Performance Work Statement Calibrate Scales

1.0 DESCRIPTION OF SERVICE.

1.1 Scope of Work: Contractor shall provide all labor, tools, and supervision required to calibrate scale equipment as reflected in the attachment 1. All scales are to be inspected on a bi-annual basis. Scales numbered 1-30 require two (2) calibrations to be performed between 01 April - 30 September and 01 October – 31 March.

Scales 31-44 require two (2) calibrations; one performed between 01 January and 31 March, and the second performed between 01 July and 30 September. All scheduled inspections shall occur at least 90 days from the previous inspection date. In addition to scheduled calibration, the contractor must also have the ability, and be available, to perform unscheduled maintenance and make repairs to the scales when necessary. Response times to unscheduled maintenance are no later than 24 hours after notification from the respective squadron representative. If notification for unscheduled maintenance occurs on Friday, Saturday or Sunday then the contractor shall perform the unscheduled maintenance the next business day.

1.2 Location of Service: Scales are located in buildings 550, 551, 509, 150, 1212 on Dover AFB, DE.

1.3 Applicable Standards: Contractor is required to Inspect, Calibrate, Adjust, and perform necessary repairs on the forty-four (44) scales that are provided on the attached spreadsheet.

1.4 Contractor must be a commercial weighing and measuring device service company licensed to perform in the state of Delaware at the time of award. Technicians performing services under this contract must be certified to perform in the state of Delaware . Contractors must submit proof of certification/licensing to perform in the state of Delaware at the time of proposal submission.

1.5 Signed and dated calibration certificate must be provided for each scale calibrated.

1.6 A squadron representative shall escort the contractor and operate conveying equipment when necessary.

2.0 GENERAL INFORMATION.

The hours of operation are from 7:00AM to 4:00PM, Monday through Friday. The contractor is not required to provide service on the following days: New Year’s Day, Martin Luther King’s Birthday, Washington’s Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day.

3.0. ACCEPTANCE CRITERIA.

Designated squadron representative will determine service acceptability, upon verification of service performed.

4.0 RECORDS MANAGEMENT.

The contractor understands that when creating, handling and maintaining records for the Air Force, either electronic or paper, you must meet the requirements established in AFRIMS Records Disposition Schedule (RDS), AFI 33-322, Records Manage Program, AFI 33-364, Records Disposition Procedures and Responsibilities, and AFM 33-363, Management of Records. In order to meet the requirement established IAW the above mentioned Air Force Instructions; regulations contact the Base Records Management Office (436 CS/ SCXK, Bldg. 919) at extension 3642 for guidance. Contractors must contact the Base Records Manager to receive training in both Air Force Records Management and the Air Force Privacy Act Program.

5.0. SECURITY

5.1 All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the governmental installation shall abide by all security instructions and directives of Dover AFB. Employees are responsible for safeguarding all government property provided for contractor use.

5.2 At the close for each work period, government facilities, equipment and materials shall be secured, lights, heat and water turned off and all doors and window secured.

5.3 The contractor and, as applicable, subcontractor shall not employ persons for work on this contract if such employee is identified as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population, nor shall the contractor or subcontractor employ persons under this contract who have an outstanding criminal warrant as identified during the Criminal Background Check

(CBI).

5.4 Contractors shall ensure their employees and those of their subcontracts have the proper credentials allowing them to work in the United States. Employees and subcontractors later found to be undocumented or illegal aliens will be remanded to the proper authorities.

5.5 The contractor shall not be entitled to any compensation for delays or expenses associated with complying with the provisions of this requirement. Furthermore, nothing in this requirement shall excuse the contractor from proceeding with the contract as required.

5.6 All contractors and subcontractors when working in a Controlled, Restricted or other sensitive areas must be escorted at all times. The military agency or unit responsible for the project or work is responsible for providing the escorts. The Contractor shall follow existing procedures and instructions for obtaining entrance to restricted or controlled areas.

6.0. CONTRACTOR BADGE POLICIES.

6.1 All requests for contractor badges will be submitted through the Base Contracting Squadron Office and/or the requesting agencies base point of contact. The Contracting Squadron Office or base point of contact will then fill out an Operational Risk Management Assessment Form (ORM) for the requesting agency and submit it to the Pass and Registrations section for approval IAW DAFBI 31-101. As a minimum the ORM will be submitted 72-hours prior to the requested date of employment.

6.2 All contactor employees who’ll be granted unescorted access to the installation are required to consent to a Criminal Background Investigation (CBI) prior to being granted entry to the installation. Continued employment is contingent upon successful completion and favorable reporting of the criminal background check.

6.3 Contractor badges will be issued for a maximum period of one year. Prior to reissuing new badges, all old badges must be returned to Pass and Registration for destruction. Also before a new badge will be issued an ORM must be resubmitted to the Pass and Registration section for approval again. In the event a badge is lost or stolen, the badge holder must accomplish the Loss/Theft of Identification Worksheet and provide it to his/her supervisor. The supervisor of the contractor will investigate the loss and report in writing the circumstances in which the badge was lost to the 436th Contracting Squadron and 436th Security Forces Squadron and submit a new request for badge renewal per paragraph 1.8.4.1. before a new badge can be reissued.

