1. The 1st Special Operations Contracting Squadron at Hurlburt Field, Florida intends to establish a Firm Fixed Price contract for Medical Information Management and Information Technology Support Services.

3. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.
4. The Request for Quotation (RFQ) number FA441721Q0027 shall be used to reference any written quote provided under this RFQ.
5. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-07 effective 10 Sep 2021, DFARS Change 30 August 2021, and Air Force Acquisition Circular 2021-0726 effective 13 Sep 2021.
6. In accordance with FAR 19.1505, this requirement is set-aside for Women-Owned Small Businesses. The North American Industry Classification System (NAICS) code for this project is 541513 with a size standard of $30,000,000.

7. The contractor shall provide Medical Information Management and Information Technology Services in accordance with the Performance Work Statement (PWS). Please see Attachment 3, Quote Sheet, it contains the description of requirements for the Contract Line Item Numbers (CLIN) items to be acquired and the date and place of the period of performance.
8. Addendum to FAR Provision 52.212-1, Instructions to Offerors – Commercial Items (JUN 2020) applies to this acquisition and the following addendum applies:
INSTRUCTIONS TO OFFERORS
Offerors are cautioned to follow the detailed instructions fully and carefully, as the Government reserves the right to make an award based on initial offers received, without discussion of such offers. Non-responsive offers will not be evaluated.
1.0. GENERAL INSTRUCTIONS
1.1. Minimum requirements. As a minimum, offers must include the following information to be eligible for award:
(1) The name, address, and telephone number of the offeror
(2) Acknowledgment of all Amendments
(3) Price Submission
(4) Technical Submission
(5) Past Performance submission to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information)
1.2. Offerors are responsible for submitting proposals (including any revisions, and amendments) so as to reach the Government office designated in this RFQ by the time specified in the RFQ.
Only electronic versions of the proposals will be accepted. Offers must be submitted through e-mail, in accordance with this RFQ.
2.0. SPECIFIC INSTRUCTIONS
The following instructions are to aid in the evaluation process.
2.1. Organization/Number of Copies/Page Limits. A complete proposal shall consists of three
(3) volumes: Volume I, Price; Volume II Technical; Volume III Past Performance.

