STATEMENT OF WORK
REPLACE FLOORING
IN BUILDING 610
MALMSTROM AFB, MT
1. GENERAL
1.1. Summary of work: The contractor shall provide all labor, equipment, tools, accessories, and materials (carpet and related items shall be purchased from mandatory Tier I contracts listed in paragraph 2.1) to properly remove existing and install approximately 396 square yards of carpet tile to include 1135 linear feet of 4” cove base, using the direct glue down method of installation in building 610, on Malmstrom AFB, MT. Additionally, the contractor will be responsible for moving all furniture, in phases, to areas in the building that are not being worked on, as well return the furniture after the carpet installation is complete to it's respective area. See attached room inventory for a complete list of all furniture.
1.2 Anti-Terrorism Awareness Training: In accordance with DoDI O-2000.16v1_AFI 31-145-O, Antiterrorism (AT) Program Implementation, ensure Level I - AT Awareness Training is completed by all contractors within 30 days of requiring access, and annually thereafter. The training will be provided by the government. The objective of the training is to provide personnel with requisite knowledge necessary to remain vigilant and enable employment of AT tactics, techniques, and procedures.
1.3 Force Protection: Contractor personnel are required to abide by all real-world Force Protection Condition (FPCON) measures, random anti-terrorism measures (RAMs), shelter-in-place orders, evacuations and lock-down notices as directed. Examples include mandatory badge or ID checks, facility single points-of-entry and searches of hand carried items.
2. MATERIALS
2.1. The contractor shall provide all material to complete the installation in accordance with standard commercial practices. The contractor shall purchase the carpet materials by following the guidelines of the Air Force Installation Contracting Agency (AFICA) Carpet Ordering Guide Program. A listing of approved dealers previously established under the Tier I contracts as follows: (FA8003-23-C-0003, FA8003-23-C-0004, FA8003-23-C-0005, FA8003-23-C-0006, FA8003-23-C-0007, FA8003-23-C-0008, and FA8003-23-C-0009).
The carpet shall be purchased only from the pre-approved Tier I Contracts as in Attachment 2 of this SOW. The contractor is required to provide proof that the carpet was purchased from one of the pre-approved Tier I contracts. Cove base shall be 4” rubber of a complementary color (color must be approved by Facility Manager prior to purchase) in all areas.
3. EXECUTION
3.1. The performance period will be 90 calendar days after order to proceed. Working hours will be from 7:00 AM to 4:30 PM, Monday through Friday.
3.2. The contractor shall coordinate installation with the Office Manager (Anne Meyer 406-731-3558). This facility will be in use during installation.
3.3. The contractor shall install the flooring as recommended by the manufacturer, using the direct glue down method. The contractor shall provide all materials required to clean, patch, level, and prepare substrate for the new flooring. Flooring shall be installed in accordance with attached floor plan.
The contractor’s performance must meet or exceed The Carpet and Rug Institute’s Standard for Installation of Commercial Carpet, CRI 104-2002. Installation must be performed in strict compliance with all local, state, and federal regulations. The Occupational Safety and Health administration Hazard Communication Standard must be followed.
The contractor shall move all occupying furniture to accommodate the installation of the carpet with coordination with the Office Manager Ms. Anne Meyer (see sec 3.2), as well as move it back in place once the installation is complete.
3.4. The government will inspect and accept the areas as the contractor completes them.
If any areas are found to be unacceptable when inspected by the government, the contractor shall re-perform until the newly installed area meets standard commercial practices.
3.5. Cove Base, Edge Guards and Transition Strips. In all area’s base will be replaced with 4” rubber. The contractor shall provide and install aluminum bend-down type edge guard and rubber /vinyl transition strips for direct glue down carpet where required in all other areas.
3.6 The contractor shall provide 48 carpet tiles for attic stock to the Facility Manager, for future repairs.
4. CLEAN UP
4.1. Clean up of the job site shall be the responsibility of the installation contractor. The contractor will perform a final vacuum on all carpeted surfaces when completed.
4.2 Waste generated by the Contractor in performance of the services specified with this SOW shall be disposed of by contractor off base in compliance with State and Local ordinances.

The following list of contractors are the pre-approved carpet vendors for the purchase of carpet product in accordance with the Tier I Contracts awarded by the Air Force Installation Contracting Agency (AFICA). All carpet Products must be referred to and purchased from one to the seven vendors below. These contracts have a locked in and not-to-exceed (NTE) pricing for carpet and is hereby mandated to be the only source options for purchase of carpet products used a Malmstrom AFB, Montana.
