PERFORMANCE WORK STATEMENT

USSTRATCOM TURNSTILE RESTORATION & MAINTENANCE

Date: 14 March 2016

1.0 DESCRIPTION OF SERVICES. The contractor provides all management, tools, supplies, equipment, and labor necessary to inspect, and repair all turnstiles and related equipment in the USSTRATCOM Complex, Offutt AFB, NE. Refer to Table 2 for a list of turnstiles. Accomplish maintenance and inspection actions in a manner to ensure continuous turnstile operations. Perform all maintenance and work in accordance with typical commercial practices and/or manufacturer specifications.

1.1. PLACE: USSTRATCOM, Offutt AFB:

1.2. PERIOD OF PERFORMANCE: 12- month Base, to begin at contract award, plus 4-one year option periods.

1.3 TASKS:

1.3.1 INSPECTIONS

1.3.1.1. INITIAL INSPECTION. Upon award of the contract, the contractor and the government shall conduct a joint inspection on all turnstiles to determine the working order and condition of all equipment in its pre-existing state. A report shall be submitted NLT 10 days after this inspection. The Contractor will replace missing items, repair all items not in working order, and perform preventative maintenance on all turnstiles listed in Table 2. The contractor will notify the Government that work has been completed. The Government will then certify the necessary repairs have been made.

1.3.1.2. REGULAR INSPECTION. Contractor shall conduct semiannual inspections on all turnstiles and submit a report NLT 10 days after inspection is completed. Report shall list each turnstile and include a detail of the turnstile’s condition of all functional components, recent damage, aesthetics, and overall status as well as reporting on all corrective actions taken on 100% of the turnstiles.

1.3.2. SERVICE CALLS

1.3.2.1. EMERGENCY CALLS. The contractor will be required to be on call 24/7 for emergency calls. The Contract Officer Representative (COR) will notify the contractor of required emergency repairs. For the purposes of this specification, an emergency repair situation is any condition that requires immediate action to eliminate or prevent: (1) loss of force protection measures; (2) restore critical and mission essential operations. Contractor shall respond to emergency repair service calls and arrest the emergency within 2 hours of notification. Contractor shall complete the work within 48 hours after notification. It is anticipated that number of emergency service calls will be 6 per year. In the event that an emergency call is initiated and it is determined that the turnstile has been damaged by Government personnel or equipment, the contractor shall immediately notify the contract COR who then must validate or invalidate the contractor’s claim and provide further direction.

1.3.2.2. ROUTINE CALLS. The contractor will be required to be on call during normal duty hours for routine repair actions. The COR will notify the contractor of required routine repairs. Routine repair calls are defined as actions to be conducted during normal business days and hours. The contractor shall respond to routine calls before the close of business of the next duty day. The contractor shall complete the work within 3 duty days of notification.

The total anticipated number of routine calls is 12 per year.

1.3.2.3. COORDINATION. In the event that a service call is initiated and the Contractor reasonably believes the operational deficiency results from the access control system, the Contractor shall contact the COR. The COR will evaluate the situation, and if necessary arrange a joint inspection with an access control system representative, see 1.3.4.4.

1.3.3. MAJOR MAINTENANCE AND REPAIRS

1.3.3.1. Should a replacement part exceed the $200.00 threshold established in paragraph 1.3.6 REPLACEMENT PARTS or should the time necessary to accomplish required repairs exceed the performance periods established for emergency and routine calls (48 hours or 3 duty days respectively), indicating a major maintenance effort, the Contractor shall notify the COR for further direction. In the event a circumstance arises for special order parts, the COR can allow additional time for the contractor to meet the repair requirements emergency and routine calls. For example; if it takes one day to diagnose and order necessary parts, the contactor will have two days to complete the repair once the parts are received.

1.3.4. PREVENTATIVE MAINTENANCE CHECKS AND SERVICES (PMCS).

1.3.4.1. Contractor shall perform all Preventative Maintenance Checks and Services (PMCS) as required by the manufacturer’s recommendations and provide a report documenting said PMCS every 6 months.

1.3.4.2. Contractor shall ensure that the turnstiles remain functional and free from general debris that will render them inoperable.

1.3.4.3. The Government reserves the right to self-perform day-to-day adjustments and maintenance to the turnstiles as required and in lieu of placing a service call to the contractor.

1.3.4.4. All turnstiles operate as part of an access control system. The Government reserves the right to install and/or service personal identification systems, such as biometrics and/or card readers, which directly interact with the turnstiles. If the Contractor determines that any action is required by the Government relating to these systems, the Contractor shall notify the COR.

