PERFORMANCE WORK STATEMENT

FOD/LITTER PATROL

WHITEMAN AIR FORCE BASE, MO

3 OCTOBER 2019

1. DESCRIPTION OF SERVICES

1.1. The work covered by this project consists of the Contractor furnishing all labor, supervision, quality control, and services to perform all operations with providing Foreign Object & Debris (FOD)/Litter Patrol services at various locations thru out Whiteman Air Force Base (WAFB), MO.

1.2. Servicing Requirements: FOD Litter Patrol will include the following as a minimum:

1.3. Collect and dispose of FOD/Litter/Refuse and fallen tree branches (up to 4 feet/ 48 inches in length) in Government provided containers. An example of FOD/Litter/Refuse is cigarette butts, paper, cans, bottles, boxes, and general trash. The Contractor shall collect and dispose of 100% of FOD/Litter in the Government provided bins.

This requirement must be at 100% compliance, with no trash bags present at the time of surveillance. Failure to comply will reflect negatively in rating of performance.

1.4. In performance of this contract the contractor will provide their own vehicle that is capable of navigating to all identified locations on Whiteman Air Force Base. If weather is inclement or conditions require; a four wheel drive/ low speed vehicle may be necessary to execute all required tasks IAW this Statement of Work. All costs associated with the operations and maintenance of the equipment will be borne by the contractor.

1.5. All operators of vehicles shall be properly licensed and insured. Vehicles shall also be registered and insured.

2. STATEMENT OF WORK

2.1. Weekly Service: Provide FOD/Litter Patrol services in the designated locations identified on the map provided. The Contractor should render services within the hours to emulate the Contracting Officer Representative.

Services should be rendered at minimum three times a week between the days of Monday through Friday and the hours of 0700 and 1600. Excused absence has to be obtained in writing from the Contracting Officer.

2.2. Contractor Availability: The Contractor is expected to perform services as requested in paragraph 1.2.1. If Contractor cannot perform on a specific day, notification to COR and Contracting Officer should be made within ten

(10) business days or as soon as knowledge of absence is known. This request will assist in ensuring that any change of personnel will be able to be processed and added to the Entry Authorized List (EAL) in advance. Failure to coordinate for known absence will result negatively in review of performance. The Contractor must also respond to any request of services within 48 business hours of notification.

2.3. Location of Services: FOD/Litter Patrol areas shall be specified by the Government provided map. The locations are the Base Common areas, office facilities, and industrial areas. Example locations are business facilities and playground/park areas, running tracks, along roadways and parking lots. Patrolling areas can be coordinated with the COR and a base route map will be provided by the Government.

4. GOVERNMENT FURNISHED PROPERTY AND SERVICES

4.1. The Government will provide reasonable services and utilities for the contractor to perform the required tasks IAW the SOW.

4.2. The Government will provide bins in which the Contractor will be allow to disposal of FOD.

4.3. Toilet Facilities: Use of existing toilet facilities in the immediate work area only will be permitted. Contractor personnel will ensure facility cleanliness is maintained at all times.

4.4. Security Police and Fire Protection: The Government will provide general on-base security police and fire protection services. Security Police phone number is 687-3800 for emergencies and for routine calls. Fire Protection emergency phone is 911.

4.5. Telephone: The Government shall not provide telephone services. If available, contractor may use Government phones in case of emergencies or during field and site work. No long distant telephone calls will be authorized.

5. HOURS OF OPERATION

5.1. The contractor shall perform the services required under this contract during the following hours: 0700 – 1600 hrs, unless approved by the Contracting Officer.

6. FEDERAL HOLIDAYS

6.1. The following is a list of Federal Holidays observed by this installation:

New Year’s Day, 1 January or the Friday preceding or Monday following:

Martin Luther King’ Birthday, 3rd Monday in January, President’s Day, 3rd Monday in February, Memorial Day, Last Monday in May, Independence Day, 4th of July or the Friday preceding or Monday following

Labor Day, First Monday in September, Columbus Day, 2nd Monday in October, Veterans Day, 11 November or the Friday preceding or Monday following

Thanksgiving Day, 4th Thursday in November, Christmas Day, 25 December or the Friday preceding or Monday following.

7. BILLING

7.1. Invoices/Service Tickets/Logs: Invoices shall detail date of service, building number and name of location(s) serviced, cost of service at each building, and total cost. Service tickets/logs at a minimum the tickets/logs shall reflect the date, time, and type of service (approx. number of gallons removed) that was performed with a comments section that allows for description of services provided and notation of any repairs that the unit being serviced may require. A copy of the tickets/logs shall be sent to Service Contracts Office in 509 CES. Invoices Service will be billed thru WAWF (Wide area work flow).

8. SECURITY

8.1. The contractor and his/her employees shall be subject to all base instructions, including but not limited to all vehicle traffic and security instructions. The contractor and his/her employees shall be subject to all base instructions, including, but not limited to, all vehicle traffic, airfield, and security instructions. The contractor shall ensure each employee obtains a contractor pass (CAC Card). This shall be accomplished by initiating application procedures through CE Service Contracts office. All employees shall have the contractor pass in their possession at all times when on base. Contractor employees will be required to obtain and display identification badges.

Anticipate delays in getting commercial vehicles on base and allow time for commercial vehicles to reach their destination by driving designated routes at posted speed limits throughout the base. Procedures for commercial vehicle access to the base are subject to change without prior notice.

9. INSPECTIONS

9.1. All work performed, the methods and manner of performance, all areas used by the Contractor, and all equipment, materials, and supplies used for the work will be subject to inspection at any and all times by the Contracting Officer's authorized representatives. The Contracting Officer will issue all notices of unsatisfactory conditions or services in writing to the Contractor.

9.2. The Contracting Officer will have access to any and all the Contractor's equipment and sites of operation for inspection purposes.
