Statement of Work
Add Electrical Service for Turbo Chef Fryer at 14 MAFs (A-01 – F-01 and H-01 – O-01)

OVERVIEW:
Contractor will be responsible for removal and replacement of an existing electrical circuit.
Replace existing 20A, 208/240VAC circuit with 30A 208/240VAC circuit. Replace 10 gauge wiring with 8 gauge wiring along with all incidental related work.
The performance period for this project shall not exceed 120 calendar days.
GENERAL:
Accomplish all work in accordance with the Contract Specifications and Drawings, Manufacturer’s written recommendations, and all applicable building codes, standards and practices in the industry.
On a daily basis or more often as necessary, clean all work areas of debris as well as
Contractor tools, equipment, and materials. This includes the exterior/interior of adjacent buildings and parking lot/access road.
The Contractor is responsible for verification of all quantities, dimensions, and site conditions.
Any change in conditions or modifications with this project must be presented to the
Contracting Officer (CO) and the Project Manager prior to any change. After the change is agreed on, a FIELD CHANGE REPORT will be completed by the Contractor and processed through the Contracting Office. The FIELD CHANGE REPORT will reflect whether the change or modification will be a no cost Mod or a cost Mod. The FIELD CHANGE REPORT will also have all agreeing parties’ signatures.
The Contractor will be responsible to provide and maintain the information required to obtain a current Entrance Authorization Letter (EAL) for all Contractor personnel working on the Missile Alert Facility (MAF). Changes to EAL will require up to twenty working days to process.
Prior to starting any work, all items listed on the attached AF Form 66 shall be submitted on individual AF Form 3000. This includes Safety Data Sheets (SDS) for all materials used on the project.
If required, the Contractor shall obtain any necessary AF Form 592 – Welding, Cutting, and Brazing Permit (aka “burn permit”), at least three days prior to construction at the sites.
Contact Missile Engineering at (307) 773-2438 for requirements, initiation and completion of permit. Contractor shall have the completed permit control number prior to any burning/welding operations.
The Contractor will keep a copy of the most current Statement of Work and Drawings on jobsite.
Working hours will be between the hours of 7:30 a.m. (start processing on site) and 4:00 p.m.
(terminated work off site) excluding Saturdays, Sundays, and Federal Holidays. An Air Force inspector/escort will be provided during these work hours.
SECURITY REQUIREMENTS
Access to the Missile Alert Facilities (MAFs) is controlled by the U.S. Air Force. All contracted personnel entering the sites shall be on an Entry Authorization List (EAL) created by the Government.
1. Contractor/A&E shall provide complete written, valid, and legible data that shall include legible photocopies or scanned electronic documents to be used to produce the initial EAL prior to their initial commencement of work at the site for this project. Twenty (20) working days are required to process, initially issue, add to, correct, and renew EALs. Any delays caused by submission of incorrect, outdated, and invalid information will be at no cost to the Government. Missile Engineering shall provide to the Contracting Officer and the Contractor/A&E the expiration date for each EAL, when the date is made effective.
a. Contractor/A&E submitted data shall include the following for each employee (to include prime, subcontractor and delivery/testing services employees):
1) Project GHLN number, contract number, name of project, and Contracting Officer.
2) Employers name and address (city and state)
3) One form of ID as follows:
i. Legible photocopy or electronically scanned Valid Driver's license, State photo ID card, or official state/federal photo ID card with the individual's name.
b. Identification cards that expire during the construction period must be updated and photocopies or scanned copies must be sent to the Contracting Officer or his/her authorized representative 20 days prior to expiration or the person shall be dropped from the EAL. The Contractor/A&E is responsible for maintaining an accurate valid list.
c. All of the following will need to be provided for each vehicle: license, state, VIN, make, model, color and year.
Site Entry:
1. In order to gain entry onto the project facility, the Contractor/A&E personnel must be:
a. Listed on an effective EAL.
b. Identified by escort personnel.
c. Able to produce, for verification, the exact form of ID given for the EAL process.
d. Submit to physical and electronic search of person and materials.
e. Physically escorted on-site by an authorized escort official and remain under escort full time while on site.
f. Not in possession of any electronic device or cellular telephone at Launch Facilities (LFs).
2. Upon arrival, the escort will identify all Contractor/A&E personnel and check identification. Processing onto and off the sites may require approximately thirty (30) minutes each and shall not be considered delay time. The processing onto and off of the site shall be during normal working hours, between 0730 hours (7:30 a.m.) and 1600 hours (4:00 p.m.).
Vehicle Identification:
1. Company Identification (logo) must be clearly, legibly, and identifiable at a minimum of 30-foot distance and displayed on each side of all vehicles and equipment brought onto or operated on site. Vehicles and equipment without such identification may be denied access to the site.
2. The access road leading from the main road adjacent to the site is under military control. The area off to the sides of the access road is either private property, federal, state, or county property. Parking vehicles on the access road is prohibited unless permission has been obtained from the government. Vehicles may be parked in the Air Force access area but any such parked vehicles must be clear of the boundary fence by a minimum of twenty-five feet (25’) and a minimum of fifty feet (50’) from the centerline of the access road. Permission to use additional offsite parking and storage areas must be obtained in writing from the landowner and a copy must be submitted to the Contracting Officer prior to the Notice to Proceed for Site Work.
PRINCIPLE FEATURES OF WORK PER CSI DIVISION
NOTE: This scope is a general overview of intended work.
CSI 01 – General Requirements
The Contractor shall submit according to the AF Form 66 a Project Work Plan. The Project Work Plan shall detail duration of work at the site. The work plan shall also identify periods that power will be unavailable. The contractor will protect, clean, repair, or replace adjacent surfaces and existing materials from waste, debris, and damage.
1. The Contractor shall ensure that work done on electrical systems is done by an individual who is licensed to work on electrical systems in the state where the work is to be done.
CSI 26 – Electrical
A. The contractor shall provide all labor, equipment, and materials to remove existing 20A circuit and install new 30A circuit from electrical panel LCPD to kitchen. Exiting 10 gauge wiring will be replaced with 8 gauge wiring and a new NEMA 6-30P outlet.
B. Contractor shall adhere to all local, state, and federal regulations, including NEC requirements, while performing this work.
MAF Driving Distances The work shall be for the systems located at Missile Alert Facilities (MAFs) A01, B01, C01, D01, E01, F01, G01, H01, I01, J01, K01, L01, M01, N01 and O-01 located throughout Wyoming, Nebraska and Colorado, with distances from FE Warren AFB as indicated below.
