ADDENDUM TO 52.212-1, Instructions to Offerors--Commercial Items. PROPOSAL PREPARATION INSTRUCTIONS A. 52.212-1(c) Period of acceptance of offers is hereby changed from 30 calendar days to 60 calendar days.

B. 52.212-1(l) Debriefing, is deleted in its entirety- any requests for award information will be provided in accordance with FAR 13.106-3(d) and FAR 15.503(b)(2).

C. To assure timely and equitable evaluation of the offer, the offeror must follow the instructions contained herein. The offer must be complete, self-sufficient, and respond directly to the requirements of this solicitation. The response shall consist of two (2) separate parts; Part I – Price, Part II – Technical Capability.

D. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional data will be requested. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit information other than cost or pricing data to support a determination of price reasonableness and affordability of the price.

E. Specific Instructions:

1. PART I – PRICE QUOTE – Submit one (1) copy.

(a) Insert proposed unit and extended prices in the SF 1449 Solicitation. The extended amount must equal the whole dollar unit price multiplied by the number of units. In accordance with the pricing exhibit, the proposal(s) must be submitted for a base year plus four (4) Option Years and one (1) 6-month extension.

2. PART II – TECHNICAL CAPABILITY – Technical capability consist of the following subfactors. Contractor shall submit information requested for each subfactor to be considered for award. Submit one (1) copy.

(a) Subfactor 1 – Quality Control Approach: Demonstrate preliminary quality control approach to ensure all services listed on the Service Summary (SS) (Paragraph 6.0) are performed and inspected in accordance with the standards of the PWS. The final quality control plan will be submitted within (10) days of award of the contract. Quality Control Approach shall not exceed 8 pages, letter-size 8.5 by 11, 1” margins, 12 pt. Font, Times New Roman.

(b) Subfactor 2 – Prior experience: Prior experience will be based on recent, relevant contracts only. Recent contracts are those that were completed within 3 years from the date of the solicitation. Provide any information currently available which demonstrates experience for relevant past efforts/contracts. Relevant contracts include those that are similar in scope and magnitude of effort and complexity to this solicitation requirements. Relevant experience includes performance of efforts involving Integrated Solid Waste Management duties that are similar or greater in scope, magnitude, and complexity of the effort described in this solicitation. The government will evaluate the extent of offeror's experience deemed relevant to the requirements of this solicitation. Recent experience is work that is currently being accomplished or has been accomplished in the last three (3) years.

(1) Prior Experience Surveys: Provide a list of no more than five (5), of the most relevant contracts performed for Federal agencies and commercial customers within the last three (3) years. The government will use information submitted by the offeror and any other sources of information available to the Government to assess prior experience (i.e. FAPIIS, CPARS). For a description of the characteristics or aspects the Government will consider in determining relevance, see addendum to FAR 52.212-2 "Basis for Contract Award." The evaluation of prior experience information will take into account prior experience information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the instant acquisition. Furnish the following information for each contract listed:

(i) Company/Division name

(ii) Product/Service

(iii) Contracting Agency/Customer

(iv) Contract Number

(v) Contract Dollar Value

(vi) Period of Performance

(vii) Scope of work, magnitude and complexity of the effort

(viii) Verified, up-to-date name, address, FAX & telephone number of the contracting officer

(c) Subfactor 3 – Crisis Action Plan: The offeror shall provide with its offer a written plan describing how it will continue to perform the essential contractor services listed in Attachment 1 PWS, Continuation of Essential Contractor Services During Crisis (Para 8.6), dated 5 June 2019 during periods of crisis. The offeror shall–

(1) Identify provisions made for the acquisition of essential personnel and resources, if necessary, for continuity of operations for up to 30 days or until normal operations can be resumed;

(2) The plan shall at a minimum address the following:

(i) Challenges associated with maintaining essential contractor services during an extended event, such as a pandemic that occurs in repeated waves;

(ii) The time lapse associated with the initiation of the acquisition of essential personnel and resources and their actual availability on site;

(iii) The components, processes, and requirements for the identification, training, and preparedness of personnel who are capable of relocating to alternate facilities or performing work from home;

(iv) Any established alert and notification procedures for mobilizing identified “essential contractor service” personnel; and

(v) The approach for communicating expectations to contractor employees regarding their roles and responsibilities during a crisis.

(vi) The Crisis Action Plan shall not exceed 10 pages, letter-size 8.5 by 11, 1” margins, 12 pt. Font, Times New Roman.

