PERFORMANCE WORK STATEMENT (PWS)

WASHER AND DRYER SERVICE

1. DESCRIPTION OF SERVICES. The Contractor shall furnish all equipment, parts, labor, tools, supplies, materials and management necessary to install, maintain and repair Contractor owned washers and dryers at various locations on Altus AFB as shown in Appendix A.

1.1. EQUIPMENT SPECIFICATIONS. All washers shall meet current Energy Star requirements when installed. Dyers are desired to meet Energy Star, but are not required to meet Enery Star at this time. Washers installed after the contract award phase-in period are required to meet the existing regulation at time of installation.

1.1.2. All equipment shall be installed in accordance with (IAW) the National Electric Code (NEC), National Mechanical Code and other applicable codes. All gas operated equipment shall be installed IAW American National Standards Institute (ANSI) Z223.1, National Fuel Gas Code.

1.1.3. Equipment installed shall be manufactured on or after 2013 and be in “good” condition.

Good condition is defined as no scratches, dents, dings, discoloring of paint or rust. Equipment that is no longer in “good” condition shall be replaced by the Contractor at no additional cost to the Government using the requirements in paragraph 1.1.

1.1.4. The equipment must be convenient to operate and of rugged construction to withstand heavy use. All washers and dryers in the same building shall be of like make, model and color with a factory baked enamel finish. Equipment shall not be coin operated.

1.2. MACHINE REQUIREMENTS.

1.2.1. Washers. Washing machines shall have a dry clothes capacity of 15 pounds minimum.

Each washing machine shall be rated at 120 VAC, 60 Hz. Amperage rating of machines shall not exceed 15 amps. Each washing machine shall be equipped with a timer and have three (3) temperature selections with a minimum of two (2) cycles. Washers shall require no bolting down. Machines shall be designed so that if an unbalanced load condition occurs during spin phase, this condition will be automatically compensated for within the machine. The Contractcor shall furnish all water hoses and electrical cord/plug connections for the washer. All washers must not exceed 31 ½ inches in width.

1.2.2. Stacked Electric Washer Gas Dryer Combo Machines. Washer dryer combo machines shall be rated at 120 VAC, 60 Hz, 15 amp maximum power rating. Each washer shall have a timer and three (3) temperature selections with a minimum of two (2) cycles. Gas dryers shall have a dry clothes capacity of 15 pounds minimum; be equipped with a timer and have a minimum of three cycles. Gas dryers shall have an electric ignition for the burners, a gas pressure regulator, a safety thermostat on combustion chambers and a minimum input gas capacity of 20,000 BTU/hr. The Contractor shall furnish water hoses, electrical cord/plug connections and flexible gas connectors for each gas dryers. Gas connections shall be American

Gas Association (AGA) approved 3/8” to ½” to include all fittings. Combo units are considered two machines (pockets), but shall be billed a one (1) unit.

1.2.3. Dryers. Dyers shall have a dry clothes capacity of 15 pounds minimum. Each machine shall be equipped with a timer; have a minimum of three (3) cycles. Dryers will have a door-operated safety switch which stops the drum and source of heat whenever the door is opened.

Machines must have an accessible lint screen. All dryers must not exceed 31 ½ inches in width.

1.2.4. Gas Dryers. Gas dryers shall operate on 120 VAC, 60 Hz, 15 Amp maximum power rating. Gas dryers shall have an electric ignition for the burners, a gas pressure regulator, a safety thermostat on combustion chambers and a minimum input gas capacity of 20,000 BTU/hr.

The Contractor shall furnish all electrical cord/plug connections and flexible gas connectors for each gas dryers. Gas connections shall be AGA approved 3/8” to ½” to include all fittings.

1.2.5. Stacked Gas Dryers. Each gas dryer shall operate on 120 VAC, 60 Hz, 15 Amp maximum power rating. Gas dryers shall be provided with electric ignition for the burners, have a gas pressure regulator, safety thermostat on combustion chambers and a minimum input gas capacity of 20,000 BTU/hr. The Contractor shall furnish all electrical cord/plug connections and flexible gas connectors for each gas dryers. Gas connections shall be AGA approved 3/8” to ½” to include all fittings. Stacked dryers are considered two machines (pockets), but shall be billed as one (1) unit.

1.2.6. Electric Dryers. Electric dryers shall be rated at 220 VAC, 60 Hz. The Contractor shall furnish all electrical cord/plug connections.

1.3. EQUIPMENT INSTALLATION. All machines furnished under this contract shall be phased in during the first thirty (30) calendar days after contract award. The installation schedule shall be coordinated with the Contracing Officer (CO) and the Contracting Offier Representative (COR) prior to commencement of work on utility hook-ups. All machines required under this contract shall be fully installed and operational within thirty (30) calendar days of the awarded contract.

