PERFORMANCE WORK STATEMENT

STERILIZER EQUIPMENT PREVENTATIVE MAINTENANCE

341st MEDICAL GROUP

MALMSTROM AIR FORCE BASE, MONTANA

26 NOVEMBER 2018

SUMMARY OF CHANGES

1 DESCRIPTION OF SERVICE.

1.1 The contractor shall provide all personnel, equipment, tools, materials, vehicles, supervision and services necessary to perform periodic, scheduled preventive maintenance, calibration, and support on the following equipment:

Equipment is located within the 341st Medical Group (341 MDG), Building 2040, Malmstrom Air Force Base, Montana.

2 SCHEDULE.

2.1 The contractor will perform calibrations for each sterilizer and washer annually in accordance with (IAW) currently established Defense Medical Logistics Standard Support (DMLSS) maintenance schedules.

2.2 The contractor will perform preventative maintenance, to include all parts and labor, IAW the manufacturer’s technical literature for the washer one time annually.

2.3 The contractor will perform preventative maintenance, to include all parts and labor, IAW the manufacturer’s technical literature for the sterilizers four (4) times annually - a quarterly basis.

2.4 Biomedical Equipment Technician (BMET) Office. Scheduling will be coordinated through the BMET Office. All performance shall occur during regular business working hours. The contractor will provide two (2) weeks’ notice to the BMET Office prior to propose service dates by calling 406-731-7375.

2.5 Base Access. Access to Malmstrom Air Force Base (MAFB) will require an Entry

Authorization List (EAL) to be submitted for each employee for a security clearance. The contractor shall submit an updated EAL to the 341st Contracting Squadron (341 CONS) no later than 30 days in advance of any performance start date.

2.6 In the event rescheduling is necessary; the contractor shall give the BMET Office 48 hours’ notice.

3 SERVICE SUMMARY.

3.1 The contractor service requirements are summarized into performance objectives that relate directly to mission items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. The performance thresholds are critical to mission success and calculated annually within each period of performance.

4 GOVERNMENT FURNISHED PROPERTY AND SERVICES.

4.1 The MDG will provide the following equipment, supplies, and services. The use of

Government furnished property and services is restricted to official use as required by this agreement. 341 MDG will:

4.2 Maintain the equipment site in a clean and sanitary condition and ensure the equipment is cleaned and decontaminated after contact with blood or other potentially infectious material.

4.3 Provide necessary housekeeping and cleaning services to maintain the equipment free from dirt, refuse, contamination, or waste of any kind.

4.4 Maintain the equipment site and environment (including temperature and humidity control, incoming power quality, and fire protection system) in a condition suitable for operation of the equipment.

4.5 Operate the equipment in accordance with the Operator's Guide. Make normal operator adjustments to the Equipment as specified in the Operator's Guide.

4.6 Provide access and escort to the required equipment in order for the service technician to satisfactorily complete the tasks required according to this SOW.

4.7 Provide Facility Management support to shut down internal systems or utilities that are connected to the equipment, as required.

4.8 Allow access to and use of the following administrative support equipment and services: copy machines and Class "A" telephone lines for local or toll-free calls.

5 SPECIFIC TASKS.

5.1 The contractor shall provide all personnel, equipment, tools, parts, transportation, related administrative support, and any other items and services, including but not limited to travel, per diem, necessary to perform the services required by this contract.

5.2 The contractor shall ensure that all service technicians are fully qualified and trained to service the equipment covered under this contract.

5.3 The service technician(s) shall check-in and check-out with the 341 MDG BMET prior to performing any services within the facility and at the completion of services prior to departing the facility.

5.4 The contractor shall prepare an Existing Deficiency Report listing all deficiencies noted during the inspection. The Report shall be provided to the BMET within ten (10) business days after the inspection.

5.5 If deemed appropriate by the Government, the contractor shall correct all deficiencies noted in the Report. The Government may decide that not all deficiencies need immediate repair. The contractor shall invoice for any deficiency repairs separately from the cost of maintenance services under the contract.

5.6 The contractor shall perform preventative maintenance, annual calibrations, and intervening repair services on all the equipment listed within para. 1 herein.

5.7 The equipment listed above shall be maintained and calibrated in conformance with the manufacturer's drawings and specification, as well as in compliance with all current federal and nationally accepted standards.

5.8 The contractor shall inspect and document certifications, annually or as required by the manufacturer, of the equipment and provide a copy of the certifications within ten (10) working days of the inspection, as required, to 341 MDG BMET.

5.9 The contractor shall respond and have a service technician on-site within four (4) hours of a call from an authorized 341 MDG individual for emergency services, unless otherwise agreed upon by both parties. Emergency services shall be provided only during 341 MDG's normal hours of operation. If insufficient time remains in the business day, the service technician shall respond at the start of the next business day.

6 GENERAL INFORMATION.

6.1 The Government shall inspect and evaluate the contractor’s performance to ensure services are received IAW requirements set forth in this contract. The BMET Office will evaluate the contractor’s performance through 100% surveillance inspections and investigating customer complaints. Results of the surveillance inspections become the official Air Force record of the contractor’s performance. When a performance threshold has not been met the BMET Office will initiate and provide 341 CONS a Contract Discrepancy Report (CDR), DD Form

2772, for issuance to the contractor. The contractor shall respond to the CDR IAW instructions provided therein and return it to 341 CONS.

