1. DESCRIPTION OF SERVICES. The contractor shall provide all management, labor, equipment, tools, and transportation necessary to provide maintenance, repair, inspection and certification of government owned elevators and personnel lifts at Lajes Field, Azores, Portugal.

1.1. SPECIFIC TASKS

1.1.1. Contractor maintenance and inspection personnel shall be certified in accordance with Decreto-Lei no320/2002, Directive no 95/16/EC, NP EN 81-1 Electric Elevators, NP EN 81-2 Hydraulic Elevators, and any other applicable industry standard codes and regulations. Contractor shall perform work in accordance with the elevator manufacturer’s recommendations and the standards set forth in this Statement of Work (SOW).

1.1.2. Upon completion of maintenance/repair services the contractor shall provide a report written in English that indicates all maintenance and/or service accomplished on each elevator or lift. Any required parts and repairs that exceed the standard maintenance work shall be identified to the contracting officer or contracting officer representative (COR).

1.1.3. The contractor shall not perform work not covered by this statement of work that will add to the cost of the contract, unless authorized in writing by the contracting officer.

1.1.4. The contractor will obtain approval from the contracting officer for additional work or parts and if determined reasonable will be authorized by modification or any other contractual methods as determined most appropriate by the contracting officer. If the contractor performs any additional work or provides parts not covered in this contract without prior approval of the contracting officer, it will be done at the contractor’s own risk and expense.

1.1.5. Preventive Maintenance and Examinations of Elevators and Lifts: Monthly, the contractor shall perform complete preventive maintenance, examination, and repair of all elevators and lifts to ensure reliable and continuous safe operation. In addition to performing the manufacturer’s recommended maintenance, work shall include, but not limited to replacing light fixtures, tubes or bulbs, checking and replacing smoke or heat detectors, as applicable. The contractor shall be responsible to furnish the following parts and supplies: all replaceable type contacts, contact insulators, contact springs, copper braids, all oils and greases, and cleaning materials. The contractor is also liable for replacement parts with a value up to 145.00€. Replacement parts with an individual value above 145.00€ shall be invoiced as “Reimbursable Items” under the assigned CLIN.

1.1.6. The contractor shall annotate the maintenance records in the cabs of all elevators and lifts immediately following completion of preventative maintenance.

1.1.7. The contractor shall prepare and submit a written report within two (2) days of the results of the preventive maintenance and examination to the contracting officer representative. This report shall include a checklist which states all items inspected/serviced for each unit. It shall also include a certification statement on the condition of each unit serviced and any repairs needed. If repairs require work considered over and above routine maintenance work, the contractor shall notify the contracting officer representative for appropriate action. The contractor shall commence repair work only after notification from the contracting officer.

1.2. REPAIRS/SERVICE CALLS.

1.2.1. Routine Service. The contractor shall report to the work location and survey the repair within four (4) hours from a repair or service call, or first thing next duty day if notified with less than four hours to respond within duty hours. The contractor shall provide the contracting officer an estimated time to repair and the total cost of repairs. All repair work shall be in accordance with normal commercial practices using parts specified by the elevator manufacturer or items of equal or better quality. All replacement parts shall be original manufacturer's replacement parts or equal. All parts shall be new.

If the contractor is unable to perform all repairs within duty hours of the same day, contractor shall return next day during duty hours to proceed with pending task.

1.2.2. Urgent and Emergency Service. The contractor shall respond within two (2) hours from calls for a service of urgent or emergency nature. Service of this type consists of problems that endanger life or property, problems that have an adverse impact upon personnel or equipment, and/or problems that inhibit mission accomplishment. Emergency calls may require work outside normal duty hours.

1.2.3. The contractor shall maintain a log of all interim calls and repairs reflecting the name of the person that placed the call, time of call, nature of the call (Emergency, Urgent, or Routine), building number, problem, solution, time in and time out of facility, parts replaced, and a breakdown of labor hours and material costs. The contractor shall provide a copy of the repair log to the contracting officer representative at least monthly.

1.3. INSPECTION AND TESTS. A Portuguese government certified inspector shall perform inspections and tests in accordance with Decreto-Lei no 320/2002. The contractor is responsible for costs and scheduling of these inspections with the government inspector and shall inform the COR of the inspection’s status.

1.3.1. Any other tests and inspections required by safety, fire, or elevator codes or called for by the government shall be the responsibility of the contractor. The results should conform to the requirements of the standards listed in Paragraph 1.3 and shall contain the same warranty as specified in 1.4. Inspection results should be posted in accordance with Paragraph 1.4.

