall be relegated to appendices and may not be reviewed.

Firms submitting proposals shall limit submission to data essential for evaluation of proposals so that a minimum of time and monies are expended in preparing information required by the RFP. Data submitted must reflect the designer's interpretation of criteria contained in the RFP. Unnecessarily elaborate or voluminous brochures or other presentations, beyond those sufficient to present a complete and effective response, are not desired.

Elaborate artwork, expensive paper and bindings, and expensive/extensive visual and other presentation aids are unnecessary.

Technical proposals will be evaluated for conformance with the minimum RFP criteria. While the intent is to keep the pre-award design effort to a minimum, proposals must provide adequate detail for effective evaluation. It must also form sufficient basis for a fair and reasonable price proposal.

Proposal Clarity and Organization: Proposals shall be clear, concise and organized, while presenting all required information. No required material shall be incorporated by reference. A proposal that does not conform to these requirements may be considered non-responsive.

Revisions: At the request or approval of the Contracting Officer, proposal revisions for written portions of the proposal, including catalog cuts and specifications, shall be submitted as page replacements with revised text readily identifiable, e.g. bold face print or underlined. The source of the revision, e.g. Error, Omission, or Clarification

(EOC), amendment or other Contractor-initiated change, shall also be indicated for each revision. Revised pages shall be numbered, dated, submitted in same number of copies as the original proposal submittal, and a different color page than the original.

Electronic Material: One electronic copy on CD-ROM of Volume I and Volume II is to be submitted with proposal. All textual material, catalog cuts, and other non-drawing material shall be in Adobe Acrobat Portable

Document Format (.pdf), arranged in the same order as the hard copy version with each section or part book marked.

All drawings shall be formatted in accordance with Appendix A1 – Special Design Considerations for Dyess AFB, Abilene, TX .

Restrictions Incomplete proposals - Failure to submit all the data indicated in this section may be cause for determining a proposal incomplete and, therefore, not considered for further technical evaluation or for subsequent award.

(2) Binding: All volumes shall be submitted in three pronged binders (maximum ring size is 3-inches).

Text: Text shall be single spaced and printed black on white paper (black and white requirement does not apply to graphics, photos, etc.). Paper shall be 8 ½ x 11.

VOLUME PROPOSAL FORMAT

All proposals shall be submitted in seven parts, or volumes.

Volume I: The first volume shall be submitted in a single 3-ring binder labeled as Volume I with tabs separating the factors shown on the table below. The original plus 4 copies of Volume I shall be submitted. VOLUME I

SHALL NOT EXCEED MORE THAN 20 SINGLE-SIDED OR 10 DOUBLE-SIDED PAGES, EXCLUSIVE OF

THE COVER SHEET, TABLE OF CONTENTS, AND APPENDICES. COMPANY NAME AND/OR LOGO IS

ONLY AUTHORIZED ON THE COVER PAGE.

Volume II: The second volume shall contain the original plus 4 copies of the offeror‟s past performance data from

Federal, State and Local Government and private industry contracts from the past three (3) years; not to exceed the 9 most recent projects.

Volume „s III, IV, V, VI: (See information in paragraph below).

The third, fourth, fifth, and sixth volumes (designs for seed projects), except for drawings, shall be submitted in four

3-ring binders labeled as Volume III, Volume IV, Volume V, and Volume VI as shown on the table below.

Drawings shall include 4 full size sets. Each drawing shall be identified with the appropriate Sequence and Sheet

Numbers in the lower right hand corner. Firms are encouraged to provide INFORMATIVE DRAWING NOTES to convey important features of their design. Drawing information shall present basic concepts, arrangements, and layouts. Arrangements, layout plans, and notes may be combined together on single sheets in order to simplify presentation, so long as clarity is maintained. The original plus 4 copies of Volume III, Volume IV, Volume V, and Volume VI shall be submitted.

Volume VII: The seventh volume shall include Seed Project pricing as described in section L-7 of this solicitation.

The original plus 4 copies shall be submitted.

REFERENCED PUBLICATIONS

Unified Facilities Criteria (UFC) and Unified Facilities Guide Specifications (UFGS) can be downloaded from the

Whole Building Design Guide at the following address: http://www.wbdg.org.

Obtaining other referenced publications such as Federal and Military specifications, Military Standards, and industry standards (i.e., ASTM, ANSI, ACI, NFPA, building codes) will be the responsibility of each offeror.

MAILING REQUIREMENT FOR SPECIAL MARKING OF PROPOSAL DATA

Envelopes or other cover for material submitted in response to this RFP must be presented so that they may be easily identified. At a minimum, the outside cover must show:

VOLUME NUMBER

Submit the proposal in the format specified. Oral, faxed or telephonic proposals or modifications will not be considered. Mail or deliver the proposal to the address listed in the Solicitation.

