ADDENDUM TO FAR 52.212-1, INSTRUCTIONS TO OFFERORS – COMMERCIAL

ITEMS (JAN 2018)

Addendum to 52.212-1(b), Submission of offers. This paragraph is tailored as follows:

Quotations shall contain the following information:

• RFQ Number;

• Time Specified for Receipt of Quotations;

• Name, Address, and Telephone Number of Quoter;

• Name and Email address of Representative authorized to discuss quote;

• Any Discount Terms; and Acknowledgement of all Solicitation Amendments (if applicable);

• A completed copy of the representations and certifications at FAR 52.212-3 (see FAR

52.212-3(b) for those representations and certifications that the offeror shall complete electronically)

Quotations shall also include the following required documentation:

1. Price. Quoted prices must be submitted using the Pricing Schedule on the 1449 on page 3. Quoters shall insert unit prices in the attached pricing schedule, rounded to the nearest cent (for example 1.02 not $1.0231), and sign acknowledging the terms and conditions of the RFQ. Quoters rejecting the terms and conditions may be excluded from consideration. Note: Department of Labor (DOL) Wage Determination 2015-4532, Rev.

11, dated 07/16/2019, at Attachment 1, applies.

2. Technical. Offeror must provide the following certifications with their quote:

(a) A Baccalaureate degree from a regionally accredited American College or University demonstrated by a sealed official transcript.

(b) minimum of 2 year of professional work or volunteer experience demonstrated by a resume with, and verifiable by, at least one professional reference.

3. Past Performance. Offeror must provide a list of no more than three (3) recent and relevant contracts performed for Federal agencies or commercial customers as a prime contractor that demonstrates the ability to successfully perform the scope and breadth of the requirements as described in the PWS for this solicitation. Recency and Relevancy is defined in Attachment 3, Addendum to FAR 52.212-2. Any past performance references in excess of the first three submitted will not be evaluated. Submit the following information for each contract submitted for past performance evaluation:

• Company/Division name;

• Product/Service;

• Contracting Agency/Customer;

• Contract Number;

• Contract Dollar Value;

• Period of Performance;

• Verified, up-to-date name, address, e-mail address, telephone number of the

Government evaluator, contracting officer or customer POC responsible for procuring your services;

• Comments regarding compliance with contract terms and conditions; and

• Comments regarding any known performance deemed unacceptable by the customer, or not in accordance with the contract terms and conditions, the resolution(s) implemented and results.

• Explanation of how the scope of the referenced contract is relevant to the PWS in this solicitation.

Offerors' quotation shall conform to the required CLIN structure as referenced above. Any offeror not conforming to the required PWS and to this solicitation may be deemed non-responsive and therefore ineligible for award.

Addendum to FAR 52.212-1(c), Period for Acceptance of Offers. The paragraph is tailored as follows: The Offeror agrees to hold the prices in its offer firm for 60 calendar days from the date specified for receipt of offers.

Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors; failure to meet a requirement of the solicitation may be excluded from consideration. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale; and documents submitted in response to this solicitation must be fully responsive to and consistent with requirements of this solicitation, Government standards and regulations, and Evaluation Factors for Award and Attachment 3 , Addendum to FAR 52.212-2.