2.2. Pages exceeding the page limitations will not be read or evaluated and will be removed from the offer.
3.0 VOLUME I – PRICE
3.1. Pricing must be submitted on the Quote Sheet, Attachment 3. Submit one (1) copy.
3.2. Fully burdened labor hour prices for all Contract Line Items to include options years must be submitted. Include any discount terms.
3.3. Offerors shall round fully-burdened labor hour rate to no more than two decimal places.
4.0 VOLUME II – TECHNICAL
MANAGEMENT PLAN
Offerors shall submit a Management Plan that clearly demonstrates offerors' understanding of the managing and staffing requirements of the PWS.
The Management Plan shall detail how the offeror intends to recruit, retain, and replace sufficient personnel who have the knowledge, experience, and skills to perform Information Management / Information Technology support services as required in the PWS.
The Management Plan shall detail how the personnel proposed will meet duties described in sections 1.4.2.7 through 1.4.2.9, 1.4.3.1, 1.4.3.4, 1.4.3.7 and 1.4.4.13.
The Management Plan shall address how the offeror plans to meet and maintain the qualifications for each designated job description outlined in paragraphs 4.9 of the PWS.
The Management Plan shall also address how the offer will meet and maintain the secret security clearance requirements and the guidelines of the required DD Form 254 detailed in paragraphs
4.7.1 through 4.7.4.
The Management Plan shall identify how the offeror intends to continue uninterrupted services during personnel absences, such as sickness, vacations, etc. without any degradation in services provided.
Quality Control Plan
Offerors shall submit a written plan clearly describing the Quality Control Procedures the offeror
4 | F A 4 4 1 7 2 1 Q 0 0 2 7 plans use to ensure compliance with PWS paragraph 4.2 and Service Summary (PWS section 2.0).
The offeror shall identify the procedures the offeror plans to use to analyze, prevent and monitor defects in the quality of service performed as well as corrective actions which shall be performed to ensure nonperformance and continual repeat of defective services do not occur, and compliance with the contract terms and conditions.
The offeror shall include a detailed schedule of inspections that the offeror plans to conduct per month, quarter and year to conform with section 4.2.2
5.0. VOLUME III – PAST PERFORMANCE
5.1. Submit a list of no more than five (5) recent and relevant contracts performed for Federal agencies or commercial customers as a prime contractor that demonstrates the ability to successfully perform the scope and breadth of the requirements as described in the PWS for this solicitation. Any past performance references in excess of the first five (5) submitted will not be evaluated. Information on each contract is limited to no more than 3 pages per contract listed and fifteen (15) pages total for Volume III. Submit one (1) copy.
5.2. Only references for relevant (same or similar) type of contracts are desired.
5.3. Provide a summary of the previous contracts described above, not to exceed 3 pages of past performance information per reference. The summary should explain what aspects of each contract are deemed recent and relevant as related to the requirements of this RFQ. Failure to provide required relevancy description may impact the confidence rating. The summary should include:
a) Company/Division name
b) Name of project (Contract number, if applicable)
c) Services/ Number and type of worker(s) provided
d) Name and address of customer or Government agency
e) Name, address, telephone, email address of the customer contact or contracting officer
f) Dollar value
g) Period of performance/Completion date
h) Comments regarding compliance with contract terms and conditions.
i) Comments regarding any known performance deemed unacceptable to the customer, or not in accordance with the contract terms and conditions, the resolution(s) implemented and results.
5.4. Proposals shall provide recent and relevant information regarding an offeror’s actions under previously awarded contracts.
a) Recency Assessment.
An assessment of the past performance information will be made to determine if it is recent. To be recent, the effort must be ongoing or must have been performed during the past three (3) years from the issuance month of this RFQ.

b) Relevancy Assessment The Government will conduct and in-depth evaluation of all recent performance information obtained to determine how closely the services provided under those contracts relate to the scope and magnitude/complexity of the Hurlburt Field AFB requirement described in this RFQ.
Offerors shall identify what elements of each contract are deemed relevant to the PWS in this RFQ and indicate what percentage of service was performed by the offeror (prime), subcontractor (if applicable) and teaming contractor (if applicable). The past performance questionnaires and information obtained from other sources will be used to establish the degree of relevancy of past performance. Consideration will be given to efforts that are considered to be of the type that are similar in scope and magnitude/complexity when compared to the services described in this solicitation.
5.5. Completed Past Performance Questionnaires, Attachment 4 of the RFQ. Submit one
(1) PPQ per contract referenced in Volume III. Any past performance questionnaires received in excess of the one (1) per contract referenced will not be evaluated. Once all questionnaires are completed by the evaluator, the information contained in the questionnaires shall be considered source selection sensitive and not releasable to you, the offeror. If the offeror claims there is no past performance, previously completed questionnaires and CPARS from other source selections may be submitted in lieu of Attachment 4, PPQ, for the five (5) contracts referenced in Volume III. Altered or substituted questionnaires will not be evaluated. Also, Past Performance Questionnaires will only be accepted electronically by email from Project Owners or their authorized representatives. Past Performance Questionnaires will not be accepted directly from Prime Contractors or Subcontractors. Offerors are responsible to ensure questionnaires are transmitted to their references, and to indicate which references the questionnaire was sent to.
5.5.1. Offerors shall track the questionnaires and ensure that the individuals below receive them by the RFQ due date. It is also the sole responsibility of offerors to inform the evaluators of any extensions the Government approves to the date and time proposal are due to be submitted.