52.212-2 Evaluation – Commercial Items

BASIS FOR AWARD

1.Award will be made in accordance with FAR Subpart 13.5. This acquisition will utilize the Lowest Price Technically Acceptable (LPTA) technique to make a best value award decision. The application of the LPTA technique in the contract award selection and approval process is as follows.
2.Price: Initially, all offers will be ranked according to price, including option prices. An offeror's proposed prices will be determined by multiplying the quantities identified in the schedule by the proposed unit price for each Contract Line Item Number (CLIN) to confirm the extended amount for each. All extended CLIN amounts will be totaled for evaluation purposes. Individual pricing for each line must be completed. Price will be evaluated for reasonableness. Reasonableness will be determined based on prices submitted by the competition, current market conditions, and comparison to the Government estimate, if applicable. If the government deems this information is not sufficient to determine reasonableness, the government may use other techniques as appropriate to determine price reasonableness for the acquisition.
3.Technical Capability: Starting with the lowest priced quote the subfactors for technical capability shall be evaluated by the Government on an acceptable/unacceptable basis. For an offeror to be eligible for award all subfactors must have a rating of acceptable. If the offeror is determined to be unacceptable the evaluation will stop and the next lowest quote will be evaluated. This will continue until a quote with acceptable technical capability is found.

Subfactor 1 – Quality Control Approach: The technical team will evaluate the Quality Control Approach to determine if the approach appropriately and adequately addresses all services listed in the SS (Para 6.0) of the PWS. The offeror’s Quality Control Approach will be assigned a rating of acceptable/unacceptable base on the following:

Acceptable: Quality Control Approach appropriately and adequately addressed all services listed in the Services Summary of the PWS.

Unacceptable: Quality Control Approach does not appropriately and adequately address all services listed in the Services Summary of the PWS.

Subfactor 2 – Prior Experience: The contracting officer will seek relevant and recent (within 3 years from the date of solicitation experience information based on (1) the information provided by the offeror and (2) data independently obtained from other Government and commercial sources. Relevant experience includes performance of efforts involving Integrated Solid Waste Management that are similar or greater in scope, magnitude, and complexity of the effort described in this solicitation.

The purpose of the prior experience evaluation is to allow the Government to assess the offeror's ability to perform the effort described in this solicitation based on the offeror's demonstrated present and prior experience. Relevant information will be determined based on the following:

Offerors relevant prior experience will be assigned a rating of acceptable/unacceptable based on the following:

Acceptable: The offeror has recent and relevant prior experience.

Unacceptable: The offeror does not have recent and/or relevant prior experience.

Subfactor 3 – Crisis Action Plan: The technical team will evaluate the Crisis Action Plan to determine if the approach appropriately and adequately addresses all services listed in the Continuation of Essential Contractor Services During Crisis (Para 8.6) of the PWS. The offeror’s Crisis Action Plan will be assigned a rating of acceptable/unacceptable base on the following:

Acceptable: Crisis Action Plan appropriately and adequately addressed all aspects Continuation of Essential Contractor Services During Crisis (Para 8.6) of the PWS.

Unacceptable: Crisis Action Plan does not appropriately and adequately address all aspects Continuation of Essential Contractor Services During Crisis (Para 8.6) of the PWS.

5.Offerors may be asked to clarify certain aspects of their offer (for example, the relevance of prior experience) to which the offeror has not previously had an opportunity to respond. Clarifications conducted to resolve relevance of the prior experience or minor or clerical errors will not constitute discussions and the contracting officer reserves the right to award a contract without the opportunity for revisions.
6.The Government intends to award a contract without discussions with respective offerors. Therefore, each initial quote must address information in accordance with the paragraphs above and should contain the offeror's best price. The Contracting Officer, however, reserves the right to enter discussions if it is determined to be in the best interest of the Government.
7.Award will be made to the lowest priced technically acceptable offeror who is responsive to the terms of the solicitation.

SITE VISIT

A site visit will be held on 20 August 2019 at 9:00 AM CST. The site visit will start at the visitor center. If you are planning to attend the site visit, notify SSgt Larry Gilpatrick, at larry.gilpatrick@us.af.mil at least 24 hours in advance. On the day of the site visit arrive at the visitor center by 8:30 AM CST, to allow time for processing base passes prior to the start of the site visit.

NOTE: Attendance to the site visit is not mandatory, offerors or quoters are highly encouraged to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable. In no event shall failure to inspect the site constitute grounds for a claim after contract award.
