PERFORMANCE WORK STATEMENT (PWS)

INSTALL 54TH HELICOPTER SQUADRON TEMPORARY FACILITY

Minot AFB, ND 7 December 2017

PERFORMANCE WORK STATEMENT

FOR INSTALL 54TH HELICOPTER SQUADRON

TEMPORARY FACILITY

1. DESCRIPTION OF SERVICES. The Contractor shall provide all labor, materials, tools, supplies, equipment, parts, vehicles, supervision, transportation, and other items necessary for a temporary facility for the 54th Helicopter Squadron at Minot Air Force Base in a manner that will ensure continuous and safe operation. The Contractor shall perform to the standards in the contract as well as comply with all local, state, and federal regulations. The location of install for the temporary facility is identified in Appendix A of this document.

1.1. GENERAL SITE INFORMATION. The site for the temporary facility and the adjacent parking lot are located on Minot AFB. Minot AFB personnel will do a security/criminal background check on each employee requesting access to the base. The Contractor shall assume responsibility of all equipment identified in this document.

1.2. TEMPORARY FACILITY INSTALLATION. Contractor shall provide all labor, materials, tools, supplies, equipment, parts, vehicles, supervision, transportation, and other items necessary to install and maintain a 13,000 square foot building. The contractor will also be required to Deconstruct and remove at the end of the lease.

1.2.1. STRUCTURE. Contractor shall provide a structure that includes the following: one auditorium, two restrooms, 3 briefing rooms, a mission planning room, 14 office spaces, a break room, a mechanical room, and a communication equipment room. The facility shall have a foundation capable of accommodating 100 personnel with furniture and equipment.

All support for the foundation shall be provided by the contractor. The contractor will supply keys for each room.

The temporary facility (TPF) roof will consist of 45 mil EPDM roofing with gutters/downspouts drained away from buildings. All interior partitions will be moisture/mold resistant vinyl-wrapped gypsum board with tile flooring. Color selections have been made from factory stock samples and shown on 100% drawings.

The TPF shall be constructed in accordance with International Building Code 2015 (or newest version as superseded) for construction of temporary facilities with duration greater than 180 days, and UFC 1-200-01, “DoD Building Code (General Building Requirements)”, dated 2016/06/20, as applicable. The design of the TPF will be Type V Construction, Occupancy Type B, 89 MPH nominal wind load, wind exposure category B, 50# Floor loads, and 20# Roof Loads per IBC 2015 for Building, Plumbing, Mechanical, Electrical and Energy Conservation codes. All means of egress will comply with ABA requirements and Life Safety Code for new Business Occupancies.

The project will comply with ABA Accessibility Guidelines for Building and Facilities as published in the Federal Register July 23, 2004 and amended August 5, 2005. The Thermal package will include Fiberglass Insulation with the following values:

Insulation shall maintain a minimum of R-11 for interior walls, R-19 for floor and exterior walls, and R38 for roof assemblies.

1.2.1.1 Plumbing Details

1. All plumbing and fixtures will be installed in accordance with International Plumbing Code 2018.

2. Supply line will use PEX-B or equal and lead-free push-to-fit or compression fittings

3. Drain, waste and vents will use PVC or equal government approved substitute

4. All water heaters will have check-valve at entry to water and each fixture will have shut-off made from heavy duty 100% brass.

5. Water Heater is electric (6 gallon) with back-flow preventer and expansion tank and inline circulator.

6. Water Closets will be Tank Type, low-flow dual flush

7. Lavatories will be wall mounted w/ADA faucet

8. Service sink will be plastic Mop sink w/ADA faucet

9. Mirrors and ADA grab bars will be provided

1.2.1.2 Doors and Windows Details

1. 3068 exterior doors Rim Type Panic Hardware /w pull and key, and door closer will be installed at entrances to the facility.

2. Exterior doors will be 18-gauge steel w/16-gauge frames painted to match interior with jamb depth as required to fit walls and windows.

