RFP Attachment 5: Past Performance Questionnaire (PPQ) The 90th Contracting Office of FE Warren AFB, WY is in the process of competitively selecting a source for a Maintain Base Interior and Exterior Paint program. This requirement is the performance of interior and exterior painting and related surface preparation including, but not limited to the following: preparation of surfaces to be painted, applying paint and protective coatings, minor cosmetic repairs to surfaces and lead based paint remediation.
One of the considerations in proposal evaluation is the verification of the offerors' past performance on contracts or other work efforts which reflect the offeror's ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the offeror, for the evaluation of the offeror's performance on those contracts or work efforts.
Our areas of interest regarding the offeror are summarized in the enclosed questionnaire. In order to meet the acquisition milestones, we request your written response no later than the date established in the RFP for receipt of proposals. This schedule will allow us sufficient time to analyze the data prior to the start of negotiations.
To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically and e-mailed to Thomas.carrigan.1@us.af.mil or Arthur.Makekau@us.af.mil. If mailing, the POCs may return the completed questionnaires via mail directly to:
90th Contracting Squadron 7505 Barnes Loop, Bldg 208 F.E. Warren AFB, WY 82005-2860
If mailing, the outside envelope must be marked as follows:
NOTE: TO BE OPENED BY ADDRESSEE ONLY
The completed questionnaires should be contained in a second envelope marked with the mailing address and the following legend:

Once the Present/Past Performance Questionnaires are completed by your POCs, the information contained therein shall be considered source selection sensitive and shall not be released to you, the offeror. Therefore, any exchange/contact between the offeror/joint venture member and/or critical subcontractor and its own POCs in regards to comments made on the questionnaire is not permitted.
Your assistance is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Source Selection.
