Miscellaneous text in this section has been modified to:

ADDENDUM to 52.212-2

M-1 BASIS FOR CONTRACT AWARD: This requirement will be awarded to the responsible Offeror whose quote conforming to the solicitation is determined to represent the best value for the Government based on quotes meeting price, technical acceptability and acceptability standards for all non-cost factors (i.e. technical experience, Attachment 3). This acquisition is being conducted in accordance with FAR Part 13 Simplified Acquisition Procedures. By submission of its offer, the Offeror accedes to all solicitation requirements, including terms and conditions, representations and certifications, technical requirements, in addition to those identified as evaluation factors or sub-factors. The Offeror accedes to the terms and conditions of the model contract and all such offers shall be treated equally except for their prices, technical evaluation and past performance evaluation. An "unacceptable" rating in any factor will result in the overall technical proposal being rated "unacceptable "and the proposal will therefore be ineligible for award. The Government shall utilize information provided in attachment 3 Technical Experience Questionnaire as well as information available from Government Past Performance collection systems when identifying the offer that represents the Best Value.

Negative Past Performance information identified by the Government could result in an "Unacceptable" technical Experience rating. Technical Experience is relatively the same in importance as Price and Technical Acceptability. While the Government will strive for maximum objectivity, the selection process is by its nature subjective. Therefore, professional judgment is implicit throughout the entire process. The evaluation process shall proceed as follows: The following documents will be evaluated for completeness and compliance with the requirements of the RFQ.

Please refer to the instructions in 52.212-1 Addendum. Incomplete items may be a basis for rejection:

1. Part I, Price

(a) Signed copies of the SF 1449 Solicitation / Contract / Order for Commercial Items document. Is the SF1449 completed and signed by an official authorized to bind the company? (Blocks 12, 17a, 26, 30a, 30b, 30c)

(b) Completed Section B, Price Schedule, SF1449. Is the price schedule accurately and completely filled in by the offeror? (Unit Price and Extended amount for all CLINs 0001-5001.

(c) Solicitation Amendments (if applicable). Did the offeror acknowledge all amendments to the RFQ?

2. Part II, Technical Quote: The technical quote shall consist of:

a) Copy of the high school diploma of the person / employee who will be performing the Catholic Music Director, Protestant Music Director, Catholic Musician & Protestant Musician Services in accordance with the Performance Work Statement (PWS), Attachment 1. Limited to a twenty (30) page submission.

M-2 PRICE EVALUATION. Quotes will be ranked from lowest to highest Total Evaluated Price (TEP). An offeror's TEP will be determined by totaling the Amount for all CLINs 0001-5004 & 9991-9994. Offerors shall enter the TEP in Block 26 (Ignore the statement "For Govt. Use Only") of the solicitation SF 1449. The offeror is responsible for ensuring that the value in Block 26 of the SF 1449 is the total of all CLINS provided in the solicitation. (Note: (To prevent rounding issues, only whole numbers are allowed to be entered for unit prices and extended amount). It is the responsibility of each contractor to verify their calculations. Due to funding limitations any offer proposing a yearly price (including Basic or Option Periods) in excess of $45,000.00 or a Total Evaluated Price over $250,000.00 will not receive further evaluation and will be ineligible for award.

Interchanges. Offerors are cautioned to submit sufficient information and in the format specified in 52.212-1 Addendum. Offeror's may be asked to clarify certain aspects of their proposal. Communication conducted to resolve minor or clerical errors will not constitute discussions and the Contracting Officer reserves the right to award a contract without the opportunity for proposal revision. The Government intends to award a contract without interchanges with respective offerors. The Government, however, reserves the right to conduct interchanges with one, none or all offerors if deemed in its best interest.

M-3 TECHNICAL ACCEPTABILITY.

1. All responsive offers will be evaluated in accordance with the procedures found at FAR 13.106-2 Evaluation of Quotations or Offers. An "unacceptable" rating in any factor will result in the overall technical proposal being rated "unacceptable" and the quote will therefore be ineligible for award. The following factors will be utilized:

Factor 1. a) Copy of the high school diploma of the person / employee who will be performing the Catholic Music Director, Protestant Music Director, Catholic Musician & Protestant Musician Services in accordance with the Performance Work Statement (PWS), Attachment 1. Factors 2a, 2b, 2c & 2d. Technical Experience: At least (1) year of experience performing Music Director, Catholic Musician & Protestant Musician Services in accordance with the Performance Work Statement (PWS), Attachment 1 . An offer received without evidence that the contractor has at least one (1) year of experience performing Music Director, Catholic Musician & Protestant Musician Services in accordance with the Performance Work Statement (PWS), Attachment 1 within the last five (5) years preceding the release of this request for quote, will receive a rating of "unacceptable" and will be ineligible for award. If different personnel are utilized for each service, separate resumes for each person must be submitted. The resume must include the name of the person proposed to perform the service.

Technical Evaluation Upon receiving a rating of "Acceptable" for all technical factors (Factor 1), the TEP Lowest Technically Acceptable offer shall be given an experience rating of Good, Acceptable, or Unacceptable according to the criteria of the Schedule Ratings (indicated below). A rating of unacceptable will result in the offer being ineligible for award. If the lowest TEP Technically Acceptable offer receives a rating of Good the Government shall conclude evaluations and make an award decision. No further offers will be evaluated. However, the Government may at its discretion evaluated the next lowest TEP Technically Acceptable offer if deemed in the Government's best interest. If the lowest TEP Technically Acceptable Quote receives a rating of Acceptable for Technical Experience, the Government will continue to evaluate offers until an offer receives a rating of Good.

If no offer receives a rating of Good, the Government shall award to the Lowest TEP Technically Acceptable offer receiving a rating of Acceptable, pending a Determination of Price Fair and Reasonable by the Contracting Officer.