3. The modular buildings have NO exterior windows.

1.2.1.3 Mechanical Details

1. Heating: Space Temperature: 70 degrees

2. Cooling: Space Temperature: 75 degrees

3. The Data/Comm room will have dedicated Mini-Split unit

4. Supply diffusers in each zone based on diffuser throw shown on 100% drawings

5. Digital programmable thermostats locations shown on 100% drawings

6. Thermostats will have a range from 70 degrees to 75 degrees in the winter, and 72 to 78 degrees in the summer.

7. Design of HVAC system shown on 100% drawings

8. Supply and returns will utilize aluminum or galvanized steel duct

1.2.1.4 Electrical Details

All electrical assemblies shall comply with NFPA 70, National Electric Code 2017.

1. T-8, 32W, 2’x4’ four bulb lay-in fluorescent fixtures or LED equivalent will be used.

2. A minimum of 50 candelas per square-foot for the entire facility will be provided.

3. Lights switches will be provided in all rooms.

4. LED exit lights with battery backup at each exit will be provided as required by NFPA 101.

5. Wall pack light fixtures connected to a photocell will be provided on the outside of the facility.

6. 120V-20A duplex outlets will be provided throughout the facility for electrical load.

7. Fire Alarm System provided per NFPA 13. FACP to be surface mounted.

8. Data / Communications outlets and locations shown on 100% drawings submitted with proposal.

9. CFM exhaust fans will be provided in restrooms per codes

1.2.1.5. AUDITORIUM. Capable of accommodating 100 personnel, space for speaker, projector screen, and lectern Approximately 2,700 sq ft. 3 data/3 communication outlets to suffice.

1.2.1.6. TWO RESTROOMS. One for males and one for females. ADA compliant.

Approximately 400 sq ft total for both.

1.2.1.7. THREE BRIEFING ROOMS. Two briefing rooms suitable for 6 people approximately 450 sq ft for both total and one room suitable for 12 people approximately 1,250 sq ft . 2 data/2 communication outlets per room to suffice. Also, there shall be a conference room for the commander and their staff that is approximately 400 sq ft with 4 data and 4 communcations outlets to suffice.

1.2.1.8. MISSION PLANNING ROOM. Must accommodate 4 workstations, map preparatory area, large map printer, and office material equipment storage approximately 320 sq ft. 6 Data /6 communication outlets to suffice.

1.2.1.9. OFFICE SPACES. Office spaces should accommodate teams ranging between 4 and 12 personnel. Total personnel will be 91. Eight offices shall be approximately 360 sq ft and house 6 individuals with 6 data and 6 communications outlets to suffice and four offices shall be approximately 300 sq ft and house 4 individuals with 4 data and 4 communications outlets to suffice. Additionally, there should be four office spaces for command type individuals that should be at least 150 square feet each with 2 data/2 communication outlets to suffice. Finally, there should be an office for the commander’s support staff that is approximately 480 sq ft with 6 data and 6 communications outlets to suffice.

1.2.1.10. BREAK ROOM. Shall include both upper and lower cabinets and a standard size kitchen sink. Break room shall be able to accommodate three refrigerators and four, four person tables with chairs and approximately be 400 sq ft with 2 data/2 communication outlets to suffice.

1.2.1.11. MECHANICAL ROOM. Mechanical room must accommodate the housing for the utilities and operations for the building. Approximate size is 200 sq ft.

1.2.1.12. THREE EXTERIOR ENTRANCES WITH VESTIBULES. One entrance should be approximately 72” wide. All vestibule doors shall have cypher locks as well as an additional two interior doors.

1.2.1.13. WATER FOUNTAIN. Shall be included in the facility.

1.2.1.14. EXTERIOR RAMPS. The contractor shall provide exterior stairs and ramps for the three exterior doors. Width and ramps should be sized appropriately to the door size.

Applicable non-permanent walkways are to be provided by the contractor. Temporary ramps, decks and stairs made from Pressure Treated wood will be installed at the trailer entrances to meet ABA requirements.