5.5.2. Lack of any past performance will not automatically disqualify an offeror, but it is a factor that is considered in the Best Value Performance Price Tradeoff.
5.5.3. If the offeror claims there is no past performance, then that status must be identified to the contracting officer not later than the date/time proposals are due from all offerors.
5.5.4. The Government reserves the right to use both the information provided in the offeror's Past Performance submittals and information obtained from other sources available to the

Government, to include, but not limited to, the Past Performance Information Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or other databases; interviews with Program Managers and Contracting Officers; the Defense Contract Management Agency (DCMA), and commercial sources and/or nongovernment sources, such as any commercial business or customers referenced by the offeror in any of the past performance information disclosed.
5.6 Signed Teaming Agreement - If Teaming Agreements are contemplated they must comply with the appropriate Federal Regulations (13 C.F.R. Part 121 (Size Regulations), Part 124 (8(a) & SDB), Part 125 (Government Contracting Programs), and/or Part 126 (HUBZone). The teaming members must provide complete information as to relevant and recent past performance information on previous teaming agreements. If this is a first time joint effort, each party to the teaming agreement must provide information on all relevant contracts as specified in paragraph 5.1.
5.6.1 The maximum number of references combined shall not exceed 5 total.
5.6.2 The Government will recognize the integrity and validity of formal contractor teaming agreements, provided, the agreements are identified and company relationships are fully disclosed in an offer. A teaming agreement must establish each party's role in the proposal preparation process and will be incorporated into the contract. Failure to clearly define roles and/or provide a teaming agreement with a proposal shall make teaming arrangements and related subcontractor past performance ineligible for evaluation and consideration of award. The prime contractor shall remain fully responsible for contract performance, regardless of any teaming agreement between the prime contractor and its subcontractors. Teaming Agreements must provide the following information as part of their proposal, Volume II (b), not later than the date/time proposals are due:
5.6.2.1 Clearly establish roles of each party (who is prime and who is subcontractor, who is responsible for what tasks, contract administration, proposals, work management, etc.)
5.6.2.2. Provide for protection of competition-sensitive proprietary information. (Subcontractor past performance cannot be disclosed to the prime offeror without the subcontractor's consent).
Provide a letter from all subcontractors that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor, only if it is being used as part of the past performance evaluation.
5.6.2.3 Identify each member's share of the prospective contract, 50/50, 51/49, etc.
5.6.2.4 Exclusivity. Assurance that the team member will not be replaced for the duration of the contract, any exceptions should be identified. Assurance that the team members are not teaming with another firm for the same procurement.

5.6.2.5 Statement of acknowledgement that the Prime Contractor is responsible for adhering to contract terms and conditions and daily management. The Prime Contractor is obligated to negotiate in good faith and responsible for conveying mandatory government terms and conditions to subcontractors.
5.6.2.6 The prime contractor shall remain fully responsible for contract performance, regardless of any teaming agreement between the prime contractor and its subcontractors.
5.6.3 At the time of initial contract offer and at time of award, each business must be a small business. In order to submit an offer on a contract, each business concern in a teaming agreement must be a small business under the designated NAICS size standard, and must also be small when combined in the aggregate of all teaming affiliates.
6.0. RELATIONSHIP BETWEEN INSTRUCTIONS AND EVALUATION
Your attention is directed to the functional relationship between the sections: Instructions, Conditions, and Notices to Offers and Evaluation Factors for Award of this RFQ. Instructions, Conditions, and Notices to Offers provides information for the purpose of organizing the proposal and is not intended to be all-inclusive. Evaluation Factors for Award describes evaluation factors for award. Since the Government evaluation of proposals will cover areas identified in Evaluation Factors for Award, proposals should address all such areas of evaluation.
7.0. AMENDMENT OF SOLICITATION PRIOR TO CLOSING
The Government reserves the right to revise or amend the performance work statement, or the solicitation prior to the proposal closing time. Such reservations or amendments will be communicated by amendments to the RFQ and posted on Contract Opportunities via SAM.gov.
If such amendments require material changes in quantities or prices, the proposal closing date may be postponed by enough days to enable offerors to revise their proposals. In such cases, the amendment will include an announcement of the new closing date and time. Offerors MUST acknowledge all amendments in their proposal, either by providing signed copies of the amendments with their proposal, or by separate letter which includes a reference to the solicitation and amendment numbers. Amendments must be included with Volume I.
8.0. QUESTIONS
All questions regarding this RFQ must be submitted in writing. All questions and subsequent answers will be posted to Contract Opportunities via SAM.gov. It is the responsibility of the contractor to continuously monitor the site for updates. All questions must be submitted electronically to the individual(s) listed below by 14 October 2021 at 2:00 PM CST. Any questions submitted after this period may not be answered.