1.3.1. The Contractor shall inspect the area where the machines will be installed. If the Contractor finds utility issues (such as broken water pipes, slow drains, etc.) that would prevent the installation of the machines, the Contractor shall contact the COR to report the issue.

1.3.2. The Contractor is responsible for connection of washers and dryers to existing electrical outlets, gas hook-ups, drains, water pipes and vents, to include supplying any necessary utility hoses for hook-up, vent adapters, inlet clamps and vent hoses. The Contractor shall furnish all flexible gas lines, all flexible water hoses and flexible dryer hoses to connect all equipment to building main lines.

1.3.3. Each machine shall be labeled with a unique ID number for easy identification.

1.3.4. New dormitories are expected to be built and ready for occupancy during the term of this contract. This may require additional washer and dryers be added to the contract.. Equipment may be added to buildings upon notification by the CO by issuance of a contract modification.

1.4. MAINTENANCE AND REPAIR. Prior to performing scheduled maintenance, the Contractor shall check in with the COR. The Contractor shall perform operational inspections and routine maintenance to ensure the machine and all parts are operating properly. The Contractor shall perform all maintenance during duty hours. Machines that cannot be repaired to a “good” condition shall be removed and replaced by the Contractor’s within ten (10) duty days.

All maintenance and repairs shall be at no cost to the Government.

1.4.1. The COR shall notify the Contractor of all washer/dryer failures requiring repair service by email. The Contractor shall respond on-site to all service repair calls within two (2) duty days.

1.4.2. Washers. During scheduled maintenance, the Contractor shall inspect and repair/replace all supply hoses, splitter connections, drain lines, power cords, filters, agitators, drive belts, dials, knobs, switches, tighten/replace screws, bolts and other machine parts, as required, to ensure proper and safe operation. The Contractor shall level the machines as required.

1.4.3. Dryers. The Contractor shall inspect and repair/replace all electrical connections, power cords, knobs, dials, switches, exhaust hoses/ducts and other machine parts, as required, for proper operation.

1.4.4. Ducts. The Contractor shall inspect and clean all ducts on an annual basis. Fixed duct work shall be cleaned from the dryer to the outside vent. All dirt, debris, lint, etc., shall be removed from inside of the duct work.

1.4.5. The Contractor shall ensure housekeeping practices are performed to prevent any buildup of lint in the interior and exterior components of the machines and in the utility spaces behind the machines.

1.4.6. The Contractor shall notify the COR (580-481-7756/5674/5675) if he believes a defect exists in the utilities service the washers and dryers.

1.4.7. Any machine that becomes unsightly (dented, soiled, rusted, etc.) shall be refinished or replaced at no additional cost to the Government. Washing machines will be kept clean between the top of the drum and the washer cabinet where dirt, lint and moisture can collect. Machines that are unserviceable shall be replaced at no additional cost to the Government.

1.5. EQUIPMENT RELOCATION/MOVEMENT. After the initial thirty (30) days of the contract, dormitory or temporarily lodging buildings may be scheduled for renovations or demolition during the performance period of the contract. The Contractor may be required to install machines, relocate machines to other buildings or remove machines upon direction by the

CO.

1.6. REPORTS. Service reports shall be given to the COR within ten (10) duty days from completion of scheduled maintenance/operational inspection or service repair calls. The report shall include the unique machine ID #, a complete list of service performed and any repairs that were made to each machine.

1.6.1. The Contractor shall submit a machine inventory report within 10 calendar days after initial installation. The report shall include location building number, unique machine ID number, manufacturer, model number and serial number. Any changes to machine inventory or locations shall be documented on the inventory and submitted to the COR within 10 calendar days of change.

2. SERVICE SUMMARY (SS). The Contractor service requirements are summarized into performance objectives that relate directly to mission essential services. The stated performance thresholds are the minimum acceptable levels of service for each stated requirement. All aspects of the Contractor’s performance are subject to Government surveillance and absence of any contract requirement from the service summary table shall not detract from its enforceability nor limit the rights or remedies of the Government under contract clauses.

2.2. QUALITY ASSURANCE. The CO and COR will assess the Contractor’s performance and adherence to standards using the SS Table. The COR shall use 100% surveillance, periodic surveillance, random sampling and/or customer complaints as dictated by the SS Table. If inspection reveals unacceptable performance, the COR shall notify the CO and Contract Administrator (CA). The CA will then notify the Contractor and require re-performance of the service (referred to as “call back”). The Contractor shall be allotted time as agreed to by all parties, but no longer than 48 hours, to return to Altus AFB and accomplish corrective call back work.

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES.

3.1. GOVERNMENT FURNISHED PROPERTY. The Government shall furnish real property including electrical outlets, in wall fixed ductwork, at wall plumbing shut-off connections and at wall gas shut-off connections. The Government will furnish without cost to the Contractor a reasonable amount of electricity and potable water in connection with the contract performance. The Contractor shall practice energy conservation in the Government provided facilities.