1.2.1.15. OPERATIONS AREA. Located in the main corridor, capable of accommodating an operations counter, two personnel, multiple radio equipment, and storage for flight related material that is approximately 320 sq ft with 4 data/4 communications outlets to suffice.

1.2.1.16. TRAINING ROOM. Capable of accommodating 15 individuals as well as a 6’ x 6’ table approximately 450 sq ft with 6 data and 6 communications outlets to suffice.

1.2.1.17 TESTING ROOM. Approximately 200 sq ft with 2 data and 2 communications outlets to suffice.

1.2.1.18 CIRCULATION. Ten percent of the building’s size shall accommodate for circulation.

1.3. UTILITIES. The contractor shall coordinate and connect the structure to utility connections provided through utility POCs provided in paragraphs 1.3.1 – 1.3.3 below.

The contractor shall ensure that all utilities are properly working prior to completion of performance. The contractor will coordinate with utility providers for installation and provide the material and equipment for those connections. The contractor shall also be responsible for any incidental utilities.

1.3.1. ELECTRIC. Verendrye Electric, POC John Westby, 1-800-472-2141

1.3.4. COMMUNICATION. Contractor shall provide 120 data wall jacks along with all material and equipment for operation of those lines. Also, the contractor will have to provide a communications room to house all equipment. Lastly, the contractor will have to move the 6 MM fiber from building 879 to the temporary facility. The contractor will be responsible for engineering, installation and testing as required.

1.3.4.1 GREY MARKET. Contract shall meet Compliance with FIR 15-02; Prevention of Grey Market Cisco Products Entering the USAF IT Infrastructure. Contractor must procure product from authorized Cisco channel. Vendor will provide original manufacturer warranty.

1.3.4.2 The contractor shall install 6 MM temporary fiber from Building 879, TF-879 FO- 0802, 1-12 MM fiber patch panel with ST connector to LC connector in temp facility.

Terminate and test the temp fiber accordingly.

1.3.4.3 The contractor shall install either two post racks or VRS-8-30 vertical wall mounted racks depending on site walkthrough and situation of the temp facility.

1.3.4.4 The contractor shall install three new Cisco 3850 PoE switches to support approximately 120 users. The Contractor shall coordinate with 5 CS/SCOI (MSgt Christian Bird 701-723-1040) to procure the correct model/version of the Cisco 3850 PoE switch and APC UPS.

1.3.4.5 The contractor shall install High Density CAT 6 patch panel and patch cord accordingly. Test and label all connectivity’s operational. Data/Communications outlets and locations estimated per location within the facility. Office, MSN Planning Office, Auditorium, Briefing Room 1, Briefing Room 2, Briefing Room 3, etc.

1.3.4.6 All cables shall be tested and labeled appropriately and naming scheme shall also be coordinated with the 5CS/SCX.

1.3.5. FIRE DETECTION/SUPPRESSION. Fire detections system must be compatible with Monaco D21 Fire Management System. Fire extinguishers and cabinets will be installed in accordance with NFPA codes and standards and will be mounted in an appropriate box that is flush or semi-flush mounted. If pre-installed, fire suppression system must have Fire Design Engineering stamp for the layout for the facility, in accordance with UFC 3-600-01, NFPA 101, NFPA 72, and NFPA 13. The fire alarm system shall be compatible with the installation's existing fire alarm reporting system.

1.4 FACILITY DETAILS.

1.4. LEASE. The temporary facility will be leased to the Government under this contract.

The contractor will provide operations and maintenance of the facility during its use.

1.4.1 FACILITY MANAGEMENT. The contractor shall provide coverage and support to the operations and functions of the facility. The contractor shall assess and correct all deficiencies and work requests provided by the Government in a timely manner.

1.5. SITE TEARDOWN. At the end of the contract, the contractor shall remove all structures, equipment, and material to its original state.

1.6. OPERATION/MAINTENANCE INSTRUCTIONS - Contractor shall submit the manufacturer’s published instructions. Maintenance of the facility shall be coordinated with and provided by the contractor over the terms of the lease.