9. Addendum to FAR 52.212-2 Evaluation - Commercial Items (OCT 2014) Evaluation Factors for Award
1.0. BASIS FOR CONTRACT AWARD.
1.1 Basis for Contract Award. The Government will review proposals received to determine if they are responsive to the RFQ requirements. A responsive proposal is one that meets all of the terms, conditions, and specifications in the RFQ. The offeror must provide all documents listed and comply with all RFQ. Only responsive proposals will be evaluated for award.
This acquisition will utilize the Best Value approach, in which the Government seeks to award to an offeror who provides the greatest confidence that they will meet the Government's requirements affordably. Award will be made to the offeror providing an offer that is deemed most advantageous to the Government. This Performance Price Tradeoff approach will include an integrated assessment where technical and past performance, when combined, are significantly more important than price. The trade-off process permits tradeoffs among price and non-price factors and allows the Government to award to a higher priced offeror when the decision is consistent with the evaluation factors and the Contracting Officer reasonably determines the higher priced offeror outweighs the price difference. To arrive at a best value decision the CO will integrate the evaluation of the factors described in this RFQ. While the Government will strive for maximum objectivity, the tradeoff process is by nature, subjective;
therefore professional judgement is implicit throughout the ordering process.
1.2. Discussions and Clarifications. The Government intends to evaluate proposals and make award without discussions. Offerors may be given the opportunity to clarify certain aspects of their offers (e.g., defining the relevance of the offeror's past performance and adverse past performance information to which an offeror has not previously had an opportunity to respond) or to resolve minor clerical errors. Therefore, the offeror's initial proposal should contain the offeror's best terms from a past performance and price standpoint.
1.3. Options. For price evaluation purposes, only the offerors price will be ranked by adding the total of all Contract Line Item Numbers (CLINs). Evaluation of the CLINs will not obligate the Government to award all CLINs. In the best interest of the Government, the Government reserves the right to award All, Some or None of the CLINs.
2.0. EVALUATION FACTORS
The following evaluation factors will be used to evaluate proposals. Evaluation of proposals will occur using the Performance Price Tradeoff approach as defined in paragraph 1.1
Factor 1: Price Factor 2: Technical Factor 3: Past Performance
In accordance with FAR 15.304(e), all evaluation factors other than Price, when combined, are significantly more important than price.