3.2. GOVERNMENT FURNISHED SERVICES. The Government shall provide police and fire protection. In the event of a medical emergency, call “911” for transportation of an injured employee by civilian ambulance to a local hospital. The Government shall provide emergency medical treatment for life-threatening emergencies only until civilian ambulance arrives.

3.3. DAMAGE TO GOVERNMENT FACILITIES AND PROPERTY. The Contractor shall repair, at his expense, any damage to Government property from the movement of washers and dryers into or out of any buildings. These repairs shall be made by a craftsman of the particular trade in an approved manner after approval by the Contracting Officer.

3.3.1. The Contractor shall reimburse the Government if details of formal investigation of fire or other related or unrelated structural damage result in pecuniary liability assessed to the Contractor. The Contractor shall replace all damaged or lost Government Furnished Facilities, Government Furnished Equipment and Government Furnished Materials at no cost to the Government.

4. GENERAL SERVICES.

4.1. CONTRACTOR PERSONNEL. Contract personnel shall present a neat appearance.

Contract personnel shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wear of distinctive clothing, overcoats or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection.

4.2. CONTRACTOR QUALIFICATIONS. The Contractor shall provide qualified personnel to install and repair washing machines, electric dryers, gas dryers, gas piping and all other work to be performed under this PWS.

4.3. SECURITY REQUIREMENTS. The Contractor shall comply with Altus AFB security measures. These regulations are available for review in the contracting office. Security and base access requirements are contained in clause 5352.242-9000 “Contractor Access to Air Force Installations”. Contract employees will be required to obtain and display identification badges.

Contractor employees will provide information to assist in the completion of the background check. This information includes, but is not limited to: full name (first, middle, last), social security number, date of birth, address, driver’s license number and state of issue, and the number of years employee has been a resident of the State of Oklahoma. This information will be protected under the procedures listed in the Privacy Act of 1974. A background check must be completed prior to unescorted access while on Altus AFB. If the national agency background check reveals a Contractor employee or prospective employee with an adverse criminal history or as a potential threat to the health, safety, operational mission of the installation, then the Government reserves the right to restrict access of that individual.

4.3.1. Contractor Badges. The Contractor is responsible for tracking and retrieving badges from employees upon their separation, termination and/or completion of business on Altus AFB, OK. A list of current Contractor employees issued security badges must be provided/submitted to the CO or COR. Badges from employees who are no longer employed on the contract must be retrieved and turned in to the CO or COR within 30 days of the individual’s separation or termination. Employees who abuse their badge privileges by attempting base access beyond the dates, times, locations or specified work and other limitations may be subject to revocation of base access. Contractors must take necessary steps to ensure that badge use is controlled and that badges are turned in to the CO or COR when an employee is no longer employed by the Contractor. Failure to control or turn in badges may result in contract termination or a decision not to exercise the option periods of this contract.

4.3.2. Anti-Terrorism. Altus Air Force Base has an antiterrorism program that is responsible for helping secure and protect the base and its personnel from terrorist attacks. As a Contractor working on Altus Air Force Base, you and your sub-contractors are required to obey all orders from Security Forces and adhere to all security measures implemented as a result of Force Protection Condition changes (security posture changes). Additionally, you and your sub-contractors are expected to report to base security forces (481-7444) any suspicious activities, packages or items you see while conducting work on Altus Air Force Base. Suspicious activities could include personnel conducting surveillance of the installation, unauthorized personnel requesting access to the installation or somebody asking a lot of questions about the base. In an effort to help familiarize you, your employees, and sub-contractors about antiterrorism, Altus Air Force Base has an antiterrorism binder with all of the pertinent information for awareness training available for review at the Pass and Registration Office located at the south gate (building #428) of Altus Air Force Base, Oklahoma.

4.3.3. Vehicles. The Contractor shall provide and maintain vehicles for transportation to meet the requirements of this contract. All Contractor vehicles used in the performance of this contract shall be readily identified as property of the company and be maintained to present a professional appearance at all times. Contractor personnel shall operate vehicles in compliance with base traffic regulations. Any violations of base traffic regulations are considered federal offenses and are adjudicated at federal court in Oklahoma City, OK. The current policy for Contractor personnel receiving a Driving Under the Influence (DUI) charge, on base, shall result in that person having their driving privileges on base revoked for a minimum of one year. See clause AFFARS 5352.242-9000, entitled “Contractor access to Air Force Installations”. All vehicles shall be registered, insured, and operated by a licensed operator at all times.

4.4. VANDALISM. If vandalism is suspected, the Contractor shall contact Security Forces (481-7444) to file a report. The Contractor shall also contact COR, who will notify the CO of the suspected vandalism. The Contractor shall send a copy of the completed Security Forces report to the CO.