2. GOVERNMENT FURNISHED SERVICES.

2.1. GOVERNMENT FURNISHED UTILITIES. The Government will furnish water and sewage as necessary for accomplishment of work in accordance with this contract unless unavailable.

2.2. UTILITY CONSERVATION. The Contractor shall adhere to all base level utility conservation practices or requirements. The Contractor shall be responsible for operating under conditions that prevent waste of utilities.

2.3. TELEPHONE SERVICE. The Government shall only provide base and local commercial telephone service. Long distance service, if desired, shall be at the Contractor’s expense.

2.4. SECURITY, FIRE, AND MEDICAL SERVICES. The Government will provide police and fire protection. In the event of a medical emergency, commercial ambulance service for transporting an injured employee to a local hospital is available at the cost of the Contractor.

2.5. SOLID WASTE COLLECTION AND DISPOSAL. The Contractor shall dispose of trash or solid waste generated from accomplishment of services detailed in this PWS.

Government-owned dumpsters located on the base will not be utilized by the Contractor.

The Contractor shall dispose of all trash/waste off-site and in accordance with existing local, state, and federal regulations.

3. GENERAL INFORMATION.

The Contractor is solely responsible for compliance with OSHA standards for the protection of their employees. The Contractor shall ensure specific safety requirements in Air Force Occupational Safety and Health (AFOSH) standards, Air Force technical orders, and CEM21-SM80-19FM Handbook are complied with by Contractor Personnel when non-compliance would clearly present the potential to harm or damage government resources (see AFI 21-101, paragraph 2.16.). The Air Force is not responsible for ensuring that Contractors comply with “personal” safety requirements that do not present the potential to damage government resources. All work will be performed by personnel who are trained and qualified for the systems and equipment.

3.1. ENVIRONMENTAL COMPLIANCE. Contractor shall be knowledgeable of and comply with all applicable Federal, State and local regulations and requirements regarding environmental protection.

3.2. INSPECTION. After the project is completed, the Government will inspect the site before accepting the facility. The Contractor must return to fix any deficient work they performed that does not pass inspection until inspection is passed/certified.

3.3. INSTALLATION INFORMATION

3.3.1. HOURS OF OPERATION. Working hours for the Contractor will be between the hours of 7:30 a.m. and 6:00 p.m. The Contractor will not work Saturdays, Sundays, and the days federal holidays are observed. All work must be completed within this time period.

3.3.2.Work scheduled, but not accomplished due to weather, exercises, or actual alert will be accomplished promptly after said reasoning.

3.3.2. FEDERAL HOLIDAYS.

New Year’s Day - 1 January Martin Luther King Day - 3rd Monday in January Washington’s Birthday - 3rd Monday in February Memorial Day - last Monday in May Independence Day - 4 July Labor Day - 1st Monday in September Columbus Day - 2nd Monday in October Veteran’s Day - 12 November Thanksgiving Day - 4th Thursday in November Christmas Day - 25 December

3.3.3. If these holidays fall on Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed. If a holiday falls on a scheduled service day, the Contractor will be responsible for rescheduling services for the first day after the holiday observance.

3.4. HEIGHTENED SECURITY AND FPCONs. During periods of heightened security and Force Protection Conditions (FPCONs), the Contractor shall still be responsible for continuing performance unless notified by the Contracting Officer that project has been temporarily suspended or restricted. During times of heightened security, delays at the entry gates should be expected.

3.5. SECURITY REQUIREMENTS

3.5.1. When working adjacent to the project site, all security regulations shall be obeyed, motorized vehicles shall be equipped with fire extinguishers, and no smoking will be permitted in other than prescribed smoking areas. No powder-actuated tools will be permitted.

3.6. CONTRACTOR VEHICLES AND EQUIPMENT

3.6.1. Contractor vehicles shall have company name visibly displayed on each vehicle.

3.6.2. At the end of each workday and the completion of the site, the Contractor shall cleanup work area debris and shall ensure they remove all of their equipment, tools, and work/contract materials.

