AF FORM 813 CONTINUATION SHEET
1. Project Name:
Demolition of Building 13022
2. Project Number:
XUMU131066
3. Title of Proposed Action
Demolition of Vandenberg Chapel (B13022)
4. Purpose and Need for Action:
The purpose of the action is to demolish an abandoned structure that was built in 1942 and has outlived its life cycle.
AFMAN 32-1084 supports the divesting of inadequate buildings that are in excess to mission requirements. This action will result in enhanced base bea tification.
5. Description of Proposed Action and Alternatives (DOPAA):
Building 13022 is a wood structure built in 1942 on a post and beam foundation and is estimated to be 4,900 square feet.
The demolition of this facility will include removal of all ACM material by a licensed asbestos abatement contractor as stated in asbestos inventory by Tetra Tech EM Inc 2008-2009 survey.
The demolition of this facility will include the wood structure, post and beam foundation, basement, surrounding vegetation, irrigation system, and approximately 2148 square feet of surrounding concrete walkways. All utilities serving the facility will be cut and capped as close to the building as possible and abandoned in place. The main-line of the irrigation system will be cut and capped.
All ground depression resulting from demolition process will be filled with approved material to match existing grade and hydro-seeded with an approved seed mix to blend with the surrounding environment.

Location and Site Map:
See Attachment 1
Construction Footprint:
See Attachment 1
Construction Schedule:
This is scheduled for FY21
5.4. Ground Disturbance:
Ground disturbance will result from the removal of the building foundation, surrounding concrete sidewalks/ paving vegetation removal and the capping of utilities serving the facility.
5.5. Construction Equipment:
Construction equipment include small excavator equipment, backhoe , loading equipment dump trucks and small graders.

The contract between the U.S. Government and Contractor will describe in detail how solid waste generated during normal construction activities will be handled. Generally, solid waste is hauled off site by the Contractor. If required, log books can be developed identifying all types of solid waste listing quantity, unit of measure, and the disposal site
Alternatives:
CoA 1: Status Quo – This building will remain abandoned and continue to deteriorate.

The ACM survey in the file was a snapshot of the HMMIS file not the complete survey. Please use the complete Tetra Tech survey I added to the file as this will need to be used by the ACM abatement contractor. Renovations and demolitions require permits. Asbestos abatement and demolition activities are a permitted processes! Asbestos disturbance, abatement, and demolition work orders, work clearances, and projects must be reviewed by 30 CES/CEIEC. These processes are regulated by 40 CFR 61 the National Emission Standards for Hazardous Materials (NESHAP) and the Santa Barbara Air Pollution Control District (SBAPCD). Other regulations that affect projects are OSHA and Cal-OSHA asbestos standards in 29 CFR Parts 1910, 8 CCR, 29 CFR 1926, and 40 CFR 763 for asbestos materials in schools and banned asbestos. Under NESHAP and SBAPCD, prior to commencement of a renovation or demolition project, the area shall thoroughly be inspected for the presence of asbestos.
Also prior to project start, all regulatory notifications and abatement plans must be reviewed by 30 CES/CEIEC. You must also provide 30 CES/CEIEC a copy of the approved SBAPCD Notification for Renovation and Demolition form ENF-28.

If the proponent is unable to accommodate these requirements, the proposed action will require additional environmental analysis and this CATEX will not apply.
Comments, Demolition of Chapel 2 (B13022) 813 21-009
Implement Best Management Practices (BMPs) to prevent soil, chemicals or other pollutants from entering into the storm water system, natural surface water drainages or groundwater. BMPs will include erosion and sediment controls, storm drain inlet protection, tracking controls, vehicle and equipment fueling and maintenance, spill prevention and control, solid waste management, liquid waste management, concrete waste management, stockpile management and septic waste management as applicable. BMPS shall be effectively implemented and maintained as described in a current California Stormwater BMP Manual (California Stormwater Quality Association or similar).
1.1.1 - CONTAIN ALL CONSTRUCTION TRASH. PROPERLY DISPOSE OF ANY TRASH AS SOLID WASTE.
1.1.2 - ANY NON-BIODEGRADABLE SEDIMENT CONTROLS SHALL BE REMOVED PRIOR TO PROJECT CLOSE-OUT.
Exposed soils shall be permanently stabilized with vegetation to prevent erosion.
Prepare soil prior to any revegetation efforts. The soil surface shall be loosened and amended as needed to sustain plant life or weed-free topsoil added to a sufficient depth to allow for seed germination. Apply weed-free wood fiber mulch covering 80% of soil surface. Use irrigation as needed to achieve germination as appropriate for the seed mix and the season.
Begin maintenance immediately after seeding or vegetation has been installed. Inspect the vegetation at least once a week and perform needed maintenance promptly. Water, reseed, mulch and perform other operations necessary to promote plant growth and vigor, establish self-sustaining vegetation and prevent bare soil spots. Bare spots shall not be in areas subject to significant erosion. Repair erosion in all seeded areas. Eroded areas may need to be filled with amended topsoil and replanted with the same plant species. Maintain the performance of erosion control materials and protect installed areas from traffic.

1.0 SITE STRUCTURE
Building 13022 is a single-story, approximately 4,196 sq. ft. structure. The building is constructed of a wood frame on a stone base. Interior finishes include drywall walls. Flooring materials include carpet, sheet vinyl flooring, and floor tile with associated mastic. Bathrooms have ceramic tile on the walls and floors.
The building was split up into three areas; the main chapel, the choir loft and the basement. The main choir loft includes a kitchen, offices, a chapel, and bathrooms. The choir loft is a small seating area above the main chapel on the north side of the building. The basement is below the main chapel in the southwest corner of the building and is accessible from the exterior of the building. Homogenous materials were identified in each section and sampled accordingly.
2.0 FINDINGS
Twelve homogenous materials were identified in the assessment areas with a total of 16 samples being analyzed by the laboratory. Samples that exceed the EPA-defined asbestos containing material (ACM) level of 1.0% by polarized light microscopy (PLM) testing are summarized in Table 1 for all areas. Table
2 identifies all samples taken in each area that according to laboratory results do not contain asbestos.
The laboratory report shown in Appendix C contains the PLM results for all the suspect ACM samples collected from the building.

Based on these sample analytical results, Tetra Tech recommends no further action be taken if the non-
ACMs listed in Table 2 are disturbed.
3.0 ASSUMPTIONS AND DEVIATIONS
The following suspect and/or assumed ACMs were identified but not sampled for one or more of the following reasons: limitations on access; limitation on destructive sampling in a specific area because of use; prospect of damage unable to be repaired, thus leaving the possibility of a future asbestos fiber release; or previous analytical reports identifying the material as containing asbestos (PACM).
• Destructive Sampling Limitations: Paper (vapor barrier) under the wood floors in the main chapel area, and possible electrical wiring wrap throughout all areas of the building (safety issue).
• PACM: Thermal system insulation (TSI) in the basement area on the air ducts, and 9x9 floor tile in the offices under the carpet of the main choir area and under the carpet in the choir loft.
The roof was not accessible, but appeared to be covered with wood.
Samples of the suspect ACMs not sampled during this survey should be taken by a licensed asbestos inspector, prior to any disturbance of the materials to determine if the material contains asbestos.