Personnel, Organization, PMP (Volume I): At a minimum, the submission shall be prepared in a form consistent with the evaluation criteria for award set forth in Section M of this solicitation. All information the Offeror deems appropriate to address these criteria should be included in the proposal in an orderly format following the evaluation criteria. To facilitate evaluation, Volume I should be specific, detailed, and clearly demonstrate that the Offeror has a thorough understanding of the requirements for the accomplishment of this effort. Statements that the Offeror understands, can, or will comply with the specifications, and phrases such as "standard procedures will be employed", or "well known techniques will be used" etc., will be considered unacceptable. Offerors should note that data previously submitted under any prior or existing contract or solicitation will not be considered. Therefore, such data should not be relied upon, nor incorporated in the Technical proposal by reference.

PERSONNEL – SUBMITTAL REQUIREMENTS (VOLUME I)

PERSONNEL consist of three Subfactors: Cover Letters, Organizational Charts/Ladder Diagram and the Program

Management Plan.

(a) COVER LETTER

The cover letter will be used to ensure the Offeror acknowledges solicitation and amendments.

(b) ORGANIZATION CHARTS

Provide Offeror‟s proposed onsite (Dyess AFB) organization chart and offsite, i.e. Home Office organization chart with supporting data presenting interrelationships, lines of authority, etc. Present how

Offeror plans to execute the “seed” project as well as staff up for possible future projects.

(c) PROJECT MANAGEMENT PLAN (PMP)

The Offeror shall provide a comprehensive Project Management Plan (PMP) developed specifically for implementation of this Contract. The PMP shall discuss the management approach used for design, site clearing and demolition, construction, completing/turn-over of project. The information in the PMP shall make it clear that the Offeror has the ability to deliver a quality product and effectively manage the designers, consultants, and subcontractors on the team, as well as the ability to coordinate all work throughout the design and construction phases. The PMP shall include an explanation of the total project team management approach for both the design team and the construction team.

It shall include:

1. Management Approach

2. Sub-Contractor Management

3. Quality Control Procedures

4. Schedule development and adherence (Phased Turn-Over)

5. Organization Chart

6. Acquisition of Environmental Permits

7. Safety

8. Preparation and submission of record (i.e. as-built) documents

9. Contract closeout

10. The team's computer-aided drafting and design (CADD) capabilities

Include a one page maximum executive summary of each the design quality control plan and the construction quality control plan. Complete quality control plans for design and construction shall be submitted to Contracting Officer for approval after award.

L-5 PREPARATION OF PAST PERFORMANCE (VOLUME 2):

a. Past performance (Volume II): When addressing past performance evaluation criteria set forth in Section M of the solicitation, the Offeror shall provide, as a minimum, a list of their past performance data from Federal, State and

Local Government and private industry contracts from the past three (3) years; not to exceed the 9 most recent projects. All the items identified below shall be addressed in the listing. Elaborate listings and extensive comments are not required or requested.

(1) Contract number, type;

(2) Whether the offeror performed as the prime or a subcontractor;

(3) Name of contracting office and verified name, address, phone/fax number of point(s) of contact

(4) Project Title and/or brief description of construction project.

(5) Initial price and final or current price, Contract start date, scheduled completion date, and current or final completion date.

(6) Percent complete as of the date of submission of the proposal; and

(7) Brief comments, if desired.

Should Offerors wish to provide information on problems encountered on the identified contracts and their corrective actions in accordance with FAR 15.305(2), they shall submit such information at the same time references are submitted.

(8) On the above list, the Offeror shall identify recent past performance consisting of multi-tasked construction projects of varying complexity, performed within the past three (3) years. No more than the nine (9) most recent of these projects will be considered. Uncompleted projects with work-in-progress may be included, but shall not exceed three (3) of the nine (9) questionaires. Relevant projects are defined as a broad range of maintenance, repair, alteration or new construction of real property showing the capability to perform and manage a wide scope of construction trades including, but not limited to, design, carpentry, road repair, roofing, excavation, interior and exterior electrical, heating, ventilation, air-conditioning, plumbing, sheet metal, painting, demolition, concrete masonry, and welding. Each project exceeding $500,000.00. In selecting the references, Offerors should consider the recency and relevancy of the past performance information to the work required under this solicitation.

(9) The Offeror shall then email or fax Attachment # 2, Past Performance Questionnaire, to those agencies/firms responsible for the solicitation and administration of those identified projects. Prior to forwarding the questionnaire to each reference, complete Section A, Contractor Information, on the questionnaire. Your references shall complete the questionnaire and email it to Mr. Anthony Rogiano at anthony.rogiano@dyess.af.mil or fax it to 325-696-8676. Offerors are responsible to ensure that their reference sources receive, complete, and return the questionnaires on time. The evaluation may take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontract work where your firm performed major or critical aspects of the requirement when such information is relevant to the instant acquisition.