3.6.3. STORAGE - If approved, mobile equipment must be stored during non-working hours, except noon lunch period or intermittent break periods during the day, in the area set aside on Government property at the site or be removed off-site.

3.7. ENTRY AND SITE REGULATIONS

3.7.1. The Contractor’s employees’ information must show each employee’s:

a) Full name exactly as it appears on the credentialed photo identification (ID)

b) Date of birth (DOB).

c) Full Social Security Account Number (SSAN).

d) Citizenship.

e) The Government issuing agency name and the control number (exactly as it appears on the credentialed photo ID) of the Government-issued credentialed photo ID.

f) U.S. Government-issued credentialed photo IDs must have a picture of the individual, individual’s DOB and a control number. The following are acceptable government issued credentialed photo IDs: U.S. Federal (i.e., U.S. passport or U.S. passport card), or state, commonwealth, territory, protectorate, the District of Columbia, US sovereign insular area or a tribal organization recognized by the US Department of the Interior (i.e., state- issued driver’s license, state-issued photo identification, or Tribal ID card) IAW the Real ID Act.

3.7.2. Each employee must have their identity proofed and their access vetted through the National Crime Information Center database and the Security Forces Information Management System using their name, DOB, and full SSAN. The employees’ DOBs and full SSANs will only be used for this proofing and vetting process.

3.7.3. Contractor personnel who cannot provide proper identification, will not be permitted on the site. Delays caused by lack of proper data or proper identification shall be at no cost to the Government.

3.8. HAZARDOUS MATERIAL IDENTIFICATION. Safety Data Sheets (SDSs) are required as specified in the latest version of Federal Standard No. 313 (including revisions adopted during the term of the contract) for any other material designated by a government technical representative as potentially hazardous and requiring safety controls. SDSs, container sizes, and material quantities must be submitted by the Contractor upon contract award at least 15 working days prior to bringing any materials onto government property.

Failure to provide SDSs or certificate when requested could result in the Contractor being considered non-responsive and result in termination of the contract. The Contractor is responsible to provide 5 CES/CEIE with usage.

The Contractor shall not use products that are or contain Toxic 17 chemicals, Extremely Hazardous Substances (EHS), Ozone Depleting Substances (ODS), and/or Persistent Bio-accumulative and Toxic (PBT) chemicals.

3.8.1. HAZMAT APPROVALS. The Contractor will be responsible for purchase of all approved hazardous materials. No materials may be used without prior approval through the CO to the base Environmental Office (5 CES/CEIE). The Government (5 CES/CEIE, Environmental Office) will review and approve or deny use of all chemicals that the Contractor requests to bring on to Minot AFB or any property under its control. No materials can be stored or left at Government facilities between visits.

3.8.2. SPILL RESPONSE. The Contractor will be briefed on the site-specific spill response procedures prior to start of work. The Contractor is responsible to report to the workplace Government inspector or facility manager and promptly cleanup all spills and dispose of all associated waste in a manner consistent with current environmental regulations. Contractor shall have a mobile spill kit large enough to contain a spill at least 10% bigger than the largest container of cleaning product. In the event that it is necessary to utilize Government material, equipment or personnel to clean up a Contractor-caused spill, the Contractor shall be required to reimburse the Government for all associated costs.

3.8.3. HAZARDOUS MATERIAL/WASTE MANAGEMENT. Contractor is

responsible for all wastes and waste disposal costs associated with PWS and must remove all waste from site when they leave. All wastes will be properly disposed of according to local, state and federal environmental regulations. Criteria for identifying hazardous waste is contained in Subpart C of 40 CFR, Part 261.

3.8.4. HARD HATS. Contractor shall adhere to OSHA requirements found in 29 CFR 1910 and 1926. Hard hat inspection criteria are found in 29 CFR 1910.135 and 1926.100.

Industry and manufacturer standards shall be met on hard hats in order to meet ANSI Z89.1 standards. Hard hats shall be provided by Contractor.