3.0. PRICE FACTOR
The offeror’s Cost/Price proposal will be evaluated to determine if it is complete, fair and reasonable, and no unbalanced pricing exist. Offerors whose price is determined to be incomplete or unreasonable will not be considered for award. Additionally, an offer may be rejected if the Contracting Officer determines unbalanced pricing poses an unacceptable risk to the Government.
3.1. Completeness. The Government will review the Price proposal for completeness and compliance with the RFQ.
3.2. Reasonableness. Must represent a price to the Government that a prudent person would pay in the conduct of competitive business. The Government may use various price analysis techniques and procedures to ensure a fair and reasonable price.
3.3. Unbalanced Pricing. The Government will analyze each proposal to determine whether they are unbalanced with respect to prices proposed on different CLINs for the same or similar services in the same or similar quantities with respect to option pricing.
3.4. Total Evaluated Price (TEP). The offeror’s TEP will be used for evaluating the price proposal. The TEP will be calculated as follows:
Total Evaluated Price = Base Period (CLIN 0001) + Option Period 1 (CLIN 1001) + Option Period 2 (CLIN 2001) + (Total Evaluated Price for Option to Extend Services IAW FAR 52.217- 8)
Quantities identified will be multiplied by the unit price to confirm the extended price and then the extended price for all CLINs will be added together for a total for that specific period. Unit prices shall be used in the event that extended prices are calculated incorrectly. If unit prices are extended past two (2) decimal points, the Government will calculate using two (2) decimal points.
3.4.1. Total Evaluated Price for Option to Extend Services IAW FAR 52.217-8. The Government will calculate the Total Evaluated Price for Option to Extend Services IAW FAR 52.217-8 by adding the sum by multiplying the following by the unit prices proposed;
CLIN 0001 – 12 months CLIN 1001 – 6 months CLIN 2001 – 6 months
The TEP for the 52.217-8, Option to Extend Services six month extension is included in the TEP for evaluation purposes only. The Option to Extend Services is not considered part of the contract period of performance and resultantly these totals will not be included in the contract

If the Government exercises FAR 52.217-8, Option to Extend Services, the extension will not exceed six (6) months total and CLINS will be added to the contract at the time FAR 52.217- 8 is exercised at a monthly price equal to the preceding period of performance's monthly price in accordance with FAR 52.217-8. The Government will add CLINs priced in award with FAR 52.217-8 "Option to Extend Services" at the time it's exercised.
4.0 TECHNICAL FACTOR
Offers will be evaluated for technical acceptability. Only those offerors determined to be technically acceptable will be considered for further evaluation. The rating assigned to this factor will be based on the offeror's technical volume. The validity and thoroughness of the offeror's technical volume will be evaluated as specific evidence of the offeror's understanding and capability to perform this requirement. An overall rating will be assigned; failure to be assigned an acceptable rating in any element will render the entire technical volume unacceptable.
4.1. Evaluators shall assign a rating, which falls within one of the following definitions according to the degree which the offeror's technical proposal meets the minimum requirement described below:
4.1.1. Acceptable: Proposal meets the requirements of the solicitation.
4.1.2. Unacceptable: Proposal does not meet the requirements of the solicitation.
4.2. Subfactor 1: Management Plan. The Management Plan will be rated acceptable if it meets all the requirements of paragraphs 4.2.1 through 4.2.6. below.
4.2.1. The Management Plan clearly demonstrates a reasonable approach to managing and staffing the contract based on the requirements of the PWS.
4.2.2. The Management Plan clearly demonstrates how the offeror intends to recruit, retain, and replace sufficient personnel who have the knowledge, experience, and skills to perform Information Management / Information Technology support services as required in the PWS.
4.2.3. The Management Plan clearly demonstrates how the personnel proposed will meet duties described in sections 1.4.2.7 through 1.4.2.9, 1.4.3.1, 1.4.3.4, 1.4.3.7 and 1.4.4.13.
4.2.4. The Management Plan clearly demonstrates how the offeror plans to meet and maintain the qualifications for each designated job description outlined in paragraphs 4.9 of the PWS.
4.2.5. The Management Plan clearly demonstrates how the offer will meet and maintain the secret security clearance requirements and the guidelines of the required DD Form 254 detailed in paragraphs 4.7.1 through 4.7.4.
4.2.6. The Management Plan clearly demonstrates identifies how the offeror intends to continue uninterrupted services during personnel absences, such as sickness, vacations, etc. without any degradation in services provided.

