Replace Carpet 2nd & 3rd Floors SFG Bldg. 34 Performance Work Statement
3 February 2025
1. Description of Services: The Contractor shall provide all personnel, equipment, tools, materials, supervision, and other items necessary to perform all work required for the purchase and installation of carpet tiles, composite carpet planks, and vinyl wall base, to include the demolition and disposal of the existing carpet and wall base on the 2nd & 3rd floors of the Security Forces Group (SFG), Bldg. 34, Francis E. Warren AFB, Wyoming. See attached drawings for locations/measurements of new carpet tile, composite carpet planks, and vinyl wall bases. The Contractor will not be required to move and replace furniture or floor items for carpet demo and installation.
2. Requirements:
2.1 Carpet Tiles:

2.4 Service Requirements: The Contractor shall provide the following incidental service requirements.
2.4.1 Demolition: There is existing sheet carpet, carpet tiles, and wall base throughout the 2nd & 3rd floors of the building to be removed. The Contractor is responsible for off-base disposal of all demolition or cutting materials due to demolition. The approximate demolition of existing carpet all areas is 19,129 sq ft. The approximate demolition of existing vinyl wall base is 4,225 LF.
2.4.2 Delivery and Storage: The Contractor shall deliver all materials to Building 34 for installation.
Contractors shall protect materials during delivery, storage, handling, and installation per manufacturer's printed instructions. Materials should be placed on blocking and covered when necessary to protect from water, dust, direct exposure to sunlight and other elements. Materials should be delivered in original containers or packaging with labels intact. Storage locations shall not interfere with work of other trades. Acceptable temperature and humidity conditions must be maintained. Work areas shall be enclosed, watertight, and heated as necessary to protect finish materials during and after installation.
2.4.3 Installation: The carpet tiles and composite carpet planks flooring will be installed on the plywood covering the historic flooring and sub-flooring. All new flooring will be glued to this plywood. This is a historic building with many historic features and materials. If any cultural materials are discovered or damaged during construction, work in the area shall halt immediately, the State Historic Preservation Office (SHPO), FE Warren Cultural Resources Manager (CRM), and CE Project Manager must be contacted, and the materials or changes are to be evaluated by a historian meeting the Secretary of the Interior’s Professional Qualification Standards (48 FR 22716, Sept 1983). No changes or removal of cultural materials will be allowed without written approval by SHPO and CRM.
In the event where existing plywood has raised or become unfastened, use no more than one ¾ inch staple or nail per 8” fasten plywood to floor. Contact CE Project Manager or CRM for further guidance or concerns.
The carpet tile, composite carpet planks, and vinyl wall base installation is in rooms and hallways on the 2nd & 3rd floors only, as shown in the attached drawings. The Contractor will not be required to move and replace furniture or floor items for carpet demo and installation. Any furniture, floor items, computers, printers, fax machines, or copiers will be moved as required by the government.
Please, allow one week notice for moving items prior to installation activities.
The Contractor shall repair holes, cracks, depressions, and rough areas using materials suitable for the purpose intended; apply latex underlayment or sand raised areas and ridges to smooth, level surface. Sweep the floor clean of all foreign materials before installation.
The Contractor shall remove existing vinyl wall base and replace it with new vinyl wall base. Repair any damage caused by the removal of vinyl wall base to include re-painting as needed.
The Contractor shall provide transition strips of low-profile design where carpet tiles or planks meet uneven surfaces and pose tripping hazards. Submit low-profile transition strip design for approval.
Allow for approximately 60LF of transition strips. If transitions of flooring are of even height and do not pose a tripping hazard, then no transition strip will be required.
Install the carpet tile or composite carpet planks wall to wall in the areas indicated in the drawings.
Spread adhesive with a notched trowel per manufacturer’s instructions. Installed carpet tiles or composite carpet tiles shall be smooth, uniform, and secure.
2.4.4 Clean-up: When complete, remove all debris, scraps, and foreign matter. Remove any soiled spots or adhesive with appropriate spot remover. Clean area as required to remove all small debris, dust footprints, etc. Provide 5% attic stock of carpet tiles and composite carpet planks that are to be left in the building.