However, the proposal should clearly define who accomplished the work if performed by other than the prospective contractor (e.g., a key personnel) and define the role (e.g., prime contractor, subcontractor, subtier subcontractor, etc.) performed by the Offeror. The government shall consider the information provided from the Offeror and may well consider any other source when evaluating the Offeror‟s past performance.

L-6 PREPARATION OF SEED PROJECTS (VOLUME’S III, IV, V, AND VI):

“Seed” Project Concept Designs: Provide concept design (Drawings, specification outlines, table of contents, catalog cut sheets, manufacturer/supplier recommendations, schematics, process flow diagrams, supporting data, etc.) as required to clearly present contractors‟s proposed design concept to comply with the government‟s requirements and permit offeror to establish his price for the seed project. The Offeror shall provide his proposed project schedule for design, site clearing and demolition, submittals, and construction work for the seed project.

Prepare in the form of a time-scaled (bar chart) summary network diagram and graphically indicate sequences proposed to accomplish each general work operation including design and design reviews, site clearing and demolition, construction, final clean-up of premises, appropriate interdependencies among various activities. The proposed completion time will be a contract requirement

The Offeror shall provide a verification statement that the Contractor has read the contract requirements and that the number of days includes all design time, Government review time (2 weeks) of all design submittals, construction time, and demolition time necessary to complete the project. The duration shall reflect the design and design review requirements.

Provide design (Drawings, specification outline, table of contents, catalog cut sheets, manufacturer/supplier recommendations, schematics, process flow diagrams, supporting data, etc.) as required to clearly present contractors‟s proposed design concept to comply with the government‟s requirements for the “seed” project‟s and permit offeror to establish his price.

As applicable to the “seed project’s”, address the following:

(1) Design Narrative. If applicable, provide a description of how the desired architectural character is achieved through the use of form, scale, and proportion. Indicate how materials are used to add pattern and definition to the design. Discuss the material, material quality, equipment proposed, the energy efficiency of the materials and equipment, energy-saving features, and each of the design disciplines‟(architectural, civil, structural, mechanical, electrical, etc.) systems and the rational for the selected systems, such as:

a) Structural system and the rationale behind the selection of the proposed system, including identification of major structural materials and systems.

b) Plumbing system and the rationale behind the selection of the proposed fixtures and equippment

Heating, Ventilation and Air Conditioning system and rationale behind the selection of the proposed system.

c) Electrical system and the rationale behind the selection of the Interior power distribution systems and the rationale behind the selection of the proposed system, fixtures, and equipment. Identify electrical characteristics of power supply (phase, voltage, KVA). Provide description of panels, protection devices and typical loading of circuits. Identify type of wire. Discuss communication systems (telephone, cable

TV, etc.) and the rationale behind the selection of each system.

d) Discuss maintenance and accessibility considerations in the selection and layout of the mechanical and electrical systems.

(2) Design Drawings: (AS APPLICABLE)

(3) Architectural Floor Plans – (Scale 1/8” = 1‟-0”). Provide overall dimensions, room description, room dimensions and areas. Identify types of materials and show all columns, partitions, openings, and roof overhangs.

(4) Exterior Elevations - (Scale 1/8” = 1‟-0”). Provide drawings to present design intent, identify exterior materials, and show proposed detailing. Show all sides. Indicate door and window sizes and configurations, porches, decks, and exterior steps. Provide major horizontal and vertical dimensions.

(5) Typical Interior Elevations – (Scale 1/4” = 1‟-0”).

(6) Wall Section - (Scale 3/4” = 1‟-0”). Provide a typical wall section showing foundation, wall composition, and floor and roof systems. Identify materials, finishes, thermal insulation, vapor protection, and wall fire ratings.

(7) Finish and Equipment Schedules - Provide interior finish schedule for all rooms and door and window schedules. Include ceiling heights on the interior finish schedule.

(8) Schematics/process flow diagrams

SITE DESIGN: Provide site design (Drawings, specification outline or table of contents, catalog cut sheets, manufacturer/supplier recommendations, schematics, process flow diagrams, supporting data, etc.) as required to clearly present contractors‟s proposed design concept to comply with the government‟s requirements for the “seed” project and permit offeror to establish his price. As applicable to the “seed project”, address the following:

Site Analysis Narrative. Discuss the existing site conditions, proposed grading, paving, erosion control, storm drainage, utilities, and power distribution system and the rationale behind the design and selection of proposed systems, materials, and equipment. Discuss the electrical service to the site, as applicable to the project. Also address environmental conditions, prevailing winds, solar effect, and the relationship of the site to the surrounding environment.

