About this file

This is a Past Performance Questionnaire (PPQ) template designed for the 60th Contracting Squadron at Travis Air Force Base to evaluate contractor performance on access control system replacement projects. The document requests that recent customers of prospective offerors complete the questionnaire to provide performance ratings on similar work. The PPQ is structured to gather offeror information, respondent company/organization details, and specific contract performance data including contract name, description, performance period, and total contract value.

The questionnaire employs a five-point performance rating scale (1=Unsatisfactory through 5=Exceptional) across six evaluation categories: quality of service, on-time delivery of service, management of service, communication, contract terms and conditions adherence, and overall contractor performance. Respondents must provide narrative explanations for any ratings of 1, 2, or 5, and answer whether the contractor was issued a Corrective Action Report or similar deficiency report. The template also includes a yes/no question regarding willingness to award future contracts to the same contractor. Completed questionnaires must be submitted as PDF files to the points of contact identified in the solicitation. This template is referenced in relation to solicitation FA442726Q1128 for Building 179 Access Control System Replacement and Intrusion Detection System Upgrade, a Total Small Business Set-Aside opportunity with an RFQ response deadline of September 2, 2026.
