INTRODUCTION. This Quality Assurance Surveillance Plan (QASP) has been developed to evaluate the contractor's actions while implementing this performance work statement (PWS). This document is designed to provide an effective surveillance method of monitoring the contractor's performance for each listed objective on the Service Delivery Summary (SDS) for the rental of washers and dryers.

The QASP provides a systematic method to evaluate the services that the contractor is required to furnish.

This QASP is based on the premise that the government requires a quality standard for the rental of washers and dryers. This QASP is the best means of achieving that objective.

The contractor, and not the government, is responsible for the management and quality control actions required to meet the terms for the rental of washers and dryers requirements. The role of the government is quality assurance to ensure contract standards are achieved.

In this contract the quality control program is the driver for product quality. The contractor is required to develop a comprehensive program of inspections and monitoring actions. The first major step to ensuring a “self-correcting” contract is to ensure that the quality control program provides the measures needed for the contractor's success.

Once the quality control program is approved, careful application of the process and standards presented in the remainder of this document will ensure a robust quality assurance program.

SERVICE DELIVERY SUMMARY:

Installation/Transition. Provide and install washers and dryers in accordance with approved installation plan

1.2 All washers and dryers shall be installed in the allotted approved timeframe

Service Calls. Repair and replace washers and dryers within 2 days 1.3 All repairs shall be completed 98% of the allotted time

Removal/Relocation. Install, relocate or remove washers and dryers within 7 days 1.4

All work completed within the allotted time 98% of the time

Reports/Documentation. Provides reports and documentation 1.5

All reports and documentation are current/available 100% of the time.

Manpower Reports. Provides annual labor data documentation. 1.6 All required documentation provided 100% of the time.

Equipment Removal. Remove washers and dryers in accordance with approved removal plan.

1.2 All washers and dryers shall be removed in the allotted approved timeframe

SURVEILLANCE: The Contracting Officer’s Representative (COR) will ascertain the performance objectives through inspections during each service cycle as well as complaints from the using activity personnel. These complaints will be forwarded to the contractor's Quality Control Inspector (QCI) for correction.

STANDARD: The contractor is responsible to meet the firm standards in the above SDS. A Notice of Violation (NOV) is absolutely not permitted. The contractor shall perform all work required by in a satisfactory manner in accordance with the appropriate PWS paragraph. The COR shall not consider the services complete until all deficiencies have been corrected.

PROCEDURES: The COR will review performance objectives annually and periodically to ensure the contractor complies with the appropriate paragraphs of the PWS. The COR will also inspect all complaints form the using activity personnel to ensure the contractor complies with the appropriate paragraphs of the PWS. The COR shall record the results of inspection, noting the date and time of inspection. If inspection indicates unacceptable performance, the COR will notify the contract manager or the QCI of the deficiencies for correction. The contractor shall be given one hour after notification to begin correction of the unacceptable performance. Additional time might be approved if the COR considers that additional time is warranted. If the deficiencies corrections are not initiated within the required time frame, or if the deficiencies are not corrected in a timely manner, the COR shall notify the Contracting Officer (CO) for rectification. Recurring customer complaints are not permitted for any of these service items. If repeated customer complaints are received indicating the same deficiency during the service performance period, the COR shall contact the CO for appropriate action.