3. Performance Period:
The Contractor shall complete this project within one hundred twenty (120) calendar days of contract award. Installation is to be scheduled and coordinated with the government facility manager and CE project manager. All work shall be done in phases to allow relocation of personnel and furniture.
4. General Information:
4.1 Quality Control: The Contractor is required to control the quality of service in delivery and installation of the carpet tiles, carpet planks, and wall base. The overall control of quality must meet the specified performance thresholds for each requirement in the Services Summary.
4.2 Quality Assurance: The Government will evaluate the Contractor’s performance by appointing a representative(s) to monitor performance to ensure services are received. The Government representative will evaluate the Contractor’s performance through on-site inspections and receipt of customer complaints. The Government will inspect each task as completed. The Government will investigate complaints received from customers. The Government representative shall make final determination of the validity of customer complaint(s) in cases of disagreement with the customer(s).
**Damaged/scratched/stained/or soiled materials shall not be accepted. The Contractor is responsible for replacement of materials at no additional costs to the Government. **
4.3 Security Requirements: The Contractor shall be responsible for all passes issued to his/her employees under this contract. Background checks are mandatory and pre-vetting applications are required for Contractors entering the base. Applications shall be coordinated through the Contracting Office. Follow the following procedures:
• Driver/Delivery Contractor(s) shall register in https://dbids-global-enroll/dmdc.mil/
• Driver/Delivery Contractor(s) shall receive QR code after registration.
• Forward QR code print out to the Contracting Office POC’s via emailing liezel.mcintyre@us.af.mil and erika.farms.1@us.af.mil no later than 7 calendar days prior to base arrival.
• Contracting Office will provide digital passes to Driver/Delivery Contractor(s) via email.
Driver/Delivery Contractors shall utilize their respective digital passes to gain entrance to the base.

4.4 Physical Security: The Contractor shall be responsible for safeguarding all Government property provided for Contractor use. At the close of each work period, Government facilities, property and materials shall be secured. Contractor is responsible for the secure storage of their company tools.
4.5 Base Access: Large trucks and trailers shall enter the base with a signed copy of their contract through Gate 5, Commercial Gate, on Central Avenue Cheyenne, WY. Digital passes issued by the Contracting Office shall be utilized to pre-vet Contractors.
4.6 Work Schedule: The Contractor shall coordinate delivery and installation with the 90th CES
Project Manager and 90th Contracting Squadron at least 14 calendar days in advance of shipment arrival.
4.7 Hours of Operations: 1.9. The building will be occupied throughout the duration of the project.
The Contractor shall perform the services required under this contract during the hours of 7:30 a.m.
to 4:30 p.m., Monday through Friday, excluding Saturdays, Sundays, and legal holidays. Any other work hours must be approved by the Contracting Officer.
4.8 Recognized Holidays: The Contractor is not required to provide service on the following days:
New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veteran’s Day, Thanksgiving Day, and Christmas Day. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on Sunday, it is observed on Monday.
4.9 Safety Requirements: In performing work under this contract, the Contractor shall: Conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work. Record and report promptly (within one hour), to the Contracting Officer or designated representative, all available facts relating to each instance of damage to government property or injury to either Contractor or Government personnel.
In the event of an accident/mishap, take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property and preserve evidence until released by the accident/mishap investigative authority through the Contracting Officer.
4.9.1 Hazardous Materials (HM) Approval: The Contractor shall submit a Materials Safety Data Sheet (MSDS) for each chemical and/or compound proposed carpet or vinyl installation to the 90CONS/PKA for review and approval prior to delivery to the base. An HM is defined in Federal Standard DED-STD-313D, Tables I and II. The definition of materials presenting a health or physical hazard is found in OSHA 29 Code of Federal Regulations 1910.1200(c).
4.9.2. Hazardous Materials Management:
All HM shall be managed, stored, and monitored in full compliance with all applicable local, state, federal and Air Force regulations. Contractors who bring hazardous materials onto FEW AFB without authorization are responsible for any punitive or regulatory actions. Contractors are responsible for proper disposal of containers/hazardous material. Flammable liquids shall be stored in an approved fireproof storage cabinet. All chemicals shall be secured and properly stored when not in use.
