CONTRACTOR'S QUALITY CONTROL

1. GENERAL.

1.1. Contractor(s) shall be required to institute a comprehensive Quality Control Program in order to assure the Government that all materials and workmanship are in strict accordance with the provisions of the contract. Within fifteen (15) calendar days of contract award, the contractor shall submit a Quality Control Plan for Government review, approval, and incorporation into the contract. The contractor's Quality Control representative shall meet at least the minimum standards as required by federal, state, and local construction practices; while at the same time performing only Quality Control functions.

1.2. Establish and maintain a QC program as described in this section. The QC program consists of a QC Organization, a QC Plan, a QC Plan Meeting, a Coordination and Mutual Understanding Meeting, QC meetings, three phases of control, submittal review and approval, testing, completion inspections, and QC certifications and documentation necessary to provide materials, equipment, workmanship, fabrication, construction and operations which comply with the requirements of this Contract. The QC Program shall cover on-site and off-site work and shall be keyed to the work sequence. No work or testing may be performed unless the QC Manager is on the work site. The QC Manager shall report directly to an officer of the firm and shall not be the same individual as, nor be subordinate to the project superintendent or the project manager.

The QC Manager, Project Superintendent and Project Manager must work together effectively.

Although the Quality Control Manager is the primary individual responsible for quality control, all three individuals will be held responsible for the quality of the work on the job. The Project Superintendent will be held responsible for the quality of production work.

The contractor is responsible for quality control and shall establish and maintain an effective quality control system in compliance with the Contract Clause entitled "Inspection of Construction." The quality control system shall consist of plans, procedures, and organization necessary to manage all delivery orders to produce end products, which comply with the contract requirements. The government will hold the contractor contract/project manager responsible for the quality of work on the job and is subject to removal by the CO for non-compliance with quality requirements specified in the contract.

3. PAYMENT.

3.1. Separate payment will not be made for providing and maintaining an effective Quality Control program, and all costs associated therewith shall be included in contractor's coefficient.

4. QUALITY CONTROL PLAN.

4.1. In accordance with the paragraph titled "Quality Control" in Section H of the solicitation, the contractor shall furnish for review by the government the contractor's Quality Control (QC) Plan proposed to implement the requirements of the Contract Clause entitled "Inspection of Construction." The plan shall identify personnel, procedures, control, instructions, test, records, and forms to be used. Construction will be permitted to begin only after acceptance of the QC Plan or acceptance of an interim plan applicable to the particular feature of work to be started.

Work outside of the features of work included in an accepted interim plan will not be permitted to begin until acceptance of a QC Plan or another interim plan containing the additional features of work to be started.

4.2. Content of the QC Plan.

The QC Plan shall include, as a minimum, the following to cover all construction operations, both on site and off site, including work by subcontractors, fabricators, suppliers, and purchasing agents:

A) A description of the quality control organization, including a chart showing lines of authority~ As a minimum, the staff shall include a QC manager whose sole responsibility is to implement the QC plan.

B) The name, qualifications (in resume format), duties, responsibilities, and authorities of each person assigned a QC function.

C) A copy of the letter to the QC manager signed by an authorized official of the firm which describes the responsibilities and delegates sufficient authorities to adequately perform the functions of the QC manager, including authority to stop work which is not in compliance with the contract. The QC manager shall issue letters of direction to all other various quality control representatIves outlining duties, authorities, and responsibilities. Copies of these letters will also be furnished to the Government.

D) Procedures for scheduling, reviewing, certifying, and managing submittals, including those of subcontractors, off site fabricators, suppliers, and purchasing agents. These procedures shall be in accordance with Section 01301 SUBMITTAL PROCEDURES.

E) Procedures for tracking preparatory, initial, and follow-tip control phases and control, verification, and acceptance tests including documentation.

F) Procedures for tracking construction deficiencies from identification through acceptable corrective action. These procedures will establish verification that identified deficiencies have been corrected.

G) Reporting procedures, including proposed reporting formats.

H) A listing of outside organizations such as, architectural and consulting engineering firms that will be employed by the Contractor and a description of the services these firms will provide.

I) Testing laboratory information required by the paragraphs entitled "Accredited Laboratories" or "Testing Laboratory Requirements", as applicable.

J) A Testing Plan and Log that includes the tests required, referenced by the specification paragraph number requiring the test, the frequency, and the person responsible for each test.

K) Procedures to identify, record, track and complete rework items.

L) Documentation procedures, including proposed report formats.