3.9. WEAPONS, FIREARMS, AND AMMUNITION. Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their Contractor-owned or privately owned vehicle while on Minot AFB.

3.10. CONTROLLED UNCLASSIFIED INFORMATION (CUI). The contractor does not require access to any CUI or other sensitive information and is not authorized any access in the performance of these duties.

3.11. FOR OFFICIAL USE ONLY (FOUO). The Contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements.

This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

3.12. PERSONNEL. Contractor personnel shall present a neat appearance. Contractor personnel shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wear of distinctive clothing, overcoats, or hats bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection. The Government is authorized to restrict the employment under the contract of any Contractor employee or prospective Contractor employee who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population.

3.12.1. CRAFTSMEN. Only skilled craftsmen in the trades required for the job shall be performing the work. All work shall result in the highest standards of workmanship, safety, and quality. The contracting officer may, in writing, require the Contractor to remove from the project any employee deemed to be incompetent, careless, or otherwise objectionable to personnel safety, equipment, or the quality of workmanship.

3.12.1.1. WELDING, CUTTING, BRAZING, ETC. Before "Hot Work" (welding, cutting, brazing, etc.) is going to take place, the Contractor shall obtain a welding permit (AF Form 592) from Base Fire Department, within ten working days after award, before any open flame or welding operation is conducted. The Contractor shall allow five workdays for processing the permit. No welding actions shall occur without the welding permit in hand and on site. The welding permit shall be renewed every 30 days. The Contractor shall conduct his work such that, after any welding operation, a one-hour (minimum) fire watch in the area in which welding has occurred is conducted. If digging, the Contractor shall call 811 and obtain a Minot AFB dig permit with all signatures.

3.13. REPORTING REQUIREMENTS. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, Contractor personnel, resources, and classified or unclassified defense information. Contractor personnel shall report any sensitive information (classified or CUI) discovered.

3.14. SAFETY REQUIREMENTS AND REPORTS. The contractor shall perform work in a safe manner as required by OSHA Safety and Health Requirements. Provide a verbal report to the CO as soon as possible of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. Provide a completed copy of required Accident Investigation Reports to the CO within five calendar days of each occurrence.

3.15. DAILY CLEANUP AND DISPOSAL OF CONSTRUCTION DEBRIS

3.15.1. The Contractor shall at all times keep the work area clean of all foreign matter not essential to the project completion. The Contractor shall clean up at the end of each day (within the working hours defined in paragraph 5.1) all foreign matter not essential to the project completion. The Contractor shall return all areas to the condition of cleanliness that existed before work commenced. Contract completion time shall include final cleanup of the premises in accordance with (IAW) the terms and clauses contained in this contract. It is essential that all equipment and materials clear (by a minimum of 25 feet) the perimeter fence and/or perimeter fence line during all non-working hours.

3.15.2. SITE DAMAGE. Contractor shall be responsible for all damage incurred during this project and shall return damaged areas to original condition at no additional cost to the Government. Any damage done will be the responsibility of the Contractor to repair or replace to acceptable standards.

3.15.4. DRAWINGS. Drawings shall be provided to the CO. Drawings shall illustrate how the specified materials are integrated into the work as a whole. Adjacent materials that affect the complete installation of the specified products shall be shown and identified.

3.16. MAINTENANCE AND REPAIRS. The Government expects the Contractor to perform at the acceptable levels throughout the life of the contract. Contractor shall provide facility maintenance and repairs. Contractor shall ensure that repairs and maintenance are done within 48 hours of time of notification of issue from the government facility manager.

Contractor shall furnish all materials and labor necessary to repair and maintain temporary facility. Contractor shall respond to trouble calls within three (3) hours of emergency notifications.

4. APPENDICES. The Government will make all publications, forms, references and report formats listed available upon request. Publications can be accessed on-line at http://www.e-publishing.af.mil/. Supplements or amendments to listed publication from any organizational level may be issued during the life of the contract.