4.3 Subfactor 2: Quality Control Plan: Offerors provides a written plan clearly describing the Quality Control Procedures the offeror plans use to ensure compliance with PWS paragraph 4.2 and Service Summary (PWS section 2.0).
4.3.1. 5.0 PAST PERFORMANCE FACTOR
The Past Performance evaluation will result in an overall performance confidence assessment as defined below. This performance confidence assessment represents the Government's judgment of the probability of an offeror successfully accomplishing the proposed effort based on the offeror's demonstrated past and present performance. The performance confidence assessments are defined as follows:

SATISFACTORY CONFIDENCE: Based on the offeror's recent/relevant performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort.
UNKNOWN CONFIDENCE (NEUTRAL): No recent/relevant performance record is available or the offeror's performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned. The offeror may not be evaluated favorably or unfavorably on the factor of past performance.
LIMITED CONFIDENCE: Based on the offeror's recent/relevant performance record, the Government has a low expectation that the offeror will successfully perform the required effort.
NO CONFIDENCE: Based on the offeror's recent/relevant performance record, the Government has no expectation that the offeror will be able to successfully perform the required effort.
5.1. Past/Present Performance Questionnaires and Past Performance Information Retrieval System (PPIRS) Submissions. Offerors shall provide information regarding past or current contracts (including Federal, State, local government, and private) for efforts similar to the Government requirement as stated in this solicitation through either questionnaires or evaluations from PPIRS as identified in the Evaluation Factor for Award.

5.1.1. In the event an Offeror submits a Past Performance Questionnaire and it is subsequently determined that the referenced project has been formally evaluated in PPIRS, the ratings in PPIRS shall take precedence.
5.1.2. In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available, the offeror will not be evaluated favorably or unfavorably on past performance. Therefore, the offeror shall be rated as “Unknown Confidence (Neutral)”.
5.1.3. Adverse past performance is defined as past performance information that supports a less than satisfactory rating on any evaluation element or any unfavorable comments received from sources without a formal rating system. Offerors may be asked to clarify or respond to adverse past performance information to which the offeror has not previously had an opportunity to respond.
5.2 Past Performance Evaluation Process. The past performance evaluation considers each offeror’s demonstrated recent and relevant record of performance in supplying products and services that meet the contract’s requirements. Performance confidence is assessed at the overall Past Performance factor level after evaluating aspects of the offeror’s recent past performance, focusing on performance that is relevant to the tasks outlined in the PWS of the RFQ. The Government reserves the right to use both the information provided in the offeror's Past Performance submittals and information obtained from other sources available to the Government, to include, but not limited to, the Past Performance Information Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or other databases; interviews with Program Managers and Contracting Officers; the Defense Contract Management Agency (DCMA), and commercial sources and/or nongovernment sources, such as any commercial business or customers referenced by the offeror in any of the past performance information disclosed.
Past performance regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement may or may not be considered as highly as past performance information for the principal offeror.
Past performance regarding minor members of Joint Ventures will be considered in accordance with the member's share of the prospective contract.
5.2.1. Recency Assessment. An assessment of the past performance information will be made to determine if it is recent. To be recent, the effort must be ongoing or must have been performed during the past three (3) years from the issuance month of this solicitation. Past performance information that fails this condition will not be evaluated.
5.2.2. Assessment. The Government will conduct and in-depth evaluation of all recent performance information obtained to determine how closely the services provided under those contracts relate to the scope and magnitude/complexity of the Hurlburt Field AFB requirement described in this RFQ. Offerors shall identify what elements of each contract are deemed relevant to the PWS in this RFQ and indicate what percentage of service was performed by the offeror (prime), subcontractor (if applicable) and teaming contractor (if applicable). The past performance questionnaires and information obtained from other sources will be used to

