About this file

This file is a Past-Present Performance Questionnaire template designed for contractors to document their relevant experience and qualifications in response to the 60 FSS IT Specialist Travis AFB solicitation. The questionnaire serves as an appendix to the RFQ and does not specify required products or services; rather, it functions as an evaluation tool for assessing contractor capability and past performance. Contractors are expected to complete this questionnaire as part of their technical proposal to demonstrate experience with similar IT technician services, relevant project history, and organizational capacity to perform the contracted work over the base year plus four option years with an optional six-month extension.

The questionnaire template is intended to support the evaluation criteria outlined in the Performance Work Statement (PWS) sections addressing tasks, qualifications, duty hours, and security requirements. As a standard evaluation document, it requires contractors to provide documented evidence of past performance on comparable contracts, references from previous clients, and details on personnel qualifications. Submission of the completed questionnaire is required by the RFQ response deadline of August 14, 2026 at 12:00 PM PDT, with all responses submitted electronically via email to the contracting officers at Travis Air Force Base, Air Mobility Command.