1.3.5. DAMAGE DUE TO NEGLIGENCE. The contractor will not be responsible for repairs/damage caused by government personnel/equipment colliding with the turnstile. Instances of impact/collision will be estimated separately and repair work will not be started until an agreement has been reached and the contractor has received notification by the COR to proceed.

1.3.6. REPLACEMENT PARTS. The contractor shall be responsible for the procurement and costs of each specific component/part that is less than or equal to $200.00. Any specific component/part that exceeds $200.00 will require coordination with the COR/CO at which time a determination will be made on how the contractor will proceed. The contractor shall not purchase, replace, procure, or install any part that exceeds $200.00 without prior approval by the COR or CO. Thus, each component/part that exceeds $200.00 will either provided by the government or an approval to reimburse the contractor will be sought. The government will not be responsible for the reimbursement of component/parts that exceed $200.00 unless prior approval by either the COR or CO has been established. For example, Turnstile A requires a new control head body and a new cam assembly. The cost of the control head body is $200.01 and the cost of the cam assembly is $200.00. The contractor shall be responsible for the cost of the cam assembly at $200.00. However, the contractor shall seek approval before procuring/purchasing the control head body at $200.01, since it exceeds the cost threshold of $200.00. The labor associated with the alignment and installation of any parts is part of the ongoing maintenance requirement and the Government will not be responsible for any labor costs.

1.3.4

Tabulated report documented the manufacturers’ recommended preventative maintenance and aligning those tasks with the Contractor’s accomplished work during that period. Report required for 100% of the turnstiles every 6 months.

3.0 GENERAL INFORMATION

COR, to monitor performance to ensure services are received. The COR will evaluate the contractor’s performance through intermittent inspections of the contractor’s quality control program and receipt of complaints from base personnel.

3.2 HOURS OF OPERATION. Normal operations are Monday through Friday (0730 to 1630 except for Federal Holidays). Inspection and routine maintenance shall be performed during normal duty hours. Emergency repairs shall be accomplished 24 hours a day, seven days a week.

3.3. SECURITY REQUIREMENTS.

3.3.1 Contractor personnel will obtain and possess at all times while on Offutt AFB and USSTRATCOM Complex, an authorized identification card and restricted area badge. It is estimated approximately 2 days processing for these ID cards are required for each contractor employee prior to contract start date. Each contractor employee will be required to fill out a DD Form 1172, Application for Uniformed Services Identification Card, prior to issuance of an ID card. This form will be available from the COR or the Contracting Office.

3.3.2. The contractor shall request a United States Air force (USAF) restricted Area badge for all personnel who require unescorted entry into restricted areas for specific work for the duration of the contract. While paperwork for badges is being processed, escorts will be provided by the J43 Engineering Division on an interim basis for areas that requires access. The contractor will wait no longer than 20 minutes after initial notification for an escort to gain access into the restricted area. It is the contractor's responsibility to fully document all cases where entry into restricted areas was not granted. The contractor personnel shall have proper identification prior to entry into restricted areas. It is the responsibility of the J43 Engineering Division to brief all contractor personnel working on the USSTRATCOM Complex on procedures and policies of each restricted area and document the briefing for the USSTRATCOM files.

3.3.3. The contractor shall obtain a favorable National Agency Check (NAC) investigation for its employees for unescorted escort entry into restricted areas. This will be accomplished by completing the Electronic Questionnaires for Investigations Processing (e-QIP) via the internet by each individual. After completion, call for an appointment with the 55th CES/CEC for the processing of AF Form 2583 and also Resource Protection Section, 55th Security Police, Bldg 323, 1st floor, Rm 105, phone 294-5252, who will initiate the NAC.

3.3.4. The contractor is responsible for controlling and accounting for all identification cards and restricted area badges. When employees leave the employment of the contractor, the contractor is responsible to recover the ID cards and all other entry/security documents from the terminated employee and immediately (within one working day) deliver the items to the Contract COR. When restricted area badges are involved, notify the 55th CES/CER security Officer. If lost or failure to turn in these items within 10 calendar days, the government shall deduct the amount of $50.00 per item, per employee from the next applicable invoice. This fee is to cover the government administration and inventory cost.

4.0 General Requirements

4.0.1. FY11 NDAA Section 8108, Contractor Inventory

“The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the USSTRATCOM Turnstile Restoration & Maintenance via a secure data collection site. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil.

Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.”

*Reporting Period: Contractors are required to input data by 31 October of each calendar year.

Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.
