PERFORMANCE WORK STATEMENT
MAF REFUSE COLLECTION
1. DESCRIPTION OF SERVICES. The Contractor shall provide all personnel, equipment, tools, materials, vehicles, supervision, and other items and services necessary to perform refuse removal services as defined in the Performance Work Statement (PWS) at the 15 locations for the Minot Air Force Base
Missile Alert Facilities (MAF). The contractor shall perform to the standards in the contract as well as all local, state, and federal regulations. Estimated workload factors are in Appendix A and collection areas are in Appendix B.
1.1. Routes/Schedules Submittals. The contractor shall establish vehicle routes and collection schedules based on a 10 day collection cycle. They are to be submitted to the Contracting Officer (CO) and
Contracting Officer Representative (COR) five (5) calendar days prior to start of contract performance for the base year and every option year if the option is exercised. The Contracting Officer will review the schedule for approval.
1.1.1. Unscheduled/Changes/Reduction of Collections. The Contractor must coordinate all changes to the schedule with the COR and the CO 10 days in advance prior to approval. Frequency tables are located at
Appendix A. The Government reserves the right to reduce the frequency of collections.
1.1.2. Inclement Weather Schedule. The Contractor shall collect refuse material during periods of inclement weather. However, in cases of severe weather, the CO or COR may authorize exceptions. When exceptions are granted, the Contractor shall make up all missed collections within 24 hours after the severe weather has terminated or travel is authorized on public road.
1.1.3. Route Parameters. The Contractor shall establish routes to each Missile Alert Facility using attached map. Collection shall be made between the hours of 0730 and 1630 on the day of the week specified.
Collection outside these hours shall require prior approval of the COR or the on-duty MAF personnel.
1.1.4. Points of Collection. Collection stations for refuse are shown in Appendix B. Dumpsters shall be located outside the fenced area of each MAF. The Contractor shall position bulk containers for customer ease in depositing refuse. The Contractor shall also position containers to minimize interference with adjacent parking areas, roadways, overhead utilities, trees, and other potential obstructions.
1.1.5. Prohibited Waste. Customers will not dispose of Hazardous Waste or items covered by the MAF
Recycling program.
1.1.6. Maintaining Containers and Collection Area. When collecting refuse, the Contractor shall return the containers to their original location after servicing, in an upright position with the lids securely in place
(closed). The Contractor will also be responsible for immediately cleaning up any spills, debris, etc., which may occur during servicing of containers. All refuse on the ground within ten feet of the container, whether spilled by the Contractor or placed there by on-site personnel, shall be picked up by the Contractor during collection. The Contractor shall collect debris placed at collection stations; for example, tree branches, sacks, cartons, boxes, cans, tied bundles, or other containers. The Contractor will be responsible for keeping collection areas free of refuse and debris.
1.2. Off-Base Disposal. The Contractor shall transport and dispose of all solid waste at a licensed off-base disposal site selected by the contractor. Disposal shall be in accordance with existing local, state, and federal regulations. The Contractor shall be responsible for any permit or fees associated with the use of off-base disposal locations.

3. GOVERNMENT FURNISHED PROPERTY/SERVICES. There is no Government furnished property or services in this contract.
4. CONTRACTOR FURNISHED EQUIPMENT. The Contractor shall provide all vehicles and dumpster containers needed to perform the requirements of the contract. The Contractor-provided, industry standard dumpster containers must be pre-approved by the CO prior to placement and before contract performance is to begin. Typical capacity of containers located at the MAFs is 6 cubic yards. For security purposes, the contractor provides a padlock for each container and duplicate keys to the COR or on-site
MAF personnel.
4.1. Contractor Maintenance Requirements. The Contractor shall furnish containers that are fully serviceable with working doors, hinges, lids, and hasps. The containers shall remain easily accessible to customers and in good, workable condition. The Contractor shall maintain all contractor containers ensuring they are free of unpleasant odors, dirt, debris, and pests. The Contractor shall furnish replacement containers for any container removed from the MAF unless previously approved by the COR or the on-duty
MAF personnel.
5. CONTRACTOR PERSONNEL. The Contractor shall provide a Contract Manager who shall be responsible for the performance of the work. The name of this person, and an alternate or alternates, who shall act for the Contractor when the Manager is absent, shall be designated in writing to the Contracting
Officer.
5.1. The Contract Manager or alternate shall have full authority to act for the Contractor on all contract matters relating to daily operation of this contract.
5.2. The Contract Manager or alternate shall be available during normal duty hours (0730-1730) within 24 hours to meet, on the installation, with Government Personnel (designated by the Contracting Officer) to discuss problem areas. After normal duty hours, the Manager or alternate shall be available within 4 hours.
5.3. The Contract Manager and alternate or alternates must be able to read, write, speak and understand
English.
5.4. The Contractor shall not employ persons for work on this contract if such employee is identified to the
Contractor by the Contracting Officer as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.