M) List of definable features of work. A definable feature of work (DFOW) is a task which is separate and distinct from other tasks and requires separate quality control requirements. A DFOW could be identified by different trades or disciplines or by an item or activity on the construction schedule. Although each specification section could be considered a definable feature of work there frequently is more than one definable feature of work under a particular section. The list shall be cross-referenced to the contractor's Construction Schedule and the specification sections. For projects requiring a Progress Chart, the list of definable features of work shall include but not be limited to all items of work on the schedule.

N) Procedures for performing the Three Phases of Control. For each DFOW, provide the DFOW's Preparatory and Initial Phase Checklists. Each list shall include a breakdown of quality checks that will be used when performing the quality control functions, inspections, and tests required by the contract documents. The Preparatory and Initial Phases and meetings shall be conducted with a view towards obtaining quality construction by planning ahead and Identifying potential problems for each definable feature of work.

0) Procedures for Identifying and documenting the completion inspection process.

Include in these procedures the responsible party for punch out inspection, prefinal inspection, and final acceptance inspection.

4.3. Coordination Meeting.

After the Pre-construction Conference, before issuance of any delivery order under this contract, and prior to acceptance by the Government of the QC Plan, the contractor shall meet with the CO or authorized representative and discuss the contractor's quality control system. The QC Plan shall be submitted for review a minimum of 5 calendar days prior to the Coordination Meeting.

During the meeting, a mutual understanding of the system details shall be developed, including the forms for recording the QC operations, control activities, testing, administration of the system for both on site and off site work, and the interrelationship of contractor's management and control with the government's quality assurance. Minutes of the meeting shall be prepared by the government and signed by both the contractor arid the CO. The minutes shall become a part of the contract file. There may be occasions when subsequent conferences will be called by either party to reconfirm mutual understandings and/or address deficiencies in the QC system or procedures which may require corrective action by the contractor.

4.4. Acceptance of Plan.

Government acceptance of the contractor's plan is required prior to the award of any delivery order. The government reserves the right to require the contractor to make changes in his QC Plan and operations including removal of personnel, as necessary, to obtain the quality specified.

4.5. Notification of Changes.

After acceptance of the QC Plan, the contractor shall notify the CO in writing of any proposed change. Proposed changes are subject to acceptance by the CO.

4.6. Quality Control Organization.

General: The requirements for the QC organization are a QC manager and sufficient number of additional qualified personnel to ensure contract compliance. The contractor shall provide a QC organization that shall be at the site at all times during progress of the work and with complete authority to take any action necessary to ensure compliance with the contract.

QC manager: The Contractor shall identify as QC manager an individual within his organization at the site of the work who shall be responsible for overall management of QC and have the authority to act in all QC matters for the contractor. The QC manager shall be a construction person with a minimum of five (5) years in related work. This QC manager shall be on the site at all times during construction and will be employed by the prime contractor. The QC manager shall be assigned no other duties. The contractor shall identify an alternate for the QC manager in the phm to serve in the event of the manager's absence.

QC Inspectors: In addition to the QC manager, the contractor shall provide as part of the QC organization inspectors to assist the QC manager for every CSI discipline including: electrical, mechanical, civil, structural, environmental, architectural, and materials technician. These individuals shall be directly employed by the prime contractor; be responsible to the QC manager; be physically present at the construction site during work on their areas of

TRAVIS AFB - SABER tit 'ontract FA4427-10-R-OI02 responsibility; and have the necessary education and/or experience to be qualified to inspect their trade. These individuals may perform other duties but must be allowed sufficient time to perform their assigned quality control duties as described in the Quality Control Plan.

Advisors: The contractor shall have access to professional advisors for each discipline capable of physically visiting the job site within 48 hours of notification to design and resolve specific technical issues as required. These professional advisors shall have a minimum education and/or experience in accordance with the experience matrix listed herein.

Additional Requirement: In addition to the above experience and education requirements the QC manager and QC inspectors shall have completed a course in Construction Quality Management for Contractors or have comparable experience in construction quality control.

Organizational Changes:. When it is necessary to make changes to the QC staff the contractor shall revise the QC Plan to reflect the changes and submit the changes to the CO for acceptance.

The contractor shall adjust the number of inspectors assigned to the organization dependent upon the number of projects underway and their complexity. However, the number of inspectors assigned shall be sufficient to allow each inspector to thoroughly inspect each project in progress at least once a day and to meet any additional QC functional requirements as specified herein.

5. THREE PHASES OF CONTROL

The three phases of control shall adequately cover both on-site and off-site work and shall include the following for each definable feature of work.