6.2 The Point of Contact and service technicians must be able to read, write, speak, and understand English fluently.

6.3 The contractor shall not employ any person who is an employee of the United States

Government if the employment of that person would create a conflict of interest. The contractor shall not employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with DoD Directive 5500.7 and Air Force policy.

6.4 When requested by the contractor, the Government will provide Anti-Terrorism Awareness

Training, AT level-1, awareness training in accordance with AFI 10-245. The objective of the training is to provide personnel with requisite knowledge necessary to remain vigilant for possible terrorist actions and enable employment of AT tactics, techniques, and procedures. AT awareness training will be provided by an authorized instructor.

7 HOURS OF OPERATION.

7.1 The contractor shall provide services during 341 MDG's normal business hours, which are

Monday through Friday, 7:30 AM to 4:30 PM MST, excluding Federal holidays and designated base down days. A base down day is defined as a day authorized by the 341st Missile Wing Commander or higher authority in which organizations on Malmstrom Air Force Base are closed. 341 MDG participates in a base down day if approved by the 341 MDG Commander.

7.2 Federal Holidays. The contractor shall not provide routine services on the following holidays recognized by Malmstrom AFB.

7.3 Work scheduled but not accomplished because of base closure due to weather, exercises, or actual alert, will be coordinated with the BMET Office and accomplished as soon as possible after reopening the base.

8 ENVIRONMENTAL CONTROLS.

8.1 The contractor shall be knowledgeable of and comply with all applicable Federal, State, Interstate, and local laws and permits, regulations, and requirements regarding environmental protection. This includes Air Force regulations, operating procedures and permit conditions. Contractor operations are subject to environmental compliance inspections without notice. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply as such laws come into effect.

8.2 The contractor shall take all necessary precautions to protect Government and private property. Damage caused by the contractor shall be repaired to the satisfaction of the Contracting Officer or designated representative without additional cost to the Government.

8.3 The waste water shall be disposed of through drains that have grease traps or grease interceptors to capture the grease. Disposing of water through the floor drain, in most facilities, is an acceptable method of disposal. The contractor shall verify with the FM.

Contractor shall ensure the site is returned to the same conditions it was in prior to the start of work.

8.4 The contractor shall leave the work area in a clean and safe condition and in the same condition the facility was in prior to cleaning hoods. The contractor shall dispose of all waste and materials that was accumulated at their own expense and IAW federal and state law.

8.5 Hazardous Material Identification. SDSs are required as specified in the latest version of

Federal Standard No. 313 (including revisions adopted during the term of the contract) for any other material designated by a government technical representative as potentially hazardous and requiring safety controls. SDSs must be submitted by the Contractor upon contract award. Failure to provide SDSs or certificate when requested could result in the Contractor being considered non-responsive and result in termination of the contract.

8.6 Hazard. The Government (HAZMART personnel) will inventory all chemicals that the contractor brings on to Malmstrom AFB. Any products that meet the criteria of “Hazardous Material” must be bar coded and tracked until permanently removed from Government property. The inventory will be performed prior to commencement of work. Criteria for identifying hazardous material, is contained in 261 AFI 32-7086.

8.7 Spill Response. The contractor will be briefed on Malmstrom AFB spill response procedures at the pre-performance conference. The contractor is responsible to report and promptly cleanup all spills in a manner consistent with current environmental regulations, in the event that it is necessary to utilize Government material, equipment or personnel to clean up a contractor caused spill, the contractor shall be required to reimburse the Government for all associated costs.

8.8 Hazardous Material/ Waste Management. The contractor will be briefed on Malmstrom

AFB Hazardous Material / Waste Management Plan at the pre-performance conference.

8.9 Hazardous Material Handling. The contractor shall have approval from the base

Environmental section prior to purchasing, handling, using, and storing any chemicals, solvents, lubricants, and other products that require MSDSs. The contractor shall identify these materials and products on Enterprise Environmental Safety and Occupational Health- Management Information System (EESOH-MIS) generated Air Force Form 3952 Chemical/Hazardous Material Authorization Request Form. The contractor shall provide one copy of the MSDSs for each item to Environmental section for review prior to any chemicals being brought onto Malmstrom AFB. The contractor shall maintain one copy of the MSDSs for each hazardous material line item used within the work center.

8.10 The contractor shall not use products that are or contain Extremely Hazardous

Substances (EHS), Ozone Depleting Substances (ODS), and/or Persistent Bio accumulative and Toxic (PBT) chemicals. Any HM containing one of these banned substances will not be allowed on base.

8.11 Environmental. The contractor shall appoint a primary and alternate HAZMAT and

Hazardous Waste monitor. Monitors are responsible for training all contractor personnel regarding hazardous material containers maintained within Malmstrom AFB and MSDSs immediately upon receipt of new chemicals, products, or materials. The contractor shall submit MSDSs to HAZMART as required.