6.4 Base Contracting Office or base point of contact will immediately notify Security Forces, Pass and Registration when a contractor’s employment has been terminated. The Agency Chief or Site Supervisors are responsible for notifying and returning the contractor’s badge to Pass and Registration when this occurs. At no time will a contractor contact Security Forces directly regarding badge denial; they need to contact the Contracting Squadron.

7.0. INSTALLATION ACCESS.

7.1 The contractor shall obtain personal contractor identification badge for all employees and vehicle passes for all contractor and personal vehicles requiring entry onto Dover AFB from Pass and Registration, for the duration of the contract. Employees are only permitted to enter the installation during the date and time periods indicted on their contractor badge.

7.2 Vehicle registration, proof of insurance and a valid driver’s license must be presented for all vehicles while operating on the installation. All vehicles entering the installation or sensitive areas are subject to search.

Any refusal or non-consent by an employee will result in termination of their base access and immediate confiscation of this access badge.

7.3 During Force Protection Condition (FPCON) Normal and Alpha, personnel without base issued badges must be sponsored onto the installation.

7.4 During FPCON Bravo, Charlie and Delta, personnel without base issued identification shall be physically escorted onto the installation.

7.5 During Higher FPCONs (Charlie and Delta) the base will curtail non-essential operations/functions;

access by Non-Essential Contractors will be suspended as the direction of the Installation Commander.

8.0 FLIGHTLINE DRIVING. Contractors will have to adhere to the provisions in DAFBI 13-202 if operating on the airfield.

9.0 CONTRACTOR MANPOWER REPORTING.

9.1 The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for scale calibration via data collection site. The contractor is required to completely fill in all required data field at http://www.ecmra.mil.

9.2 Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year, which runs 1 October through 30 September * of each calendar year. Contractor may direct questions to the CMRA help desk.”

9.3 *Reporting Period: Contractors are required to input data by 31 October of each year.

9.4 Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.

9.5 User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link.

However, user manuals for government personnel and contractors are available at the Army CMRA link at http://www.ecmra.mil.

10.0 SOLID AND HAZARDOUS WASTE REMOVAL. The contractor shall dispose of all wastes generated in the performance of their task. No solid waste materials shall be placed in any trash receptacle on http://www.ecmra.mil/

DAFB. If any Hazardous or Special Waste is generated the Contractor will coordinate with the 436 Civil Engineering Natural Resources Flight (436 CES/CEAN) prior to disposal.

11.0 OPSEC.

11.1 436th Contracting Squadron unit OPSEC coordinators will state OPSEC requirements on DD Form 254, contracts and SOW/PWSs with sufficient detail to ensure complete contractor understanding of the exact OPSEC provisions or measures required by the organization. If the OPSEC block is checked on the DD Form 254, the 436th Contracting Squadron unit OPSEC coordinators shall:

11.1.1 Task the contractor to develop an OPSEC program plan to address how the contractor plans to protect critical and sensitive contracted information, and upon organization acceptance, implement the OPSEC program plan.

11.1.2 Provide OPSEC guidance for the contractors to use in developing their own OPSEC plan.

11.2 Based on the classification guidance or OPSEC requirements in the prime contract, the prime contractor is responsible for preparation of DD Forms 254 for any subcontracts. This should be done in coordination with the 436th Contracting Squadron unit OPSEC coordinators and unit security manager.

11.3 For a contractor to effectively comply with OPSEC provisions of the contract, the organization must provide the following guidance:

11.3.1 Organization’s critical information.

11.3.2 Adversaries’ collection threat information as it applies to the organization’s mission and the contract.

11.3.3 Operations security guidance (at a minimum, the organization will provide a copy of this instruction).

11.3.4 Specific OPSEC measures the organization requires (as appropriate).

12.0 ANTITERRORISM AWARENESS LEVEL I TRAINING.

12.1 DoD Contractors supporting a Base Service Contract (e.g., Custodial Services, Ground Maintenance, etc.) are highly encouraged to complete AT Awareness Level I training IAW AFI 10-245, Antiterrorism, Standard 25, by completing the―Force Protection computer based training (CBT) course through the Advanced Distributed Learning System (ADLS) site or the Defense Technical Information Center (DTIC) at https://atlevel1.dtic.mil/ for individuals without access to government Non-classified Internet Protocol Router Network (NIPRNET) computer and who do not possess a CAC card.

13.0 SOLID and HAZARDOUS WASTE REMOVAL.

The following information is required for tracking of hazardous materials on Dover AFB. For contracts exceeding six months, this form is required to be filled out on a monthly basis and returned to the government representative (COR).

For contracts less than six months, this form is required at the beginning and at the completion of the work. The COR will provide a copy to the HAZMART pharmacy. This information is required to comply with State, Federal and Air Force laws and regulations.