5.1. Preparatory Phase

Notify the Contracting Officer at least 2 work days in advance of each preparatory phase. This phase shall include a meeting conducted by the QC Manager and attended by the QC specialist, the superintendent, and the foreman responsible for the definable feature. Document the results of the preparatory phase actions in the daily Contractor Quality Control Report. Perform the following prior to beginning work on each definable feature of work:

a. Review each paragraph of the applicable specification sections;

b. Review the Contract drawings;

c. Verify that appropriate shop drawings and submittals for materials and equipment have been submitted and approved. Verify receipt of approved factory test results, when required;

d. Review the testing plan and ensure that provisions have been made to provide the required QC testing;

e. Examine the work area to ensure that the required preliminary work has been completed, including ensuring any required excavation permits have been issued;

f. Examine the required materials, and equipment and sample work to ensure that they are on hand and conform to the approved shop drawings and submitted data;

g. Discuss construction methods, construction tolerances, workmanship standards, and the approach that will be used to provide quality construction by planning ahead and identifying potential problems for each definable feature of work; and

h. Review the safety plan and appropriate activitY hazard analysis to ensure that applicable safety requirements are met, and that required Material Safety Data Sheets (MSDS) are submitted. .

5.2. Initial Phase

Notify the Contracting Officer at least 2 work days in advance of each initial phase. When construction crews are ready to start work on a definable feature of work, conduct the initial phase with the QC Specialists, the superintendent, and the foreman responsible for that definable feature of work. Observe the initial segment of the definable feature of work to ensure that the work complies with Contract requirements. Document the results of the initial phase in the daily Contractor Quality Control Report. Repeat the initial phase for each new crew to work on-site, or when acceptable levels of specified quality are not being met. Perform the following for each definable feature of work:

a. Establish the quality of workmanship required;

b. Resolve conflicts;

c. Ensure that testing is performed by the approved laboratory, and

d. Check work procedures for compliance with the Safety Plan and the appropriate activity hazard analysis to ensure that applicable safety requirements are met.

5.3. Follow-Up Phase

Perform the following for on-going work daily, or more frequently as necessary until the completion of each definable feature of work and document in the daily Contractor Quality Control Report:

a. Ensure the work is incompliance with Contract requirements;

b. Maintain the quality of workmanship required;

c. Ensure that testing is performed by the approved laboratory;

d. Ensure that rework items are being corrected; and

e. Perform safety inspections and document on daily reports.

5.4. Additional Preparatory and Initial Phases

Additional Preparatory and Initial Phases shall be conducted on the same definable features of work if the quality of on-going work is unacceptable, if there are changes in the applicable QC organization, if there are changes in the on-site production supervision of work crew, if work on a definable feature is resumed after substantial period of inactivity, or if other problems develop.

5.5. Notification of Three Phases of Control for Off-Site Work

Notify the Contracting Officer at least two weeks prior to the start of the preparatory and initial phases.

6. SUBMITTAL REVIEW AND APPROVAL

Procedures for submission, review and approval of submittals are described in section entitled "Submittal Procedures."

7. TESTING

Except as stated otherwise in the specification sections, perform sampling and testing required under this Contract.

7.1. Accreditation Requirements

Construction materials testing laboratories performing work on SABER construction contracts will be required to submit the following:

a. A copy of the Certificate of Accreditation and Scope of Accreditation by an acceptable laboratory accreditation authority. Construction materials testing laboratories performing work for Navy construction contracts must be accredited by one of the laboratory accreditation authorities. The laboratory's scope of accreditation must include the ASTM standards listed in the paragraph entitled "Construction Materials Testing Laboratory Requirements" as appropriate to the testing field. The policy applies to the specific laboratory performing the actual testing, Ilot just the "Corporate Office".

7.2. Construction Materials Testing Laboratory Requirements

Provide an independent testing laboratory accredited by an acceptable accreditation program to perform sampling and tests required by this contract. Testing laboratories that have obtained accreditation by an acceptable laboratory accreditation authority listed in the paragraph entitled "Laboratory Accreditation Authority": submit to the Contracting Officer, a copy of the Certificate of Accreditation and Scope of Accreditation. The scope of the laboratory's accreditation shall include the test methods required by the Contract. For testing laboratories that have not yet obtained accreditation by an acceptable laboratory accreditation authority listed in the paragraph entitled "Laboratory Accreditation Authority" submit an acknowledgment letter from one of the laboratory accreditation authorities indicating that the application for accreditation has been received and the accreditation prqcess has started, and submit to the Contracting Officer for approval, certified statements, signed by an official of the testing laboratory attesting that the proposed laboratory, meets or conforms to the ASTM standards listed below as appropriate to the testing field. All testing/sampling shall be performed by approved laboratory personnel and not by QC specialist or the QC manager.

a. Laboratories engaged in testing of construction materials shall meet the requirements of ASTM E 329.

b. Laboratories engaged in testing of concrete and concrete aggregates shall meet the requirements of ASTM cion.

c. Laboratories engaged in testing of bituminous paving materials shall meet the requirements of ASTM D 3666.

d. Laboratories engaged in testing of soil and rock, as used in engineering design and construction, shall meet the requirements of ASTM D 3740.

e. Laboratories engaged in inspection and testing of steel, stainless steel and related allows will be evaluated according to ASTM A 880.

f. Laboratories engaged in nondestructive testing (NDT) shall meet the requirements of

ASTME 543.

g. Laboratories engaged in Hazardous Materials Testing shall meet the requirements of OSHA and EPA.