B) SUPPORTING DATA:

Provide supporting data design (Drawings, specification outline or table of contents, catalog cut sheets, manufacturer/supplier recommendations, schematics, process flow diagrams, supporting data, etc.) as required to clearly present contractors‟s proposed design concept to comply with the government‟s requirements for the “seed” project and permit offeror to establish his price. As applicable to the “seed project”, address the following:

Product Literature. The Offeror shall provide product literature indicating Construction Materials, Products, Equipment, and Systems to indicate specific make and model of the proposed materials, products, equipment, and systems. In addition, the Offeror shall provide product literature for all items listed in this attached form including manufacturer‟s descriptive literature, technical data, performance charts and curves, catalog cuts, etc. The technical data proposed shall meet the specific seed project requirements. Organize the product literature in accordance with

CSI Master Format and provide a table of contents.

L-7 PREPARATION OF PRICE PROPOSAL (VOLUME VII):

a. VOLUME VII must set forth all pricing information necessary to provide a meaningful basis for the

Government's analysis and evaluation of price and technical acceptance as set forth in Section M of this solicitation.

The Offeror‟s are to provide detailed cost estimates for all 4 SEED projects. Additionally, for the 2 electrical projects (FNWZ12-0023 Primary Overhead Conductors and FNWZ10-0038 Provide Air Break Switches) the

Offeror shall submit a detailed bill of materials (BOM) along with material submittals (to include catalog cuts/fact sheets). A 100% design would no longer be required. The Government IS NOT requesting certified cost or pricing information. While price will not be rated and color-coded, price will be evaluated in terms of completeness and reasonableness. Volume VII shall contain the following:

1. Section A. Complete in its entirety the "Offeror" portion of the Standard Form 1442 (Page 2). An official having the authority to bind your company contractually must sign and date the SF 1442. One copy of the SF

1442 must bear an original, authorized signature.

Complete ALL required certifications and representations and include in this area of Volume VII.

2. Section B. The Government reserves the right to request additional pricing information after receipt and evaluation of proposals. Contractors shall submit a proposal for all four of the following seed projects. If contractors fail to submit proposals for all four, they will be deemed nonresponsive.

Attachment 5. The project is titled as FNWZ 11-0020, Construct Handicap Ramp, Bldg. 7215. The specifications, drawings, and DOL wage determinations are all included.

Attachment 6. The project is titled as FNWZ 11-0053, Repair Hanger Door Feeder Duct, Hanger 5110. The specifications, drawings, and DOL wage determinations are all included.

Attachment 7. The project is titled as FNWZ 12-0023, Repair Primary Overhead (Reconductor). The Statement of

Objectives (SOO), drawings, and DOL wage determinations are all included.

DEFINITION OF DESIGN COSTS

Design costs are limited by law to not exceed 6% of the construction cost. “Design” means defining the construction requirement (including the functional relationships and technical systems to be used such as architectural, environmental, structural, electrical, mechanical, and fire protection), producing the technical specifications and drawings.

DEFINITION OF NON-DESIGN COSTS

For the purpose of this proposal, the following items are to be considered as non-design tasks.

The following is a list of items which are not considered to be part of preparation of designs, plans, and specifications.

(1) Initial site visits: This is for work needed to gather information from the site/installation to begin the design.

(2) Field and topographic surveys, property, boundary, utility and right-of-way survey.

(3) Subsurface explorations and borings: soils and materials testing and resultant report.

(4) Preparation or verification of as-built drawings: drawings of existing facilities (record drawings) are maintained by the Government. These may be obsolete or inadequate at the beginning of a project involving rehabilitation.renovation of the facili ty. The effort to verify existing conditions and update exisiting drawings is not subject to the 6% design limitation.

(5) The services of consultants, where not specifically applied to the preparation of working drawings or specifications.

(6) Title II services: These services done during the construction phase of a projects, such as; review of shop drawing submittals, construction surveillance, estimates for construction modifications, etc.

(7) Models, renderings, or photographs of completed designs.

(8) Reproduction of designs for review purposes: Most projects are designed in stages such as completing a percentage of the project and then pausing to review the work completed. Copies of design data and drawings are reviewed and considered before proceeding. Costs for reproduction and distribution of the review documents are not part of the statutory consideration.

(9) Travel and per diem allowances in connection with excludable services: All travel and per diem costs will be excluded from the 6% stautory limitation.

(10) Studies, testing, and recommendations for pavement or foundation designs.

(11) Color Boards with materials samples.

(12) Work required in preparing for and obtaining permit applications.

(13) Labor and computer costs for life cycle energy analyses.

(14) Administration costs for a Small Business Subcontracting Plan.