5.5. Contractor Personnel shall present a neat appearance and be easily recognized as Contractor employees. This shall be accomplished by wearing distinctive clothing bearing the name of the company.
6. GENERAL INFORMATION.
6.1. Occupational Safety and Health (OSHA) Standards. The Contractor is solely responsible for compliance with OSHA standards for the protection of their employees. The Contract Manager shall ensure specific safety requirements in Air Force Occupational Safety and Health (AFOSH) standards and Air
Force technical orders are complied with by Contractor Personnel when non-compliance would clearly present the potential to harm or damage government resources (see AFI 21-101, paragraph 9.16.). The Air
Force is not responsible for ensuring that Contractors comply with “personal” safety requirements that do not present the potential to damage government resources.
6.2. Environmental Compliance. Contractor shall be knowledgeable of and comply with all applicable
Federal, State and local regulation and requirements regarding environmental protection.
6.3. Contract Manpower Reporting. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the MAF
Refuse Services via a secure data collection site. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil.
6.3.1. Reporting inputs will be for the labor executed during the period of performance for each
Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.
7. SECURITY REQUIREMENTS. Dumpsters must be locked with a padlock to prevent unauthorized use of the dumpster. Duplicate keys for each dumpster must be provided to the COR or on-site MAF personnel.
8. HOURS OF OPERATIONS. The Contractor shall perform services between the hours of 0730 - 1630.
8.1. Holidays. The Contractor may be required to provide services on all holidays except Thanksgiving, Christmas and New Year’s Day.
9. RECORDS MANAGEMENT: All records, files, documents, regardless of media (e.g., paper, electronic, etc.), as described in the PWS, that are the responsibility of the Contractor are the property of the Government and shall remain so upon termination or completion of the contract. The
Contractor shall keep these items current and maintain and dispose of them in accordance with the requirements established in Air Force Instruction (AFI) 33-322, Records Management Program, Air Force
Manual (AFMAN) 33-363, paragraph 6.4, AFMAN 33-364, paragraph 2.13, and the Air Force Records
Disposition Schedule which can be provided by the COR or CO upon request. Records shall be turned over to the Government upon completion of the contract unless otherwise stated. All records are subject to the Freedom of Information and Privacy Act. The Minot AFB Records Management Office
(5 CS/SCXR), 723-3529/3186, will provide assistance and training to the Contractor to ensure compliance with Federal record keeping requirements.
10. QUALITY CONTROL. The Contractor shall provide a Quality Control Plan (QCP) to the
Contracting Officer, for acceptance, within 10 days of contract award notification. The Contracting Officer will notify the Contractor of acceptance or required modifications to the plan before the contract start date.
The Contractor shall make appropriate modifications and obtain acceptance of the plan by the Contracting
Officer before the contract start date.
10.1. The plan shall include a description of the methods to be used for identifying and preventing defects in the quality of service performed before the level of performance becomes unacceptable as outlined in the
Service Summary. The plan shall include methods and procedures for identifying and preventing defects in the quality of service.
10.2. A description of the inspection system to cover all services listed on the Service Summary (SS). The description shall include specifics as to the areas to be inspected on a scheduled and unscheduled basis, frequency of inspections, and the title and organizational placement of the inspectors.
10.3. Develop, implement and maintain a comprehensive quality control program that assures compliance with all requirements of this PWS. Safety violation(s) and deviation(s) from established technical data will constitute an unsatisfactory condition(s) and will be documented as such. If the Contractor exceeds the http://www.ecmra.mil/ error rate as listed on the Service Summary, they may be subject to a lower performance evaluation rating.
The QCP shall also include procedures for training, qualification and any required certification of employees prior to performance of assigned duties under this contract. Procedures shall include assignment of management responsibility for employee task qualification to specific positions/individuals within the
Contractor’s organization, as well as the quality control process that will be utilized to ensure compliance.
11. QUALITY ASSURANCE. The COR will perform all customer complaint inspections of the MAF
Refuse services to ensure that the required work is being performed. The COR shall validate all customer complaints and strive to have Contractor successfully re-perform all unacceptable performance in a timely manner. The intent of the contract is to have the service provided rather than taking any reduction of payment. However, the Contractor’s satisfactory re-performance of services shall be at no additional cost to the government and shall not remove the status of the validated customer complaint as it applies to the monthly performance threshold stated in the SS.
11.1. When the CORs investigation of a complaint indicates defective performance, the COR will require the Contract Manager or Representative to initial the observation. The initialing of the observation does not necessarily constitute concurrence with the observation, only acknowledgment that he or she has been made aware of the defective performance. Government surveillance of tasks not listed in the SS or by methods other than those listed in the SS (such as provided for by the Inspection for Services Clause) may occur during the performance period of this contract. Such surveillance will be done according to standard inspection procedures or other contract provisions.
11.2. The Contracting Officer may require the Contract Manager to meet with the Contracting Officer, Contract Administrator, COR, and other Government Personnel as deemed necessary. The Contractor may request a meeting with the Contracting Officer when he or she believes such a meeting is necessary.