Consideration will be given to efforts that are considered to be of the type that are similar in scope and magnitude/complexity when compared to the services described in this solicitation. The Government will use the following relevancy definitions when assessing recent contracts:
VERY RELEVANT: Present/past performance effort involved essentially the same scope and magnitude of effort and complexities this RFQ requires. The offeror recruited, retained and replaced as necessary the following positions in a secure military and medical setting: System Administrator, Network Administrator and Cybersecurity Specialist (ISSO) on the same contract or task order. Responsible for Information Management / Information Technology Support Services with a workload of at least 400-500 users with at least 500-600 devices.
RELEVANT: Present/past performance effort involved similar scope and magnitude of effort and complexities this RFQ requires. The offeror recruited, retained and replaced as necessary the following positions in a secure military setting: System Administrator, Network Administrator and Cybersecurity Specialist (ISSO) on the same contract or task order. Responsible for Information Management / Information Technology Support Services with a workload of at least 300-400 users with at least 400-500 devices.
SOMEWHAT RELEVANT: Present/past performance effort involved some of the scope and magnitude of effort and complexities that this solicitation requires. The offeror recruited, retained and replaced as necessary the following positions (or positions with the same/similar duties) in a secure setting: System Administrator, Network Administrator and Cybersecurity Specialist (ISSO) on the same contract or task order. Responsible for Information Management / Information Technology Support Services with a workload of at least 300 users with at least 400 devices.
5.2.3. Quality of Performance. All recent past performance that is determined to be somewhat relevant, relevant, or very relevant will be reviewed to determine the quality of the offeror's performance, general trends, and the usefulness of the performance. This information will be used by the evaluation team in determining the overall performance confidence assessment rating.
5.2.4. More recent performance will have a greater impact on the Performance Confidence Assessment than less recent effort. A strong record of relevant past performance may be considered more advantageous to the Government than an "Unknown Confidence" rating.
Likewise, a more relevant past performance record may receive a higher confidence rating and be considered more favorably than a less relevant record of favorable performance.
5.2.5. Past performance regarding predecessor companies, key personnel who have relevant experience, or sub-contractors that will perform major or critical aspects of the requirement will not be considered as highly as past performance information for the principal offeror.
5.2.6. If a Teaming Agreement is submitted as part of the proposal, each teaming offeror's past performance will be evaluated separately using the same evaluation methods described above.
The Prime Contractor's past performance will be weighted slightly more than equal based on
14 | F A 4 4 1 7 2 1 Q 0 0 2 7 their overall responsibility for contract management. The separate ratings of each party in the Teaming Agreement will be combined to determine an aggregate PERFORMANCE ASSESSMENT CONFIDENCE RATING as defined above.
6.0. RFQ REQUIREMENTS, TERMS AND CONDITIONS
Offerors are required to meet all solicitation requirements, such as terms and conditions, representations and certifications, and technical requirements, in addition to those identified as factors or sub factors. Failure to comply with the terms and conditions of the solicitation may result in the offeror being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and must provide complete supporting rationale.
7.0. CLARIFICATIONS, DISCUSSIONS AND NEGOTIATIONS
The Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offerors initial offer should contain the offeror’s best terms from a price and technical standpoint. Offerors may be asked to clarify certain aspects of their proposal (for example, relevance of past performance information). Communications (Clarifications) conducted to resolve minor or clerical errors will not constitute discussions and the Contracting Officer reserves the right to award a contract without the opportunity for proposal revision.
8.0. EVALUATION PROCESS
All offerors will be ranked in order of price. The lowest priced, responsive, technically acceptable, offer shall be evaluated for past performance. If the lowest priced technically acceptable offer is judged to have a Substantial Confidence rating, that offer represents the best value for the government and the evaluation process stops at this point. Award shall be made to that offeror without further consideration to any other offers. If the lowest priced, responsive, technically acceptable offeror is not judged to have a Substantial Confidence rating, the next lowest priced responsive, techncially acceptable offeror will be evaluated for past performance and the process will continue in order by price until a responsive, technically acceptable offeror is judged to have a Substantial Confidence rating or until all offerors are evaluated. The Government shall then make an integrated assessment best value award decision based on all evaluated offerors.

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days.