4.9.3 Hazardous Materials (Asbestos): If potentially suspected hazardous materials such as asbestos are encountered contact the CE Project Manager immediately. If potentially hazardous materials are discovered, the government will seek additional sampling/testing. The government will provide the sampling/testing results and take action appropriately.
4.10 Recycled Products:
4.10.1 New and Recycled Materials and Equipment Purchasing: All materials and equipment shall be new or newly manufactured using recycled materials unless otherwise indicated. All materials (both new and recycled) shall be the most suitable and highest grade and type for the intended use unless otherwise indicated. They shall be produced by a manufacturer regularly engaged in manufacturing or fabricating similar products with a history of successful production acceptable to the Contracting Office. When names of manufacturers or models are listed, the purpose is to give examples of products meeting the specifications and to provide a basis for subject to the following: (1) The product is equivalent to the specified product meets or exceeds the requirements of the specifications and is otherwise suitable for the intended use. (2) The warranty meets or exceeds that of the specified product and the requirements of the specifications. (3) The Contractor coordinates the installation and makes any necessary changes at no additional cost to the Government. (4) The Contractor waives claims for any time extension associated with the use of the product
4.10.2 Recycled Product Purchases: Contractors must comply with guidelines and requirements of The Air Force Affirmative Procurement Program for the purchase and use of recycled material. This is required by Presidential Executive Order and Section 6002 of the Resource Conservation and Recovery Act (RCRA) (42 U.S.C. 6962). The use of recycled products is mandatory unless they are not available within a reasonable time frame, or if the cost exceeds the cost of a non-recycled item or if they do not meet the technical requirements of the project, or if they are not available from at least two (2) sources so as to maintain a satisfactory level of competition. The construction materials included under this requirement are all types of carpet made of polyester fiber and floor tile containing recovered rubber or plastic. The use of other recycled products is encouraged.
4.10.3 Recycled Product Reporting: Contractors must compile a "Recycled Products Report" for the Resource Recovery and Recycling Manager at 90 CES/CEV listing the total quantity recycled products used. Submit the report through the COAR at the end of the contract period or at the end of the calendar year, whichever comes first.
4.10.4 Affirmative Procurement Guidance: The use of recycled material shall follow requirements based upon Department of the Air Force Space Command Affirmative Procurement Program Policy and Guidance Letter dated 8 April 2002. The main legal requirements for an Affirmative Procurement Program (APP) are derived from Executive Order 13101 dated 14 September 1998 (http;//www.ofee.gov) and the Resource Conservation and Recovery Act (RCRA) Section 6002 (http://www.ornl.gov/ornlp2/sec6002.htm). This policy requires action by the Federal Government or its authorized agents to make more efficient use of natural resources by maximizing recycling and preventing waste whenever possible. Specifically, it mandates procurement of the five EPA guideline items; calls for review of government plans and specifications to eliminate virgin material requirements; requires recycling programs and requires consideration of: recovered materials use, reuse of products, life cycle cost analysis, ease of recycling, and use of environmentally preferable products.
4.11 Warranty: The work done under this project requires a written warranty letter for one (1) year from the conclusion of the work, covering the assurance for all labor, materials furnished, and work performed.
5. Contractor Personnel:
5.1 Contract Manager: The Contractor shall provide a Contract Manager who shall be responsible for the performance of the services. The name of the Contract Manager shall be provided to the CO in

http://www.ofee.gov/ http://www.ornl.gov/ornlp2/sec6002.htm) writing prior to the beginning of the performance period. The Contract Manager must be able to read, write, speak, and understand English. The Contract Manager shall have full authority to act for the Contractor on all contract matters relating to daily operations of this contract.
5.2 Contractor Employees: The Contractor shall not employ persons for work on this contract if such employee is identified to the Contractor by the CO as a potential threat to the health, safety, security, general wellbeing or operational mission of the installation and its population. Contractor personnel shall present a neat appearance and be easily recognized as Contractor employees. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges which contain the company name and employee name in English.