7.3. Laboratory Accr~ditation Authorities

Laboratory Accreditation Authorities are the National Voluntary Laboratory Accreditation Program (NVLAP) administered by the National Institute of Standards and Technology, the American Association of State Highway and Transportation Officials (AASHTO) program, ICBO Evaluation Service, Inc. (lCBO ES), and the American Association for Laboratory Accreditation (AALA) program.

Furnish to the Contracting Officer, a copy of the Certificate of Accreditation and Scope of Accreditation. The scope of the laboratory's accreditation shall include the test methods required by the Contract.

7.4. Capability Check

The Contracting Officer retains the right to check laboratory equipment in the proposed laboratory and the laboratory technician's testing procedures, techniques, and other items pertinent to testing, for compliance with the standards set forth in this Contract.

7.5. Test Results

Cite applicable Contract requirements, tests or analytical procedures used. Provide actual results and include a statement that the item tested or analyzed conforms or fails to conform to specified requirements. If the items fail to conform, notify Contracting Officer immediately.

Conspicuously stamp the cover sheet for each report in large red letters 'CONFORMS" or "DOES NOT CONFORM" to the specification requirements, whichever is applicable. Test results shall be signed by a testing laboratory representative authorized to sign certified test reports. Furnish the signed reports, certifications, and other documentation to the Contracting Officer via the QC Manager. Furnish a summary report offield tests at the end of each month.

Attach a copy of the summary report to the last daily Contractor Quality Control Report of each month.

7.6. Test Reports and Monthly Summary Report of Tests

The QC Manager shall furnish the signed reports, certifications, and a summary report of field tests at the end of each month to the Contracting Officer. Attach a copy of the surniriary report to the last daily Contractor Quality Control Report of each month. '

8. Design Submittals.

The QC manager shall review all design submittals as specified in Section 01300 SUBMITT AL PROCEDURES. The QC organization shall be responsible for certifying that all submittals are in compliance with the contract requirements.

9. ,Documentation.

The Contractor shall maintain current records providing documented evidence that required quality control activities and/or tests have been performed. These records shall include the work of subcontractors and suppliers and shall be on a form acceptable to the CO that includes, as a minimum, the following information:

Contractor/subcontractor and their area of responsibility.

Operating plant/equipment with hours worked, idle, or down for repair.

Workperformed each day, giving location, description, and by whom.

Test and/or control activities performed with results and references to specifications/drawings requirements. List deficiencies noted along with corrective action.

Quantity of materials received at the site with statement as to acceptability, storage, and reference to specifications/drawings requirements.

Submittals reviewed, with contract reference, by whom, and action taken.

Off-site surveillance activities, including actions taken.

Job safety evaluations stating what was checked, results, and instructions or corrective actions.

Instructions given/received and conflicts in plans and/or specifications.

Contractor's verification statement.

These records shall indicate a description of trades working on the project; the number of personnel working; weather conditions encountered; and any delays encountered. These records shall cover both conforming and deficient features and shall include a statement that equipment and materials incorporated in the work and workmanship comply with the contract. The original and one copy of these records ih report form shall be furnished to the government daily within 24 hours after the date(s) covered by the report, except that reports need NOT be submitted for days on which no work is performed. Reports shall be signed and dated by the QC manager. The report from the QC manager shall include copies of test reports and copies of reports prepared by all subordinate quality control personnel.

10. Interface with Government Inspectors.

The contractor shall host a weekly meeting with government inspectors to address the status of all projects in progress. The meeting shall cover the progress of all work with regard to their respective project schedule, any problems encountered and corrective actions taken, and anticipated delays, if any. The contractor shall provide documentation of these meetings to the CO through the SABER Chief for official record.

11. Notification of Noncompliance.

The CO will notify the contractor of any detected noncompliance with the foregoing requirements. The contractor shall take immediate corrective action after receipt of such notice.

If the contractor fails or refuses to comply promptly, the CO may issue an order stopping all or part of the work until satisfactory corrective action has been taken. No part of the time lost due to such stop orders shall be made the subject of claim for extension of time or for excess costs or