4.5. SAFETY. The Contractor shall comply with all safety and health requirements necessary for the protection of personnel, facilities and equipment including, but not limited to, all applicable Occupational Safety and Health Administration (OSHA) standards.

4.5.1. The Contractor shall replace any machine, that when used, would violate fire or safety regulations or that would pose a risk to life or property. The machine shall be removed from the inventory immediately and replaced with the same model machine (or suitable substitution) within ten (10) duty days. An updated machine inventory shall be sent to the CO and COR within ten (10) duty days after replacement.

4.6. ENVIRONMENTAL AND HAZARDOUS MATERIAL/WASTE (HM/W)

REQUIREMENTS. Hazardous Materials (HM) are defined in 49 CFR 172.101 and 40 CFR 301-304. Absolutely no HM shall be brought onto Altus AFB until that material is authorized for use by the Civil Engineer Environmental Element at (580) 481-7156. The Contractor shall submit a list of chemicals with attached Material Safety Data Sheets (MSDS) and other supporting documentation for each HM prior to their use to the CO, the Environmental Element, and COR. The Contractor shall take appropriate actions to comply with waste minimization and pollution prevention practices and policies.

4.6.1. Notification of Environmental Spills. In the event that the Contractor spills or releases any hazardous substances or hazardous waste into the environment, the Contractor shall immediately report the incident to the Fire Department, the COR and the CO. The Contractor shall be liable for the costs of clean up and remediation of any spills or the release of such substances into the environment.

4.6.2. Contractor Environmental Awareness Training. The Contractor shall ensure all employees that work on Altus AFB complete the Altus AFB Environmental Management System (EMS) Awareness training IAW AFI 32-7001, paragraph 5.6, Environmental Management. The EMS Contractor Awareness training can be accessed at http://www.altus.af.mil/altusafbenvironmentalnews.asp. This is a one-time requirement for all current and new Contractor/contract employees working on Altus AFB.

4.7. HOURS OF OPERATION. The Contractor shall perform services during normal duty hours which are 0730 to 1630, Mondays through Fridays, excluding federal holidays. The Contractor must request/coordinate to work weekends or holidays through the CO and COR.

4.7.1. Recognized Holidays. The Contractor is not required to provide service on federal holidays. The holidays are:

If these holidays fall on Saturday, the preceding Friday will be observed. If these holidays fall on Sunday, the following Monday will be observed. If a holiday falls on a scheduled service http://www.altus.af.mil/altusafbenvironmentalnews.asp day, the Contractor will be responsible for rescheduling services for the first day post the holiday observance.

4.7.2. Base Closures. Work scheduled but not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base.

4.7.3. Performance of Service During Crisis or Heightened Security. Services at Altus AFB are not considered mission essential and service will not continue in the event of a crisis or heightened security caused by a national emergency, natural disaster or other cause, as necessary, in support of the Air Force mission. Contractor employees with approved background checks will be allowed access to the base for performance of work, except during “Charlie” conditions.

If the “Charlie” condition exists for an extended duration, some services may be required. The CO may direct that services continue on a case-by-case basis.

4.8. PHASE-IN. The Contractor shall submit a phase-in installation schedule to the CO and COR at the contract pre-performance meeting. Phase-in of machines shall include removal of old machines and complete installation of new machines. The Phase-In process shall be completed at no extra cost to the Government. The entire installation shall be completed not later than 30 calendar days from the notice to proceed. The Contractor shall use existing washer/dryer outlets and all debris generated from installation shall be removed during the 30-day period. The schedule may be altered to accommodate cooperation of Contractors phasing-in and phasing-out, provided the Contractor ensures that no laundry facility or building has less than 20% of washer and dryer appliance capacity for more than 24 hours. The schedule is tentative and can be adjusted when coordinated with the CO or COR. The Contractor shall supply all necessary repair parts, tools, equipment and vehicles to perform proper installation.

4.8.1. Machine Labels. All machines will have a unique ID # identified on a waterproof self-adhesive label. Washer identification shall include the letter “W” with four digits and numbered consecutively, in sequence, thereafter (i.e. W0001). Dryer identification shall include the letter “D” with four digits and numbered consecutively, in sequence, thereafter (i.e. D0001). The label may include the COR phone number to report broken or malfunctioning equipment.

4.9. PHASE-OUT. Phase-out shall include the complete removal of old machines. The Phase-Out process shall be completed at no extra cost to the Government. The Contractor shall submit a phase-out schedule to the CO and COR 30 days prior to the end of the contract. It is understood that if the contract is awarded to another vendor other than the incumbent Contractor, the Contractor agrees to fully cooperate with the subsequent Contractor and Government personnel to ensure that no laundry room has less than 20% of washer and dryer appliance capacity for more than 24 hours. Phase-out shall be completed within 30 days of contract end date.