1.3.2. If an elevator or lift presents a threat to personal safety or property, or violates fire or safety regulations, it will be shut down. The contractor shall notify the contracting officer representative (COR) or contracting officer immediately and shall place a red tag to note the problem(s), corrective action required, date, and either mechanic, inspector, or quality control inspector’s names.

1.4. WARRANTY. Upon completion of each installation; renovation; examination or quality control inspection the contractor shall warrant that the elevators and material lifts are in compliance with all mandatory requirements. The contractor shall use a warranty plate or sticker, displayed in a conspicuous location (e.g., inside passenger elevator cab) to cover this requirement. If the contractor makes major changes in the configuration of the unit, the contractor is responsible for providing new wiring diagrams or as-built drawings.

2. SERVICE SUMMARY (SS). The contract service requirements are summarized in performance standards that relate directly to mission success. The performance threshold briefly describes the minimally acceptable levels of service required for each requirement. The SS provides information on contract requirements, the expected level of contractor performance (performance threshold) and the expected method of assessment by the government to ensure services have been provided. These thresholds are a key element to mission success.

Procedures as set forth in the FAR 52.212-4 (a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance, will be used to remedy all deficiencies. During the first initial 30 days of the contract, two additional errors on each task shall be allowed in an effort to identify normal phase-in problems, except for any periodic service.

3. GENERAL INFORMATION

3.1 Security Forces and Fire Protection. In the event of an emergency you may dial 911 from an on base phone, or 295-571-911 from an off base line or cell phone.

3.2 Personnel Language Requirements. Contractor manager shall speak and write fluent English.

3.3 Quality Control Plan. The plan shall include, as a minimum, a description of the inspection system covering all services listed in the SOW. The quality control plan shall address the frequency of inspections, procedures to identify, prevent, and ensure non-recurrence of defective services.

3.4 Records. The contractor shall maintain all records of inspections. Records shall include the dates of inspections, what was inspected, discrepancies found, and corrective action taken. This documentation shall be kept and made available when requested by the contracting officer or his designated representative.

3.5 Work Schedules. The contractor shall arrange his performance of work to minimize interference with normal conduct of government business. The contractor shall establish a work schedule, which includes all required tasks, for each area for all buildings. Any changes made to the scheduled tasks and/or days of performance shall be submitted to the contracting officer in writing at least five days prior to the effective date.

3.6 Hours of operation. Normal Base operations hours are Monday through Friday, 0800 to 1700. There may be mission situations that require the contractor to work other than normal hours. Some of these situations are military exercises, contingency operations, or emergencies. When the contractor’s access to a work area conflicts with the Air Force’s mission requirements, the contractor shall (re) schedule the work to minimize any disruptions. Such scheduling may require work to be accomplished at times other than normal duty hours.

3.7 National Portuguese Holidays. The contractor is not required to provide routine services on the following

Down/Family/Goal/Minimum Manning Days. The contracting officer representative shall coordinate with facility managers and the contractor, to schedule work on the following days: Goal, Down, Family and/or Minimum Manning days. If access and/or required escort(s) are provided, then the contractor shall perform the regularly scheduled service.

3.8 Prohibitions: The contractor shall not use any of the following hazardous material: Lead-based paints containing 0.5 percent or greater, by weight, of lead. Products containing Class I Ozone Depleting Chemicals (ODC's) are not to be used. Paints, paint thinners, solvents, or strippers with 3.5 pounds per gallon or more volatile organic compound content.

3.9 Asbestos/Lead: If suspected asbestos or lead based paint-containing materials are encountered when performing any work under this contract, the contractor shall immediately stop work and notify the contracting officer or QAP. Work will resume only after the area has been cleared by testing or by a certified government inspector.

3.10 Base Access. Contractor personnel shall maintain current base passes at all times.

3.11 Smoking Policy: All contractor employees shall comply with the Air Force smoking policy (AFI 40-102

Tobacco Use in the Air Force, section 2) which states smoking is not allowed in any DoD facility.

4. SUBMITTALS: The contractor shall submit items listed below a minimum of 10 days after pre performance conference.

a. Inspection, Certification, and Maintenance Schedule - Contractor shall provide a detailed inspection and maintenance schedule (task, day, and time) and Periodic (month) schedule for each elevator and lift to be serviced. Any changes shall be submitted to contracting officer 10 days prior to the change.

b. Safety Data Sheet (SDS) - Contractor shall provide an English & Portuguese SDS for each chemical to be used by contractor employees.
