CONTRACTOR ACCESS TO AIR FORCE IN STALLATIONS (NOV 20 12)
(a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.
(b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver's license, current vehicle registration, valid vehicle insurance certificate, and [insert any additional requirements to comply with local security procedures] to obtain a veh icle pass.
(c) During perfonnance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.
(d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with [insert any additional requirements to comply with AFI 31-10 I, Integrated Defense, and AF! 31-50 I, Personnel Security Program Management] citing the appropriate paragraphs as applicable.

CONTRACTOR/VENDOR ACCESS TO BARKSDALE AFB
AJI commercial vehicles are required to enter Barksdale AFB through the East Gate, just off 1-20 and Industrial Drive. This includes construction, food, supply, FedEX, and UPS deliveries, as well as contractors entering with company trucks. Contractors entering in their privately owned vehicles with an approved pass will be allowed entry at any gate. Hours of operation for the East Gate are 0500-2200 Monday through Friday, Saturday 0600- 1200.
Background Checks for Contractor Personnel Requiring Entry/Access to Barksdale AFB (Jan 2005)
Security Forces will conduct a National Crime and Information Center (NCIC) background check on all contractor or subcontractor personnel prior to granting access to the installation.

CELL PHONE POLICY
The use of cell phones while operating a moving vehicle on Barksdale AFB is prohibited, unless the phone is operated in a "hands free" mode. "Use of cell phones" includes placing calls, retrieving stored information from the phone, programming the phone, talking on the phone, and receiving calls, regardless of the source. Violators may be issued a citation and assessed 3 points on their base driving record. Additional points may be assessed if cell phone use is found to be a contributing factor in a motor vehicle accident.

(b )(I) By submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the SAM database prior to award, during perfonnance, and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation.
(2) The offeror shall enter, in the block with its name and address on the cover page of its offer, the annotation " DUNS" or "DUNS +4" followed by the DUNS or DUNS +4 number that identifies the offeror's name and address exactly as stated in the offer. The DUNS number will be used by the Contracting Officer to verify that the offeror is registered in the SAM database.
(c) If the offeror does not have a DUNS number, it should contact Dun and Bradstreet directly to obtain one.
( I) An offeror may obtain a DUNS number--
(i) Via the internet at http://fedgov.dnb.com/webform or if the offeror does not have internet access, it may call Dun and Bradstreet at 1-866-705-57 1 I if located within the United States; or
(ii) If located outs ide the United States, by contacting the local Dun and Bradstreet office. The offeror should indicate that it is an offeror for a U.S. Government contract when contacting the local Dun and Bradstreet office.
(2) The offeror should be prepared to provide the following infonnation:
(i) Company legal business.
( ii) Tradestyle, doing business, or other name by which your entity is commonly recognized.
(iii) Company Physical Street Address, City, State, and Zip Code.
(iv) Company Mailing Address, City, State and Zip Code (if separate from physical).
(v) Company Telephone Number.
(vi) Date the company was started.
(vii) Number o f employees at your location.
(viii) Chief executive officer/key manager.
(ix) Line of business (industry).
(x) Company Headquarters name and address (reporting relationship within your entity).
(d) If the Offeror does not become registered in the SAM database in the time prescribed by the Contracting Officer, the Contracting Officer will proceed to award to the next otherwise successful registered Ofteror.
(e) Processing time, which normally takes 48 hours, should be taken into consideration when registering. Offerors who are not registered should consider applyit1g for registration immediately upon receipt of this solicitation.

52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS (APR 20 14)
(a) North American Industry Classification System {NA ICS) code and small business size standard. The NAICS code and small business size standard for this acquisition appear in Block 10 of the solicitation cover sheet {SF 1449). However, the small business size standard for a concern which submits an ofter in its own name, but which proposes to furni sh an item which it did not itself manufacture, is 500 employees.
(b) Submission of offers. Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted on the SF 1449, letterhead .stationery, or as otherwise specified in the solicitation. As a minimum, offers must show--
{I) The solicitation number;
(2) The time specified in the solicitation for receipt of offers;
(3) The name, address, and telephone number of the offeror;
(4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary;
(5) Terms of any express warranty;
(6) Price and any discount terms;
(7) "Remit to" address, if different than mailing address;
(8) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically);
(9) Acknowledgment of Solicitation Amendments;
(I 0) Past perfonnance infonnation, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information); and
( 11) Jf the offer is not submitted on the SF 1449, include a statement specifying the extent of agreement with all tenns, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the tenns and conditions of the solicitation may be excluded from consideration.
(c) Period for acceptance of offers. The offeror agrees to hold the prices in its offer finn for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation.
(d) Product samples. When required by the solicitation, product samples shall be submitted at or prior to the time specified for receipt of offers. Unless otherwise specified in this solicitation, these samples shall be submitted at no expense to the Government, and returned at the sender's request and expense, unless they are destroyed during preaward testing.
(e) Multiple offers. Offerors are encouraged to submit multiple offers presenting alternative tenns and conditions or commercial items for satisfying the requirements of this solicitation. Each offer submitted will be evaluated separately.
(t) Late submissions, modifications, revisions, and withdrawals of offers:
( 1) Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in the solicitation by the time specified in the solicitation. Jf no time is specified in the solicitation, the time for receipt is 4:30 p.m., local time, for the designated Government office on the date that offers or revisions are due.
(2)(i) Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of oITers is " late" and will not be considered unless it is received before award is made, the Contracting Officer detennines that accepting the late offer would not unduly delay the acquisition; and--
